Psychoeducational and Social Support Interventions

In Chapter 9, “Psychoeducational and Social Support Interventions: Theory and Practice,” the history, theory, and application of both of these therapeutic approaches with caregivers and older adults are provided. While differences in structure and context exist, the underlying theoretical perspectives of these approaches are comparable, with goals and outcomes of enhanced coping, increased competence, and decreased stress. The chapter describes the use and practice of psychosocial education to increase knowledge, impart information, and foster skill development among older adults. Likewise, the methods employed in social support interventions to provide opportunities for older individuals to share experiences, foster connections with others in similar life circumstances, and enhance receipt of needed emotional and instrumental support are explained. Case studies demonstrating the use of both approaches with older care providers are included. Finally, the chapter outlines the range of environmental settings in which psychoeducational and social support interventions are commonly employed.

Gaps in social support resources in later life may arise when older adults lose social network members due to illness, death, or residential relocation. Gaps also may arise when social networks remain intact but are not well suited to meet older adults' intensifying support needs, such as needs for extended or highly personal instrumental support. Significant gaps in support resources are likely to require adaptive responses by older adults. This discussion highlights theoretical perspectives and illustrates empirical findings regarding the nature and effectiveness of older adults' responses to gaps in their social support resources. The literature examining these issues is relatively small and, as a result, is ripe for further development. Promising directions for future research are suggested.

SummaryThis study investigated gender difference in the effects of social support, including emotional support and instrumental support (such as help when sick and financial assistance), and social activities on perceived health of middle-aged and older adults in South Korea. Data were acquired from 3771 men and 4954 women aged 40 years and older who participated in the 2005 cross-sectional survey of the Seoul Citizens Health and Social Indicators Survey. Using multiple regression analysis, both age- and gender-specific differences related to social support and engagement in social activities and self-rated poor health were examined. Poor emotional support from close friends, relatives or someone with whom one could talk about worries was strongly associated with poor self-rated health in men, with the greatest effect in older men. Lack of engagement in social activities was associated with self-rated poor health in older adults, especially in older men. Poor instrumental support was associated with perceived poor health only in middle-aged women. As a health improvement strategy for men aged 65 years and older especially, emotional support should be considered. Measures should be considered for encouraging social activities by older adults, particularly older men.

ABSTRACT Objectives: The aim of this study was to examine social network characteristics and social support (emotional and instrumental support) and to determine how those factors differed between relocating older adults and nonrelocating older adults who were affected by the 2015 flood in South Carolina. Methods: Twenty-five community-dwelling elderly (CDE) were interviewed between December 2015 and May 2016 to learn about their experiences in the immediate aftermath of the flood. Ego-centric network data were collected with a focus on social network members and the types of flood-related support that these network members provided. Results: Ten of 25 CDE relocated because of the flood. All CDE were more likely to receive social support from female network members and family members than from other acquaintances. Relocating CDE received significantly less emotional support in comparison to nonrelocating CDE. The odds of receiving instrumental support were higher, but nonsignificant, among relocating CDE in comparison to nonrelocating CDE. Conclusions: The findings around the support provision are concerning particularly because of the additional psychological burden that relocation can place on flood-affected, older adults. Recommendations for public health preparedness strategies are provided in addition to future research directions for examining the well-being of flood-affected, older adults.

AbstractIn this article we examine the association between national welfare regime and the propensity of middle–aged and older individuals with adult children of their own to provide social support to aged parents. Using data from mature adults (50+) in 26 European countries, we examine whether older and younger generations compete for the time resources of the middle “sandwiched” generation, and whether national policy context shapes this competition. Contrary to expectations, we found that sandwich generation members were less likely to provide support to their parents in Conservative–Mediterranean and East European regimes, but more likely to do so in universalistic Social–Democratic regimes. This evidence supports the hypothesis that well–developed welfare states “crowd–in” family support to older individuals. Middle generation members who provided social support to their adult children tended to provide to their older parents as well. This was particularly true in the two regimes where resources and public benefits tend to be more generous and may be interpreted as state benefits that reduce intergenerational competition. Findings are discussed in terms of the capacity of state policies to shape the allocation of family resources to older adults where extended family lineages have become the norm.

Purpose The purpose of this paper is to explore the quality of social relationships of older Filipino church members by determining their perceptions of support and non-support in their social network. Design/methodology/approach A qualitative research approach with semi-structured interviews was utilized. A purposive sample of six Filipino older adults (ages 60-89) were invited and agreed to participate in the study. Findings The themes found in the study confirmed the types of support outlined in social convoy theory. Several unique nuances in the types of support and non-support between the interactions of older adult Filipinos with people very close to them, somewhat close to them, and merely acquainted with them were identified. The themes of support include instrumental support, emotional care, social connectedness, and companionship during engagement in activities. Themes characterising lack of support include disrespect and lack of understanding, constraining one’s actions, helplessness in responding to the other’s needs, non-dependability and non-reliability, difficulty in maintaining social connections, making it difficult to play a desired or expected role. Research limitations/implications The study’s limitations are the small sample size, the quality of support explored only through nominating two members of each level of closeness in the older adults’ social convoy, sample size adequacy to reach data saturation, and the lack of data on support reciprocity that may influence the respondents’ perceptions of support and non-support. Practical implications The findings of the paper point to possible interventions to improve social support for the older population. The road map for those interested in developing interventions should also put some emphasis on older adults’ needs in their continued societal engagement. Interventions may involve facilitating role transitions and providing social support systems attuned to the needs of the elderly. Social implications The authors speculate that this lack of support experienced by the older adults relates to the loss of societal roles, especially as this relates to their identity, meaning, and changes in social interactions within their communities. It is therefore important that interventions be planned to provide structures for older adults’ transitions in their re-engagement in society and into the work-force, thereby reducing this sense of role ambiguity and providing them with more positive identities in their communities. Originality/value The results suggest another form of support distinct from the emotional and instrumental support elaborated in previous work. This support is identified as companionship from within the social network that allows older adults to sustain engagement in meaningful activities. The study’s results further suggest a lack of clarity in societal roles, i.e. a sense of role ambiguity, which older adults may experience in the transitioning from adulthood to later adulthood.

Abstract Individuals are taking more responsibility for their retirement. However, economic stress generated by inadequate planning may pose a grave threat to well-being in retirement. Drawing on stress theory, this study examined how different types of social support mediate the relationship between economic stress and life satisfaction. We used the data from the 2013 and 2014 Korean Retirement and Income Study. Our sample was restricted to older Korean adults in retirement 55 to 96 years of age who were head of household (N=1,672). Confirmatory factor analysis was used to evaluate the measurement model for six latent constructs: 1) economic stress; 2) emotional support; 3) informational support; 4) instrumental support; 5) appraisal support; 6) life satisfaction. Structural equation modeling (SEM) was used to test this hypothesized model. The results revealed that both measurement model (CFI=0.985, TLI=0.983, RMSEA=0.036, SRMR=0.035) and structural model (CFI=0.978, TLI=0.984, RMSEA=0.030, SRMR=0.039) fit the data well. Standardized results from the SEM model adjusting for sociodemographic variables showed that economic stress directly predicted life satisfaction (β=-0.39, p<0.001) and two of four social support types directly predicted life satisfaction (instrumental support β=0.29, p<0.001; appraisal support β=0.25, p=0.004). Analyses demonstrated that economic stress may lead to lower levels of life satisfaction directly and indirectly through its effect on instrumental (β=-0.07, p<0.001) and appraisal support (β=-0.04, p=0.013). These findings will help inform policymakers and institutions of the need to alleviate economic stress and increase particular types of support with potentially more serious impact on the well-being of older adults in retirement.

Abstract For older adults wanting to maintain good health and stay active after retirement, volunteering is an important activity. Social capital is important factor for volunteering. Social support as a social capital, is a contributing factor that is important and needed by older adults who volunteer. Also as a result from volunteering, older adults can increase their social support through volunteering. This study examined whether emotional and instrumental social support mediate volunteering on both relational and life satisfaction. This study used the 6th additional wave of the Korean Retirement and Income Study (2016). Subjects for this study are over 60 years old and the sample size is 280. For data analysis Baron and Kenny's triangular regression analysis and the Sobel test were used for data analysis. Demographic variables were controlled. Volunteer variables such as volunteering asked by others or self-motivated, whether only one type of volunteering or more, professional volunteering, and volunteer hours were used as independent variables. Emotional and instrumental social support were used as mediators. Relationship satisfaction and life satisfaction variables were used as dependent variables. Emotional and instrumental support partially mediate volunteering asked by others to influence relational and life satisfaction. In addition, emotional support and instrumental support mediate “more than one kind of volunteering” to influence relationship satisfaction. As such, emotional and instrumental support through volunteering has a mediating effect on relationship satisfaction and life satisfaction.

Abstract Social support is fundamental to human survival, and is significantly involved in the attainment and maintenance of good health and wellbeing. Previous studies have often considered social support as a singular, non-dimensional construct. While this is important and enlightening, the method of adding up individual aspects to create a singular, non-dimensional construct has produced little understanding of these aspects/dimensions of social support and their implications for health. This study examined three dimensions or types of social support-affective, confidant, and instrumental support-and their associations with physical and mental health in older adults. Data for this study were obtain from Utah Fertility, Longevity, and Aging (FLAG) study. Participants involved 325 older adults, aged 50 years or older. Results showed a significant, strong positive correlation between affective support and physical and mental health, and weak association between confidant support and physical and mental health. The correlation between instrumental support and physical and mental health was moderate. After controlling for the influence of socio-demographic variables, affective and instrumental support significantly predicted physical and mental health. Confidant support was not a significant predictor of either physical or mental health. The findings suggest both affective and instrumental support might be relatively more important to the health and mental wellbeing of older adults than confidant support, underscoring the relative importance older adults attach to quality rather than confidant support, which essentially is quantity of social ties.

Language plays an important role in defining identities in older adulthood. Both self-perception and others' perceptions of older adults are reflected through language used by older and younger adults (see recent texts: de Bot & Makoni, 2005; Harwood, 2007; Hummert & Nussbaum, 2001; Nussbaum & Coupland, 2004). In this review, we outline key theoretical perspectives on the study of communication with older adults and provide evidence supporting these perspectives within the context of age stereotypes, intergenerational communication, cross-cultural communication, and health care encounters. Given that communication is an interactive process, we discuss how older adults use language and communication to respond to age stereotypes and adaptively cope with age-related losses. We also discuss communication interventions aimed at improving interactions between care providers and older adults, and opportunities that technology brings to enhance communication within and across generations.

Abstract Sustaining a hip-fracture is a life-changing event negatively affecting older adults. Although, social support is a known determinant of health outcomes, the relationship between social support and living situation of older adults with hip fracture remains under researched. For this study social support is conceptualized using the Finfgeld-Connett framework, where social support is seen as being composed of emotional and instrumental support. The objectives were to examine the relationship between two domains of social support and living situation: 1) after discharge; 2) 3-months after discharge; and 3) 6-months after discharge from an inpatient rehabilitation facility in a sample of older adults with hip fracture. Emotional support was measured as frequency of interaction with someone one week prior to hip fracture, whereas instrumental support was measured as help received in instrumental activities of daily living. Logistic regression was performed to examine the association between social support and living situation. Majority of study participants (N=139) were older (mean age 81.31), female (77.70%), had no cognitive impairment (68.35%), were not married (58.99%), and lived with someone (51.80%) in their own house (71.95%). Older adults with more emotional support were more likely to be discharged home, however little can be said about the effect of the association (OR 6.80, 95% CI 1.08, 22.31, P<.001). Persons receiving more instrumental support had less odds of living at home 3-months (OR 0.41, 95% CI 0.21, 0.78; P=.007) and 6-months after discharge (OR 0.59, 95% CI 0.38, 0.91, P=0.017). Social support is important for older adults during recovery.
