Aim. Assess the medical therapy quality in patients with chronic heart failure (CHF) and patients' adherence to the treatment depending on the previous observation in a specialized medical center as part of an of an outpatient registry.Materials and methods. An analysis of the medical therapy quality in patients with CHF was carried out as part of the COMPLIANCE prospective observational study (NCT04262583). 72 patients with CHF verified according to the protocol were included in the study. The average age of the patients was 69.1±9.5 years (31% of women and 69% of men). Patients were divided into groups: those who first applied to a specialized department during the period of the study inclusion, or those who were previously observed in a specialized department. The general adherence assessment to medical therapy was carried out using the original questionnaire «The adherence scale of the National Society for Evidence-Based Pharmacotherapy» which was supplemented with questions to assess the actual adherence to specific medical drugs recommended for patients with CHF.Results. According to the results of the study, beta-blockers were prescribed to 70 (97.2%) patients. Angiotensin converting enzyme inhibitors (ACEi)/angiotensin receptor blockers (ARBs) were recommended in 68 (94%) patients. Mineralocorticoid receptor antagonists were included in therapy in 6 out of 9 patients who were shown to be prescribed (66.6%). The choice of medical drugs within the group was not always adequate. For example, ACEi/ARBs with proven efficacy in patients with CHF were prescribed only in 72% of patients. Comparative analysis of adherence to medical therapy between patients of the selected groups demonstrated a higher adherence to the recommended therapy in patients who were previously observed in a specialized center.Conclusion. The medical therapy quality for patients with CHF doesn't always comply with current clinical guidelines. The choice of a medical drug within a group is not always adequate. Regular observation in a specialized center contributes to a higher adherence to the recommended therapy.

Driven by the theory-building around the role of the non-verbal components to communication, we aimed to understand how therapists experience the therapeutic process using a facial mask. The empirical evidence of the power of non-verbal communication to engage therapists and clients in therapeutic work, develop a positive and collaborative relationship between them, and display empathy is quite large. A mixed-methods approach was adopted, drawing from the therapists’ participation in an online survey. A sample of 137 psychotherapists with different therapy orientations and years of clinical practice participated in the study. Therapists conducted face-to-face therapy wearing face masks with existing and/or new clients. We performed an exploratory analysis, using descriptive statistics, to explore the psychotherapists’ evaluations regarding perceived impact of face masks on different therapy quality dimensions. In a complementary rationale, we analyzed the therapists’ perspectives on their experience wearing face masks using the thematic analysis methodology. Results show that among 137 psychotherapists, 114 were attending both existing and new clients, whereas only 13 were seeing exclusively existing clients and 10 were working exclusively with new clients. Despite no major differences were found between conditions regarding the perceived impact of face masks on different therapy quality dimensions and strategies adopted, the qualitative analysis allowed us to expand the quantitative results and deepen understanding of psychotherapists’ experience. Based on general and typical patterns, we propose two distinct models to describe the therapist’s experiences narrative when working with existing or new clients wearing face masks. Based on the results, we propose some recommendations to clinical practice in similar conditions.

Epidemiological and clinical data have shown clear differences in several aspects of cardiovascular disease, particularly in the case of coronary artery disease (CAD), between men and women, including risk factors, response to therapy, quality of care, and natural history.[...]

The article presents the first experience of analyzing the treatment quality of hospitalized patients with heart failure based on electronic health records (EHR). We analyzed EHR of patients hospitalized in three large hospitals in Kirov. The results of the analysis indicated insufficient detailed information in the EHR, which complicates analyzing the accuracy of diagnosis and therapy quality. In particular, attention is drawn to the disproportionate number of patients with heart failure with reduced and preserved ejection fractionю This, apparently, is due to the low prevalence of assessing brain natriuretic peptides and conducting Doppler echocardiography. A separate part of the analysis is devoted to assessing the therapy quality in patients with concomitant diabetes. Despite the study limitations, the presented results can be useful for improving the quality of EHR filling for performing further observational clinical trials.
