Background Numerous studies on involuntary weight loss (IWL) have been published since the 1980s, although most of them have included small samples of patients with specific symptoms. The aim of the present study was to determine the causes, demographic and clinical characteristics and mortality at 12 months in patients attended at a rapid diagnostic unit (RDU) for isolated IWL. Methods A single-center retrospective observational study including all patients presenting to the RDU for isolated IWL between 2005 and 2013. The following data were recorded: demographic and clinical variables, results of complementary tests (blood tests, x-rays, computed tomography scan and digestive endoscopy), main diagnosis and vital status at 12 months. Results Seven hundred and ninety-one patients met the criteria for IWL. Mean age was 67.9 years (SD 4.7), 50.4% were male and mean weight loss was 8.3 kg (SD 4.7). The cause for IWL was malignant disease in 23.6% of patients, non-malignant organic disease in 44.5%, psychiatric disorder in 29.0% and unknown in 3.2%. Overall mortality at 12 months was 18.6% (95%CI: 16.1–21.6). The mortality rate was highest in the group with malignancy (61.1%; 95%CI: 54.2–68.2). Conclusions Almost a quarter of all patients attended at the RDU for IWL were diagnosed with cancer. Mortality at 12 months was higher in this group than in the other three. Malignancy should therefore be ruled out during the first visit for patients attended for IWL.

A child’s language development is one of the main factors of the personal formation. The level of personal development determines the formation of social and cognitive skills in children. Speech development disorders, in particular mastering of syntagmatic constructions with different structural complexity, complicates children’s communication with peers and adults and impedes written language development. The purpose of this study is to examine the basic parameters of the compositional rhythmic speech structure (movement, rhythm and space) and to determine its development level in preschool children with multiple disabilities (disabilities in sensory, motor and speech systems). In order to examine these parameters a special technique consisting of four diagnostic units (“rhythmic movement”, “rhythmic space”, “rhythmic pronunciation” and “complex rhythm of speech”) was developed. First, an empirical study for each diagnostic unit was conducted, and the findings suggested that majority of preschool children with sensory, motor and speech disorders have underveloped compositional structure of speech, which requires further targeted interventions. The study found that preschool children with multiple disabilities have a reduced ability to pronounce words with different structural complexity, impaired perception of lexical units and a disturbed ability to perform dynamic and rhythmic serial movements, which indicates an insufficient level of motor, rhythmic and spatial skills development. Then, a correlation analysis was performed, the results of which indicate that the diagnostic units – “rhythmic movement”, “rhythmic space”, “rhythmic pronunciation” – are inextricably linked. When the skills assessed by the above-mentioned diagnostic units are developed, the skills, assessed by the “Complex rhythm of speech” diagnostic unit can begin their development as well. Determination of the child’s functioning level in each diagnostic unit will allow describing the effective strategies to influence parameters of compositional rhythmic speech in children with multiple disabilities based on potential capabilities and their maximal implementation in the speech therapy process.

AbstractBackgroundCOVID-19 infection is particularly aggressive in frail patients, as cancer patients. Therefore, the more suitable management of the oncological patient requires a multidisciplinary assessment, to identify which patients should be treated, as inpatients or outpatients, and which treatments can be procrastinated.ConclusionsThe role of radiologist is crucial, and, all cancer patients who need an imaging evaluation will need to be studied, using the most appropriate imaging tools related to the clinical question and paying a special attention to preserve public health. Guidelines are necessary in the correct organization of a radiology unit to manage patients with suspected or confirmed COVID-19 infection, and whenever possible, a satellite radiography center with dedicated equipment should be used to decrease the transmission risk.

AbstractThe aim of this research was to determine ex vivo the influence on accuracy of five different embedding media, for investigative and educational purposes, and one electronic apex locator. 110 human extracted mature roots of permanent single-rooted human teeth were used. The roots were embedded in alginate, stick sponge, 2% agar–agar and 6% and 12% gelatin. The actual working length to the physiological foramen was determined under a stereo-microscope (16 ×) and the electronic working lengths with the Elements Diagnostic Unit and a K-file ISO 10. The accuracy ranges of the accumulated measurements, when allowing a ± 0.5 mm tolerance, went from 98.2% (6% and 12% gelatin), 93.7% (alginate), 92.8% (2% agar–agar) to 91.7% (sponge). The exact measurements at the physiological foramen ranged from 80.0% (6% gelatin), 76.5% (2% agar–agar), 71.8% (12% gelatin), 68.2% (alginate) to 64.5% (sponge). Although relatively seldom (n = 24), measurements with deviations of more than ± 0.5 mm were also observed; thus, the accuracy of the working length determination results per se can be considered as clinically acceptable. The results of this research allow a recommendation of the investigated embedding media for electronic working length determination models for educational and research purposes in endodontics.

Aim: to study the changes in the critical parameters of tear film stability and autorefractometry in patients with mild dry eye syndrome (DES) after instilling artificial tear products of different viscosity. Patients and Methods: thirty patients aged 23–70 with mild bilateral DES were examined using the Visionix VX120+Dry Eye multi-diagnostic unit equipped with the TF-Scan to fix and assess tear film stability and tear breakup time (TBUT). The critical refraction and tear film parameters were evaluated by keratometry, corneal topography, TBUT, and tear meniscus height measurements. Next, an artificial tear product was instilled, i.e., group 1 (14 patients, 28 eyes) received a 0.15% sodium hyaluronate-based preparation, and group 2 (16 patients, 32 eyes) received a 0.4% sodium hyaluronate-based preparation. The examination was repeated after 30 sec and 5 min. Controls were seven healthy individuals (14 eyes). Results: in DES, breaks in the tear film appeared earlier and affected greater corneal surface area. A "Destabilized tear film area" parameter of the Visionix VX120+Dry Eye contributed significantly to the knowledge of tear film functionality, thereby empowering the diagnostic armamentarium in DES. As early as 30 sec after instillation of both artificial tear preparations, breakup-free time tended to increase, while the area of tear film destabilization tended to reduce. After 5 min, this trend continued to increase, particularly after instilling 0.15% sodium hyaluronate-based preparation. At the same time, changes in keratometry readings (i.e., sphere and cylinder) and astigmatism axis orientation were reported. Corneal refractive power (more in the flattest axis) increased, in particular, after the instillations of 0.15% hyaluronic acid. The range of changes was 0.00–0.75 D for the sphere power, 0.00–0.50 D for the cylinder power, and 0–69 degrees for astigmatism axis orientation. Conclusions: our findings with the Visionix VX120+Dry Eye multi-diagnostic unit demonstrate a significant variation of autorefractometry readings in DES depending on the extent of ocular surface hydration. These phenomena raise questions about the accuracy of autorefractometry readings. Therefore, autorefractometry should be performed after normalizing tear film volume and stability. AIM: The present in-vitro study aims to compare the accuracy of root canal working length determination between the third generation and fourth generation electronic apex locators. MATERIAL AND METHODS: Fifty extracted single-rooted single canal teeth were selected for the study, and a definite coronal plane was prepared. Actual working length (AL) was measured using a stereomicroscope under 8X magnification. Electronic working length measurements were recorded using Root ZX (EL1) and Elements Diagnostic Unit (EL2) apex locators. One-way ANOVA test was carried out to analyse the data among the experimental groups. RESULTS: The results of the one-way ANOVA test showed that difference in the working length determined by either apex locators (EL1 and EL2) and actual length determined under a stereomicroscope (AL) was statistically not significant. The independent ‘t’ test comparing between groups EL1 and AL; and EL2 and AL showed that working length determined by either of the apex locators (EL1 and EL2) and actual length determined under a stereomicroscope (AL) was statistically not significant. CONCLUSION: In this in vitro study, the Root ZX and Elements Diagnostic Unit apex locators are equally accurate for determination of working length when compared to actual working length.
