We develop a new age-period-cohort model for cancer surveillance research; the theory and methods are broadly applicable. In the new model, cohort deviations are weighted to account for the variable number of periods that each cohort is observed. Weighting ensures that the fitted rates can be naturally expressed as a function of age × a function of period × a function of cohort. Furthermore, the age, period, and cohort deviations are split into orthogonal quadratic components plus higher-order terms. These decompositions enable powerful combination significance tests of first- and second-order age, period, and cohort effects. The regression parameters of the orthogonal quadratic polynomials (global curvatures) quantify how fast on average the trends in the rates are changing. Importantly, the global curvature for cohort determines the least squares slope of the expected annual percentage changes by age group versus age (local drifts), thereby providing a powerful one-degree-of-freedom test of age-period interactions. We introduce new estimable functions, including age gradients that quantify the rate of change of the longitudinal and cross-sectional age curves at each attained age, and gradient shifts that quantify how the cross-sectional age trend varies by period. We illustrate the new model using nationally representative multiple myeloma incidence. Comprehensive proofs are given in technical appendices. We provide an R package.

Objective: This study examines trends in the mortality of Alzheimer’s disease and other dementias in China from 1990 to 2019. Methods: The data were drawn from the Global Burden of Disease Study 2019 (GBD 2019), and an age–period–cohort model was used for analysis. Results: The net drift was .152% (95% confidence interval [CI]: .069%, .235%) per year for men ( P < .05) and .024% (95% CI: −.078%, .126%) per year for women. The local drift values were below 0 in both genders for people aged 45–54 years ( P < .05), and above 0 for males aged 60–94 years and females aged 60–79 years ( P < .05). In the same birth cohort, the risk of mortality of Alzheimer's disease and other dementias exponentially increases with age for both genders. Conclusion: More rapid and effective efforts are needed to mitigate the substantial impact of Alzheimer's and other dementias on the health of China’s elderly.

Written reflections of 113 occupational therapy clinical doctoral students who graduated from an online program between 2007 and 2013 were analyzed for themes which reflected transformative learning and characteristics of curricular design which promoted transformative learning. Qualitative analyses of written reflections were performed. Several themes emerged which are presented using the framework of Person/Learner, Environment/Learning Context, and Occupation/Engagement in Learning Activities. Strategies such as active learning; assignments that directly apply to students’ work settings; implementation of a cohort model; and use of reflection, dialog, and project implementation appear to be effective in facilitating transformative learning in an online clinical doctoral program.

AbstractAlthough many studies have described society-wide changes in adult women's care-giving for ageing family members, few have examined how this informal eldercare has changed across care-givers’ lifecourse and the temporal changes across different time periods and birth cohorts. Using a hierarchical age–period–cohort model to disentangle the confounding effects of age, period and cohort, this study examines the lifecourse and temporal changes in women's informal eldercare in China that can be attributed to the processes of ageing and social transformation. It advances understanding of the extent to which urbanisation and education have affected women's care-giving over time. The results reveal positive age, period and cohort effects which, together, indicate that the tradition of xiao (filial piety) is in a stable state or even resurgent. Higher education and urban residency are both positively related to women's care-giving behaviour. As women age, the gap between residency differences in eldercare decreases. This study supports the argument that xiao culture is resilient and resistant to change, and that education can function as an enabler to promote it.

This chapter discusses the successes, strengths, and lessons learned during a five-year international Ed.D. program, which took place from 2007 to 2012 in Belize through a partnership with the Consortium for Belize Educational Cooperation. The objectives of the chapter are to (1) provide a brief history and explanation of the program including an overview of the Belize educational system, (2) explain how the program filled a need for both our institution and the country of Belize, (3) discuss the strengths and lessons learned in this cohort model for international educators, (4) offer a framework for other educational leadership preparation programs that might attempt international cohort-model doctoral programs, and (5) suggest implications for improving domestic practices through faculty and student participation in an international doctoral program.
