Applications related to the use of leeches in medical therapies date back for centuries. We present here a case of self-administered leech application for chronic low back pain who subsequently developed complication of chronic subdural hematoma. We are not aware of similar report in the literature. This was a 34-year-old man who was admitted to our Emergency Department with headache, right side numbness, and intermittent difficulty in speaking. During the neurological examination, it was observed that the patient had difficulty in speaking and pronouncing words intermittently while taking his medical history. Motor examination was normal. The patient did not report any recent trauma. There was no known history of systemic or hereditary disease, drug or substance abuse. Cranial CT and MR imaging showed a left frontoparietal chronic subdural hematoma. Further history was obtained that leech was applied to the patient for low back pain in the last 45 days, two times at 15 days intervals. The patient’s drowsiness and speech difficulties improved after the surgical operation. On one month follow-up, neurological examinations were normal. Our patient shows that leech therapy can be complicated by subdural hematoma.

COVID-19 has grossly impacted lives of people across the globe. In particular, children have also been affected due to closure of schools, therapy, and day care centers. Families have been challenged with new circumstances, and mental health professionals are coming up with novel ways to help these families who have children with mental health issues. This article describes experiences of families who have children with a diagnosed neurodevelopmental disorder with comorbid mental health difficulties and their ways of coping with the pandemic challenges. The series will throw light on ground level experiences of families during the pandemic, give insights into their ways of adapting, and brings out problem areas which healthcare professionals must work on, to design novel ways of care. The case series is novel and a similar report has probably not been presented from India or other low and middle income countries.

Colleagues at the Institute of Educational Technology at the Open University produce an annual report, Innovating pedagogy, now in its 9th edition, which looks at new and emerging approaches to teaching, learning, and assessment in a wide range of settings and subject areas. They highlight ten innovative pedagogies, and provide a research-informed summary of each one, as well as a list of further resources for those who want to find out more. Over the past few years, this has become an influential publication and we have long thought that it would be very useful to have a similar report that focused specifically on language teaching, learning, and assessment.

ABSTRACT We report an unusual case of a 24-year-old girl with a history of recurrent hypokalemic paralysis episodes and skin lesions on the lower limbs and buttocks, both of which had an acute evolution. In subsequent investigations, the patient also had nephrocalcinosis, nephrolithiasis, hyperchloremic metabolic acidosis and persistent alkaline urinary pH. The findings were consistent with distal renal tubular acidosis as the cause of hypokalemic paralysis. Clinical findings, immunological tests and the result of skin biopsy suggested primary Sjögren's syndrome as an underlying cause. The patient developed azotemia due to obstructive nephrolithiasis. All the features presented in this case are an unusual manifestation of distal renal tubular acidosis; so far, we are not aware of a similar report in the literature.

AbstractWe aimed to characterize the relationship of the oral microbiome with diabetes in Pakistan. Saliva samples were collected from diabetic patients (n = 49) and healthy individuals (n = 55). 16S metagenomics saliva was carried out by NGS technology. We observed that the phylum Firmicutes (p-value = 0.024 at 95% confidence interval) was significantly more abundant among diabetic patients than among the controls. We found that the abundance of phylum Actinobacteria did not significantly vary among both groups in contrast to a similar report from the USA (Long et al., 2017). On genus level, acidogenic bacteria Prevotella (p-value = 0.024) and Leptotrichia (p-value = 1.5 × 10−3); and aciduric bacteria Veillonella (p-value = 0.013) were found to be in higher abundance in diabetic patients. These bacteria are found in dental biofilm and involved in the metabolism of fermentable carbohydrates. Stratified analysis by gender revealed healthy and diabetic females to be more divergent. Abundance of Prevotella (p-value = 4.4 × 10-3) and Leptotrichia (p-value = 0.015) was significantly associated with male patients. A comparison of oral bacteriome between two groups revealed the dominance of acidogenic and aciduric bacteria in diabetics which suggested the involvement of these eubacteria in oral dysbacteriosis in diabetes mellitus.

The article considers the specifics of the activities of international non-governmental organizations to ensure the gender equality principle. Attention is drawn to the practice of activities of such international non-governmental organizations as: «Gender and Development Network» (GADN), Equality Now, Promundo, which directly fulfill the mission of advocating for women's interests, guaranteeing their equal opportunities with men in all spheres of public life. Women's international non-governmental organizations are best known for two different but often interrelated activities: firstly, it is the provision of services for women in need, and secondly, it is the organization of informational outreach activities and public campaigns to implement the social transformations needed to achieve gender equality. INGOs are important partners of national governments in ensuring gender equality; act as experts, diplomats, advocates for specific issues or groups, protesters and activists; they are involved, for example, in the development of international rules, standards and regulations; use their experience and special authority to inform societies and national governments on particular issues and raise people's awareness about specific problems; promote new ideas and encourage governments to implement them; can act as representatives of national governments in times of crisis; awarded the Nobel Peace Prize for «spreading authoritative information and raising public awareness». Ukraine's positioning in the leading international rankings, which consider gender equality as an integral component, the basis of a democratic society, is examined. It is about such reports as the Human Development Index, the Freedom House Report, the Global Gender Gap Report. It is concluded that, according to the 2020 Global Gender Gap Report, in Ukraine a positive dynamics is observed compared to a similar report for 2014, however, the worst is the situation with the political component (women's participation in decision-making) – 83th place with a value that is almost approaching zero, that is, continuous gender inequality.

AbstractA 79-year-old patient was admitted to the emergency room with transitory monoparesis in the left hand and dysphasia. The brain computed tomography (CT) and magnetic resonance imaging (MRI) showed a spontaneous right convexity subarachnoid hemorrhage (cSAH). Digital subtraction angiography (DSA) confirmed an asymptomatic occlusion of the right internal carotid artery (ICA) . Cases related to stenosis have already been described, but there is no similar report of a case related to occlusion, even though the pathophysiology of both entities is similar. Atraumatic SAH has been associated with intracranial and extracranial artery stenosis.
