Sleep disordered breathing (SDB) is a common, yet under-recognized and undertreated condition in pregnancy. Sleep disordered breathing is associated with pregnancy complications including preeclampsia, gestational diabetes, preterm birth, as well as severe maternal morbidity and mortality. The identification of risk factors for SDB in pregnancy may improve screening, diagnosis, and treatment of SDB prior to the onset of pregnancy complications. The goal of this study was to determine whether fetal sex increases risk of SDB in pregnancy. A cohort of singleton (N = 991) pregnant women were recruited within 24 to 48 hours of delivery and answered questions regarding SDB symptoms by questionnaire. Women who reported frequent loud snoring at least 3 times a week were considered to have SDB. Hospital records were reviewed to extract information on fetal sex and pregnancy complications including preeclampsia, pregnancy-induced hypertension, gestational diabetes, preterm delivery, and low birth weight. Women carrying male fetuses were significantly more likely to have SDB (β = .37, P = .01, OR: 1.45 [95% CI: 1.09-1.94]). Fetal sex was associated with increased risk of hypertensive disorders of pregnancy (defined as preeclampsia and/or pregnancy-induced hypertension) among women with SDB in pregnancy (β = .41, P = .02, OR: 1.51 [95% CI: 1.08-2.11]). Fetal sex did not increase risk of preterm birth, low birth weight, or gestational diabetes among women with SDB in pregnancy. Women carrying male fetuses were approximately 1.5 times more likely to report SDB in pregnancy compared to women carrying female fetuses, and women with pregnancy-onset SDB carrying male fetuses were 1.5 times more likely to have hypertensive disorders of pregnancy compared to women with SDB carrying female fetuses. Confirmation of fetal sex as a risk factor may, with other risk factors, play a role in identifying women at highest risk of SDB complications in pregnancy.

Background: To assess the changes occurring in the placenta due to anemia, pregnancy induced hypertension (PIH), Gestational Diabetes Mellitus (GDM) and whether these changes were reversible with treatment when they were diagnosed in early stages. Subjects and Methods: The study is conducted in the Department of Obstetrics and Gynecology, DVVPF’s Medical College, Ahmednagar on 200 anemic patients, 180 PIH patients and 650 GDM patients. Placentae were collected, stored in a 10% formalin solution. Morphological changes in respect to shape, weight, diameter, thickness, number of cotyledons and site of insertion of the cord are observed and documented. Results: In anemic patients who has undergone treatment, there were a smaller number of irregular (37/100) and round (63/100) round placentae were seen, whereas in the untreated anemic group, round (09/100) round and irregular (91/100) irregular placentae were seen. In PIH group with treatment, round (57/90) placentae and irregular (33/90) placentae were seen, whereas, in the untreated group, 13/100 round/oval placentae were observed and 77/100 irregular placentae were seen. Conclusion: Morphology of placenta is on error mode in untreated anemia, PIH and GDM. Diagnosis in the first trimester and on consequent treatment till the delivery is resulting in decreasing of irregular morphology of placenta and wellbeing of newborns.

Abstract Both gestational diabetes mellitus(GDM) and pregnancy induced hypertension (PIH) would influence the gestation significantly. However, the causation between these two symptoms remains speculative. 16,404 pregnant women were identified in Harbin, China in this study. We investigated the evaluate the causal effect of GMD on PIH based on the statistic inference theory. The statistical results indicated that GDM might cause PIH. Also, this case study demonstrated that the decrease temperature might also cause hypertension during pregnancy, and the prevalence rate of GDM increased with age. However, the prevalence of diabetes did not show a remarkable difference in varied areas and ages. This study could provide some essential information that will help to investigate the mechanism for GDM and PIH.

The epidemic of obesity is spreading worldwide and subsequently, rate of obesity during pregnancy has also increased. Maternal overweight and obesity are widely associated with adverse pregnancy outcomes. Recurrent miscarriage is an important reproductive health issue, because it affects many couples. So the present study is planned to study the relationship between maternal obesity and pregnancy outcome in women with recurrent miscarriages. Observational Cross sectional study was conducted in a tertiary care hospital. 111 Postnatal women between 18 to 44 years of age with history of two or more miscarriages less than 20 weeks of gestation in previous pregnancy were included in the study. First trimester weight at the first visit (registration) was recorded, BMI was calculated & women were divided into obese and non obese groups. The outcome of present pregnancy was noted as Mode of delivery, Gestational diabetes mellitus, Pregnancy induced hypertension, Preterm delivery etc. Statistical tests were used to quantify the risk. Gestational diabetes (OR= 13.6) and pregnancy induced hypertension (OR=4.2) were significantly associated with obesity in women with recurrent miscarriages. [At 95% CI] The incidence of LSCS and preterm delivery was more in overweight and obese mothers, though not statistically significant. Maternal obesity significantly contributes to poor prognosis for the mother and the baby during delivery. Hence the women of this group should be regarded as ‘high risk’ and counselling and the risk assessment should be done during ANC visits.

Objectives. To evaluate pregnancy outcomes and its determinants in women with polycystic ovary syndrome (PCOS).Methods. Two-hundred and twenty pregnant PCOS and 594 healthy women were followed from early pregnancy. Incidences of gestational diabetes mellitus (GDM), pregnancy-induced hypertension (PIH), preterm birth, twinning, and fetal growth restriction (FGR) were determined.Results. The incidence of GDM was notably higher among all PCOS combined (54.9%; OR: 2.9, 95% CI: 2.0–4.1) and PCOS subgroups, whether they conceived spontaneously (51.5%; OR: 3.3, 95% CI: 2.0–5.4), or via IVF-ET or ovarian stimulation, compared with controls (14.3%;P<0.001). The incidence of PIH was also higher among all PCOS (10.4%; OR: 2.2, 95% CI: 1.1–4.4) and the subgroup conceiving spontaneously (11.8%; OR: 2.6, 95% CI: 1.1–6.2;P<0.001) but not for those conceiving with IVF-ET (9.1%) or ovarian stimulation (9.4%). Lean women with PCOS (BMI<24kg/m2) had higher incidences of GDM (51.1% versus 14.5%; OR: 5.6, 95% CI: 3.4–9.0) and PIH (8.9% versus 3.2%; OR: 3.0, 95% CI: 1.3–7.1) than lean controls. PCOS woemn with normal glucose tolerance had higher risk for PIH than their comparable control group (OR: 4.0, 95% CI: 1.3–11.7).Conclusion. This study suggested that PCOS is an independent risk factor for the development of GDM and PIH. This trial is registered withChiCTR-RCC-11001824.
