Plasminogen Activation in Diabetes Mellitus: Normalization of Blood Sugar Levels Improves Impaired Enzyme Kinetics In Vitro

SummaryWe have demonstrated previously that fibrin enhanced plasmin formation by the vascular plasminogen activator was significantly impaired, when components isolated from the plasma of three uncontrolled diabetic patients (type I) were used to study plasminogen activation in vitro. In the present study it can be demonstrated that functional properties of the vascular plasminogen activators as well as of the plasminogens from the same three diabetic patients are significantly improved after normalization of blood sugar levels and improvement of HbAlc values. Most pronounced the Km of diabetic vascular plasminogen activator in the presence of fibrin returned to normal values, and for diabetic plasminogen the prolonged lag period until maximal plasmin formation occurred was shortened to almost control values. From these data we conclude that the observed abnormalities of in vitro fibrinolysis are not primarily associated with the diabetic disease, but might be secondary to metabolic disorders caused by diabetes.

Abstract Background and Objectives: There is a need to re assess the value of low-GI food and its specific components. The effect of one of them, namely low-glycemic-index sugar, is unclear, as its impact is usually confounded when reported in the literature. This study attempts to breach this gap, shedding light on its effect in type 2 diabetic patients and evaluate if it could be considered as part of a dietary plan.Subjects and Methods: The blood sugar level of twenty (20) type 2 diabetics was monitored using a Continuous Glucose Monitoring system during two phases; firstly, an initial period of 5 days, whereby all the included patients were taking their usual dietary meals. Subjects were then randomized into two groups of equal size before embarking on a second phase; 10 subjects were instructed to eat prepared, portioned and delivered meals, the difference for the second group being that low-GI sugar was used for the preparation. Results: Compared to baseline (day 1), blood sugar dropped by 18% for the group with low-GI sugar and 13% for those who consumed sugar with a normal-GI. The variation in sugar levels was also more contained in the interventional group. A by-product of the study design shows that constant glucose monitoring could raise awareness, and may foster reduction in blood sugar levels. Portioned food was capable of reducing blood sugar levels, with elevated levels of compliance just after start. Conclusions: Even though the ultimate aim is to reduce sugar consumption by diabetic patients, the intake of a low-GI sugar seems to be less harmful than normal sugar. Compared to using normal sugar for the preparation of portioned foods, the use of a low-GI sugar is encouraged as part of a wider plan for the management of diabetic patients.

Background: Necrotizing fasciitis (NF) is a uncommon and serious infection involving the skin and subcutaneous tissues of lower and upper limbs, perineal area (Fournier’s gangrene), and the abdominal wall with high morbidity and mortality. Early diagnosis and aggressive treatment can reduce the mortality rate of NF. The objectives of this study are to study the etiolopathological, microbiological factors determining the outcome of necrotizing fasciitis.Methods: It is prospective study and was conducted in JSS Medical College and Hospital in October 2008 to October 2010. Total 50 patients diagnosed with necrotizing fasciitis were admitted to JSS Medical College and Hospital. Demographic data, type of co morbidities, site of infection, clinical features with microbiology and laboratory results, and outcomes of patients were analyzed.Results: Out of 50 patients there were 42 males and 8 females. Highest number of cases was found in the age group of 65-74 years among males, 55-64 years among females. The commonest site in the present study is lower limbs (74%). Diabetes mellitus was the commonest (76%) co morbidity. Beta hemolytic streptococci 22 (44%) was the highest to be isolated. In type I and Coagulase positive staphylococci 18 (36%) was the commonest organism isolated in type II necrotizing fasciitis. The significant risk factors were gender, comorbidties, hospital length of stay, and albumin level, leucocytosis, anemia, hypoalbumenia, low serum ferritin levels increase blood sugar levels. The mortality was 12% (8 patients).Conclusions: Patients with advance age, co-morbid conditions like diabetes mellitus, hypertension, peripheral vascular disease, osteomyelitis had high unexplained susceptibility to the disease and with a higher incidence in males. Thus, early recognition with a high index of clinical suspicion would definitely reduce both morbidity and mortality. It has gross morbidity and mortality if not treated in its early stages. Leucocytosis, anemia, hypoalbumenia, low serum ferritin levels, increase blood sugar levels were consistent findings seen in majority of the patients which resulted in increased morbidity.

SummaryAn improved sensitive, specific, precise and accurate assay of plasminogen in rat plasma was developed. It is performed in 96-well microtiter plates and can be completed within one hour. The assay is based on activation of plasminogen by human urokinase-type plasminogen activator (uPA) and simultaneous measurement of generated plasmin with the specific plasmin substrate H-D-Val-Phe-Lys-4-nitroanilide (S-2390), using purified native rat plasminogen for calibration. The concentration of S-2390 in the final reaction mixture during the whole reaction period is much greater than the K m value (≈20 µM) for rat plasmin-cleavage of S-2390 ensuring that hydrolysis of substrate follows zero order kinetics and that the substrate produces a 20-35 fold decrease in rate of inhibition of plasmin by its target inhibitors in plasma. Analogous to the human system the target plasma inhibitors of rat plasmin are shown to be plasmin inhibitor and α-macroglobulins. Tranexamic acid (0.8 mM) is incorporated in the reaction mixture resulting in a 19-fold increase in the rate of plasminogen activation and presumably an about 50-fold decrease in the rate of inhibition of generated plasmin by plasmin inhibitor. The assay is suitable for accurate measurement of plasminogen in samples obtained from animals containing pharmacological concentrations of uPA or tissue-type plasminogen activator (tPA) in their plasma when in vitro plasminogen activation is blocked at pH 5 by collecting blood in acidic anticoagulant. Judged from in vitro experiments formation of catalytic active plasmin-α-macroglobulin complexes during massive activation of plasminogen in vivo does not interfere with the assay.

Hypercoagulability is one of the possible factors reported in genesis or aggravation of vascular complications in diabetes mellitus. We therefore examined procoagulant activity (PCA) of disrupted monocytes frcm 26 patients with Type I diabetes and 6 with Type II, versus 32 control subjects (male/ female ratio = 1 in each group).Diabetes monocytes exhibited a slight but detectable PCA before any incubation or in vitro stimulation, whereas control monocytes did not. Data obtained with coagulation factor deficient plasmas or phospholipase C indicated that PCA was tissue factor (TF) alone in 22 cases and TF associated with a significant amount of factor VII/VIIa activity in 10 cases.Incubation in serum free medium led to significant raise of PCA in diabetes cells when stimulated with endotoxin or not, and in control cells only after stimulation. Furthermore, PCA appeared earlier in diabetes monocytes than in control ones, (4 hours, versus 20 hours). PCA frcm control cells was FT-like. PCA frcm diabetes cells was FT-like when no VII/VIIa activity was present on non-stimulated cells, and prothrombinase-like when VII/VIIa activity was early associated with the cells. In the latter case, trace amounts of factor X activity were also detectable. Whether factor VII and factor X activities were of plasmatic origin and associated to the cells, or synthesized in vitro by the cells remains unclear. The characteristics of PCA were net correlated with clinical features (age, diabetic complications) nor with the type of diabetes.Our data suggest that in diabetes patients, monocytes exhibit an increased PCA, possibly corresponding to a baseline stimulation, or at least a higher responsiveness to undergoing stimuli in vitro.

Diabetes is a life-long disease that occurs because of ineffectiveness or lack of the insulin hormone. Although the blood sugar, fructose and haemoglobin A1c (HbA1c) values are commonly used for diagnosis, the latter give more accurate results. The HbA1c value gives information about the blood sugar levels over the past 2 to 3 months, which is required for treating diabetes. Follow-up data of diabetic patients have been used in this study. In the classification phase, a feed-forward artificial neural network (ANN) was used to estimate the factors affecting HbA1c. The designed ANN has 26 features used as input parameters. The output layer comprises two outputs: normal (HbA1c < 6.5) and high (HbA1c ≥ 6.5). An accuracy rate of 90.33% was obtained with the proposed method. The results, which show accurate estimation of the HbA1c level parameters, will be used in future studies to investigate which parameter affects the HbA1c levels, and in what way.
