Recent Advances on Pharmacology and Chemistry of Pycnanthus angolensis over the last decade (2012-2021)

<i>Pycnanthus angolensis</i>, widely known under its trade name "ilomba", is a medicinal plant from the family Myristicaceae that has occupied a prominent place in African traditional medicine for several decades; its broad application to treat numerous diseases, including malaria, bacterial infections and most recently COVID-19. The various chemical studies undertaken on the plant identified many classes of specialized compounds, including quinone-terpenoids, lignans and isoflavonoids, as the most abundant and bioactive components. The plant is defined as a major asset in developing new potent drugs and deserves further investigation in this regard. This mini-review aims to compile the newly documented findings on the traditional uses, phytochemistry and pharmacology of<i> P. angolensis</i> over the last decade from 2012 to 2021. In this regard, a literature search using the keyword <i>Pycnanthus</i> has been done without language restriction in numerous online libraries, including Scifinder, PubMed, Google Scholar, and only papers on <i>Pycnanthus angolensis</i> published after 2011 have been exploited during the writing.

Interventions for Early-Stage Pericoronitis: Systematic Review of Randomized Clinical Trials

Background: To investigate the efficacy and safety of interventions for early stage pericoronitis. Methods: We searched for randomized controlled trials (RCTs) in databases from inception to July 2020, without language restriction. RCTs assessing adolescents and adults were included. Results: Seven RCT with clinical diversity were included, so, it was not possible to conduct meta-analyses. Individual study data showed an improvement in oral health quality of life in favor of topical benzydamine versus diclofenac capsule (Mean difference (MD) −1.10, 95% Confidence interval (CI) −1.85 to −0.35), and no difference between topical benzydamine and flurbiprofen capsule (MD −0.55 95% CI −1.18 to 0.0). There was no difference between diclofenac and flurbiprofen capsules (MD 0.55, 95% CI −0.29 to 1.39). An imprecise estimate of effects was found for all outcomes, considering (i) oral versus topic pharmacological treatment, (ii) different oral pharmacological treatments, (iii) pharmacological treatment associated with laser versus placebo laser, (iv) pharmacological treatment associated with different mouthwashes, and (v) conventional treatment associated to antimicrobial photodynamic therapy versus conventional treatment, with low to very low certainty of evidence. Conclusions: Until future well-designed studies can be conducted, the clinical decision for early stage pericoronitis should be guided by individual characteristics, settings and financial aspects.

HUNTINGTON’S DISEASE: CURRENT ADVANCES AND FUTURE PROSPECTS

Huntington’s disease is a neurodegenerative disease which is caused by dominantly inherited cytosine-adenine-guanine trinucleotide repeat expansion in the huntingtin gene of chromosome 4. Present survey reveals 2.7 per 100000 people are affected by huntington’s disease worldwide. The symptoms present with these patients are progressive motor, cognitive and psychiatric disorders. The early symptoms are chorea and loss of balance. This review aims to observe the present data available concerning huntington’s disease, symptoms, age of onset, risk factors, benefits of early diagnosis and genetic attribution. There is no cure for the disease. The article searched, selected and reviewed were from google scholar, medscape, NIH MedlinePlus, PubMed database using MeSH terms huntington’s disease, recent therapeutic advancement from 2003 to July 2021 with no language restriction and additional studies were included from the reference lists of relevant articles. The present review provides clinical features, diagnosis, symptomatic management and ongoing research. Hence this review will have an impact to create awareness for the society and researchers to find future treatment for Huntington’s disease.

Annual incidence of osteoporotic hip fractures in Iran: a systematic review and meta-analysis

Background Osteoporosis (OP) is progressively becoming a global concern with the aging of the world’s populations. Osteoporotic fractures are associated with significantly increased mortality rates and a financial burden to health systems. This Meta-analysis aims to estimate the annual incidence of osteoporotic fractures in Iran. Methods A comprehensive systematic literature search was performed through Medline (PubMed), Embase, Scopus, Web of Science, and Google Scholar to identify studies which contain an investigation of the incidence of osteoporotic fractures in Iran up to December 3rd 2020, with no time and language restriction. For the risk of bias assessments of studies, the Joanna Briggs Institute (JBI) critical appraisal checklist for studies reporting prevalence data was used. The pooled estimation of the incidence of osteoporotic fractures in population aged≥50 years was calculated using random-effects meta-analysis, and the heterogeneity of included studies was quantified with the I2 statistic. Results In all, 6708 papers were initially retrieved from the electronic databases, among which seven studies were included in the meta-analysis. The pooled standardized annual cumulative incidence of hip fractures was estimated as 138.26 (95% CI: 98.71–193.65) per 100,000 population and 157.52 (95% CI: 124.29–199.64) per 100,000 population in men and women, respectively. Conclusion This study showed a high incidence rate of osteoporotic hip fractures in Iran. Early detection and treatment of individuals with higher risks of primary fragility fractures at primary health care as well as implementing fracture liaison services to prevent secondary fractures are highly recommended. The results suffer from the following limitations: first, a low number of studies that were eligible for inclusion; second, the lack of population-based studies; and presence of highly heterogeneous studies despite the use of a random effect model.

Decreased urinary uromodulin is potentially associated with acute kidney injury: a systematic review and meta-analysis

Background Urinary uromodulin (uUMOD) is one of the novel biomarkers for predicting AKI. However, currently available publications showed inconsistent results. We designed this meta-analysis to evaluate the potential association between uUMOD and AKI. Methods We searched research articles with no language restriction in Medline, Web of Science, Cochrane Library, Embase, and 3 Chinese datasets from inception to February 2021. We used random-effects models to estimate the standardized mean difference (SMD) between patients with AKI or not, while the leave-one-out method and random-effects meta-regression to evaluate the sensitivity and the impact of potential confounders such as age and surgery. Results The meta-analysis comprising 3148 subjects from 11 studies showed that the uUMOD of the AKI group is significantly lower than the non-AKI group (SMD: − 0.71; 95% confidence interval (CI), − 1.00, − 0.42, P < 0. 001, I2 = 78.8%). Subgroup analysis revealed the difference is also significant in a different age, surgery condition, and assay time but not acute rejection (AR) group, especially in children (SMD: − 1.21, 95% CI: − 1.80, − 0.61; P < 0.001) and patients undergoing surgery (SMD: − 1.03, 95% CI: − 1.75, − 0.30; P < 0.001). Lower uromodulin is associated with higher odds for AKI incidence (odds ratio = 2.47, 95% CI: 1.12, 5.47; P < 0.001, I2 = 89%). Meta-reggression found that age was associated with the SMD of uUMOD. The study outcome was reliably confirmed by the sensitivity analysis. Conclusion The present study suggested a negative association between uUMOD and AKI especially in children and surgical patients.

Virtual reality therapy to control burn pain: systematic review of randomized controlled trials

Background Virtual reality therapy has been shown to be an excellent alternative to non-pharmacological treatment for the control of burn pain. Objective To evaluate the effects of virtual reality therapy on pain control in people who have suffered burns published in the scientific literature. Method Systematic review carried out as recommended by Cochrane®. The search was carried out in the Embase, PubMed, Lilacs and Cochrane Library databases, in the period from March 2021. Randomized clinical trials were included without language restriction and year of publication. The risk of bias was assessed using the Cochrane® tool. Results Of the 3755 articles found, only 17 articles were selected for reading in full. Of these, only four articles met the inclusion criteria. The results of the studies showed that the use of virtual reality therapy reduced the intensity of pain in children and adolescents with burns, despite the fact that most results are not statistically significant. No selected study had a high risk of bias. Conclusions Virtual reality therapy has been shown to be effective in controlling pain, reducing the time spent thinking about it and greater distraction during the procedures. However, most randomized clinical trials results were not statistically significant in at least one of the moments when pain was assessed. It is noteworthy that randomized clinical trials are still necessary to administer virtual reality therapy, especially in adults.

Mushrooms (Basidiomycetes) as a significant source of biologically active compounds for malaria control

Mushrooms represent a large family of fleshy fungi that have been of high interest since ancient ages due to their medicinal and nutritional importance. Therefore, it can represent a significant source of bioactive compounds in malaria control. The few numbers of studies on <i>in vitro</i> antiplasmodial and insecticidal properties of their extracts and chemical constituents led to interesting results reported in numerous scientific publications. This review aims to provide a comprehensive compilation of literature up to 2021 on the antiplasmodial, insecticidal as well as cytotoxic chemical constituents of medicinal mushrooms that can help in the management of malaria both against the parasite <i>Plasmodium falciparum</i> and the mosquitoe <i>Anopheles stephensis</i> acting as a vector of malaria through its bites. For this purpose, some searches have been done in some online libraries using keywords like Basidiomycete, mushroom, malaria, <i>Plasmodium</i>, <i>Anopheles</i> and antiplasmodial without language restriction. Among the reported compounds, 51 selected ones displayed significant antiplasmodial potency with IC₅₀ values lower than 10 μM against <i>P. falciparum</i> strains sensitive or resistant to chloroquine. For instance, ganoderic acid AW1 demonstrated a strong antiplasmodial activity with IC₅₀ of 257.8 nM against <i>P. falciparum</i> D6, while strong activities were displayed by ganoweberianones A (IC₅₀ = 0.050 μM) and B (IC₅₀ = 0.46 μM) against <i>P. falciparum</i> K1. Moreover, some mushroom methanol extracts demonstrated good larvicidal and ovicidal activities against <i>Anopheles stephensis</i>. This paper provides further insights into the development of new antiplasmodial drugs or new potent eco-friendly pesticides to control mosquito vectors.

Modal context restriction for multiagent BDI logics

We present and discuss a novel language restriction for modal logics for multiagent systems, called modal context restriction, that reduces the complexity of the satisfiability problem from EXPTIME complete to NPTIME complete. We focus on BDI multimodal logics that contain fix-point modalities like common beliefs and mutual intentions together with realism and introspection axioms. We show how this combination of modalities and axioms affects complexity of the satisfiability problem and how it can be reduced by restricting the modal context of formulas.

Substandard and falsified medical products: bibliometric analysis and mapping of scientific research

Objective Substandard and falsified (SF) medical products are a global public health threat. The presence and spread of SF drugs negatively affect (1) patients’ safety and health outcomes, (2) national economy, (3) public trust in the healthcare system, and (4) the international fight against serious health challenges such as malaria and antimicrobial resistance. The objective of the current study was to investigate and provide a snapshot analysis of the evolution and developmental patterns of global research publications on SF products. Methods A bibliometric approach was adopted using terms such as fake, falsified, counterfeit, substandard, and others. No language restriction was made. The study period was from 1900 up to 2020. The search strategy was validated and implemented using Scopus database. Results The search strategy retrieved 978 documents authored by 2861 researchers from 100 different countries and published in 421 different journals. The retrieved documents received 11,237 citations (11.5 citations per document) with an H-index of 53. The 978 documents retrieved from Scopus were published from 1961 to 2020, giving an average of 16.6 publications per year. The present study indicated that research on SF medical products: (a) has experienced a steep growth from 2001 to 2012 followed by a steady-state growth; (b) was disseminated in a wide range of journals, mainly in the fields of the pharmaceutical industry, analytical chemistry, public health, infectious diseases, and internal medicine; (c) was published by scholars with diverse and distant geographical backgrounds; (d) was mainly produced in the United States, United Kingdom, and Germany; (d) has fragmented research networks and a limited number of researchers per network; (e) has limited cross-country collaboration except for that between the US and the UK in one hand and countries in the Mekong region in the other hand; (f) emphasized on medications related to malaria and sexual stimulants; and (g) received relatively inadequate funding. Conclusions Research on SF medical products is important and should remain a priority to ensure good quality of medications. Research activity in the field needs to be encouraged in world regions such as Africa and the Middle East where drug regulations are unsatisfactory and cross-border trade of illegal medications is common.

Natural Dietary and Medicinal Plants with Anti-Obesity Therapeutics Activities for Treatment and Prevention of Obesity during Lock Down and in Post-COVID-19 Era

Overweight and obesity have become global epidemics, especially during the lockdown due to the COVID-19 pandemic. The potential of medicinal plants as a better and safe option in treating obesity and overweight has gained attention in recent years. Obesity and overweight has become a major public health concern, and its incidence rising at an alarming rate. Obesity is one of the major types of metabolic syndrome, resulting in various types of problems such as hypertension, diabetes, dyslipidemia, and excess fat accumulation. The current searching was done by the keywords in main indexing systems including Scopus, PubMed/MEDLINE, the search engine of Google Scholar, and Institute for Scientific Web of Science. The keywords were traditional medicine, health benefits, pharmaceutical science, pomegranate, punicalin, punicalagin, and ellagitannins. Google Scholar was searched manually for possible missing manuscripts, and there was no language restriction in the search. This review was carried out to highlight the importance of medicinal plants which are common in traditional medicinal sciences of different countries, especially Asia to prevent and treatment of obesity and overweight during the global pandemic and the post-COVID-19 era.
