Issues of medical care quality examination are becoming more relevant at present, given the increase in the number of citizens’ appeals to the courts, an increase in the flow of patients with complaints to the Ministry of Health, and an analysis of the medical care quality is important for further improving the process of providing citizens with medical care and improving its quality level. The objective of this study was to analyze the structure of citizens’ complaints regarding the provision of medical care in the otorhinolaryngology profile, as well as to study the characteristics of quality defects in the provision of medical care.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.

Professional activity of intensivists consistently ranks high among the most «risky» areas of medicine. In 6 out of 89 criminal «iatrogenic» cases initiated by investigative authorities in the Moscow region in 2016–2018 anesthesiological or intensive care in the ICU was the subject of investigation. In 4 of these 6 cases, iatrogenic complications were detected during the forensic examination. Intensivist is considered an attending doctor and therefore holds responsibility for defects in the provision of medical care and its adverse outcome.The purpose of the study is to outline the professional risks in the work of an intensivist and identify ways to reduce them by using a case study with a fatal outcome, which led to criminal proceedings.Materials and methods. We studied and analyzed the criminal case files which included the complaint of the patient’s relatives with the prosecutor’s office, the explanations and interrogation records of the patient’s relatives (4) and healthcare workers (6), the minutes of the Morbidity and Mortality conference, the expert report on patient care quality issued by health insurance company, the final report of sanitary and epidemiological examination, autopsy protocol, emergency call files (3), the inpatient medical records, outpatient records and the final report of Commission of forensic medical examination.Results. Defects in the provision of medical care were not identified. The differential diagnosis was performed correctly. The severity of the patient's condition was due to brain infarction and associated abnormalities. Underdiagnosis of botulism did not affect the outcome of the disease and was not the cause of death of the patient.Conclusion. The reasons for filing a complaint with the Prosecutor's office and initiating a criminal case were organizational shortcomings and ethical and deontological aspects.

Abstract Background: Guideline-based medical care has been identified to improve outcomes in stroke. However, data acquisition and medical quality management during hospital stay still need to be improved in China. We have developed a computer-based medical data collecting system, together with automated calculation of Key Performance Indicators (KPIs) and regular individualized education, and thus aim to explore whether it can improve the medical care quality of acute ischemic stroke (AIS) during hospital stay in stroke centers.Methods: The individualized quality improvement based on the Computer Analysing system to improve Stroke management quality Evaluation (CASE) trial is a prospective, multi-center, historical control study among 30 stroke centers in China. In this trial, the data is directly extracted from the saved original medical record of each AIS patient during hospital stay, regardless of different Electronic Medical Record System (EMRS) in each center. Then the automated calculation of KPIs and the regular education via tele-conference per month allows the clinicians to examine the causes of non-compliance of guideline-based care and develop programs to decrease their frequency. Discussion: We compare KPIs between pre-intervention stage and post-intervention stage (without or with education) among stroke centers. If proved effective, this approach might be generalized around China and even worldwide, where a unified EMRS is difficult to be applied and in-patient care needs to be improved.Trial registration: clinicaltrials.gov, ID: NCT03684629. Aim. The medical care quality remains one of the most pressing issues of the health care system, especially at the stage of reforming of the medical sector. Medical-statistical information comprising reporting forms is the basis for analyzing the status of medical care quality and incorporating management decisions to improve it. Material and Methods. The study was performed by the means of analysis of the annual reports of the Urology Department of Municipal Non-profit Enterprise "Lviv Clinical Emergency Hospital" over a ten-year period: from 2010 to 2019. Statistical analysis included the analysis of average and relative values, as well as the calculation of indicators of the dynamic range: growth rate and absolute increase. Results and Discussion. Analysis of the use of bed stock of the Urology Department of MNELCEH during 2010-2019 showed that the average quantity of inpatient days is 251.12-340.18 days per year. Inpatient turnover was in the range of 30.28-36.55 patients per year. The indicator of surgical activity increased by 5.4 percentage points (from 33.0% in 2010 to 38.4% in 2019). There was also a positive tendency of decrease of the duration of post-surgery stay of patients at the inpatient department of the Urology Department (from 9.25 in 2010 to 8.55 in 2019). During the study period of 2010-2019, the ratio of post-surgical complications remained relatively stable and did not exceed 5.1% (the indicator is in the range of 4.38% -5.09%). The analysis of the total mortality among all treated patients was in the range of 0.06%-0.65%. Conclusion. The indicators analyzed in the annual reports make it possible to identify the main problematic issues in the work of the department. In order to provide professional specialized care, quality and rational use of property and human resources, algorithms should be developed to increase treatment of patients with malignant tumors and urolithiasis at the department, as in recent years we could observe an outflow of patients with these diagnoses to other healthcare establishments. There is a need for individual analysis of the causes of post-surgical complications and mortality. These issues can be resolved by introducing clinical audit into the work of the department

Building a modern system of rehabilitation of servicemen in Ukraine is an integral part of providing medical care in armed conflict. Rehabilitation of servicemen after gunshot wounds with soft tissue defects of the lower extremities is a common but difficult problem for surgical and rehabilitation teams. The process of rehabilitation requires the implementation of certain methodological provisions. At present, medical care for servicemen is a four-levels’ one. Rehabilitation service is provided at the third and fourth levels of medical care. The aim of the study was to improve the quality of medical care for servicemen after gunshot wounds with soft tissue defects of the lower extremities by introducing a system of staged rehabilitation. Organizational and methodological bases of the system of staged rehabilitation of wounded servicemen with soft tissue defects at different levels of medical care have been developed. Two models of rehabilitation depending on the severity of the injury and the tactics of surgical treatment are proposed. The division of the stage of early sanatorium rehabilitation for the wounded with severe soft tissue injuries depending on the stage of surgical treatment and the formation of a "skin patch" is justified. The introduction of the proposed models of rehabilitation of wounded with soft tissue defects in the practice of early sanatorium rehabilitation will provide increasing of medical care quality.

Abstract - Background: Enlarged financial and managerial resources spending on medical improvement had rarely to decrease the crowed waiting line or increase the patient satisfaction. Investigating patients’ perceived value (PPV) gain an insight of patients’ satisfaction. PPV is a valuable perspective for hospitals to strategically improve medical care quality and performance from functional and emotional sides. - Methods: According to the theory of perceived value, an empirical study had been carried out by field survey and data collection in 7 well-known hospitals in Zhejiang Province China. 2586 questionnaires with valid data were analyzed according to PPV.- Results: Beside of the importance of functional values (effectiveness of treatment effect, reasonable and accurate prices, standardization and normalization, convenience and accessibility), the emotional values (communication with doctors/nurses, comfortable environment and facilitates) were highlighted. The preferences of patients’ perceived value were influenced by patients’ background features, and then the differentiation of patients’ satisfaction was proceeded. These patients, who are young, female, outpatient, light ill condition, high educational level and high income, tend to be relatively high demanding on medical care services but in high dissatisfaction level. Additionally, the findings show an advantage to pass the reasonability of waiting time to patients’ thought.- Conclusion: Classified convenience receiving approaches according to differentiated PPV and background features of patients, equipped up with e-enabled health care environment, can bring worthwhile patients’ satisfaction in Chinese hospitals .
