Abstract Purpose of Review Coffee is a very popular drink and an estimated 2.25 billion cups worldwide are consumed daily. Such popularity of coffee makes it the most consumed drink next to water. Numerous studies have shown a beneficial effect of habitual and moderate coffee consumption on the functioning of the nervous, digestive, and cardiovascular systems, as well as on kidney function. Taking into account the very high prevalence of arterial hypertension in the world (31.1% of adults), much controversy has been raised about the influence of coffee consumption on blood pressure and the risk of arterial hypertension. Moreover, there have been extensive discussions about the safety of coffee consumption for hypertensive persons. Recent Findings There are over 1000 chemical compounds in coffee. The best characterized of these are caffeine, chlorogenic acid, trigonelline, kahweol, cafestol, ferulic acid, and melanoidins. These compounds have bidirectional influences on blood pressure regulation. The results of numerous studies and meta-analyses indicate that moderate and habitual coffee consumption does not increase and may even reduce the risk of developing arterial hypertension. Conversely, occasional coffee consumption has hypertensinogenic effects. Moderate habitual coffee consumption in hypertensive persons does not appear to increase the risk of uncontrolled blood pressure and may even reduce the risk of death from any cause. Summary Moderate and habitual consumption of coffee (1-–3 cups / day) does not adversely affect blood pressure in most people, including those with arterial hypertension.

Introduction. The high prevalence of diseases of the circulatory system of workers in various sectors of the economy, the lack of data on the effects of harmful production factors on the formation of cardiovascular diseases in petrochemical industry workers justify the importance of conducting special studies on these petrochemical complex plants with the aim of assessing cardiovascular risk with the subsequent development of preventive measures. Material and methods. The objects of the study were workers in the production of monomers (ethylene, propylene, ethylbenzene, isoprene, divinyl) of petrochemical enterprises. Hygienic studies of the working conditions of workers included an assessment of the factors of the working environment and the labor process using generally accepted methods. Comprehensive clinical examination of workers included periodic examinations (n = 2634) with calculation of total and relative cardiovascular risk using the SCORE system, using questionnaire data, a Reeder L questionnaire and an in-depth clinical diagnostic medical examination (n = 101) with the definition total cardiovascular risk. Results. The most significant occupational factors of the labor process for workers in the main profession - panmen were established to include the combination of a chemical factor with the occupational noise and labor intensity, with an overall assessment of working conditions 3.2. The class of working conditions of workers of the comparison group (locksmith in instrumentation and automation corresponds to the permissible). There were revealed a high prevalence of arterial hypertension and other factors of cardiovascular risk. A higher dependence of the arterial hypertension level on age and length of service in the group of panmen was established when compared to the mechanicians of instrumentation and automation. Using mathematical models of multiple regression, the likely dynamics of the level of arterial hypertension and the left ventricular myocardial mass index were calculated under the influence of increasing age and other risk predictors. Conclusion. The results of a clinical study showed workers with severe arterial hypertension, a high and very high risk of death from cardiovascular disease on the SCORE scale, a high and very high overall cardiovascular risk, signs of dyslipidemia, obesity, high stress levels to require close attention. It is important to timely diagnose lesions of target organs, to conduct rational prevention aimed at reducing risk factors. According to the results of the study, there were developed preventive measures aimed at reducing cardiovascular morbidity and cardiovascular risk, prolonging the longevity of workers in petrochemical industries.

Aim. To study the clinical potential and feasibility of the assessment of cardio-ankle vascular index (CAVI) at medical prevention departments and units.Material and methods. The study included 66 patients (13 men and 53 women; mean age 52,26±11,2 years) with very low to very high total cardiovascular risk (CVR) levels and SCORE index 0–25% (mean SCORE index 3,18±4,34%), who underwent volume sphygmography and automatic CAVI measurement.Results. There was a moderately strong, statistically significant correlation between CAVI and age; CAVI and systolic blood pressure (SBP); and CAVI and SCORE. While higher CAVI values were observed among non-smokers and patients with arterial hypertension, these differences were not statistically significant.Conclusion. As a method for arterial stiffness assessment, CAVI measurement is easy to perform, does not require any special preparation, and can be used at medical prevention departments and units as a part of a more detailed preventive examination. A moderately strong, statistically significant correlation was observed between CAVI and SCORE, as well as between CAVI and such SCORE components as age and SBP. However, further research is needed in order to clarify the possibility of CAVI inclusion in prognostic models.

Arterial hypertension (AH) remains one of the most common diseases in the world. Reducing cardiovascular risk of mortality from cardiovascular complications is a priority in the treatment of hypertension. Targets of hypertension therapy is to achieve SBP/DBP <140/80 mm Hg. article, regardless of cardiovascular risk and comorbidity. The choice of antihypertensive therapy depends on blood pressure levels, the presence or absence of concomitant diseases, lesion of target organs. Effective and long-lasting control of AH proved by the use of combinations of antihypertensive drugs, including antagonists and ACE inhibitors. The purpose of the study. Evaluation of clinical efficacy and safety of fixed combination antihypertensive therapy using lisinopril combined with amlodipine in patients with hypertension. Material and methods. The study included 30 respondents from essential hypertension II-III degree (ESH/ESC, 2018) aged 35 to 75 years. Patients were treated with the schema a (lisinopril 10 mg, amlodipine 5 mg) and scheme b (lisinopril 20 mg, amlodipine 10 mg) fixed combination therapy with the drug VivaCor® once in the morning. The period of observation was 3 months. At the time of inclusion and at the end of the study all respondents was performed echocardiography, daily monitoring of blood pressure, evaluation of laboratory and instrumental methods of research. Results and discussion. The combination of lisinopril with amlodipine has a fairly high antihypertensive activity, provides a significant cardioprotective effect and is an integral part of antihypertensive therapy in the long-term strategy of administering patients with high hypertensive / very high total cardiovascular risk. Conclusions: 1. The use of a fixed combination of lisinopril and amlodipine in patients with hypertension instead of free or other two-component combinations leads to its target level within 3 months of therapy. 2. Taking a fixed combination of lisinopril and amlodipine is effective and safe. 3. Therapy using the combined drug VivaCor® in patients with high / very high risk of hypertension prevents further pathological remodeling of the heart. 4. The combination of amlodipine and lisinopril is metabolically neutral and well tolerated by patients. Keywords: arterial hypertension, lisinopril, amlodipine, VivaCor.

In almost 36% of patients, the syndrome of arterial hypertension (AH) is determined by a combination of psychoemotional disorders with autonomic dysfunction. The aim of the study was to study the features of psychoemotional disorders and autonomic homeostasis in 78 middle-aged patients with uncomplicated hypertension and their correction using Carvelis in complex antihypertensive therapy.

Background. We investigated the association between blood pressure control and common cardiometabolic risk factors from a global and regional perspective.Methods. In the present analysis of a large cross-sectional i-SEARCH study, 17.092 outpatients receiving antihypertensive treatment were included in 26 countries. According to clinical guidelines for the management of arterial hypertension, patients were classified based on the level of seated systolic/diastolic blood pressure (SBP/DBP). Uncontrolled hypertension was defined as SBP/DBP ≥140/90 mmHg for non-diabetics, and ≥130/80 mmHg for diabetics.Results. Overall, mean age was 63.1 years, 52.8% were male, and mean BMI was 28.9 kg/m2. Mean SBP/DBP was 148.9/87.0 mmHg, and 76.3% of patients had uncontrolled hypertension. Diabetes was present in 29.1% with mean HbA1c of 6.8%. Mean LDL-cholesterol was 3.2 mmol/L, HDL-cholesterol 1.3 mmol/L, and triglycerides 1.8 mmol/L; 49.0% had hyperlipidemia. Patients with uncontrolled hypertension had a higher BMI (29.4 versus 28.6 kg/m2), LDL-cholesterol (3.4 versus 3.0 mmol/L), triglycerides (1.9 versus 1.7 mmol/L), and HbA1c (6.8 versus 6.7%) than those with controlled blood pressure (P<0.0001for all parameters).Conclusions. Among outpatients treated for arterial hypertension, three quarters had uncontrolled blood pressure. Elevated SBP/DBP and uncontrolled hypertension were associated with increasing BMI, LDL-cholesterol, triglycerides, and HbA1c, both globally and regionally.
