This chapter traces the practical and conceptual origins of eight problematic commitments including the perception that there is an inherent moral dilemma at the heart of research with humans and the tendency to conceptualize research as a private transaction between researchers and participants without clear connections to the requirements of a just social order. It introduces readers who are new to research ethics to key cases and documents relating to domestic and international research and illustrates how they gave rise to the problematic views that produce conceptual and practical tensions in the field. The chapter frames the questions that will be addressed in subsequent chapters, including issues about research risk; the role of paternalism in research ethics; and requirements relating to responsiveness to host community health needs, the standard of care, and post-trial access in international research.

This chapter reviews a series of arguments that purport to show that there is an inherent moral dilemma in research with humans and that this conflict produces a corresponding social dilemma, known as the prisoner’s dilemma. If these arguments are sound, it would show that dual requirements of the egalitarian research imperative outlined in chapter 4 cannot be satisfied in practice. This chapter argues that these arguments fail and that their intuitive force in this area is bolstered by two dogmas of research ethics: the claim that the ethical norms in this area derive from the role-related obligations of medical professionals and the claim that research is an inherently utilitarian undertaking. This chapter demonstrates that rejecting those dogmas creates a space for reconciling the production of socially valuable information with respect for research participants as free and equal.

While many European Union/European Economic Area (EU/EEA) countries recently expanded human papillomavirus (HPV) vaccination to boys, HPV vaccine supply is currently limited for girls in low- and middle-income countries (LMIC) that are severely affected by HPV. Globally, about 50% of countries have introduced HPV vaccination. Some LMIC with high burden of cervical cancer have not yet introduced HPV vaccination, or are reaching suboptimal vaccination coverage. While WHO issued a call for cervical cancer elimination in 2018, a global shortage of HPV vaccines is currently predicted to last at least until 2024. We reviewed national policies of EU/EEA countries and recommendations of the World Health Organization (WHO) Strategic Advisory Group of Experts on immunisation to discuss current challenges and dose-sparing options. Several EU/EEA countries have extended HPV vaccination to boys and the European Cancer Organisation has issued a resolution for elimination of all HPV-associated cancers in both sexes. The European Centre for Disease Prevention and Control concluded in its 2020 guidance that cost-effectiveness of extending routine vaccination to boys depends on several context-specific factors. The extension of HPV vaccination to boys in EU/EEA countries may affect global availability of vaccines. Temporary dose-sparing options could be considered during the COVID-19 post-pandemic period.

The presented research focuses on verifying the confluent application of concept mapping (CM) and socio-scientific issues (SSI) according to the value-laden and moral dilemma orientation to construct problem-solving performance. This research sets up some perspectives for all 146 participants, including 139 students and 7 experts. All findings reveal that the design of SSICM contexts includes a rebuttal process and incense claim to improve students' argument response (16.4%), to increase content knowledge and illuminate their science learning by argumentations. To develop an assessment tool with high validity and reliability (Cronbach's α > .9) and find positive presentations of all learning attitudes in the SSICM context, learning environment and results will concern the best argumentation process. Students’ interview responses and SWOT analysis of teachers indicate that SSICM's use of argument in the classroom is a real benefit. The research provided a better paradigm of attempts to combine analytical and academic hypotheses to explain literature sources by teachers, researchers, textbook developers, and editors. Recent studies have shown that people make more utilitarian decisions when dealing with a moral dilemma in a foreign language than in their native language. Emotion, cognitive load, and psychological distance have been put forward as explanations for this foreign language effect. The question that arises is whether a similar effect would be observed when processing a dilemma in one’s own language but spoken by a foreign-accented speaker. Indeed, foreign-accented speech has been shown to modulate emotion processing, to disrupt processing fluency and to increase psychological distance due to social categorisation. We tested this hypothesis by presenting 435 participants with two moral dilemmas, the trolley dilemma and the footbridge dilemma online, either in a native accent or a foreign accent. In Experiment 1, 184 native Spanish speakers listened to the dilemmas in Spanish recorded by a native speaker, a British English or a Cameroonian native speaker. In Experiment 2, 251 Dutch native speakers listened to the dilemmas in Dutch in their native accent, in a British English, a Turkish, or in a French accent. Results showed an increase in utilitarian decisions for the Cameroonian- and French-accented speech compared to the Spanish or Dutch native accent, respectively. When collapsing all the speakers from the two experiments, a similar increase in the foreign accent condition compared with the native accent condition was observed. This study is the first demonstration of a foreign accent effect on moral judgements, and despite the variability in the effect across accents, the findings suggest that a foreign accent, like a foreign language, is a linguistic context that modulates (neuro)cognitive mechanisms, and consequently, impacts our behaviour. More research is needed to follow up on this exploratory study and to understand the influence of factors such as emotion reduction, cognitive load, psychological distance, and speaker’s idiosyncratic features on moral judgments.

Abstract Background Ethical decision-making of nurses could affect patients’ recovery and also decrease medical costs. To make ethical decisions, ICU nurses experience complicated ethical conflicts. Considering the multi-dimensional process of ethical decision-making, the present study was conducted to describe the experiences of ICU nurses regarding ethical decision making. Method The present research is a qualitative study with conventional content analysis approach that was done in 2020. Fourteen ICU nurses were interviewed using a semi-structured in-depth interview method. The interviews were recorded, transcribed verbatim, and analyzed using Granheim and Landman approach. Results Being torn by inevitable moral dilemma was defined as the main category which contained the categories of conflict with professional self, feeling squeezed between self-authority and demands of others, and Surrounded by organizational limitations. Conclusions Results of the present study showed that moral dilemma is a situation in which the nurses are forced to choose between two options based on their equipment and facilities. In these situations, the nurses would hesitate between their internal conflicts and confusion between choosing their own benefits and patients’ benefits. To prevent ethical challenges in decision making, it is necessary to educate healthcare team about ethical dilemmas and empower the personnel for encountering ethical challenges.

The sponsorship of junior sport is a contentious issue with community concerns raised about the appropriateness of some marketing targetted at children. Parents and coaches are guardians of junior sports players and often faced with a moral dilemma. On the one hand they know sponsors, who are trying to generate goodwill through providing financial or in-kind support, are crucial to the affordability of junior sports when competition for securing sponsorships is intense. On the other hand, they hold concerns that some sponsors may exploit their access to these young, vulnerable players. Guardians are caught at a crossroads with equally undesirable alternatives—financially unviable junior sports without sponsors or junior sports with potentially exploitative sponsors. Our study fills a gap in the literature where the tensions of commercial sponsorship agendas and moral concerns in sponsorship has not been well explored with our study being the first to apply the construct of moral judgements in a sponsorship model. Mixed methodology is used to examine the role that moral judgements play in determining sponsor goodwill. A qualitative study ( n = 18) informed an online survey ( n = 306) of Australian junior sports guardians. Structural equation modelling revealed that guardians’ moral judgements mediated the relationships between sponsor fit, perceived sponsor altruism and the outcome variable, sponsorship goodwill. The findings of this study extend scholarly understanding of consumers’ appraisal of sponsorships and provide useful insights to guide practitioners in sponsorship decision making, particularly in contexts that stimulate community interest or concerns.
