This article reports the findings of a research study investigating the decision process of former division one nurses in deciding to leave the profession. Semi-structured interviews were conducted wi th 29 par t icipants. For many participants, leaving nursing was not an easy decision. This article outlines the decision processes of five of the study participants to illustrate the complex psychological process associated with quitting. The insight gained may shed light on how to entice back nurses who have left the profession and address the needs of those thinking of leaving.

Introduction. Effective debriefing of simulation-based experiences is critical for learning. Approximately 33% of health professions instructors are debriefing novices. However, specific faculty development needs of novice debriefers has not been studied. This study examines how health professions instructors approach debriefing when they are new to debriefing simulation-based experiences. Methods. This pilot qualitative study used a thematic analysis approach to explore novice debriefers’ experiences in conducting post-simulation debriefings. Eligible participants engaged in one-hour semi-structured interviews. Recruitment continued until data saturation was reached. We reviewed verbatim interview transcripts, hand-coded the data, and formed codes into themes. Results. Nine novice debriefers participated. The overarching theme “I’m on my own…and they’re on their own,” reflects debriefers’ view that they are on their own, without resources. Debriefers also believe learners should identifying their own errors. Three main themes emerged: “Deep divide between me and the learners” portrays a separation between debriefers and learners in terms of expectations, roles, and responsibilities. “Winging it” depicts debriefers’ making-up their own debriefing approaches. “Debriefing quality: missing pieces of the puzzle” portrays novice debriefers unaware of criteria for effective debriefing. Conclusions. Novice debriefers in this study perceived that they were on their own, having little to no debriefing training and mentorship. Study participants expressed debriefing struggles in several areas including discussing errors, facilitating learner participation, and assessing debriefing quality. Our findings shed light on simulation as a growing specialty by health profession educators and it is critical that resources are devoted to faculty development for debriefing skill acquisition. These findings can serve as a basis for future studies on debriefer skill acquisition.

Our social media newsfeeds are filled with a variety of content all battling for our limited attention. Across three studies, we investigated whether moral and emotional content captures our attention more than other content and if this may help explain why this content is more likely to go viral online. Using a combination of controlled lab experiments and nearly 50,000 political tweets, we found that moral and emotional content are prioritized in early visual attention more than neutral content, and that such attentional capture is associated with increased retweets during political conversations online. Furthermore, we found that the differences in attentional capture among moral and emotional stimuli could not be fully explained by differences in arousal. These studies suggest that attentional capture is one basic psychological process that helps explain the increased diffusion of moral and emotional content during political discourse on social media, and shed light on ways in which political leaders, disinformation profiteers, marketers, and activist organizations can spread moralized content by capitalizing on natural tendencies of our perceptual systems.

Reaction coordinate diagrams (RCDs) are chemical representations widely employed to visualize the thermodynamic and kinetic parameters associated with reactions. Previous research has demonstrated a host of misconceptions students adopt when interpreting the perceived information encoded in RCDs. This qualitative research study explores how general chemistry students interpret points and trends on a RCD and how these interpretations impact their inferences regarding the rate of a chemical reaction. Sixteen students participated in semi-structured interviews in which participants were asked to interpret the points and trends along provided RCDs and to compare relative reaction rates between RCDs. Findings derived from this study demonstrate the diversity of graphical reasoning adopted by students, the impact of students’ interpretations of the x-axis of a RCD on the graphical reasoning employed, and the influence of these ideas on inferences made about reaction rate. Informed by analytical frameworks grounded in the resources framework and the actor-oriented model of transfer, implications for instruction are provided with suggestions for how RCDs may be presented to assist students in recognizing the critical information encoded in these diagrams.

The purpose of this study was to learn about mothers’ experiences with food choices for their pre-school children in underprivileged communities in Greater Western Sydney (GWS). A total of 20 mother-child dyads living in GWS were recruited to a qualitative study from an ongoing birth cohort study. Participants’ houses were visited for semi-structured interviews, which were recorded, transcribed verbatim, and analysed thematically. The interviews yielded five main themes: (i) food choices, nutrition, and health; (ii) accessibility and availability of foods (iii) buying time for parents; (iv) child’s age and their preference on food choices; (v) conditioning certain behaviours by family and cultural factors. Nutrition literacy, child’s preferences, unhealthy food intake by family members, child’s demand, advertising and availability of harmful foods, and time constraints were all mentioned as hurdles to mothers making appropriate meal choices for their children. However, some identified facilitators were promoting parents’ knowledge, increasing access to health educational materials, upskilling mothers to providing healthier alternatives, regulating the marketing of unhealth foods. Although, the present study identified critical factors that influence mothers’ food choices for their young children, making healthy food choices is a complex practice as it is shaped by individual, social and environmental influences.

Who, in the organization, buys the computer system? How are various departments involved in the organizational decision process? T V Seshadri and N Kinra analyse the decision processes of 30 organizations that had bought a computer system—mini, mainframe, or macro. Based on a questionnaire study and factor analysis, the authors conclude that the EDP department and Board of Directors are critical in the buying grids of the purchasing organizations. They draw implications of their findings for managers marketing computer systems.

Purpose Despite the practice of dual leadership in many organizations, there is relatively little research on the topic. Dual leadership means two leaders share the leadership task and are held jointly accountable for the results of the unit. To better understand how dual leadership works, this study aims to analyse three different dual leadership pairs at a Danish hospital. Furthermore, this study develops a tool to characterize dual leadership teams from each other. Design/methodology/approach This is a qualitative study using semi-structured interviews. Six leaders were interviewed to clarify how dual leadership works in a hospital context. All interviews were transcribed and coded. During coding, focus was on the nine principles found in the literature and another principle was found by looking at the themes that were generic for all six interviews. Findings Results indicate that power balance, personal relations and decision processes are important factors for creating efficient dual leaderships. The study develops a categorizing tool to use for further research or for organizations, to describe and analyse dual leaderships. Originality/value The study describes dual leadership in the hospital context and develops a categorizing tool for being able to distinguish dual leadership teams from each other. It is important to reveal if there are any indicators that can be used for optimising dual leadership teams in the health-care sector and in other organisations.

A sample of 12 clinical psychologists and 12 graduate students in clinical psychology performed an analogue task to investigate decision processes with respect to the judged salience of criteria for social validity. Six child cases were considered by all; each card contained information describing a dangerous behavior, information accompanied by an explicit normative refererence, the same information without a normative reference, or unrelated filler comments. Non-parametric analyses indicated that subjects consistently evaluated information about dangerous behavior as being more serious than any other concern; dangerousness was ranked first 94.4% of the time. Subjects did not distinguish between information with explicit normative referents and the same information without any such referents. Students and clinicians did not differ in their response to these categories of information. The results demonstrate the application of a fixed-order problem-solving method to study the clinical-decision process and suggest the importance of criteria for social validity in this sequence.

AbstractBackground Following evidence published in the Pharmacists in Emergency Departments (PIED 2016) study Health Education England funded novel advanced clinical practitioner training for pharmacists (ACP-p), to support service delivery.Objective To explore experiences and clinical activity of trainee ACP-p, and opinions and recommendations of both trainees and clinical supervisors.Setting Five Urgent/Emergency Care Departments in London UK.Method Longitudinal mixed-methods study in three phases of registered UK pharmacists appointed as trainee ACP-p. Phase 1 (May-July 2019) – early semi-structured interviews and focus group using an experiences, opinions and recommendations (EOR) framework, Phase 2 (January-December 2019) – prospective recording of trainee clinical activity, standardised using bespoke spreadsheet, Phase 3 (November-December 2019) – as Phase 1 but at conclusion of training.Main outcome measure Experiences, clinical activity, opinions and recommendations of study participants.Results Twelve (92 %) eligible trainee ACP-p and five supervisors were recruited. Identified themes were: trainee personality, educational components, length of programme, support/supervision, career transition, university and placement training alignment, recommendations. Success was dependent on effective support and supervision. Clinical supervisors should be allocated adequate supervision time. Trainees, their supervisors and emergency department staff should be given a clear brief. Study participants agreed that the programme could be successful. Trainee ACP-p reported that they could manage 82 % of 713 pre-selected clinical presentations. Additional training needs include: ECGs, X-rays and CT scans.Conclusions Pharmacists can successfully train as ACP-p in this setting over a two-year period. This career transition needs careful management and clear structures. Training ACP-p is a useful way of enhancing skills and supporting clinical services to large numbers of patients.

Abstract Background Homeopathy has been experiencing a period of expansion in Brazil due to its practical relevance in the face of new global and national health demands, culminating in the launch in 2006 of the National Policy for Integrative and Complementary Practices (NPICP) by the Ministry of Health of Brazil, which standardized and regularized the position of homeopathy within the Brazilian Unified Health System (SUS). Aims To understand the impact of the guidelines proposed by the NPICP on homeopathy services in the SUS, specifically in the south-east region of Brazil, according to the perceptions of their managers. Methods This is a descriptive, exploratory research study with a qualitative approach, conducted in homeopathy services in the south-east region of Brazil, through semi-structured interviews and with data processed using content analysis. Results The data show the importance of the NPICP in regulating and offering homeopathy in the services studied. However, the NPICP's objective of promoting and fully developing integrative and complementary practices has not been achieved because it has failed to translate strategies into actions. Conclusions Though important to the development of homeopathy services in any given location, policies stated in the NPICP were revealed to have limited impact on the implementation and development of new services. Without further legislation, training programs and appropriate budget allocation, new services will be unable to thrive and their users unable to benefit from a more comprehensive approach to healthcare.

The objective of this study was to explore and describe the factors that influence the decision by pregnant women to be tested for HIV. This was achieved through a qualitative research study that was exploratory, descriptive and contextual in nature. A sample of 13 pregnant women participated. Data obtained from semi-structured interviews was analysed according to a protocol based on a combination of methods of analysis. The three main themes, namely factors that contribute to the decision by pregnant women to be tested for HIV, factors that contribute to the decision by pregnant women not to be tested for HIV and organisational factors that influence the decision by pregnant women to be tested for HIV, were divided into nine subthemes. Conclusions and recommendations to promote counselling to pregnant women being tested for HIV are provided. Opsomming Die doel van die studie was om die faktore wat die besluit van swanger vroue om vir MIV getoets te word, te verken en beskryf. Dit is gedoen deur middel van verkennende, beskrywende, kontekstuele kwalitatiewe navorsing. ’n Steekproef van 13 swanger vroue het deelgeneem. Inligting is verkry deur middel van semi-gestruktureerde onderhoude. Hierdie inligting is ontleed aan die hand van ’n protokol gebaseer op ’n kombinasie van ontledingsmetodes. Die drie hooftemas, naamlik faktore wat bydra tot swanger vroue se besluit om vir MIV getoets te word, faktore wat bydra tot swanger vroue se besluit om nie vir MIV getoets te word nie en organisatoriese faktore wat swanger vroue se besluit beïnvloed om vir MIV getoets te word, is in nege subtemas onderverdeel. Gevolgtrekkings en aanbevelings ter bevordering van berading van swanger vroue vir MIV-toetsing, word verskaf.
