Reparation in Respect of Mother’s Organized Defence Against Depression

Author(s):
Donald W. Winnicott

In this paper Winnicott discusses how false reparation can appear through a patient’s identification with the mother, where the dominating factor is the mother’s organized defence against depression and unconscious guilt. He states that the attainment of a capacity for making reparation in respect of personal guilt is one of the most important steps in the development of the healthy human being. While Winnicott’s experience tells him that a children’s out-patient department demonstrates the extent of hypochondria in mothers, he admits that there is no sharp dividing line between the hypochondria of a depressed woman and a mother’s genuine concern for her child. The task for the child is to deal with the parent’s mood, after which they can start their own lives. Winnicott discusses the task of the analyst with this sort of patient to not be experienced as depressed - and notes the importance of this relationship between the patient and the environmental mood in group work.

Homoeopathy is the system of medicine founded by Dr. Samuel Hahnemann (1755-1843) of Germany. It is based on the principle that “like cures like”. In practice, this means that a medicine capable of producing certain effects when taken by a healthy human being is capable of curing any illness that displays similar effects. In pediatric practice; attention deficit disorders (ADHD/ADD) are the most common serious psychosocial problems prompting parents to seek help for their children. Since the ability to pay attention and concentrate is a basis prerequisite of child development, forming the foundation of all learning and thinking as well as of emotional and social interaction, the suffering of these children as well as their siblings, parents, teachers, and fellow pupils is often considerable.

Aboagora 2013 discussed the complex relationships between man and machine, where not only may the human being itself be viewed as a corporeal machine, but it is also possible to interpret the machine as an extension of the human sensory system. After three days of lectures and workshops about the multifaceted relationship between man and the machine, the ontological dividing line between humans and machines was open to question. For example, while the human body can be defined as the ultimate machine – an assemblage of forces, actions and mechanisms ranging from the optics of the eye to the processes of cognition – the boundaries between man and machine may be blurred as technological devices are used as integral parts of the human body. Where do we draw the line between man and machine in such situations? The Aboagora symposium on 'The Human Machine' raised important questions about the ontological qualities and delineations of various entities.

Ayurveda is the ancient holistic science of life which gives us most detailed information on different Ahara (Diet) and lifestyles to be followed in different disease conditions as well as to maintain the optimum health of a healthy human being. Ayurveda states in full detail about the specific Ahara and lifestyles to be followed during different seasons under different climate circumstances and for different diseases. Ayurveda has scientific approach in planning the Ahara; therefore Acharya Charaka has included Ahara as first among the three supporting pillars of life. Ahara is an important component for the management of a disease in the same way as Ahara is considered as an important aid to protect the life and health. Sthoulya (Obesity/Medoroga) is a burning problem in present era due to improper Ahara and luxurious and competitive lifestyle. People have neither time nor interested to follow the dietetic regimens as well as daily regimens and seasonal regimens as described in classical texts of the Ayurveda.

In Reply.— We are happy to address Dr Roscelli's concerns regarding our paper on familial reflux screening; we are not in complete agreement with him, however. First of all, the incidence of vesicoureteral reflux in the healthy human being is accepted as being 0.4% to 1.8%.1 The only study to show a significantly higher rate was the Kollerman study referred to by Dr Roscelli. This study has been roundly criticized because it was performed with high pressure injection, and it is the only such study to show a high rate of reflux in healthy children.

It is quite likely with regard to the title of this paper that critics may say—“Why cannot the writer say clearly what she is going to talk about? The words ‘early nervous and mental,’ cover so enormous an area,” or again “Why in any case should we of the Medico-Psychological Association be troubled with the word ‘nervous’ ; surely that pertains exclusively to the Neurological Society? Why should nerves—inferior spinal things—be mentioned in the august presence of those con cerned with the brain?” and yet there is no nerve of interest in the healthy human being which is not interdependent upon, or in close association with a brain. It is inconceivable that any nerve should suffer and leave the brain unaffected, if not from impressions sent up and down, then from absence of impres sions, and it is upon this vital essential oneness of the whole nervous system that I wish to lay stress to-day.

According to experimental and clinical evidence, the experiences of previous treatments are carried over to different therapeutic approaches and impair the outcome of subsequent treatments. In this behavioral pilot study we used a change in administration route to investigate whether the effect of prior treatment experience on a subsequent treatment depends on the similarity of both treatments. We experimentally induced positive or negative experiences with a topical analgesic treatment in two groups of healthy human subjects. Subsequently, we compared responses to a second, unrelated and systemic analgesic treatment between both the positive and negative group. We found that there was no difference in the analgesic response to the second treatment between the two groups. Our data indicate that a change in administration route might reduce the influence of treatment history and therefore be a way to reduce negative carry-over effects after treatment failure. Future studies will have to validate these findings in a fully balanced design including larger, clinical samples.
