AbstractFacial trauma is common in the pediatric population with most cases involving the soft tissue or dentoalveolar structures. Although facial fractures are relatively rare in children compared with adults, they are often associated with severe injury and can cause significant morbidity and disability. Fractures of the pediatric craniomaxillofacial skeleton must be managed with consideration for psychosocial, anatomical, growth and functional differences compared with the adult population. Although conservative management is more common in children, displaced fractures that will not self-correct with compensatory growth require accurate and stable reduction to prevent fixed abnormalities in form and function.

Abstract PurposeThe present study aimed to explore the influence of ulnar bow on the surgical treatment of Bado type I missed Monteggia fracture in children.MethodsThis study is a retrospective review of 24 patients between November 2010 and March 2019. All patients were treated with open reduction of the radial head and ulnar opening wedge osteotomy without annular ligament reconstruction. The mean interval between injury onset and surgery was five months (range: 2–12 months). The average age of participants at the time of surgery was 6.4 years (range: 3–10 years). We evaluated the maximum ulnar bow (MUB) and MUB position (P-MUB) via radiography. The patients were divided into middle group (group A: 14 cases, MUB located at 40% to 60% of the distal ulna) and distal group (group B: 10 cases, MUB located at 20% to 40% from the distal end of the ulna) based on P-MUB. The mean period of follow-up was 37 months (range: 6–102 months).ResultsAt the last follow-up, all the children showed stable reduction of the radial head, and the flexion function of elbow joint improved after operation (P<0.05). Group A presented a larger the ratio of maximum ulnar bow(R-MUB) and angle of ulnar osteotomy(OA) than group B (P<0.05). There was statistically significant difference between group A and Group B in the P-MUB (P < 0.05). The osteotomy angle was positively correlated with the R-MUB (R2 =0.497,P=0.013), The osteotomy angle was positively correlated with the P-MUB (R2=0.731,P=0.000), The R-MUB is proportional to the P-MUB (R2 =0.597,P=0.002). The regression equation of P-MUB and osteotomy angle: Angle=7.064+33.227* P-MUB (R2=0.459, P =0.000).ConclusionWhen the ulnar bow is positioned at the middle ulna, a stable reduction of radial head need to be achieved through a larger angle in the ulnar osteotomy. If the position of maximum ulnar bow (P-MUB) is closer to the middle of the ulna or the ratio of maximum ulnar bow (R-MUB) is larger, the osteotomy angle is larger.

Surgeons are always in a dilemma for the management of paediatric facial fractures due to distinctive nature of growing facial skeleton. The ultimate treatment goal for paediatric fractures should be as conservative as possible with least invasive surgical approach that will access the fracture and least invasive surgical fixation that will allow stable reduction and will prevent any growth disturbances. We are presenting a case of 4- year old with panfacial fracture managed according to this principle using transmucosal titanium plates with acylic splint for sagittal split of palate.

AbstractIntroductionDuring pregnancy, the imperative to stop smoking becomes urgent due to health risks for mother and baby.AimExplore responses to a smoking-related, pregnancy-focused Risk Behaviour Diagnosis (RBD) Scale over time with Aboriginal1 pregnant women.MethodsSix Aboriginal Medical Services in three states recruited 22 eligible women: ⩽28 weeks' gestation, ⩾16 years old, smoked tobacco, pregnant with an Aboriginal baby. Surveys were completed at baseline (n = 22), 4-weeks (n = 16) and 12-weeks (n = 17). RBD Scale outcome measures included: perceived threat (susceptibility and severity), perceived efficacy (response and self-efficacy), fear control (avoidance), danger control (intentions to quit) and protection responses (protecting babies).ResultsAt baseline, the total mean threat scores at 4.2 (95% CI: 3.9–4.4) were higher than total mean efficacy scores at 3.9 (95% CI: 3.6–4.1). Over time there was a non-significant reduction in total mean threat and efficacy; fear control increased; danger control and protection responses remained stable. Reduction of threat and efficacy perceptions, with raised fear control responses, may indicate a blunting effect (a coping style which involves avoidance of risks).ConclusionIn 22 Aboriginal pregnant women, risk perception changed over time. A larger study is warranted to understand how Aboriginal women perceive smoking risks as the pregnancy progresses so that health messages are delivered accordingly.

Medial Swivel-type dislocation are dislocation of talonavicular joint (TNJ) medially and the calcaneus swivels under the talus, with the calcaneocuboid joint intact. This is a rare injury due to the strong network of ligament and tendinous structures. An 11-year-old girl presented after 1 week of injury to the left foot. She had pain, swelling on the mid-foot and inability to weight bearing. X-ray and computerized tomography (CT) scan showed medial dislocation of TNJ, with fracture of cuboid body. A closed reduction was attempted but it failed. Patient then underwent open reduction with K-wire fixation and immobilization by below knee cast for 3 weeks. After K-wire removal, the foot was stable with near normal ankle and sub-talar joint range of motion and patient started to partial weight bearing Midtarsal dislocations of the foot are rare injuries. In this case dislocation is resulted from high-energy medial forces to the forefoot. The associated cuboid fracture possibly as a result of tensile forces through the lateral structure of midfoot with adduction. ‘Swivel dislocation’ in which the TNJ dislocates, usually medially, and the calcaneus swivels under the talus, with the calcaneocuboid joint intact. A careful assessment of initial radiograph and CT scan should be done to determine type of dislocation and associated fracture. An open reduction K-wire fixation bring a stable reduction. Talonavicular dislocations are rare injuries, occur as a result of high-energy trauma. A stable reduction and good outcome are anticipated.

AbstractWe give a description of the category of ordinary K3 surfaces over a finite field in terms of linear algebra data over {{\mathbf{Z}}}. This gives an analogue for K3 surfaces of Deligne’s description of the category of ordinary abelian varieties over a finite field, and refines earlier work by N.O. Nygaard and J.-D. Yu. Our main result is conditional on a conjecture on potential semi-stable reduction of K3 surfaces over p-adic fields. We give unconditional versions for K3 surfaces of large Picard rank and for K3 surfaces of small degree.
