In this chapter we present an information system conceived for supporting managers of Public Health Care Agencies to decide the new health care services to propose. Our system is HL7-aware; in fact, it uses the HL7 (Health Level Seven) standard (Health Level Seven [HL7], 2007) to effectively handle the interoperability among different Public Health Care Agencies. HL7 provides several functionalities for the exchange, the management and the integration of data concerning both patients and health care services. Our system appears particularly suited for supporting a rigorous and scientific decision making activity, taking a large variety of factors and a great amount of heterogeneous information into account.


In health care service the treatment recommendation system is plays very important role. In health care services for decision making the Treatment Recommender Systems (TRS) presented as a complementary tools. In current process the Treatment Recommender Systems increase the technology usability and reduce overloading of information

Uncertainty in clinical decision-making is integral to the pathways chosen while applying available knowledge to a patient’s care process. This chapter explores the ways in which uncertainty can be incorporated into the understanding of better performance approaches, and is thus proposed as an enabling dimension of performance. Tracing the keystone definitions of uncertainty from Hippocrates to Osler, the discussion addresses the dimensions of decision - making appropriateness, its timeliness, the expected and actual value of the care services, and the role of systematic communication between providers of care as well as with patients. The crucial role of Health Information Technology is emphasized, and a unifying model is proposed where the inclusion of uncertainty as a dimension of performance promotes an encompassing evaluation of the quality of health care services.

The impact of the health care sector and economic vitality plays a major role in the upliftment of the welfare of the society. A country's stability and sustainability will depend on how healthy its population is (Patry, Morris & Leatherman, 2010). Therefore, the reduction in mortality rate and an increase in healthy work force that performs work duties are likely to promote economic activities which will in turn increase the country's wealth. Clients seek health care services that can improve the state of their well-being. Since health care services are needed to promote well-being, clients are particular about how and where they purchase health care services from (Akponah, Mazibuko & Krüger, 2015:153). Numerous researchers have studied the factors that determine clients' decision-making in the utilization of health care services. Andersen and Newman (2005:14) put forward that predisposing, enabling and illness level influence the clients' decision to utilize health care services. Mekonnen and Mekonnen (2002:2) report that gender as a demographic factor influences the decision-making of clients regarding the utilization of health care services. However, Williams (2005:35) maintains that client attitudes regarding their decisions to utilize and access health care services from private health care institutions are influenced by their social structure, occupation, education, ethnicity, cultural, attitudes and beliefs relating to values, knowledge and attitudes from past experience. This study investigates and answers the following questions: What factors influences clients' decision-making regarding the utilization of private health care institutions? Background Uganda continues to have a high neonatal mortality rate, with 20 deaths per 1000 live births reported in 2018. A measure to reverse this trend is to fully implement the Uganda Clinical Guidelines on care for mothers and newborns during pregnancy, delivery and the postnatal period. This study aimed to describe women’s experiences of maternal and newborn health care services and support systems, focusing on antenatal care, delivery and the postnatal period. Methods We used triangulation of qualitative methods including participant observations, semi-structured interviews with key informants and focus group discussions with mothers. Audio-recorded data were transcribed word by word in the local language and translated into English. All collected data material were stored using two-level password protection or stored in a locked cabinet. Malterud’s Systematic text condensation was used for analysis, and NVivo software was used to structure the data. Findings Antenatal care was valued by mothers although not always accessible due to transport cost and distance. Mothers relied on professional health workers and traditional birth attendants for basic maternal services but expressed general discontentment with spousal support in maternal issues. Financial dependency, gender disparities, and lack of autonomy in decision making on maternal issues, prohibited women from receiving optimal help and support. Postnatal follow-ups were found unsatisfactory, with no scheduled follow-ups from professional health workers during the first six weeks. Conclusions Further focus on gender equity, involving women’s right to own decision making in maternity issues, higher recognition of male involvement in maternity care and improved postnatal follow-ups are suggestions to policy makers for improved maternal care and newborn health in Buikwe District, Uganda.

Purpose The purpose of this paper is to identify and prioritize prime just-in-time (JIT) elements in Indian healthcare sector based on the degree of importance and difficulty. This facilitates the implementation of JIT philosophy in healthcare services without any failure as well as provides quality services to patients at low cost. Design/methodology/approach JIT elements related to health care services have been sorted out from comprehensive literature survey. Moreover, important and difficult JIT elements have been ranked on the basis of score obtained by the quantitative method using statistical tools. Furthermore, validation and prioritization of important JIT elements based on the degree of importance have been computed using the analytical hierarchy process (AHP) and best worst method (BWM). Findings The present work provides important, difficult and easy to implement JIT elements in healthcare services. Besides, this work justifies the application of decision-making tool (AHP, BWM) for the prioritization of JIT elements in the health care sector. Practical implications The present study develops a deep understanding of JIT concepts in health care services. Furthermore, it motivates professionals for implementing JIT in healthcare services with healthy situation for both hospital and supplier. Social implications The present work is beneficial for all the dimensions of sustainable development (social, economic and environmental). The implementation of JIT with a proper understanding of its elements results in an improved patient care, lesser cost of healthcare delivery, time and better management of associated medical items. This work also facilitates the proper management of inventory items together with the reduction in various Lean wastes with the proper implementation of JIT in healthcare. The reduction in various associated wastes leads to cleaner surrounding and lesser environmental degradation. Originality/value This paper outlines the need of a robust JIT approach in the healthcare sector for quality services. The efficacy of JIT, AHP and BWM has been explored to find out critical elements for successful JIT implementation in health care.
