Missed nursing care as experienced by undergraduate nursing students

Abstract. Background: In clinical settings where missed nursing care prevails, nursing students’ instructions, supervision and the joint reflection on clinical practice with mentors are also affected and nursing students become involved in dealing with missed nursing care. Aim: To explore the experience of missed nursing care in clinical placements, its meaning for nursing students, and actions they considered or took. Methods: Qualitative study based on a content analysis of nursing students’ written reports. Nursing students in three universities in Germany with experience of at least one clinical practice placement were invited to participate. Online, they answered three open questions concerning missed nursing care focusing on: (1) experiences of the phenomenon, (2) its meaning and (3) actions that had been considered or taken when dealing with missed nursing care. A content analysis was performed. Results: The recruitment criteria were met by 69 nursing students. Four main themes and 20 sub-themes were identified. The reports covered all forms of missed nursing care. Becoming involved led to intrapersonal conflicts and, feelings of powerlessness but also to learning opportunities. It challenged professional and ethical standards. Students identified a broad spectrum of actions to answer the challenge. Conclusion: Dealing with missed nursing care creates opportunities and challenges for nursing students. To build up a well-trained and sustainable nursing workforce, learning environments for nursing students must reflect the challenges and counteract deficits.

Problem-based Learning is a learner-centered approach to education which encourages student participation and group work in the learning process. This method of selfdirected learning is facilitated by the use of small-group discussions. This being the case, it is important for groups to function effectively in order for this learning to occur. These small groups are guided by a facilitator and utilize real-life problems from the clinical settings.

Introduction and Objective: Human anatomy is an essential component of the undergraduate nursing curriculum for learning the specific disciplines which deal with clinical practice. Anatomical knowledge provides assurance for the practice of clinical assessment and invasive procedures of legal competence of nurses.  The aim of the study was to analyze the correlation of the content taught in the discipline Human Anatomy with the clinical practice of undergraduate nursing students in the discipline Semiology and Semiotics in Nursing and The Care Process, as well as their assurance to start it.Methods: Quantitative descriptive study with the application of an online questionnaire to 66 undergraduate nursing students at a public education institution in the interior of São Paulo. Data analysis by number of occurrences and Chi-square test.Results: There was partial agreement about the interdisciplinarity between human anatomy and disciplines of clinical nursing practice. The students agreed to be partially assured about the procedures to start the semiological practice of different devices and to perform nursing procedures. The predominance of the superficial approach to content related to the clinical practice of the disciplines Semiology and Semiotics in Nursing and The Care Process was predominant.Conclusions: The teaching of human anatomy, along the lines offered, maintains an unsatisfactory correlation with clinical practice due to the students’ experience, interfering with learning, acting in clinical teaching and professional training.

AbstractThe experience of nursing students who make mistakes during clinical practice is poorly understood. The literature identifies clinical practice mistakes as a significant issue in nursing practice and education but there is very little research on the topic. This study used a grounded theory approach to explore the experience of undergraduate nursing students who had made at least one mistake in their clinical practice. What emerged is a theory that illuminates the process of how students move through the positive and negative elements of the mistake experience the core variable that emerged from the study was “living through the mistake experience.” The mistake experience was clearly a traumatic process for nursing students and students reported feeling unprepared and lacking the capability to manage the mistake experience. A number of recommendations for nursing education are proposed.

Background and objective: Concept mapping is a powerful instructional tool in the clinical settings that improves competency of undergraduate nursing students in interprets situations, problem solving, decision making and critical thinking in different circumstances. Also enable them to provide optimal comprehensive care for clients. This study was conducted to evaluate the effect of concept mapping on problem solving skills, competence in the clinical settings and knowledge among undergraduate nursing students.Methods: Design and Setting: A quasi-experimental design was used and data were collected from Medical & Surgical Nursing Department and Community Health Nursing Department labs in the Faculty of Nursing, Tanta University. Sample: Random sample of 60 undergraduate nursing students which are selected by using simple random method who are divided into; thirty students from second year and thirty students from fourth year. Tools: Four tools were used for data collection: Tool (I): Structure questionnaire sheet to assess students’ knowledge regarding concept mapping, Tool (II): Case study rubric for assessing concept map, Tool (III): Problem solving skills assessment sheet and Tool (IV): Perception of students about using of concept map.Results: As a result of this research, there were significant improvements among students knowledge about concept map, simulation case study rubric and problem solving skills, in addition to more than three quarter from students had positive perceptions regarding application of concept mapping in the clinical settings.Conclusions and recommendation: Based on the findings of the study, there were significant improvement in the score of knowledge, simulation case study rubric and problem solving skills post application of concept mapping in the clinical setting. Therefore, it is necessary to improve wide-spreading of concept map training guidelines for large number of undergraduate nursing students at the level of the nursing faculties.

Objetivo: descrever a construção e validação de instrumento para avaliação do conhecimento de alunos de Graduação em Enfermagem e enfermeiros sobre o cuidado de Enfermagem no perioperatório de cirurgias geradoras de estomias intestinais de eliminação. Método: pesquisa metodológica realizada após aprovação do Comitê de Ética em Pesquisa para construção dos itens a partir da literatura e diretrizes internacionais e validação de conteúdo e aparência por oito especialistas/juízes em Estomaterapia. Para as análises, consideraram-se Índice de Validade de Conteúdo ?a 80,0% e Coeficiente Kappade Fleis s?a 70%.  Resultado: o Índice de Validade de Conteúdo global foi de 94,0% e o Coeficiente Kappa de Fleiss ficou abaixo de 0,43 para a maioria dos itens, sendo necessária segunda etapa, na qual o Coeficiente Kappa de Fleiss obteve resultado perfeito 1,00. Conclusão: Foi possível construir e validar o instrumento, entretanto, necessita de outros tipos de validação para que seja utilizado.Descritores: Estomia; Estudos de Validação; Enfermagem. CONSTRUCTION AND VALIDATION OF AN INSTRUMENT FOR EVALUATION OF KNOWLEDGE ABOUT INTESTINAL ELIMINATIONObjective: To describe the construction and validation of an instrument for assessing the knowledge of undergraduate nursing students and nurses about nursing care in the perioperative period of surgery to generate intestinal elimination ostomy. Methodology: Methodological research conducted after approval by the Research Ethics Committee for construction of items from the literature and international guidelines and validation of content and appearance by eight stomatherapy experts / judges. For the analyzes, we considered Content Validity Index ≥ 80.0% and Kappade Fleis Coefficient ≥ 70%. Results: The final version of the instrument consisted of 39 items, divided into 7 domains, with overall CVI of 94% and Perfect Fleiss Kappa Coefficient 1.00, after the second round. Conclusion: It was possible to build and validate the instrument that may highlight gaps in knowledge about nursing care in the perioperative period of surgery that generates elimination intestinal ostomy.Descriptors: Ostomy; Validation Studies; Nursing.CONSTRUCCIÓN Y VALIDACIÓN DE INSTRUMENTOS PARA LA EVALUACIÓN DEL CONOCIMIENTO SOBRE LAS ESTANCIAS INTESTINALES DE ELIMINACIÓNObjetivo: Describir la construcción y validación de un instrumento para evaluar el conocimiento de los estudiantes de enfermería y las enfermeras sobre la atención de enfermería en el período perioperatorio de cirugía para generar ostomía de eliminación intestinal. Metodología: Investigación metodológica realizada después de la aprobación del Comité de Ética de Investigación para la construcción de artículos de la literatura y las directrices internacionales y la validación del contenido y la apariencia por ocho jueces / expertos en estoterapia. Para los análisis, consideramos el índice de validez de contenido ≥ 80.0% y el coeficiente de Kappade Fleis ≥ 70%. Resultado: La versión final del instrumento consistió en 39 ítems, divididos en 7 dominios, con un IVC general del 94% y el coeficiente Perfect Fleiss Kappa 1.00, después de la segunda ronda. Conclusión: fue posible construir y validar el instrumento que puede resaltar lagunas en el conocimiento sobre el cuidado de enfermería en el período perioperatorio de la cirugía que genera la eliminación de la ostomía intestinal.Descriptores: Estomía; Estudios de Validación; Enfermería.
