Protecting worker and public health involves an understanding of multiple determinants, including exposures to biological, chemical, or physical agents or stressors in combination with other determinants including type of employment, health status, and individual behaviors. This has been illustrated during the COVID-19 pandemic by increased exposure and health risks for essential workers and those with pre-existing conditions, and mask-wearing behavior. Health risk assessment practices for environmental and occupational health typically do not incorporate multiple stressors in combination with personal risk factors. While conceptual developments in cumulative risk assessment to inform a more holistic approach to these real-life conditions have progressed, gaps remain, and practical methods and applications are rare. This scoping review characterizes existing evidence of combined stressor exposures and personal factors and risk to foster methods for occupational cumulative risk assessment. The review found examples from many workplaces, such as manufacturing, offices, and health care; exposures to chemical, physical, and psychosocial stressors combined with modifiable and unmodifiable determinants of health; and outcomes including respiratory function and disease, cancers, cardio-metabolic diseases, and hearing loss, as well as increased fertility, menstrual dysfunction and worsened mental health. To protect workers, workplace exposures and modifiable and unmodifiable characteristics should be considered in risk assessment and management. Data on combination exposures can improve assessments and risk estimates and inform protective exposure limits and management strategies.

Components of the metabolic syndrome (i.e., hypertension, insulin resistance, obesity, atherosclerosis) are a leading cause of death in the United States and result in low-grade chronic inflammation, excessive oxidative stress, and the eventual development of cardiometabolic diseases (CMD). High-stress occupations (HSO: firefighters, police, military personnel, first responders, etc.) increase the risk of developing CMD because they expose individuals to chronic and multiple stressors (i.e., sleep deprivation, poor nutrition habits, lack of physical activity, psychological stress). Interestingly, heat exposure and, more specifically, sauna bathing have been shown to improve multiple markers of CMD, potentially acting as hormetic stressors, at the cellular level and in the whole organism. Therefore, sauna bathing might be a practical and alternative intervention for disease prevention for individuals with HSO. The purpose of this review is to detail the mechanisms and pathways involved in the response to both acute and chronic sauna bathing and collectively present sauna bathing as a potential treatment, in addition to current standard of care, for mitigating CMD to both clinicians and individuals serving in HSO.

AbstractThe resistance of an east border upwelling system was investigated using relative index of marine pelagic biomass estimates under a changing environment spanning 20-years in the strongly exploited southern Canary Current Large marine Ecosystem (sCCLME). We divided the sCCLME in two parts (north and south of Cap Blanc), based on oceanographic regimes. We delineated two size-based groups (“plankton” and “pelagic fish”) corresponding to lower and higher trophic levels, respectively. Over the 20-year period, all spatial remote sensing environmental variables increased significantly, except in the area south of Cap Blanc where sea surface Chlorophyll-a concentrations declined and the upwelling favorable wind was stable. Relative index of marine pelagic abundance was higher in the south area compared to the north area of Cap Blanc. No significant latitudinal shift to the mass center was detected, regardless of trophic level. Relative pelagic abundance did not change, suggesting sCCLME pelagic organisms were able to adapt to changing environmental conditions. Despite strong annual variability and the presence of major stressors (overfishing, climate change), the marine pelagic ressources, mainly fish and plankton remained relatively stable over the two decades, advancing our understanding on the resistance of this east border upwelling system.
