This is the second of two essays about five models of nursing practice delivery—total patient care, functional nursing, team nursing, primary nursing, and the attending nurse. Primary nursing and the attending nurse are discussed in this essay. The other three models were discussed in a previous issue of Nursing Science Quarterly. Each model is described and its connection with nursing discipline-specific knowledge is discussed. The extent to which each model ascribes accountability for practice also is discussed.

Abstract Background As a new surgical method for older adults with cardiac insufficiency, transapical mitral valve clamp surgery requires the cooperation of practitioners across multiple disciplines to ensure appropriate treatment and nursing care. This study aimed to explore the utility of a multidisciplinary team nursing model in the clinical treatment and nursing care of patients undergoing transapical mitral valve clamping. Methods Our sample of ten patients included four men (40%) and six women (60%), with a mean age of 71.4 ± 5.2 years. The multidisciplinary team comprised nurses that specialized in severe illness, cardiac health, rehabilitation, psychology, nutrition, and pain. The team engaged in comprehensive discussions regarding problems specific to the patients undergoing transapical mitral valve surgery, allowing them to formulate individualized nursing measures and implement precise policies. Results No serious postoperative complications occurred in any of the ten patients included in this study, and a significant improvement was noted in the cardiac status of all the patients. Color ultrasound findings at discharge indicated that the degree of reflux of all the patients was ≤2+. Among the ten patients, the Activity of Daily Living Scale scores at discharge were significantly higher than before the operation (69.0 ± 4.6 vs. 55.0 ± 5.8). In addition, the 6-min walking test results at discharge were significantly better than those observed before the operation (318.0 ± 21.7 m vs. 295.2 ± 18.4 m). Conclusions Utilization of a multidisciplinary team allows nurses across various specialties to provide more comprehensive and systematic care for patients undergoing a mitral valve clamping operation, thus promoting patient recovery.

Team nursing is a method of care delivery that employs a group of individuals led by a knowledgeable nurse to provide nursing care to a cluster of patients. This study aims to analyse nurses’ performance after undergoing training on the team nursing method in a hospital setting. The quasi-experimental pre and post-test design was used. Purposive sampling was utilized in selecting 38 nurses who agreed to participate in the study; 19 were assigned to the intervention group and another 19 to the control group. The nurses in the intervention group joined a training in team nursing as a care delivery method and were observed and supervised for six weeks. A questionnaire was used to measure nurses’ performance in providing care. Results were analysed using rate, mean, Chi Square test and Multivariate Logistic Regression Analysis. The findings revealed that most of the respondents were between 20-30 years old with more than five years of work experience. The nurses in the intervention group significantly improved their performance by 11.35% after the team nursing method training (p value 0.0001). The OR of 27.7 may suggest that the training greatly influenced improvement in their performance. Furthermore, the training may have improved nurses' work performance 21.7 times compared to those who did not join it. Objetivo: Analisar o conhecimento da equipe de enfermagem sobre trombose venosa profunda com vistas a produção de tecnologia educacional. Metodologia: Estudo exploratório com abordagem qualitativa, realizado em hospital do norte do Brasil, por meio de questionário e entrevista semiestruturada, de fevereiro a junho de 2019, com 12 profissionais de centro cirúrgico, clínica cirúrgica, sala de recuperação pós-anestésica. Análise de conteúdo do tipo temática. Aprovado pelo Comitê de Ética em Pesquisa. Resultados: Dentre os enfermeiros (seis), o mesmo quantitativo dos sexos feminino e masculino, idade entre 26 e 41 anos; tempo de trabalho de 01 há 13 anos. Em relação aos técnicos de enfermagem (seis), todos do sexo feminino, idade entre 25 e 43 anos, de 01 há 6 anos de atuação. Emergiram cinco temas: trombose venosa profunda; fatores de risco; complicações; procedimentos que prejudicam a mobilidade; cuidados pós-operatórios com os pacientes de risco. Dos temas elaborou-se o quadro de convergências entre temas e evidências para subsidiar a construção de tecnologia educacional. Conclusão: enfermeiros e técnicos de enfermagem demostraram ter entre conhecimento adequado e parcialmente adequado, o que indica a necessidade de tecnologia educacional para a equipe de enfermagem.Descritores: Trombose venosa. Objetivo: descrever a utilização do prontuário eletrônico do paciente pela equipe de enfermagem. Método: trata-se de um estudo quantitativo, descritivo, transversal, num hospital privado, com 89 técnicos de enfermagem e 11 enfermeiros. Utilizou-se um questionário para a coleta de dados, processando-os na planilha Microsoft Excel, exportando-os, posteriormente, para o programa IBM SPSS Statistics 20.2. Resultados: predominou-se a faixa etária dos 18 aos 29 anos nas duas classes profissionais e o tempo de serviço variou de 1 a 4 anos. Revela-se que a maioria dos participantes já havia realizado cursos na área da informática e, em relação ao sistema utilizado, afirmaram ser de fácil lembrança, manipulação e acesso, porém, a maior dificuldade relatada foi “o erro no sistema”. Conclusão: reforça-se, pelos resultados, a necessidade de investimentos em relação ao “erro no sistema” ou “falha na conexão”, visto que esse foi o maior problema relatados e está identificado na literatura como desvantagem do prontuário eletrônico. Resultados: el grupo de edad de 18 a 29 años predominó en ambas clases profesionales y la duración del servicio varió de 1 a 4 años. Se revela que la mayoría de los participantes ya habían tomado cursos en el área de tecnología de la información y, en relación con el sistema utilizado, se dice que es fácil de recordar, manipular y acceder, sin embargo, la mayor dificultad reportada fue "el error en el sistema". Conclusión: los resultados refuerzan la necesidad de inversiones en relación con "error del sistema" o "falla de conexión", ya que este fue el mayor problema reportado y se identifica en la literatura como una desventaja del historial clínico electrónico. AbstrakProses keperawatan adalah suatu metode yang sistematis dan Terorganisasi dalam pemberian asuhan keperawatan, yang difokuskan pada reaksi dan respons unik individu pada suatu kelompok atau perorangan terhadap gangguan kesehatan yang dialami, baik aktual maupun personal. Tujuan penerapan proses keperawatan dalam memberikan asuhan keperawatan kepada klien adalh untuk menyelesaikan masalah. Proses keperawatan juga dapat dipergunakan untuk menyusun suatu intervensi yang bertujuan mengubah suatu situasi yang lebih kondusif dalam membantu mempercepat menyelesaikan masalah klien yang sangat kompleks. Metode digunakan dalam model harus dilakukan secara hati-hati, jangan sampai memaksakan situasi agar sesuai dengan model, Beberapa jenis sistem pemberian asuhan yang sering juga disebut dengan sistem penguasaan menurut Huber (1996) yaitu : Private Duty Nursing, Metode aplikasi klien (keperawatan toal), Fuctinal Nursing (keperawatan fungsional), Team Nursing. Hasil sebagai perawat, mereka juga mempunyai keinginan untuk membantu klien dengan baik dan memberikan kontribusi yang positif terhadap perawatan pasien. Terkait pengalaman yang paling menarik dalam pembelajaran klinik, beberapa dari responden menyatakan bahwa pembelajaran klinik telah memberikan kesadaran dan kepuasan tertentu kepada mereka untuk membantu pasien. Pembahasan penggunaan proses keperawatan sangat bermanfaat bagi klien, keluarga dan masyarakat karena mendorong mereka untuk berpartisipasi secara aktif dengan melibatkan mereka ke dalam lima tahapannya. Penutup dalam perawatan klien, perawat harus melakukan pengkajian terlebih dahulu, kegiatan yang harus dilakukan perawat adalah mengumpulkan data seperti riwayat keperawatan, pemeriksaan fisik, dan pemeriksaan data sekunder lainnya.
