Objectives. This study aimed to explore the characteristics and principles of acupoints, which were applied for treating tension-type headache (TTH). Methods. Four databases were searched for the literature studies of treating TTH with acupuncture and moxibustion up to September 1, 2020. Titles, journals, authors, key words, interventions, main acupoints, and outcomes of the included literature studies were extracted and inputted into the self-established Data Excavation Platform of Acupoint Specificity for analysis. Results. In total, 128 papers containing 137 prescriptions, 89 meridian acupoints, and 7 extraordinary acupoints of treating TTH with acupuncture and moxibustion were included. The total frequency of acupoints’ application was 763 times. Fengchi (GB20), Baihui (GV20), Taiyang (EX-HN5), Hegu (LI4), and Taichong (LR3) were used most frequently. The acupoints in Yang meridians were utilized more than those in Yin meridians (66.1% vs. 17.8%), and the acupoints in the Gallbladder Meridian of Foot Shaoyang were applied most commonly. 59.9% (457/763) of the applied acupoints were on the head, face, and neck, and 31.7% (242/763) were on the four limbs. Additionally, the proportion of specific acupoints’ application was 78.2% (597/763). Conclusions. The prescription of Fengchi (GB20), Baihui (GV20), Taiyang (EX-HN5), Hegu (LI4), and Taichong (LR3) might be relatively reasonable in clinical practices of treating TTH with acupuncture, which should be verified in further studies.

Acupuncture is a medical modality based on the theory of traditional Chinese medicine, and its effect is relatively dependent on acupoint specificity. However, there is little knowledge on acupoint specificity versus acupuncture outcomes because of the deficiency of rigorous investigation on this topic, which has impeded the growing legitimacy of acupuncture in the mainstream of medicine as an evidence-based therapy. Therefore, it is of utmost importance to clarify this critical issue. The present study aims to verify the phenomenon of acupoint specificity in acupuncture-induced cardiovascular regulation and explore the biological mechanism by measuring mast cells’ degranulation and adenosine release. This study was conducted to explore the specificity of acupoints in an acute bradycardia rabbit model. After electroacupuncture (EA) stimulation at PC6, PC control (con) 1, PC con 2, LU7, LI11, and nonacupoint, only the PC6 group showed a significant improvement in relative heart rate as compared to that of the model group. There was no significant difference between the relative heart rate of other EA groups and that of the model group. Historical results also showed that the ratio of degranulated mast cells in PC6 was significantly higher than other acupoints and control points. From the results of high-performance liquid chromatography (HPLC), a transient elevation of adenosine concentration during EA was only observed on acupoints and control points ( P < 0.05 ) along the pericardium meridian. The EA-induced adjustment on acute bradycardia exhibits a relative specificity of acupoints, which may be related to mast cell degranulation and adenosine release in local acupoint areas. Increased degranulation of mast cells and augmentation of adenosine release during EA may be the mechanisms for PC6 having significantly better acupuncture effects than other acupoints and nonacupoints.

The specificity of acupoint indication (i.e., reverse inference—diseases for which an acupoint could be used) might differ from the specificity of acupoint selection (i.e., forward inference—acupoints used for a disease). In this study, we explore acupoint specificity through reverse inferences from the dataset of prescribed acupoints for a certain disease in clinical trials. We searched acupuncture treatment regimens in randomized controlled trials included in the Cochrane Database of Systematic Reviews. For forward inference, the acupoints prescribed for each disease were quantified. For reverse inference, diseases for each acupoint were quantified. Data were normalized using Z-scores. Bayes factor correction was performed to adjust for the prior probability of diseases. The specificities of acupoint selections in 30 diseases were determined using forward inference. The specificities of acupoint indications regarding 49 acupoints were identified using reverse inference and then subjected to Bayes factor correction. Two types of acupoint indications were identified for 24 acupoints: regional and distal. Our approach suggests that the specificity of acupoint indication can be inferred from clinical data using reverse inference. Acupoint indication will improve our understanding of acupoint specificity and will lead to the establishment of a new model of analysis and educational resources for acupoint characteristics.

Background: Using functional neuroimaging techniques to explore the central mechanism of acupoint specificity, the key of acupuncture theory and clinical practice, has attracted increasing attention worldwide. This review aimed to investigate the current status of functional neuroimaging studies on acupoint specificity and explore the potential influencing factors for the expression of acupoint specificity in neuroimaging studies. Methods: PubMed database was searched from January 1st, 1995 to December 31st, 2016 with the language restriction in English. Data including basic information, methodology and study results were extracted and analyzed from the eligible records. Results: Seventy-nine studies were finally enrolled. 65.8% of studies were performed in China, 73.4% of studies were conducted with healthy subjects, 77.2% of studies chose manual acupuncture as the intervention, 86.1% of studies focused on the instant efficacy and 89.9% of studies used functional magnetic resonance imaging as scanning technique. The average sample size was 16 per group. The comparison of verum acupoints and sham acupoints were the main body of acupoint specificity researches. 93.7% of studies obtained the positive results and favored the existence of acupoint specificity. Conclusion: This review affirmed the existence of acupoint specificity and deemed that the acupoint specificity was relative. Multiple factors such as participants, sample size, acupoint combinations, treatment courses, and types of acupoint could influence the expression of acupoint specificity.

In the process of treating pro-diseases with acupuncture, traditional Chinese medicine (TCM) doctors may fine-tune acupuncture prescriptions according to different prior experiences. Different prescriptions will affect the efficiency and effect of acupuncture treatment, and even excessive acupoint selection may cause psychological pressure on patients. We still lack an effective means to analyze the meridian system and acupoint specificity to clarify the mapping relationship between acupoints and diseases. Given the inability of modern medical technology to provide effective evidence support for meridians and acupoints, we combined acupuncture theory with network science for an interdisciplinary discussion. In this paper, we constructed a weighted undirected acupoint-disease network (ADN) based on clinical acupuncture prescription literature and proposed a high-specificity key node mining method based on ADN. Combined with the principle of acupoint selection in TCM, the proposed method balanced the contribution of local areas to the network based on the distribution characteristics of meridians and selected 30 key acupoints with high influence on the global topology according to the evaluation index of key nodes. Finally, we compared the proposed method with the other six classical node importance evaluation algorithms in terms of resolution, network loss, and accuracy. The comprehensive results show that the marked key acupoint nodes make outstanding contributions to the connectivity, topological structure, and weighted benefits of the network, and the stability and specificity of the algorithm guarantee the reliability of the key acupoint nodes. We consider that these key acupoints with high centrality in ADN can be used as core acupoints to help researchers explore targeted and high-impact acupoint combinations under resource constraints and optimize existing acupuncture prescriptions.

Background.Dysmenorrhea is a common problem for which acupuncture provides effective analgesia. Although acupoint selection affects the effectiveness of acupuncture, the basic rules of acupoint selection are little understood. This study aims to investigate the principles of acupoint selection and characteristics of acupoints used for primary dysmenorrhea.Methods.PubMed, China National Knowledge Infrastructure, and Chinese Biomedical Database were searched for clinical trials published in English or Chinese from January 1978 to April 2014 evaluating the effect of acupuncture on primary dysmenorrhea, with or without methods of randomization and/or control. Three authors extracted information and two reviewers inputted information on titles, journals, interventions, main acupoints, and outcomes using the self-established Data Excavation Platform of Acupoint Specificity for data mining.Results. Sanyinjiao(SP06),Guanyuan(CV04), andQihai(CV06) were used most frequently. The most frequently used meridians were Conception Vessel, Spleen Meridian of FootTaiyin, and Bladder Meridian of FootTaiyang. 67.24% of acupoints used were specific acupoints. Acupoints on lower limbs were most frequently used.Conclusion.Data mining is a feasible approach to identify the characteristics of acupoint selection. Our study indicated that modern acupuncture treatment for primary dysmenorrhea is based on selection of specific acupoints according to traditional acupuncture theory.
