Qualitative evaluation of paediatric surgical otolaryngology content on YouTube

AbstractObjectiveThis study evaluated the quality of YouTube content focusing on common paediatric otolaryngology procedures, as this content can influence the opinions and medical decisions of patients.MethodsA total of 120 YouTube videos were compiled to review using the terms ‘adenoid removal’, ‘adenoidectomy’, ‘ear tubes’, ‘tympanostomy’, ‘tonsil removal’ and ‘tonsillectomy’. The Discern criteria was used to rate the quality of health information presented in each video.ResultsThe mean bias Discern score was 3.18 and the mean overall Discern score was 2.39. Videos including US board certified physicians were rated significantly higher (p < 0.001) than videos without (bias Discern score = 3.00 vs 2.38; overall Discern score = 3.79 vs 1.55). The videos had been viewed a total of 176 769 549 times.ConclusionUnbiased, high quality videos on YouTube are lacking. As patients may rely on this information when making medical decisions, it is important that practitioners continually evaluate and improve this video content. Otolaryngologists should be prepared to discuss YouTube content with patients.

Abstract Background Given the high prevalence of dietary and herbal supplement (DHS) use in tandem with the growing ease of internet access, patients commonly search online for consumer health information about these products. One common reason for DHSs use includes weight loss. Healthcare providers need to be aware of the quality of online information about DHSs for weight loss so they can adequately counsel their patients and provide them with guidance surrounding the identification of high-quality information resources. This study aimed to assess the quality of online DHSs consumer health information for weight loss that a “typical” patient might access online. Methods Six search terms were used to generate the first 20 websites on the Google search engine in four countries: Australia, Canada, the United Kingdom, and the United States (n = 480 websites). After applying exclusion criteria, eligible websites were quality assessed using the DISCERN instrument. This tool is comprised of 16 questions, each evaluated on a 5-point scale. The averages and standard deviations for each DISCERN instrument item, in addition to overall summed scores between 15 and 75 were calculated. Results Across 87 eligible websites, the mean summed score was 44.80 (SD = 11.53), while the mean overall DISCERN score of each website was 2.72 (SD = 0.99). In general, websites detailed and achieved their specified aims and described treatment benefits. However, most websites failed to describe the impact of treatment on overall quality of life and the impact of a no treatment option. The highest-scoring websites were largely government or health portal websites, while the lowest-scoring websites were largely commercial in nature. Conclusion High variability in DISCERN instrument scores was found across all websites assessed. Healthcare providers should be aware of the fact that their patients may be accessing misinformation online surrounding the use of DHSs for weight loss. Therefore, it is important for healthcare providers to ensure that they are providing their patients with guidance on how to identify high-quality resources online, in order that safe, effective, and evidence-based decisions are made surrounding the use of DHSs for weight loss.

Purpose: YouTubeTM is one of the most popular social media platforms on the internet, and patients with chronic disease frequently use it to seek treatment options. In this study, we aimed to evaluate the quality of YouTube videos about erectile dysfunction. Materials & methods: The terms "erectile dysfunction treatment'', "erectile dysfunction surgery'', and "cure erectile dysfunction'' were entered into the YouTube search bar. A total of 56 videos were included in the study. Videos' view counts; upload dates; like, dislike, and comment counts; uploader qualifications; length; and content were recorded. Video power index (VPI), Quality Criteria for Consumer Health Information (DISCERN), and Journal of the American Medical Association (JAMA) scores were determined. Results: Thirty-two (57.1%) videos consisted of real images, and 24 (42.9%) contained animated images. Twenty-four (42.9%) videos were uploaded by physicians, and 32 (57.1%) were uploaded by non-physicians. The mean like count of the videos was 5,307 ± 17.618, the mean dislike count was 560.07 ± 1548.07, and the mean comment count was 235 ± 373. The mean VPI value of the videos was calculated as 81.19 ± 21.19, the DISCERN score was 30.5 ± 8.1, and the JAMA score was 1.23 ± 0.55. Overall quality was very poor in 24 (42.9%) of the examined videos, poor in 21 (37.5%), average in 10 (17.9%), and good in one (1.8%). Conclusion: The overall quality of YouTube content on erectile dysfunction was not sufficient to provide reliable information for patients. Physicians should warn patients about the limitations of YouTube and direct them toward more appropriate sources of information.

BACKGROUND With online health information becoming increasingly popular among patients and their family members, concerns have been raised about the accuracy from the websites. OBJECTIVE We aimed to evaluate the overall quality of the online information about scaphoid fracture obtained from Chinese websites using the local search engines. METHODS We conducted an online search using the keyword “scaphoid fracture” from the top 5 search engines in China, i.e. Baidu, Shenma, Haosou, Sougou and Bing, and gathered the top ranked websites, which included a total of 120 websites. Among them, 81 websites were kept for further analyses by removing duplicated and unrelated one as well as websites requiring payment. These websites were classified into four categories, including forum/social networks, commercials, academics and physician’s personals. Health information evaluation tool DISCERN and Scaphoid Fracture Specific Content Score (SFSCS) were used to assess the quality of the websites. RESULTS Among the 81 Chinese websites that we studied, commercial websites were the most common one accounting more than half of all websites. The mean DISCERN score of the 81 websites was 25.56 and no website had a score A (ranging from 64 to 80).The mean SFSCS score was 10.04 and no website had a score A (range between 24 and 30). In addition, DISCERN and SFSCS scores from academic and physician’s websites were significantly higher than those from the forum/social networks and commercials. CONCLUSIONS The overall quality of health information obtained from Chinese websites about scaphoid fracture was very low, suggesting that patients and their family members should be aware such deficiency and pay special attentions for the medical information obtained by using the current search engines in China.

BACKGROUND Serious public health problems, such as the COVID-19 pandemic, can cause an infodemic. Sources of information that may cause an infodemic include social networking services; YouTube, which consists of content created and uploaded by individuals, is one such source. OBJECTIVE To survey the content and changes in YouTube videos that present public health information about COVID-19 in Japan. METHODS We surveyed YouTube content regarding public health information pertaining to COVID-19 in Japan. YouTube searches were performed on March 6, 2020 (before the state of emergency), April 14 (during the state of emergency), and May 27 (after the state of emergency was lifted), with 136, 113, and 140 sample videos evaluated, respectively. The main outcome measures were: (1) The total number of views for each video, (2) video content, and (3) the usefulness of the video. RESULTS In the 100 most viewed YouTube videos during the three periods, the number of videos on public health information in March was significantly higher than in May (p = .02). Of the 331 unique videos, 9.1% (n = 30) were released by healthcare professionals. Useful videos providing public health information about the prevention of the spread of infection comprised only 13.0% of the sample but were viewed significantly more often than not useful videos (p = .006). CONCLUSIONS Individuals need to take care when obtaining information from YouTube before or early in a pandemic, during which time scientific evidence is scarce.

ABSTRACT We assessed the quality of online health and nutrition information using a Google™ search on “supplements for cancer”. Search results were scored using the Health Information Quality Index (HIQI), a quality-rating tool consisting of 12 objective criteria related to website domain, lack of commercial aspects, and authoritative nature of the health and nutrition information provided. Possible scores ranged from 0 (lowest) to 12 (“perfect” or highest quality). After eliminating irrelevant results, the remaining 160 search results had median and mean scores of 8. One-quarter of the results were of high quality (score of 10–12). There was no correlation between high-quality scores and early appearance in the sequence of search results, where results are presumably more visible. Also, 496 advertisements, over twice the number of search results, appeared. We conclude that the Google™ search engine may have shortcomings when used to obtain information on dietary supplements and cancer.

Objectives: The Internet, especially YouTube, is an important and growing source of medical information. The content of this information is poorly evaluated. The objective of this study was to analyze the quality of YouTube video content on meniscus repair. The hypothesis was that this source of information is not relevant for patients. Methods: A YouTube search was carried out using the keywords "meniscus repair". Videos had to have had more than 10,000 views to be included. The videos were analyzed by two evaluators. Various features of the videos were recorded (number of views, date of publication, "likes", "don’t likes", number of comments, source, type of content and the origin of the video). The quality of the video content was analyzed by two validated information system scores: the JAMA benchmark score (0 to 4) and the Modified DISCERN score (0 to 5). A specific meniscus repair score (MRSS scored out of 22) was developed for this study, in the same way that a specific score has been developed for other similar studies (anterior cruciate ligament, spine, etc.). Results: Forty-four (44) videos were included in the study. The average number of views per video was 180,100 (± 222,000) for a total number of views of 7,924,095. The majority of the videos were from North America (90.9%). In most cases, the source (uploader) that published the video was a doctor (59.1%). A manufacturer, an institution and a non-medical source were the other sources. The content actually contained information on meniscus repair in only 50% of the cases. The mean scores for the JAMA benchmark, MD score and MRSS were 1.6/4± 0.75, 1.2/5 ± 1.02 and 4.5/22 (± 4.01) respectively. No correlation was found between the number of views and the quality of the videos. The quality of videos from medical sources was not superior to those from other sources. Conclusion: The content of YouTube videos on meniscus repair is of very low quality. Physicians should inform patients and, more importantly, contribute to the improvement of these contents.

BACKGROUND:The aim of this study was to evaluate the information quality of YouTube videos on hallux valgus. METHODS:A YouTube search was performed using the keyword 'hallux valgus' to determine the first 300 videos related to hallux valgus. A total of 54 videos met our inclusion criteria and evaluated for information quality by using DISCERN, Journal of the American Medical Association (JAMA) and hallux valgus information assessment (HAVIA) scores. Number of views, time since the upload date, view rate, number of comments, number of likes, number of dislikes, video power index (VPI) values were calculated to determine video popularity. Video length (sec), video source and video content were also noted. The relation between information quality and these factors were statistically evaluated. RESULTS:The mean DISCERN score was 30.35{plus minus}11.56 (poor quality) (14-64), the mean JAMA score was 2.28{plus minus}0.96 (1-4), and the mean HAVIA score was 3.63{plus minus}2.42 (moderate quality) (0.5-8.5). Although videos uploaded by physicians had higher mean DISCERN, JAMA, and HAVIA scores than videos uploaded by non-physicians, the difference was not statistically significant. Additionally, view rates and VPI values were higher for videos uploaded by health channels, but the difference did not reach statistical significance. A statistically significant positive correlation was found between video length and DISCERN (r= 0.294, p= 0.028), and HAVIA scores (r= 0.326, p= 0.015). CONCLUSIONS:This present study demonstrated that the quality of information available on YouTube videos about hallux valgus was low and insufficient. Videos containing accurate information from reliable sources are needed to educate patients on hallux valgus, especially in less frequently mentioned topics such as postoperative complications and healing period.

BACKGROUND Recent emergency authorisation and rollout of COVID-19 vaccines by regulatory bodies has generated global attention. As the most popular video-sharing platform globally, YouTube is a potent medium for dissemination of key public health information. Understanding the nature of available content regarding COVID-19 vaccination on this widely used platform is of substantial public health interest. OBJECTIVE To evaluate the reliability and quality of information of YouTube videos regarding COVID-19 vaccination. METHODS For this cross-sectional study, the phrases ‘coronavirus vaccine’ and ‘COVID-19 vaccine’ were searched on the UK version of YouTube on December 10, 2020. The 200 most-viewed videos of each search were extracted and screened for relevance and English language. Video content and characteristics were extracted and independently rated against Health on the Net Foundation Code of Conduct (HONCode) and DISCERN quality criteria for consumer health information by two authors. RESULTS Forty-eight videos, with a combined total view count of 30,100,561, were included in the analysis. Topics addressed comprised: vaccine science (58%), vaccine trials (58%), side effects (48%), efficacy (35%) and manufacturing (17%). Twenty-one percent of videos encouraged continued public health measures. Only 4.2% of videos made non-factual claims. Ninety-eight percent of video content was scored to have low (60%) or medium (38%) adherence to HONCode principles. Educational channels produced by both medical and non-medical professionals achieved significantly higher DISCERN scores than other categories. The highest DISCERN scores were achieved by educational videos produced by medical professionals (64.3 (58.5-66.3)) and the lowest scores by independent users (18 (18-20)). CONCLUSIONS Overall quality and reliability of information on YouTube regarding COVID-19 vaccines remains poor. Videos produced by educational channels, especially by medical professionals, were higher in quality and reliability than those produced by other sources, including health-related organisations. Collaboration between health-related organisations and established medical and educational YouTube content producers provide an opportunity for dissemination of high-quality information regarding COVID-19 vaccination. Such collaboration holds potential as a rapidly implementable public health intervention aiming to engage a wide audience and increase public awareness and knowledge.

Abstract BackgroundManagement of primary aldosteronism (PA) has become a research hotspot in the field of endocrinology. To obtain reliable research evidence, it is necessary to establish a high-quality PA research database. MethodsThe establishment of PA research database involved two steps. Firstly, patients with confirmation of PA diagnosis between 1 Jan 2009 to 31 Aug 2019 at West China Hospital were identified and data were extracted. Secondly, patients with confirmatory testing for PA will be enrolled into a prospective cohort. Data will be prospectively collected based on the case report forms since 1 Sep 2019. We evaluated the quality of research database through assessment of quality of key variables.ResultsTotally, 862 patients diagnosed as PA were identified, of which 507 patients who had positive confirmatory testing for PA were included into the retrospective database. Among 862 patients diagnosed as PA, the mean systolic blood pressure (SBP) was 156.1 mmHg, mean diastolic blood pressure (DBP) was 97.2 mmHg. Among included patients, the mean serum potassium level was 2.85 mmol/L, and the mean plasma aldosterone concentration (PAC) was 28.1 ng/dL. The characteristics patients with positive confirmatory testing for PA were similar. Validation of data extracting and linking showed the accuracy were 100%. Evaluation of missing data showed that the completeness of BMI (95.9%), SBP (99.4%) and DBP (99.4%) were high. ConclusionThrough integrating retrospective and prospective cohort of PA, a research database of PA with high quality and comprehensive data can be established. We anticipate that the research database will provide a high level of feasibility for management of PA in China.

Recent statistics show that the World Wide Web has now grown to over 100 million sites: a phenomenal expansion in only 15 years (Mulligan 2007). It has been estimated that there are 100,000 sites offering health related information (Wilson 2002). As the amount of health information increases, the public find it increasingly difficult to decide what to accept and what to reject (Burgess 2007). Searching for information on the internet is both deceptively easy and the same time frustratingly difficult (Kiley 2002). The challenge for consumers is to find high quality, relevant information as quickly as possible. There has been ongoing debate about the quality of information aimed at patients and the general public and opinions differ on how it can be improved (Stepperd 1999). The purpose of this chapter is to provide a brief overview of the different perspectives on information quality and to review the main criteria for assessing the quality of health information on the internet. Pointers are provided to enable both clinicians and patients find high quality information sources. An understanding of these issues should help health professionals and patients to make effective use of the internet.
