Central cooling effects in patients with hypercholesterolaemia

1.A prospective study has been carried out, and 68 patients with hypercholesterolaemia have been investigated to study the effects of central cooling on serum lipid levels. 2.Central cooling was obtained by the exposure of the whole body to cold water. All patients were trained to gradually reduce the water temperature from 22 to 14 ;°C and to increase the time of exposure from 5 to 20 ;min over a period of 90 days. The 33 male and 35 female patients were aged between 40 and 60 years at entry with total cholesterol of 6.0 ;mmol/l or greater and low-density lipoprotein (LDL)-cholesterol of 4.0 ;mmol/l or greater. Thyroid-stimulating hormone, free thyroxine (FT4), total T3, total cholesterol, LDL-cholesterol, high-density lipoprotein (HDL)-cholesterol, triacylglycerols and total fat mass (determined by dual-energy X-ray absorptiometry scan) were obtained at baseline and after 3 months treatment with hydrotherapy. 3.Central cooling obtained by hydrotherapy results in a median fall in tympanic temperature from 0.2 ;°C (P< 0.001) to 0.8 ;°C (P< 0.001). We have observed in these patients a significant reduction in total cholesterol (-0.2 ;mmol/l, P = 0.006) and LDL-cholesterol (-0.2 ;mmol/l, P = 0.004). Serum FT4 level was higher than baseline results in 30 of these hypercholesterolaemic patients (15.5 ;pmol/l to 17.3 ;pmol/l) and there was no significant change in serum thyroid-stimulating hormone and total T3. 4.In conclusion, in our patients with hypercholesterolaemia we have observed a significant reduction of total cholesterol and LDL-cholesterol after body temperature regulation.

ABSTRACT Introduction Obesity is emerging as an epidemic worldwide. Obesity is associated with a number of comorbid conditions, such as diabetes mellitus, hypertension, cancer, dyslipidemia, cardiovascular abnormalities, anemia, obstructive sleep apnea, and psychosocial abnormalities. Aim This study aims at comparing the lipid profile levels of obese and nonobese men. Materials and methods This was a case—control study conducted at a tertiary care center. Totally, 80 men in the age group of 20 to 47 years attending the master health checkup were included in the study, out of which 40 men with normal body mass index (BMI) of 18 to 25 belonged to group I and 40 men with increased BMI of 30 and above belonged to group II. Lipid profile parameters, such as triglycerides (TGLs), total cholesterol, high-density lipoprotein (HDL) cholesterol, and low-density lipoprotein (LDL) cholesterol were estimated in them. The data were statistically analyzed using Statistical Package for the Social Sciences (SPSS) software version 15.0. Results Statistically significant difference was found in the total cholesterol levels with a p-value of 0.040 while the difference in LDL cholesterol was statistically highly significant with a p-value of 0.040. Conclusion Among lipid profile parameters, only total cholesterol and LDL cholesterol showed significant difference between the obese and nonobese individuals. Abstract We determined the physiological variability of total cholesterol, high- (HDL) and low-density lipoprotein (LDL) cholesterol, triglycerides, and apolipoproteins A-I and B in fasting blood samples from patients referred to the Johns Hopkins Lipid Referral Clinic. Samples were taken on each of three occasions during baseline evaluation visits before the patients were treated. The median physiological coefficients of variation (CVp) were as follows: total cholesterol, 5.0%; triglycerides, 17.8%; HDL cholesterol, 7.1%; LDL cholesterol, calculated from the previous three measurements, 7.8%; and apolipoproteins A-I and B, 7.1% and 6.4%, respectively. There were no significant differences in CVp between children (less than or equal to 18 years) and adults (greater than 18 years) for any of the measurements. On the basis of our findings, single measurements in serial samples taken on three occasions suffice to establish the patients' usual values with the following precisions (+/- 1 CV): total cholesterol, +/- 4%; triglycerides, +/- 11%; HDL cholesterol, +/- 8%; LDL cholesterol, +/- 6%; and apolipoproteins A-I and B, +/- 7%.

Background: Type II DM and obesity are metabolic disorders characterized by insulin resistance, dyslipidaemia, and metabolic stress. These features were assessed in patients using fasting plasma glucose, fasting lipid profile and serum cortisol as their markers.Materials and methods: Ninety participants were recruited and classified into 3 groups of thirty each – Obese with type II DM, Non-obese with type II DM, non-obese and non-diabetics who served as controls. Anthropometric measures of weight and height were taken using standard procedures and body mass index was calculated thereafter. Blood samples were collected after an overnight fast for the in vitro assay of serum cortisol, plasma glucose, triglycerides, total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol using enzyme linked immunosorbent assay and colorimetry as appropriate. Data obtained were analyzed statistically using ANOVA and post hoc test for comparison of variables between groups. Pearson’s correlation was performed to assess the relationship between variables and p<0.05 was considered significant.Results: Serum cortisol, plasma glucose, total cholesterol, triglycerides and LDL-cholesterol were elevated while HDL-cholesterol was reduced in both obese and non-obese subjects with type II diabetes mellitus when compared with controls. Cortisol had a significant positive association with plasma glucose, total cholesterol, triglycerides and LDL-cholesterol in obese subjects with type II diabetes mellitus while cortisol had a significant inverse relationship with HDL-cholesterol in both obese and non-obese subjects with type II diabetes mellitus.Conclusion: From this study, we conclude that elevated serum cortisol, a consequence of type II DM, accompanies dyslipidaemia in both obese and non-obese type II DM patients. It could therefore be inferred that ‘diabetic stress’ is the underlying factor of elevated cortisol in this group.

Background The prevalence of smoking in adolescentstends to increase. Smoking is associated with a higher risk ofdyslipidemia.Objective To compare the lipid profiles of tobacco-smoking andnon-tobacco-smoking male adolescents.Methods We performed a cross- sectional study in three vocationalhigh schools in Yogyakarta from January to April 2011. Dataon smoking status, duration of smoking and number cigarettesconsumed per day were collected by questionnaires. We randomlyselected 50 male smokers and 50 male non-smokers as the studysubjects.Results Mean differences between smokers and non-smokerswere 44.5 (95%CI 28. 7 to 60.1) mg/dL for triglyceride levels; 8.0(95% CI 1.0 to 14.9) mg/dL for low density lipoprotein (LDL)cholesterol; 11.8 (1.1 to 22.4) mg/dL for total cholesterol and -5.7mg/dL (95% CI -8.8 to -2.6) for high density lipoprotein (HDL)cholesterol. Mean differences (95% CI) between smokers whohad engaged in smoking for > 2 years and those who had smokedfor :S:2 years were -18.1 (95% CI -33 .9 to -2.3) mg/dL for totalcholesterol; -49.4 (95% CI -67.2 to -3 1.5) mg/dL for triglycerides.Mean differences between those who smoked > 5 cigarettes/dayand :s:5 cigarettes per day were -18 .4 (95% CI -32.8 to -4.1) mg/dL for total cholesterol and -29.1 (95% CI -53.6 to -4.6) mg/dLfor triglycerides.Conclusion Smoking more than 5 cigarettes/day significantlyincreases total cholesterol, LDL cholesterol, and triglyceridelevels, as well as reduces HDL cholesterol levels; while smokingmore than 2 years significantly increases total cholesterol andtriglyceride levels

Recent studies have demonstrated an association between low weight at birth and an atherogenic lipid profile in later life. To examine the influences of intrauterine and genetic factors, we investigated 53 dizygotic and 61 monozygotic adolescent twin pairs. Regression analysis demonstrated that low birth weight was associated with high levels of total cholesterol, low-density lipoprotein (LDL) cholesterol and apolipoprotein B (−0.17 mmol/liter per kg, P = 0.07; −0.18 mmol/liter per kg, P = 0.04; and− 0.07 g/liter per kg, P = 0.02, respectively) and with low levels of high-density lipoprotein (HDL) cholesterol (+0.04 mmol/liter per kg, P = 0.1), after adjustment for age, sex, and body mass index. Intrapair differences in birth weight were significantly associated with differences in total cholesterol, LDL cholesterol, and apolipoprotein B in dizygotic twins after adjustment for differences in current body mass index (−0.49 mmol/liter per kg, P = 0.02; −0.51 mmol/liter per kg, P = 0.01; and −0.10 g/liter per kg, P = 0.04, respectively), demonstrating that the larger the difference in birth weight, the higher these risk factors in the twin with the lower birth weight, compared with the cotwin with the higher birth weight. In monozygotic twins, however, the associations between intrapair differences in birth weight and differences in total cholesterol, LDL cholesterol, and apolipoprotein B were in the opposite direction (+0.32 mmol/liter per kg, P = 0.03; +0.23 mmol/liter per kg, P = 0.08; and +0.06 g/liter per kg, P = 0.04, respectively). The association between intrapair differences in birth weight and differences in HDL cholesterol was not significant in dizygotic twins (+0.04 mmol/liter per kg, P = 0.6) and of borderline significance in monozygotic twins (+0.11 mmol/liter per kg, P = 0.05). These data suggest that genetic factors account for the association of low birth weight with high levels of total cholesterol, LDL cholesterol, and apolipoprotein B, whereas intrauterine factors possibly play a role in the association between birth weight and HDL cholesterol.

IntroductionPositive regulatory domain containing 16 (PRDM16) protein represents the key regulator of brown adipose tissue (BAT) development. It induces brown fat phenotype and represses white adipose tissue specific genes through the association with C-terminal binding co-repressor proteins (CtBP1 and CtBP2). In healthy adults presence of BAT has been associated with lower glucose, total cholesterol and LDL (low-density lipoprotein) cholesterol levels. Our aim was to analyze the association of PRDM16 gene (rs12409277) and CtBP2 gene (rs1561589) polymorphisms with body mass index (BMI), fasting glucose level and lipid profile of adolescents.Material and methodsOur study included 295 healthy school children, 145 boys (49.2%) and 150 girls (50.8%), 15 years of age. Genotypes for the selected polymorphisms were detected by the real-time PCR method. Age, gender, height, weight, lipid profile (total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides) and fasting glucose levels were recorded.ResultsWe did not find a statistically significant association of rs12409277 and rs1561589 polymorphisms with BMI, fasting glucose and lipid profile of adolescents. We further analyzed the combined effect of the two SNPs and the statistical analysis showed that carriers of CT genotype of rs12409277 polymorphism and GG genotype of rs1561589 polymorphism had significantly lower total cholesterol (p = 0.001) and LDL cholesterol (p = 0.008) levels compared to all other groups of genotypes.ConclusionsOur study suggests that rs12409277 and rs1561589 polymorphism might have an influence on total and LDL cholesterol levels in adolescents. Larger studies should be performed in order to confirm our results.

Background and Aim: Healthy goat meat is an essential aspect in increasing consumer acceptance for this livestock product. The research aimed to examine the effect of goat meat containing low cholesterol and rich omega-6 fatty acid on the performance and blood lipid status of white rats (Rattus norvegicus). Materials and Methods: Thirty 2-month-old male white rats (R. norvegicus) weighing 195-230 g were randomly divided into three groups, with each group consisting of 10 rats. Group I was treated with a control feed (T0; BR I concentrate). Group II (T1) was treated with a mixed feed containing 50% control feed and 50% goat meat. Group III (T2) was treated with a mixed feed comprising 50% control feed and 50% goat meat with low cholesterol and rich omega-6 fatty acids. Each treatment was given ad libitum for 30 days. The variables measured were dry matter and organic matter consumption, daily body weight gain, feed conversion, triglyceride levels, total cholesterol, high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterol, and atherogenic index (AI). The data were analyzed statistically using analysis of variance in a completely randomized design. Results: The total, HDL, and LDL cholesterol levels at T0, T1, and T2 were as follows: 99.97, 35.97, and 50.43 mg/dL (total cholesterol); 108.35, 33.92, and 58.17 mg/dL (HDL cholesterol); and 101.43, 38.09, and 48.65 mg/dL (LDL cholesterol). The highest HDL and the lowest LDL cholesterol levels (p<0.05) were observed in the T2 treatment group, which had the lowest AI (1.69 vs. 1.77 and 2.19). Conclusion: The consumption of goat with low cholesterol and rich omega-6 fatty acids reduces the total cholesterol and LDL cholesterol, raises the HDL cholesterol levels, and decreases the AI.

Abstract Background Coronary artery disease (CAD) has high mortality rates and is a frequent cause of death globally. Serum lipids play a pivotal role in the development of atherosclerosis, and elevated levels of total cholesterol, low density lipoprotein (LDL) cholesterol, and triglycerides are well known risk factors of cardiovascular disease (CVD). However, there are limitations in the ability to predict CVD risk, which has led to an increased clinical interest in identifying novel risk markers. With the advances in lipidomic technology, lipoprotein subfractions may provide additional information that is missing in today's evaluation of CVD risk. Lipoprotein subfractions differ in size and density, and recent studies suggest that high density of small LDL particles provide a greater risk for CVD. Purpose To investigate whether lipoprotein subfractions are associated with the presence and extent of coronary atherosclerosis. Methods Fasting serum samples from 60 participants with suspected stable CAD were collected before scheduled coronary angiography, and analysed by nuclear magnetic resonance (NMR). The presence and extent of atherosclerosis were quantified by the Gensini Score. Participants were classified into one of three Gensini groups based on severity (&lt;20.5, normal; 20.6–30, non-significant CAD; &gt;30.1, significant CAD). Results A three-way ANOVA, adjusted for statin-use and sex, revealed statistically significant differences (p&lt;0.005) in LDL-5 particle number, LDL-5 triglycerides, and LDL-5 phospholipids between the Gensini groups. In addition, significant differences (p&lt;0.005) were found in the ratios apolipoprotein A/apolipoprotein B and LDL cholesterol/HDL cholesterol between the Gensini groups. All significant variables, identified by the three-way ANOVA, displayed the highest levels in the Gensini group with significant CAD. Conclusion Despite no difference in the traditional clinical measurements (total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides), NMR-lipidomics revealed significant differences in LDL-5 between the Gensini groups. Interestingly, our results reveal that those with significant CAD have a higher density of small LDL subfractions. Funding Acknowledgement Type of funding source: Foundation. A prospective study was conducted in the Northern region of Bangladesh, to investigate the serum lipid profile viz the level of total cholesterol (TC), Triglyceride (TG), HDL-cholesterol and LDL-cholesterol of hypertensive patients and compares them with levels of control subjects. The results revealed that serum total cholesterol, triglyceride and LDL-cholesterol were significantly markedly raised (p>0.001) whereas the level of HDL-cholesterol was significantly lower (p>0.001) in hypertensive patients as compared to control subjects. No significant changes of serum lipid profile were found between male and female hypertensive patients, but in control subjects, markedly higher levels of serum lipid profile was observed in male compared to that of female. It was concluded that hypercholesterolaemia, hypertrigyceridaemia and low density lipoprotein are the main lipid abnormalities on the incidence of hypertension in the study area. Objective: To evaluate the lipid-lowering effects, particularly changes in high-density lipoprotein (HDL) cholesterol, associated with atorvastatin use in a typical outpatient family medicine practice. Design: Retrospective case series. Setting: A community-based family medicine residency program. Patients: One hundred twenty-three patients with hyperlipidemia. Interventions: Treatment with atorvastatin to meet National Cholesterol Education Program (Adult Treatment Program) II goals. Main Outcome Measures: Fasting lipid profiles, including total cholesterol, low-density lipoprotein (LDL) cholesterol, HDL cholesterol, and triglycerides. Results: Atorvastatin lowered total cholesterol, LDL cholesterol, and triglycerides. HDL cholesterol was essentially unchanged from baseline to follow-up. In a subset of patients (∼50%), HDL cholesterol decreased by 13.1%. In the remainder of patients, HDL cholesterol increased by 10.7%. The decrease of HDL cholesterol was as much as 24 mg/dL. Conclusions: Atorvastatin is an effective agent for lowering total cholesterol, LDL cholesterol, and triglycerides. In a subset of patients, atorvastatin appeared to lower HDL cholesterol. Close monitoring of HDL cholesterol concentrations while patients are receiving atorvastatin is important.
