In recent years, immune manipulation for cancer treatment, including breast cancer, has been increasingly gaining consent, and many attempts have been made, mainly by either strengthening the immune response (IR) or by inhibiting immune evasion. Therefore, elucidating the related mechanisms is of importance due to the potential to improve the management of cancer patients by immunotherapy. This review article summarized some recent experimental studies, which have discovered novel alterations of signaling pathways related to the immune system in breast cancer. These altered signaling pathways have been grouped according to the general biological mechanism involved: tumor-initiating cells (TICs), cancer stem cells (CSCs), immune evasion, tumor growth and progression, prediction of clinical outcome and prediction of response, or resistance to chemotherapy. These altered pathways related to the immune system open clinical opportunities for the prognosis or treatment of patients. Many of these pathways are related to the origin of breast cancer and immune evasion. We recommended development of new drugs which act on these molecular pathways, and the designing of clinical trials to be carried out mainly in breast cancer patients who required adjuvant treatment.

Background Hollow spaces in the jawbone have been defined as fatty degenerative osteonecrosis of jawbone (FDOJ) and have been linked with a dysregulated immune system. Little is known about the underlying relationship. Objectives Samples of FDOJ were analyzed to assess expression of cytokines which can play a role in the pathogenesis of breast cancer (MaCa). Material and Methods Samples of FDOJ extracted from 23 patients with MaCa and 19 healthy control jawbone samples were analyzed for 7 immune messengers. Results RANTES was found to be highly overexpressed in disease samples. No change was observed in expression levels of the other immune mediators. Discussion This data provides a compelling confirmation that FDOJ produces high levels of RANTES, a cytokine implicated in MaCa and metastasis. Levels detected in FDOJ are five-fold higher than that previously reported for MaCa tissue suggesting its role as a cytokine source in MaCa. Conclusion We thus hypothesize that FDOJ may serve as an expeditor of MaCa progression, through RANTES production.

Tumor microenvironment has been increasingly proved to be crucial during the development of breast cancer. The theory about the conversion of cold and hot tumor attracted the attention to the influences of traditional therapeutic strategies on immune system. Various genetic models have been constructed, although the relation between immune system and local microenvironment still remains unclear. In this study, we tested and collected the immune index of 262 breast cancer patients before and after neoadjuvant chemotherapy. Five indexes were selected and analyzed to form the prediction model, including the ratio values between after and before neoadjuvant chemotherapy of CD4+/CD8+ T cell ratio; lymphosum of T, B, and natural killer (NK) cells; CD3+CD8+ cytotoxic T cell percent; CD16+CD56+ NK cell absolute value; and CD3+CD4+ helper T cell percent. Interestingly, these characters are both the ratio value of immune status after neoadjuvant chemotherapy to the baseline. Then the prediction model was constructed by support vector machine (accuracy rate = 75.71%, area under curve = 0.793). Beyond the prognostic effect and prediction significance, the study instead emphasized the importance of immune status in traditional systemic therapies. The result provided new evidence that the dynamic change of immune status during neoadjuvant chemotherapy should be paid more attention.

The Impact

Background : In the last five years, the prevalence of breast cancer in Indonesia has increased. The usual treatment for breast cancer is chemotherapy, which can cause physical problems that affect sleep quality. Sleep disorders are often experienced by breast cancer patients, where the impact affects the healing of the cancer and the immune system so that the condition and immune system cannot be maintained optimally in repairing body cells. Purpose: to determine the quality of sleep in breast cancer patients undergoing chemotherapy and to see which components have contributed to disturbing sleep quality. Methods: The study used a quantitative descriptive approach. Sampling technique accidental sampling with 100 respondent. The questionnaire used to measure sleep quality was the Pittsburgh Sleep Quality Index (PSQI) questionnaire. The data analysis technique used univariate analysis, there are two interpretations on the PSQI. Sleep quality is good if the score is ≤ 5, sleep quality is bad if the score is> 5. Results: Most of the respondents have poor sleep quality. This research is expected to increase the role of nursing care and educator so that it can improve the quality of life of breast cancer patients undergoing chemotherapy
