Speech Perception in a Communicative Context

By diminishing the role of communicative context, traditional tests of speech perception may underestimate or misrepresent the actual speech perception abilities of adults with a hearing impairment. This study investigates this contention by devising an assessment that may better simulate some aspects of "reallife" speech perception. A group of 31 participants with a severe-to-profound hearing impairment took part in a series of speech perception tests while wearing their hearing aids. The tests used question/answer or adjacency pairs, where the stimulus sentence was preceded by a question spoken by the participant. Four conditions were included: (a) where there was no initiating sentence, as in a traditional open-set speech perception test; (b) where the initiating question was neutral (e.g. "Why?"); (c) where there was a disruptive semantic relationship between the question and answer; and (d) where there was a strong contextual relationship between the question and answer. The time delay between the question and answer was also varied. Results demonstrated that in all conditions where there was a preceding question speech perception improved, and increasing the cohesion between the question and the reply improved speech perception scores. Additionally, time delay and the relatedness of the reply interacted. The effects of semantic context appeared to diminish over a 10-s period while other linguistic effects remained more constant. These results indicate the utility of simulating communicative environments within speech perception tests.

Purpose The study was designed primarily to determine if the use of hearing aids (HAs) in individuals with hearing impairment in China would affect their speechreading performance. Method Sixty-seven young adults with hearing impairment with HAs and 78 young adults with hearing impairment without HAs completed newly developed Chinese speechreading tests targeting 3 linguistic levels (i.e., words, phrases, and sentences). Results Groups with HAs were more accurate at speechreading than groups without HA across the 3 linguistic levels. For both groups, speechreading accuracy was higher for phrases than words and sentences, and speechreading speed was slower for sentences than words and phrases. Furthermore, there was a positive correlation between years of HA use and the accuracy of speechreading performance; longer HA use was associated with more accurate speechreading. Conclusions Young HA users in China have enhanced speechreading performance over their peers with hearing impairment who are not HA users. This result argues against the perceptual dependence hypothesis that suggests greater dependence on visual information leads to improvement in visual speech perception.

The Nucleus 24 Contour is a new cochlear implant that has recently undergone clinical trials in adults and children. The Contour uses the same electronics as the previous-generation Nucleus 24 (CI24M) but incorporates a downsized receiver-stimulator and a perimodiolar electrode array. The indications for use were expanded to include children as young as 12 months of age and children 24 months of age and older with severe to profound hearing loss who had open-set word recognition up to 30%. The Contour was successfully implanted in 256 children. The mean level of postoperative speech perception with the Contour was significantly better than the preoperative baseline with hearing aids on all measures. Children who had open-set speech perception in an audition-only condition before surgery demonstrated higher levels of postoperative performance with the Contour than children with no open-set speech perception before surgery.

Purpose The classroom acoustic standard ANSI/ASA S12.60-2010/Part 1 requires a reverberation time (RT) for children with hearing impairment of 0.3 s, shorter than its requirement of 0.6 s for children with typical hearing. While preliminary data from conference proceedings support this new RT requirement of 0.3 s, peer-reviewed data that support 0.3-s RT are not available on those wearing hearing aids. To help address this, this article compares speech perception performance by children with hearing aids in RTs, including those specified in the ANSI/ASA-2010 standard. A related clinical issue is whether assessments of speech perception conducted in near-anechoic sound booths, which may overestimate performance in reverberant classrooms, may now provide a more reliable estimate when the child is in a classroom with a short RT of 0.3 s. To address this, this study compared speech perception by children with hearing aids in a sound booth to listening in 0.3-s RT. Method Participants listened in classroom RTs of 0.3, 0.6, and 0.9 s and in a near-anechoic sound booth. All conditions also included a 21-dB range of speech-to-noise ratios (SNRs) to further represent classroom listening environments. Performance measures using the Bamford–Kowal–Bench Speech-in-Noise (BKB-SIN) test were 50% correct word recognition across these acoustic conditions, with supplementary analyses of percent correct. Results Each reduction in RT from 0.9 to 0.6 to 0.3 s significantly benefited the children's perception of speech. Scores obtained in a sound booth were significantly better than those measured in 0.3-s RT. Conclusion These results support the acoustic standard of 0.3-s RT for children with hearing impairment in learning spaces ≤ 283 m 3 , as specified in ANSI/ASA S12.60-2010/Part 1. The speech perception abilities of 15 patients were measured preoperatively using hearing aids and postoperatively using the Nucleus 22-channel cochlear implant over a period of 1, 2, or 3 years. Analysis of mean data revealed that, although the greatest amount of improvement in speech perception scores occurred between the preoperative and 3-month poststimulation evaluation, there was also significant improvement in perception of segmental features and open-set speech recognition over the 3-year time period. When individual patient data were examined, however, it was clear that these improvements were due, in large part, to the performance of a subset of patients who had measurable open-set speech recognition abilities at the time of their 3-month, poststimulation evaluation. Subjects who used the processing scheme that included coding of F1 showed significantly more improvement over time than subjects who used the original FOF2 processing scheme exclusively. It was concluded that open-set speech recognition ability at 3 months is an important prognostic indicator of continued improvement in speech perception abilities over time.

PurposeIn this article, the authors provide an overview of auditory training programs for aided listening by older adults, review criteria for evaluating effectiveness, summarize results of published studies, report on 2 training programs currently undergoing assessment, and discuss directions and needs for future research.MethodExperiments are ongoing to evaluate 2 individual, computer-based speech-perception training programs: Indiana University (IU) word-based training and the Speech Perception Assessment and Training System (SPATS). Training and control subjects are older adults with mild-to-severe hearing loss. Subjects train for about 30 hr with monaurally presented, spectrally shaped stimuli (IU) or through loudspeakers with their own hearing aids (SPATS). Displays and feedback use auditory and visual/orthographic cues. Outcome measures include objective measures of speech recognition in noise and other training benefits.ResultsSignificant improvements were observed in open-set recognition of trained sounds, words, phrases, and sentences. Large individual differences in training benefit were apparent. Generalization varied with the speech-perception task, competing noise, listening strategy, and pretraining scores.ConclusionsHigh-level evidence is needed to support the effectiveness of auditory training for older adults as a supplement to aided listening. Studies are needed to predict who will benefit from specific types of training, to assess compliance and engagement, and to discover benefits beyond communication.

Open-set word and sentence speech-perception test scores are commonly used as a measure of hearing abilities in children and adults using cochlear implants and/or hearing aids. These tests are usually presented auditorily with a verbal response. In the case of children, scores are typically lower and more variable than for adults with hearing impairments using similar devices. It is difficult to interpret children’s speech-perception scores without considering the effects of lexical knowledge and speech-production abilities on their responses. This study postulated a simple mathematical model to describe the effects of hearing, lexical knowledge, and speech production on the perception test scores for monosyllabic words by children with impaired hearing. Thirty-three primary-school children with impaired hearing, fitted with hearing aids and/or cochlear implants, were evaluated using speech-perception, reading-aloud, speech-production, and language measures. These various measures were incorporated in the mathematical model, which revealed that performance in an open-set word-perception test in the auditory-alone mode is strongly dependent on residual hearing levels, lexical knowledge, and speech-production abilities. Further applications of the model provided an estimate of the effect of each component on the overall speech-perception score for each child.

Five adults with sensorineural hearing impairment participated in a trial comparing the performance of the AVR TranSonic frequency-transposing hearing aid with that of their own conventional aids. They used the TranSonic for approximately 12 weeks, during which time systematic changes were made to the transposition parameters. Speech perception was assessed with each setting of those parameters and with the participants’ own hearing aids. Four participants obtained significantly higher scores with the TranSonic than with their own aids on at least one of the tests. However, analysis of the consonant confusions suggested that the improvement resulted mostly from the TranSonic’s low-frequency electro-acoustic characteristics. There was only limited evidence for 2 of the participants that the frequency-lowering function was effective at improving speech perception.

Abstract The paper presents the results of sentence and logatome speech intelligibility measured in rooms with induction loop for hearing aid users. Two rooms with different acoustic parameters were chosen. Twenty two subjects with mild, moderate and severe hearing impairment using hearing aids took part in the experiment. The intelligibility tests composed of sentences or logatomes were presented to the subjects at fixed measurement points of an enclosure. It was shown that a sentence test is more useful tool for speech intelligibility measurements in a room than logatome test. It was also shown that induction loop is very efficient system at improving speech intelligibility. Additionally, the questionnaire data showed that induction loop, apart from improving speech intelligibility, increased a subject’s general satisfaction with speech perception
