Is hyperemesis gravidarum associated with placental weight and the placental weight-to-birth weight ratio? A population-based Norwegian cohort study

Abstract Background Racial disparities in adverse perinatal outcomes have been studied in other countries, but little has been done for the Canadian population. In this study, we sought to examine the disparities in adverse perinatal outcomes between Asians and Caucasians in Ontario, Canada. Methods We conducted a population-based retrospective cohort study that included all Asian and Caucasian women who attended a prenatal screening and resulted in a singleton birth in an Ontario hospital (April 1st, 2015-March 31st, 2017). Generalized estimating equation models were used to estimate the independent adjusted relative risks and adjusted risk difference of adverse perinatal outcomes for Asians compared with Caucasians. Results Among 237,293 eligible women, 31% were Asian and 69% were Caucasian. Asians were at an increased risk of gestational diabetes mellitus, placental previa, early preterm birth (< 32 weeks), preterm birth, emergency cesarean section, 3rd and 4th degree perineal tears, low birth weight (< 2500 g, < 1500 g), small-for-gestational-age (<10th percentile, <3rd percentile), neonatal intensive care unit admission, and hyperbilirubinemia requiring treatment, but had lower risks of preeclampsia, macrosomia (birth weight > 4000 g), large-for-gestational-age neonates, 5-min Apgar score < 7, and arterial cord pH ≤7.1, as compared with Caucasians. No difference in risk of elective cesarean section was observed between Asians and Caucasians. Conclusion There are significant differences in several adverse perinatal outcomes between Asians and Caucasians. These differences should be taken into consideration for clinical practices due to the large Asian population in Canada.

Abstract Purpose Creatinine to body weight (Cre/BW) ratio is considered the independent risk factor for incident type 2 diabetes mellitus (T2DM), but research on this relationship is limited. The relationship between the Cre/BW ratio and T2DM among Chinse individuals is still ambiguous. This study aimed to evaluate the correlation between the Cre/BW ratio and the risk of T2DM in the Chinese population. Methods This is a retrospective cohort study from a prospectively collected database. We included a total of 200,658 adults free of T2DM at baseline. The risk of incident T2DM according to Cre/BW ratio was estimated using multivariable Cox proportional hazards models, and a two-piece wise linear regression model was developed to find out the threshold effect. Results With a median follow-up of 3.13 ± 0.94 years, a total of 4001 (1.99%) participants developed T2DM. Overall, there was an L-shaped relation of Cre/BW ratio with the risk of incident T2DM (P for non-linearity < 0.001). When the Cre/BW ratio (× 100) was less than 0.86, the risk of T2DM decreased significantly as the Cre/BW ratio increased [0.01 (0.00, 0.10), P < 0.001]. When the Cre/BW ratio (× 100) was between 0.86 and 1.36, the reduction in the risk of developing T2DM was not as significant as before [0.22 (0.12, 0.38), P < 0.001]. In contrast, when the Cre/BW ratio (× 100) was greater than 1.36, the reduction in T2DM incidence became significantly flatter than before [0.73 (0.29,1.8), P = 0.49]. Conclusion There was an L-shaped relation of Cre/BW ratio with incidence of T2DM in general Chinese adults. A negative curvilinear association between Cre/BW ratio and incident T2DM was present, with a saturation effect predicted at 0.86 and 1.36 of Cre/BW ratio (× 100).

Objective: To determine the mean placental birth weight ratio at term in primigravidae Study design: Cross-sectional study Place and Duration: Department of Obstetrics and Gynecology, Civil Hospital Karachi, duration was six months after the approval of synopsis from 1st January 2016 to 30th June 2016 Subjects and Methods: A total of pregnant women who fulfill the inclusion criteria were included in this study. After delivery, baby was weighed by using weight machine and weight of baby was also noted (as per operational definition). After expulsion of complete placenta, placental weight was measured by using weight machine. The placental-birth weight ratio (PBWR) were calculated as ratio of placental weight to neonatal weight multiplied by 100. Results: Mean ± SD of maternal age was 24.77±4.04 with C.I (24.11----25.42) years. Mean ± SD of placental weight was 505.84±99.97 with C.I (489.71----521.97) grams. Out of 150 neonatal babies 101 (67.3%) were male and 49 (32.7%) were female. Mean placental birth weight ratio was found to be 16.82±2.63 with C.I (16.39----17.24). Conclusion: It is to be concluded that placental weight increased according to the birth weight. The placental weight to birth weight ratio decreased slightly with advancing gestational age. Abstract BackgroundTo estimate Population Attributable Fractions (PAFs) and Generalized Impact Fractions (GIFs) for LBW following scenarios to remove or decrease prenatal use of caffeine or water pipe.Methods‎Using data of 861 pregnant women from a population-based prospective cohort study ‎in suburbs of Bandar Abbas city (2016-2018), PAFs and GIFs were calculated based on the relative risk scale. Practical interventional scenarios to reduce the exposure prevalence were developed for calculation of GIFs. ResultsThe cumulative incidence of LBW was 16.1%. An estimated 19% (95%CI: 6, 30%) of LBW neonates was attributed to dietary caffeine intake of >100 mg/day and 11% (95%CI: 8,14%) to water pipe smoking. Action plans to reduce caffeine intake and water pipe smoking suggested an avoidable burden of LBW cases of approximately 10.7% (95% CI: 6.6, 25.3%) and 5.7% ‎‎(95%CI: ‎5.0, 6.8%), respectively.‎ConclusionsWater pipe smoking and excessive consumption of caffeine during pregnancy decreased birth weight. Practical action plans to control water pipe smoking ‎and to prevent excessive intake of ‎caffeine ‎among pregnant women would substantially reduce LBW burden in the south of Iran. ‎
