The study aimed to conduct a cost-value analysis of the production process of a newly introduced batch of external doors in the context of value-added creation and to identify redundant processes that do not create added value and for which appropriate corrective actions could contribute to their elimination. The result of applying improvement actions following the lean management concept was the optimization time nationalized analyzed by eliminating, among others, operations related to unnecessary transport and storage of products. In addition, the optimization production process impacted both shortening the process implementation time and reducing the costs of its implementation. Further activities will be related to the use of the presented methodology to analyze the processes implemented in the company in order to increase their efficiency.

PG PESANTREN BARU is a company engaged in the manufacturing industry which began operating on July 19, 1978 with a capacity of 4,000 tons of sugar cane per day. Negligence in operating the machine in addition to causing disruption in the production process, can also endanger workers. The problem formula of this research is where it relates to the fundamental issues regarding Boiler stations, and how to minimize the risk of work accidents at Boiler stations using the HAZOP method, while the purpose of this study is to find hazard resources at Boiler stations, and minimize the use of work accidents at the Boiler station using the HAZOP method. The results of K3 research at the boiler station using the hazop method, have found 31 potential hazards which are then classified into 5 sources of hazards that have been identified at the boiler station. From the results of the risk assessment, there are 3 sources of danger that have extreme risks associated with the attitude of workers, floors, and kitchen boiler rooms, then 1 source of danger that has a higher risk on the stairs and 1 source of danger that has a risk of being exposed to oil spills . Keywords : Job Safety, Risk, HAZOP ( Hazard and Operability Study ) PG PESANTREN BARU merupakan sebuah perusahaan yang bergerak dalam bidang industri pembuatan gula yang mulai beroperasi pada tanggal 19 Juli 1978 dengan kapasitas giling 4.000 ton tebu perhari. Kelalaian dalam mengoperasikan mesin selain akan menyebabkan proses produksi terganggu, juga dapat membahayakan pekerja. Rumuan masalah penelitian ini adalah dimanakah letak resiko tertinggi sumber bahaya pada stasiun Boiler, dan bagaimana cara meminimalisir resiko terjadinya kecelakaan kerja pada stasiun Boiler dengan menggunakan metode HAZOP, Sedangkan tujuan dari penelitian ini adalah untuk mengetahui potensi sumber bahaya tertinggi pada stasiun Boiler, dan meminimalisir terjadinya kecelakaan kerja pada stasiun Boiler dengan menggunakan metode HAZOP. Hasil dari penelitian K3 di stasiun boiler menggunakan metode hazop,  telah ditemukan 31 potensi bahaya yang kemudian digolongkan menjadi 5 sumber bahaya yang sudah teridentifikasi pada stasiun boiler. Dari hasil penilaian resiko, maka terdapat 3 sumber bahaya yang memiliki resiko ekstrem yang meliputi sikap pekerja, lantai, dan ruang dapur boiler, kemudian 1 sumber bahaya yang memiliki resiko tinggi terletak pada tangga dan 1 sumber bahaya yang memiliki resiko sedang yaitu pada tumpahan oli. Kata Kunci : Keselamatan Kerja, Resiko, HAZOP ( Hazard and Operability Study )

Industrial development in Indonesia is growing rapidly. This should be balanced with the application of good HA safety risk management to workplace hazard risks. As in PT. X Lumajang Regency, in its production process has great potential hazard. One of them is in the cane knife tool at Gilingan Station that has been damaged in operation and has an employee accident impact. Therefore, to prevent the occurrence of recurrence of risk assessment, including hazard identification, risk assessment and hazard control. The purpose of this study is to conduct risk assessment using the method of failure mode effect analysis (FMEA) on the Cane Knife production process tool in PT. X Lumajang District. This research is descriptive research. The target of this research is sugar production process at PT. X with the informant is 1 person Quality Control (QC), 1 person production operators and 1 supervisor. The results show that there are 6 (six) failure modes that occur in the cane knife tool. Based on the Risk Priority Number (RPN), the highest failure mode occurs is a broken clutch string. While for risk control, quite enough. This is because there have been risk control efforts include technical control that is the installation of safety valve, administrative control is the application of SOP for workers and personal protective equipment. But for personal protective equipment only safety helmet, for other companies do not provide. The conclusion that can be drawn is the need for better risk control, especially on the damage that occurred on the clutch rope broken tool cane knife.Keywords: cane knife, failure mode effect analysis, risk assessment

The process of making a barecore still uses traditional and simple human labor and machining technology. In the production process, there are several sources of danger found, namely: machines used, work attitudes, work methods, raw materials, and the resulting sawdust disturbs environmental quality. This study aims to identify, assess the risk of hazards in the production process as well as corrective actions to control hazards using the Hazard and Operability (HAZOP) method. The research steps were hazard identification, risk assessment and corrective action. For hazard risk assessment, the OHS Risk Assessment method is used. The results of this study found that there are four sources of danger, namely the work environment, humans, materials, and machines. The results of the risk assessment calculation obtained three risk values, namely low risk with the number of findings of 13 potential hazards, medium risk with the number of findings of 78 potential hazards and high risk with the number of findings of two potential hazards. Risk control efforts to reduce potential hazards can be done by: using personal protective equipment, adding boxes as wooden material containers, making standard operating procedures, and providing first aid facilities.

Purpose The purpose of this paper is to establish an effective risk assessment approach based on the conditional value-at-risk (CVaR) in the agricultural supply chain. Design/methodology/approach This study analyzes and assesses the risks of breeding, processing, transportation and warehousing in the agricultural supply chain. The ordered weighted averaging operator is used to sort risk control factors according to their importance and determine the main risk indicators of an enterprise. The CVaR model is utilized to establish the risk loss function, and an improved genetic algorithm is employed to identify the optimal risk control portfolios in the case of the smallest risk loss. Findings Based on the approach, the optimal combination of risk control to minimize risk losses is determined. Results show that the proportion of capital investment in risk control differs at three confidence levels, and a large amount of money needs to be invested in the production process at the source. Thus, any attempt to control the risks inherent in the agricultural supply chain must begin with the production process at the source. Originality/value Supply chain risk management has become increasingly important and significant to the operation and production of enterprises in recent years. The proposed method to assess the risk in the agricultural supply chain can benefit managers in making smart decisions to control total risk.
