BACKGROUND: Although many children with medical complexity (CMC) use home health care (HHC), little is known about all pediatric HHC utilizers. Our objective was to assess characteristics of pediatric HHC recipients, providers, and payments. METHODS: We conducted a retrospective analysis of 5 209 525 children age 0-to-17 years enrolled Medicaid in the 2016 IBM Watson MarketScan Medicaid Database. HHC utilizers had ≥ 1 HHC claim. Healthcare Common Procedure Coding System (HCPCS) and Current Procedural Terminology (CPT) codes were reviewed to codify provider types when possible: registered nurse (RN), licensed practical nurse (LPN), home health aide (HHA), certified nursing assistant (CNA), or companion/personal attendant. Enrollee clinical characteristics, HHC provider type, and payments were assessed. Chronic conditions were evaluated with Agency for Healthcare Research and Quality’s Chronic Condition Indicators and Feudtner’s Complex Chronic Conditions. RESULTS: Of the 0.8% of children who used HHC, 43.8% were age <1 year, 25% had no chronic condition, 38.6% had a noncomplex chronic condition, 21.5% had a complex chronic condition without technology assistance, and 15.5% had technology assistance (eg, tracheostomy). HHC for children with technology assistance accounted for 72.6% of all HHC spending. Forty-five percent of HHC utilizers received RN/LPN-level care, 7.9% companion/personal attendant care, 5.9% HHA/CNA-level care, and 36% received care from an unspecified provider. For children with technology assistance, the majority (77.2%) received RN/LPN care, 17.5% companion/personal assistant care, and 13.8% HHA/CNA care. CONCLUSIONS: Children using HHC are a heterogeneous population who receive it from a variety of providers. Future investigations should explore the role of nonnurse caregivers, particularly with CMC.

It is said that every adversity presents the opportunity to grow. The current pandemic is a lesson to all healthcare infrastructure stakeholders to look at existing setups with an open mind. This chapter's proposed solution offers technology assistance to manage patient data effectively and extends the hospital data management system's capability to predict the upcoming need for healthcare resources. Further, the authors intend to supplement the proposed solution with crowdsourcing to meet hospital demand and supply for unprecedented medical emergencies. The proposed approach would demonstrate its need in the current pandemic scenario and prepare the healthcare infrastructure with a more streamlined and cooperative approach than before.

National statistical data shows that almost all of Indonesia’s garlic needs are imported from other countries. Local production is only less than 10% of the total national demand for garlic. The Indonesian government is currently working to increase national garlic production in order to reduce imports dependency. One of the efforts taken is to increase the planting area through mandatory planting regulations for garlic importers. Every importer is required to grow garlic in Indonesia, which, if converted, is equivalent to five per cent of the total imported garlic. Importers are given freedom regarding the planting mechanism, whether to plant by themselves or through the partnership scheme with farmers. Particularly for the partnership scheme with farmers, it is necessary to study a partnership scheme that is mutually beneficial for both parties. A study was conducted in 2018 as part of a horticultural development policy analysis research to examine the effectiveness of cooperation in implementing mandatory garlic planting regulations for importers. This research was conducted in three locations, East Lombok, Magelang, and Temanggung. This study found that each of the three locations has different cooperation schemes. Farmers are interested in forming partnerships with importers for several reasons: certainty of obtaining seeds and operational costs, and other reasons such as marketing certainty and the opportunity to get cultivation technology assistance.

The Covid-19 pandemic, which began to spread in Indonesia since March 2020, has made many activities to take place at home. The government asks all Indonesians to work from home, worship from home, and learn from home. This is done so that the virus does not spread and cause even greater casualties. Learning from home is one way to prevent students from the dangers of the Covid-19 virus. Starting from the level of early childhood education to higher education are required to carry out distance learning using technology assistance such as video calls with Hangouts, Zoom, Skype, and Webex. Future Stars Preschool also has to do distance learning using the video call method and each parent is asked to prepare their child at home so they can take online classes with written material that has been prepared in advance. Parents are also asked to take photos of their children doing homework to be emailed and the materials that have been done are sent to school at the end of the period. Schools must determine what video call application to use so that each child can take part in distance learning, as well as the method of delivering material to each child. The applications used should be easy and not burdensome for parents, and also the delivery or processing of materials can be easier for early childhood children.
