Abstract Objective This study explores the postacute-stroke management problems, particularly for patients with total dependency (Barthel Index <20), in home care service of Dr. Sardjito Hospital (SH) from the hospital personnel's and caregiver's points of view. Materials and Methods In-depth interviews with a semi-structured interview guide were conducted with hospital personnel and patients' caregivers based on the purposeful sampling. There were 10 hospital personnel that were interviewed: the director of medical service, head of home care unit, neurologists, general practitioners, nurses, and physiotherapist. There were eight caregivers who participated in the study. Statistical Analysis Data from the interviews were analyzed using systematic text condensation using Nvivo 12 plus. Results Our findings showed that all health personnel in SH agreed that home care service is a part of an integrated health care service for continuation of care. However, the preparedness by the hospital management is still lacking in infrastructures, such as standardized operational procedure, quality control, and financial system, as well as in terms of competent human resources and their welfare. In addition, the patient's family and caregiver are lacking in knowledge and independency to take care of the patients with the need of home care services' monitoring to deliver the expected home care for postacute-stroke's patient regularly. Conclusion Home care service is an act of implementing hospital obligation to fulfill the patients' rights. An established hospital policy to ensure a comprehensive home care service delivery is necessary. The capability and welfare of the health care personnel should be put into account for the standardized human resources.

Objective: In the present study, it was aimed to determine the changes in the mental states of hospital personnel at the beginning of COVID-19 pandemic, and the factors affecting these changes. Methods: The research sample consisted of 299 hospital personnel in Turkey, who could be reached with the online method. The data were collected using the “Information Form”, which was prepared in line with the literature and the pandemic process, “The Impact of Event Scale (IES)” and the “Depression-Anxiety-Stress Scale (DAS-21)”. Results: It was found that the sociodemographic characteristics, medical history, and the characteristics related to COVID-19 were effective on the participants in terms of their mental states and the way they perceived traumatic experiences. According to the multiple linear regression analyses, it was found that hyper arousal and reexperiencing predicted stress, anxiety, and depression with a rate of 58.5%, 59.0% and 60.6%, respectively. Conclusion: At the beginning of the pandemic, it was observed that the responses of frontline hospital employees to traumatic experiences affected their mental states. Psychiatric nurses should take an active role in the development and implementation of psychosocial support programs for hospital personnel. Bangladesh J Medicine July 2022; 33(1) : 19-26

Around the world, the COVID-19 pandemic has generated clinical challenges for health personnel in general and hospital personnel in particular. In Mexico, the clinical psychologists who are part of the local hospital systems have adapted professional practices to provide mental health care in COVID-19 frontline hospitals. This text describes the actions and challenges arising from treating patients, families, and health workers in six COVID-19 hospitals in Mexico. It highlights the main problems identified, strategies to address them, and the barriers encountered during this pandemic. Finally, this document may be useful for planning clinical psychological activities within COVID-19 hospitals in places where new waves of contagion appear.

Abstract Background The goal of every emergency department is to provide the highest quality services in the shortest time using limited resources. However, occupational violence is so prevalent among pre-hospital paramedic personnel that some experts claim that it is impossible to find pre-hospital personnel without an experience of violence in the workplace. Therefore, it seems necessary to investigate the causes of violence among this population group and find ways to control it. Aim The present study aimed to investigate the Violence and influencing factors among paramedic pre-hospital personnel. Method This qualitative study was conducted to explore the views of a group of pre-hospital paramedic personnel (n = 45) selected through purposive sampling. The data was collected through in-depth and semi-structured interviews and analyzed using Graneheim and Lundman’s conventional content analysis methods. The trial version of MAXQDA 16 software was used to manage the coding process. Results Based on the results of the analysis of data collected from prehospital paramedic personnel, three main categories including: human factors, organizational factors, and environmental factors and 20 subcategories were detected. Conclusion If authorities neglect violence in the workplace and do not take serious actions to prevent it, violence and, more importantly, “hostility” will gradually prevail in the workplace. It also increases the stress and anxiety of staff and consequently severely deteriorates their job performance. Hence, authorities are strongly recommended not to ignore this issue and, instead, take measures, for instance hold workshops, to train personnel about the techniques of anger and violence control.

Abstract Real world studies have demonstrated impressive effectiveness of the BNT162b2 COVID-19 vaccine in preventing symptomatic and asymptomatic SARS-CoV-2 infection. We describe an outbreak of SARS-CoV-2 infections in a hospital with high vaccine uptake. We found low secondary attack rate(7%), suggesting low infectivity of vaccinated persons with vaccine breakthrough SARS-CoV-2 infections.
