Objective: Sexual intimacy is a basic right given to all humans. However, many PWD give up on their sex life, and equally, medical professionals are not effectively prepared to deal with these issues. Design and Method: In 1996, we began a Sexual Rehabilitation Program for PWDs. We delivered about 28,000 cases of sexual counseling and education, and 1,700 cases of sex therapy for erectile dysfunction. 380 couples used the ‘sex practice room’ in our hospital. We have conducted 21 research studies, produced 3 books and 1 DVD about sexual rehabilitation. Annual seminars are being held and as well as three day workshops for couples. Results: We studied sexual function and satisfaction of 214 spinal cord injured males and 49 female partners. The significant factors affecting sexual satisfaction of the patients were erectile function and sexual satisfaction before injury. Based on the results, we developed several programs, such as the 4 week small counseling program and the ‘sex practice room’ program. 380 couples with SCI have used the sex practice room. Among them, 71 spinal cord injured males and partners participated. 41 couples had successful penilevaginal intercourse and 91.3% of them used the woman-on-top position. 90% of the users reported that the experience of using the ‘sex practice room’ was helpful. Conclusions: Since 1996, we have developed many sexual rehabilitation programs and conducted counseling, sex therapy and research. Now we can confidently conclude that the hospital based rehabilitation program is very useful and essential for persons with disabilities.

Introduction and objectives: Erectile dysfunction (ED) is a common complication after radical prostatectomy that affects quality of life. There are several therapeutic options, including intracavernous alprostadil injections (IAI). However, no specific recommendations have been made on the optimal rehabilitation strategy. In this study we evaluated a sexual rehabilitation program (SRP) with IAI for patients with ED after radical prostatectomy, assessing the rate of compliance and reasons for dropout. Methods: The sexual rehabilitation program (SRP) was offered to all patients who underwent radical prostatectomy from 1 January 2010 to 31 December 2019. The first consultations were performed by a urology specialist nurse, explaining the IAI procedure and possible complications. The program was considered successful when the patients achieved autonomy in the drug preparation with a good injection technique. A medical consultation was performed at 6 months evaluating the IAI usage and adverse events. In case of dropout, a questionnaire about reasons for dropout was performed. The primary endpoint was the rate of compliance and dropout of the program. Secondary endpoints were the reasons for dropout and adverse events. Results: 340 patients underwent radical prostatectomy at our institution, and 123 patients accepted to participate in the rehabilitation program. A total of 96 patients (78%) successfully completed the SRP, and at 6 months 60 (62.5%) still used IAI. Concerning the reasons for dropping out, the most frequent were the need of injectable therapy and pain. Regarding complications, 17 patients (13.8%) reported pain related to the injection and 1 patient (0.8%) had a priapism, managed with conservative treatment. Conclusions: Management of post-radical prostatectomy ED by a nursing program achieved good rates of patients’ self-injection accomplishment and treatment compliance. Close monitoring for dose adjustment and management of post-injection penile pain is required during the follow-up.

Abstract Background According to the ICF Model, the central goal of rehabilitation is the returning of persons with disabilities to an active and fruitful life within society. The Israel Ministry of Defense Rehabilitation Department’s occupational rehabilitation program includes assessment, professional guidance, training, and assistance integrating into employment. Aim Examining predictors for the integration of Israel Defense Force veterans with disabilities into the workforce. Methods All 1416 male veterans with disabilities who served in the military, were injured during their service, and were treated by the Israel Ministry of Defense Rehabilitation Department between 2001 and 2006 were included in this study. Data collection was based on computerized administrative data in the Ministry of Defense. The data was collected with no identifying details. Predictive variables were: health condition, personal factors, participation factors, level of functioning and structure, and variables relating to the local environment. Results Compared to a physical disability, mental and mixed (physical and mental) disabilities reduce the odds for integration into the workforce. Higher education at the time of the injury is an additional predictor for integration into the workforce. Participation in a rehabilitation track for further education to acquire a profession is one of the main predictors for integration into the workforce. Conclusion The results may enable developing more accurate intervention plans, with more efficient use of resources, including consolidation of existing information systems and monitoring the processes and outcomes of rehabilitation.

Classification of sexual assault varies based on the characteristics of the victim, perpetrator, and event. However, most studies focus on the individual characteristics of participants asked to classify the assault, the victims’ characteristics, or the event; few have examined variations in the perpetrator. Therefore, this study examined whether the occupation of the perpetrator affected the classification of the event as sexual assault. Participants included a primarily White female sample of undergraduate students ( n = 401) at a south-Atlantic university. They completed a paper-and-pencil survey containing an ambiguous sexual assault encounter where the occupation of the perpetrator (athlete, reporter, college student, or politician) was randomly varied. Participants were asked to classify whether the encounter was sexual assault and what an appropriate punishment might be. Additional measures included the Illinois Rape Myth Acceptance Scale (IRMA) and questions about personal experience with sexual assault. Overall, most participants labeled the encounter as sexual assault, indicating that college students are aware of the legal parameters for providing consent. However, this classification occurred differentially based on the occupation of the perpetrator. Participants were most likely to label the athlete as committing sexual assault, followed by the college student, politician, and reporter, respectively. Females and those who reject rape myths were more likely to label the scenario as sexual assault. A “personal apology” and a “sexual rehabilitation program” were the most common punishments selected for the perpetrator. These findings highlight potential concerns and the need for additional training when college students adjudicate sexual assault reports and determine appropriate consequences.

Persons with disabilities are at significant risk for additional or secondary disabilities. Because of this increased risk, especially for persons suffering from the “disuse syndrome,” it is important that a comprehensive health promotion component is included in the rehabilitation program for disabled individuals. Current concepts of disability are discussed as well as the distinctions between primary and secondary disabilities. Specific types and causes of disabilities are identified and described. Each type of secondary disability is assessed in terms of its “preventability.” Examples and characteristics of quality programs for health promotion for disabled persons are presented. Recommendations for further research and study are discussed to include the need for determining effective components of successful programs and ways to encourage the field of rehabilitation to incorporate health promotion into the rehabilitation process for its disabled clients.

Reporting on results from the University at Buffalo Consumer Assessments Study, this paper examines environmental problems in the homes of elderly persons with disabilities. For the study, 127 persons over the age of 60 were interviewed; all had received, or were currently receiving, services from a senior services agency or hospital rehabilitation program. An average of almost four problems per home were identified. Problems are grouped into categories by room and by area such as ‘lighting’ and ‘steps’ and described in more detail. Since all subjects had been referred to the project through an agency or hospital providing services, the results suggest that the home environment needs of elders are not adequately addressed by these service providers. An environmental interventions model, developed following an analysis of study results, is presented to guide service providers in setting priorities for recommending home modifications. Occupational therapists are qualified to provide home assessments, and must play a greater role in ensuring that these assessments and interventions are provided for non-institutionalized older persons with disabilities.
