AbstractBackgroundBody mass index (BMI) and waist circumference (WC) are commonly used markers of cardiometabolic risk. However, sagittal abdominal diameter (SAD) has been proposed to be a better marker of intra-abdominal obesity compared to WC and might better associate with metabolic disturbances in high-risk populations. The objective of this study was to compare SAD, WC, and BMI as determinants of an adverse metabolic phenotype.MethodAnthropometric and metabolic measures of 1516 overweight or obese individuals with features of the metabolic syndrome were included to examine differences between SAD, WC and BMI as measures of an adverse metabolic phenotype. Multiple linear regression and logistic regression models were used to investigate the association between SAD, WC, and BMI and markers of metabolic syndrome, insulin resistance, blood lipids, and low grade inflammation.ResultsBoth SAD and WC correlated with BMI, but as BMI increased, SAD proportionately estimated higher abdominal adiposity compared to WC (slope = 0.0037 (0.0029; 0.0046), p<0.0001). We did not find major differences between SAD, WC and BMI in explained variance in models with the different markers of metabolic risk. Furthermore, we did not find differences between SAD and WC in the ability to identify individuals with metabolic syndrome according to the International Diabetes Federation (IDF) cut-offs, but a few differences from BMI were indicated but mostly before adjustments. Moreover, the differences between SAD and WC associations were not modified by sex or degree of adiposity, but identification of individuals with a metabolic phenotype was generally better in women.ConclusionThese data indicate that SAD and WC are equally good indicators of an adverse metabolic phenotype. Thus, from a public health perspective choice of anthropometric measure may depend only on what is the most practical method in a given situation.

Background:The influence of higher physical activity on the relationship between adiposity and cardiometabolic risk is not completely understood.Methods:Between 2000–2002, data were collected on 6795 Multi-Ethnic Study of Atherosclerosis (MESA) participants. Self-reported intentional physical activity in the lowest quartile (0–105 MET-minutes/week) was categorized as inactive and the upper three quartiles (123–37,260 MET-minutes/week) as active. Associations of body mass index (BMI) and waist circumference categories, stratified by physical activity status (inactive or active) with cardiometabolic risk factors (dyslipidemia, hypertension, upper quartile of homeostasis model assessment of insulin resistance [HOMA-IR] for population, and impaired fasting glucose or diabetes) were assessed using logistic regression analysis adjusting for age, gender, race/ethnicity, and current smoking.Results:Among obese participants, those who were physically active had reduced odds of insulin resistance (47% lower; P < .001) and impaired fasting glucose/diabetes (23% lower; P = .04). These associations were weaker for central obesity. However, among participants with a normal waist circumference, those who were inactive were 63% more likely to have insulin resistance (OR [95% CI] 1.63 [1.24–2.15]) compared with the active reference group.Conclusions:Physical activity was inversely related to the cardiometabolic risk associated with obesity and central obesity.

Context: There has been an increasing interest in epidemiological and clinical studies concerning the role of uric acid in cardiometabolic diseases, especially in children and adolescents. However, these potential relationships remain undiscovered; accordingly, its pathophysiological mechanisms remain unrecognized. This study aimed to assess the potential association between Serum Uric Acid (SUA) levels and cardiometabolic risk factors in a population-based sample of Iranian children and adolescents. Objectives: This study aimed to assess the potential association between Serum Uric Acid (SUA) levels and cardiometabolic risk factors in a population-based sample of Iranian children and adolescents. Methods: The data of 595 individuals aged 7-18 years were assessed in this research. Anthropometric measurements and laboratory tests were performed according to standardized protocols. Results: The Mean±SD age of the 595 explored students was 12.39±3.07 years. The overall Mean±SD SUA level of the study participants was measured as 4.22±1.13 mg/dL, with significant gender-wise differences (4.04±0.97 mg/dL vs 4.38±1.24 mg/dL, respectively; P<0.05). The prevalence of hyperuricemia based on the 90th percentile of SUA levels was equal to 10.6%. There was a positive association between SUA levels and abdominal obesity (waist circumference: ≥90th percentile) [Odds Ratio (OR): 1.54; 95% Confidence Interval (CI): 1.26 to 1.86] and general obesity [gender-specific Body Mass Index (BMI) for >95th percentile] (OR: 2.32; 95% CI: 1.74 to 3.11). Conclusions: This study suggested BMI and waist circumference as cardiometabolic risk factors, i.e. significantly associated with SUA levels in children and adolescents.

Background: Cardiovascular disease disproportionately affects African Americans as the leading cause of morbidity and mortality. Among African Americans, compared to other racial groups, cardiovascular disease onset occurs at an earlier age due to a higher prevalence of cardiometabolic risk factors, particularly obesity, hypertension and type 2 diabetes. Emerging evidence suggests that heritable epigenetic processes are related to increased cardiovascular disease risk, but this is largely unexplored in adolescents or across generations. Materials and Methods: In a cross-sectional descriptive pilot study in low-income African American mother-adolescent dyads, we examined associations between DNA methylation and the cardiometabolic indicators of body mass index, waist circumference, and insulin resistance. Results: Four adjacent cytosine and guanine nucleotides (CpG) sites were significantly differentially methylated and associated with C-reactive protein (CRP), 62 with waist circumference, and none to insulin resistance in models for both mothers and adolescents. Conclusion: Further study of the relations among psychological and environmental stressors, indicators of cardiovascular disease, risk, and epigenetic factors will improve understanding of cardiovascular disease risk so that preventive measures can be instituted earlier and more effectively. To our knowledge this work is the first to examine DNA methylation and cardiometabolic risk outcomes in mother-adolescent dyads.

Background: Little is known about hepatic steatosis (HS) incidence and its relationship to underlying or new-onset cardiometabolic risk. This study aims to assess the incidence of hepatic steatosis in an asymptomatic population and to determine its relationship to both prevalent and new-onset cardiometabolic risk factors. Methods: We analyzed retrospective data from a voluntary employer-sponsored routine health promotion evaluation at the Preventive Medicine Center of the Hospital Israelita Albert Einstein (São Paulo, Brazil) collected from October 2004 to December 2016.Medical and demographic history, anthropometric measures including blood pressure, body mass index (BMI) and waist circumference, and fasting blood samples were obtained. Participants also had ultrasonography to assess for HS. We included data from 8,448 individuals who had complete cardiometabolic and ultrasound data at baseline and repeated all tests at least 6 months later. Results: The mean age (standard deviation, SD) of participants was 40 (9) years. Over a mean (SD) follow-up of 3.4 (2.3) years, the incidence of HS was 14.7%. As shown in the table, diabetes, poor physical activity, elevated waist circumference and cigarette smoking at baseline were independently associated with hepatic steatosis. There was an additive effect of the increasing cardiometabolic risk factors (see graph) on the risk of developing HS. Participants with new-onset cardiometabolic risk factors also had a higher risk of incident HS after accounting for baseline demographics and cardiometabolic risk factors (see forest plot). This was most pronounced for incident obesity (BMI ≥ 30 Kg/m 2 ). Conclusion: In this relatively young population, the incidence of HS was high and was both independently and collectively associated with baseline cardiometabolic risk. New-onset cardiometabolic risk tracks with incident HS. This study emphasizes the need for assessing and mitigating cardiometabolic risk in the prevention of HS.
