An estimated 29-year-old male, desert tortoise ( Gopherus agassizii ) under professional care presented for endoscopic liver biopsies as part of a diagnostic work-up for recurrent, profound anemia and hypoalbuminemia. Suspected cystoliths were identified during endoscopy but were not visible on contemporary radiographs. Dual-energy computed tomography (DECT) imaging confirmed the presence of a large urate urolith filling the majority of the bladder. Due to the recent clinical illness, the tortoise was considered a poor surgical candidate. Medical management consisting of urinary alkalinization with potassium citrate, vibration therapy, and daily warm water soaks was elected. Following institution of medical therapies, the tortoise began passing pieces of urate stone that were 0.25-2.0cm in diameter during daily soaks. A recheck DECT scan at two months indicated the urolith was approximately 1/3 the initial size, suggesting medical therapy was effectively treating the urolith. Unfortunately, after five months of medical therapy, the stone had increased in size again on the follow-up DECT scan. This case report suggests that medical management may be a potential option for large urate cystoliths in desert tortoises, though more research is needed to further refine effective therapeutics for such cases. The use of dual-energy CT imaging was essential in this case as it enabled confirmation of stone presence, identification of urolith composition, and monitoring of response to therapy.

Chronic low back pain is a highly prevalent health condition intricately linked to intervertebral disc degeneration. One of the prominent features of disc degeneration that is commonly observed with aging is dystrophic calcification. ATP-binding cassette sub-family C member 6 (ABCC6), a presumed ATP efflux transporter, is a key regulator of systemic levels of the mineralization inhibitor pyrophosphate (PPi). Mutations in ABCC6 result in pseudoxanthoma elasticum (PXE), a progressive human metabolic disorder characterized by mineralization of the skin and elastic tissues. The implications of ABCC6 loss-of-function on pathological mineralization of structures in the spine, however, are unknown. Using the ABCC6 -/- mouse model of PXE, we investigated age-dependent changes in the vertebral bone and intervertebral disc. ABCC6 -/- mice exhibited diminished trabecular bone quality parameters at 7-months which remained significantly lower than the wild-type mice at 18 months-of-age. ABCC6 -/- vertebrae showed increased TRAP staining along with decreased TNAP staining, suggesting an enhanced bone resorption as well as decreased bone formation. Surprisingly, however, loss of ABCC6 resulted only in a mild, aging disc phenotype without evidence of dystrophic mineralization. Finally, we tested the utility of oral K3Citrate to treat the vertebral phenotype since it is shown to regulate hydroxyapatite mechanical behavior. The treatment resulted in inhibition of osteoclastic response and an early improvement in mechanical properties of the bone underscoring the promise of potassium citrate as a therapeutic agent. Our data suggest that although ectopic mineralization is tightly regulated in the disc, loss of ABCC6 compromises vertebral bone quality and dysregulates osteoblast-osteoclast coupling.

Periodontal patients often report dentin hypersensitivity (DH) caused by root surface exposure or periodontal treatment. Tubular blocking technologies in toothpastes are effective for pain relief, but no specific chemical/physical agent has been reported for periodontal patients. This double-blind randomized clinical trial compared the effects of three technologies in reducing DH in periodontal patients. Eighteen (18) participants were randomly assigned into three groups: SEN (NOVAMIN technology); REG (REFIX technology); REGK (REFIX technology + potassium citrate). Periodontal patients presenting with DH were evaluated at 6 moments: T1 and T2 - immediately before and after scaling and root planing procedures (SRP); T3 - after polishing sensitive areas with their assigned dentifrice and T4, T5, T6 - after 2, 4 and 8 weeks of SRP respectively. Sensitivity was assessed by air blast (Schiff scale) and patients’ perceptions using the visual analogue scale (VAS). Data were analyzed by two-way repeated measures ANOVA complemented by the Tukey test with significance set at 5% (p <0.05). Preliminary outcomes revealed SEN, REG and REGP reduced DH in periodontal patients (n=18). All patients initially presented moderate to severe pain (64.3) and after treatment they reported mild pain (21.3). Similarly, the dentist evaluation showed significant reduction in DH with the use of the three technologies (2.26 to 0.56). No statistically significant differences were found between the three study groups for patients (p=0.751) and dentist evaluations (p=0.632). According to these preliminary outcomes, all three technologies equally reduced DH in periodontal patients. Clinical trials #NCT04422184

We report a case of an 81-year-old man with end stage renal disease (ESRD) in chronic hemodialysis with severe hypocalcaemia secondary to the intake of a parapharmaceutical containing sodium and potassium citrate and in association with poor dialysis efficiency, due to malfunction of the vascular access, which promptly resolved after discontinuation of the supplement. The patient never showed signs or symptoms of hypocalcaemia. This case highlightens the importance of a correct pharmacological reconciliation in dialysis patients in order to avoid the onset of adverse events due to the uncontrolled intake of drugs or parapharmaceuticals.

This article provides information on the utilization and development of colloidal formulations based on organic potassium citrate, an alternative, environmentally friendly composite fluid that can support the drilling and production of oil and gas wells. The activity started with a laboratory-scale literature study, the formulation and modification of organic potassium citrate, testing the characteristics and performance of liquids, and improving advanced formulations. This completion fluid based on organic potassium citrate has high solubility and good stability to other pollutant particles. The colloidal organic salt system based on potassium citrate is an alternative function of the replenishment fluid (pressure holding fluid) for cleaning/deepening oil and gas wells. Currently, widely used composite fluids are chloride (Cl-), bromide (Br-), nitrate (NO3-), phosphate (PO4-), and cesium formate. However, these salt solutions are highly corrosive and are not environmentally friendly. The swelling test and the corrosion rate test are carried out using different fluids (i.e., KCl slurry, NaCl polymer, and colloidal organic citrate). The test results show that when the test is performed at room temperature for 16 hours, the organic citrate fluid's swelling rate is lower than 10%, and the KCl and NaCl sludge is higher than 20%. The result of a seven days corrosion test at 350oF is that the citric acid organic liquid is lower than 10 mpy, while KCl and NaCl are higher than 50 mpy. This completion fluid formulation based on organic citrate salts is a new type of completion fluid that has the following specifications: density up to (1.80), corrosion rate (3.19 mpy) @ 248-350oF, pH (6-8), resistance to extreme temperature, turbidity (7 NTU), a swelling rate below 10%, and has good compatibility with formation water. Also, potassium citrate organic salts can be degraded efficiently, making them safe for the environment.
