Factors that affect general practice as a choice of medical speciality: implications for policy development

Objective This article critically appraises the range of personal, professional and social factors that affect the choice of speciality across medical students, prevocational doctors, general practice registrars and general practitioners. Methods This qualitative study applied constructs from the fields of decision theory and career theory to better understand the complex nature of choosing a speciality. In all, 47 in-depth interviews were conducted with participants at different stages of their career cycle. The data was codified and analysed using NVivo to identify key factors that influenced speciality choice. Results The research identified 77 individual findings influencing general practice as a choice of medical speciality. These were distilled into a matrix to show that factors such as money, prestige and peer interaction did not have a compelling effect, whereas clinical and academic role models, flexibility, work–life balance, scope of practice, connection with patients, training environment and practical opportunities did. Conclusion The findings indicate that the decision in relation to the choice of medical speciality is a complex cognitive process that is undertaken within a personal, social and professional context particular to each individual. What is known about the topic? Current literature aims to quantify changes in attitudes towards choice of speciality or the effect of particular variables in isolation while ignoring the complexity of this decision process and how the numerous variables compare with each other. What does this paper add? The present study is the first intergenerational research on this topic in the Australian context and the paper dismisses the role of prestige and remuneration as key drivers of choice in picking general practice as a speciality, noting that money is merely a ‘hygiene factor’. What are the implications for policy makers? A policy framework outlining 10 key principles is presented to assist policy makers seeking to affect workforce outcomes by applying policy levers to influence doctors’ choice of speciality.

Stokvel is a type of ‘rotating savings and credit associations’ found only in South Africa (SA) which have an estimated value of R49 billion and a membership of 11,5 million, in a country with a populace of 57million and an informal economy with a total value of R160 billion – yet remain a hidden sector of the economy. This confirms that there is a shortage of the consulted literature on the model for integrating the stokvels into the economic policy framework of a municipality. Therefore, developing the stokvel industry could be key to poverty alleviation, reduction of unemployment and broadening equitable access to the ownership of the economy and capital accumulation; thus, improving the livelihoods and raising the standard of living. The objective of the paper is to determine the economic contribution factors of stokvels in the economic transformation of SA at a municipality level. The paper used a mixed methods design. A sample size of 395 stokvel groups’ respondents for the quantitative research was selected using a simple random probability sampling method. The response rate was 100 percent. For the qualitative part, six policy-makers were interviewed using the purposive non-probability sampling method. The paper revealed that the money saving and investment, business opportunities and job creation, and policy development were the main economic contribution factors of stokvels in SA at the municipalities. The paper recommended for policy-makers to formulate an economic transformation policy framework inclusive of stokvels and to adopt their economic contribution factors.

Background: There has been a steady erosion of family physicians with enhanced surgical skills providing care for rural residents. This has been largely due to the lack of formal training avenues and continuing medical education (CME) opportunities afforded to those interested and attrition of those currently practicing.. Methods: A qualitative study was undertaken using an exploratory policy framework to guide the collection of in-depth interview data on GP surgeons’ training experiences. A purposive sample of GP surgeons currently practicing in rural BC and Alberta communities yielded interviews with 62 participants in person and an additional 8 by telephone. Interviews were audio recorded and transcribed then subjected to a process analysis. Results: Participants thematically identified motivations for acquiring advanced skills training, resources required (primarily in the area of solid mentorship), the most efficacious context for a training program (structured) and differences in mentorship between obstetricians and general surgeons. Conclusions: Mentors and role models were the most salient influencing factor in the trajectory of training for the participants in this study. Mentorship between specialists and generalists was constrained at times by inter-professional tensions and was accomplished more successfully within a cirriculum-based, structured environment as opposed to a learner-responsive training environment.

Purpose – This policy brief, the third in the AERI-NEPC eBrief series “Understanding validity issues around the world”, discusses validity issues surrounding International Large Scale Assessment (ILSA) programs. ILSA programs, such as the well-known Programme of International Student Assessment (PISA) and the Trends in International Mathematics and Science Study (TIMSS), are rapidly expanding around the world today. In this eBrief, the authors examine what “validity” means when applied to published results and reports of programs like the PISA. Design/methodology/approach – This policy brief is based on a synthesis of conference proceedings and review of selected pieces of extant literature. It begins by summarizing perspectives of an invited expert panel on the topic. To that synthesis, the authors add their own analysis of key issues. They conclude by offering recommendations for test developers and test users. Findings – ILSA programs and tests, while offering valuable information, should be read and used cautiously and in context. All parties need to be on the same page to maximize valid use of ILSA results, to obtain the greatest educational and social benefits, and to minimize negative consequences. The authors propose several recommendations for test makers and ILSA program leaders, and ILSA users. To ILSA leaders and researchers: provide more cautionary information about how to correctly interpret the ILSA results, particularly country rankings, given contextual differences among nations. Provide continuing psychometric or research resources so as to address or reduce various sources of error in reports. Encourage policy makers in different nations to share the responsibility for ensuring more contextualized (and valid) interpretations of ILSA reports and subsequent policy development. Raise awareness among policy makers to look beyond simple rankings and pay more attention to inter-country differences. For consumers of ILSA results and reports: read the fine print, not just the country rankings, to interpret ILSA results correctly in particular regions/nations. When looking to high-ranking countries as role models, be sure to consider the “whole picture”. Use ILSA data as complements to other national- and state-level educational assessments to better gauge the status of the country's education system and subsequent policy directions. Originality/value – By translating complex information on validity issues with all concerned ILSA stakeholders in mind, this policy brief will improve uses and applications of ILSA information in national and regional policy contexts.

Policy makers often legitimize bids for major sport events and public funding of elite sports by trickle-down effects, suggesting that hosting events, sporting success, and athlete role models inspire the population to participate themselves in sport and physical activity. According to previous review articles, empirical evidence of trickle-down effects are mixed, with several studies citing marginal or no effect. The purpose of this study is to apply a realist synthesis approach to evaluate under which conditions trickle-down effects occur (i.e., what works for whom under which circumstances?). Using rapid evidence assessment methodology, 58 empirical articles were identified in the search process and critically analyzed through the lens of realist synthesis evaluation. The analysis identified six conditions under which trickle-down effects have occurred: Event leveraging initiatives, capacity of community sport to cater for new participants, live spectating experiences, consumption possibilities on television or other media, and communities housing event venues. The findings have implications for the sustainability of sport policy decisions and public finance, as the likelihood of trickle-down effects increases with integrated planning and sustainable spending related to the above six conditions.

Purpose – The purpose of this paper is to link the multifarious problems of the elderly in a socio-economic and psychological framework. Design/methodology/approach – The universe of the sample is elderly left behind in emigrant households in Kerala. In total, 600 samples were mustered using multistage stratified random sampling method. The paper, with the aid of factor analysis, χ2 and correspondence analysis, blemish the principal factors responsible for the migration-induced exclusion of the elderly. Findings – The empirical result derived from the study shows that migration-induced exclusion is all pervasive in Kerala. The elderly left behind yearn for the presence of their children rather than the emigration and concomitant remittances. Research limitations/implications – The findings of the study are helpful to the policy makers to understand the issues faced by the elderly and include all stakeholders concerned to find a solution to tackle these problems faced by the elderly due to emigration of their children. Practical implications – The study is practically relevant in developing appropriate policy framework in Kerala as it illumines the role of the government to overcome the exclusionary trend and other manifold problems of the elderly. Social implications – The study sheds light to a new social problem developing in the state in the form of elderly exclusion owing to emigration of the young working groups in regional dimensions, demographic levels, community angles and the emerging culture of old age home in the Kerala economy and society. Originality/value – The study is a unique one and tries to situate the principal factors responsible for the emigration-induced exclusion of the elderly in Kerala with empirical evidence.

A Conference on the above topic took place at the Institute of Psychiatry, London, on 17 and 18 July 1984. The Conference was sponsored by the Department of Health and Social Security and was organized by the General Practice Research Unit. Over 100 invited clinicians, research workers and policy-makers took part. The majority of the participants were either psychiatrists or general practitioners, but representatives of all relevant disciplines attended.

AbstractObjectiveHealthy Eating Nova Scotia represents the first provincial comprehensive healthy eating strategy in Canada and a strategy that is framed within a population-health model. Five years after strategy launch, our objective was to evaluate Healthy Eating Nova Scotia to determine perceptions of strategy implementation and strategy outputs. The focus of the current paper is on the findings of this evaluation.DesignWe conducted an evaluation of the strategy through three activities that included a document review, survey of key stakeholders and in-depth interviews with key strategy informants. The findings from each of the activities were integrated to determine what has worked well with strategy implementation, what could be improved and what outputs have resulted.SettingThe evaluation was conducted in the Canadian province of Nova Scotia.ParticipantsParticipants for this evaluation included survey respondents (n 120) and key informants (n 16). A total of 156 documents were also reviewed.ResultsSignificant investments have been made towards inter-sectoral partnerships and resourcing that has provided the necessary leadership and momentum for the strategy. Policy development has been leveraged through the strategy primarily in the health and education sectors and is perceived as a visible success. Clarity of human resource roles and funding within the context of a provincial strategy may be beneficial for continued strategy implementation, as is expansion of policy development.ConclusionsKnown to be the first evaluation of its kind, these findings and related considerations will be of interest to policy makers developing and implementing similar strategies in their own jurisdictions.

Despite growing popularity of using geographical information systems and geospatial tools in public health fields, these tools are only rarely implemented in health policy management in China. This study examines the barriers that could prevent policy-makers from applying such tools to actual managerial processes related to public health problems that could be assisted by such approaches, e.g. evidence-based policy-making. A questionnaire-based survey of 127 health-related experts and other stakeholders in China revealed that there is a consensus on the needs and demands for the use of geospatial tools, which shows that there is a more unified opinion on the matter than so far reported. Respondents pointed to lack of communication and collaboration among stakeholders as the most significant barrier to the implementation of geospatial tools. Comparison of survey results to those emanating from a similar study in Bangladesh revealed different priorities concerning the use of geospatial tools between the two countries. In addition, the follow-up in-depth interviews highlighted the political culture specific to China as a critical barrier to adopting new tools in policy development. Other barriers included concerns over the limited awareness of the availability of advanced geospatial tools. Taken together, these findings can facilitate a better understanding among policy-makers and practitioners of the challenges and opportunities for widespread adoption and implementation of a geospatial approach to public health policy-making in China.

Previous research has shown that Canadian municipalities have lower visible minority proportionality rates among elected officials than other levels of government. To understand why, six visible minority city councillors and candidates from Toronto and Vancouver were interviewed. Respondents contributed their personal experiences and perspectives on issues of mirror, symbolic, and substantive representation of visible minorities. They discussed their initial involvement or interest in elite politics, their role models, and their understanding of the term “visible minority.” A broad narrative describes the complexities of political representation in practice and revealed that there were fewer entry points into elite municipal politics for individuals from marginalized groups. These entry points are further obscured by systemic barriers. Barriers were common for both cities despite differences in electoral systems and council structure. This paper recommends reimagining the “visible minority” categorization, and a policy framework that promotes the democratic ideal through institutional evaluations and acclaim for advancements.



Sustainable energy is described as energy that meets the present demand without jeopardizing the needs of future generation. Sustainable development of energy has become the main focus of recent national policies, strategies and development plans of many countries. Renewable energy (RE) is brought to play as it is deemed sustainable and has the potential to thrive in the vastly competitive market of contemporary business. Growing deployment of RE resources in businesses will provide an impetus to accelerate the drive towards clean energy as well as interest and funding for research and development works. In Malaysia, RE was introduced into the energy mix through the Five Fuel Diversification Policy in 2000 and its share has been growing ever since. This article offers a critical analysis of the developing theories linking environmental sustainability to businesses practice, and discusses their implications on Malaysia’s policy development toward RE. The extensive review presented in this work offers a useful reference for policy makers, corporate managers and researchers who have vested interest in business sustainability and renewable energy related studies.
