In 2018 and 2019 Cancer Research UK (CRUK) launched a controversial advertising campaign to inform the British public of obesity being a preventable cause of cancer. On each occasion the advertisements used were emotive and provoked frustration among the British public which was widely vocalised on social media. As well serving to educate the public of this association, the advertisements also had the secondary effect of acting as health promotion through social marketing, a form of advertising designed to influence behavioural changes. As CRUK delivered a public health message through its campaign, the advertisements should be held according to the ethical principles which underpin healthcare in the UK. This article evaluates whether the advertisements used by CRUK in 2018 and 2019 fulfilled the ethical principles of beneficence, autonomy, non-maleficence and justice. It is found that while providing an important message, the oversimplification of obesity as being the result of personal decisions ignored the complex aetiology and served to stigmatise the target demographic, potentially disengaging them from the message. Additionally, posting cancer as the consequence of obesity invokes feelings of fear due to its connotations of suffering and premature death. Based on available evidence, the use of fear in social marketing does not create sustained behavioural change. This essay recommends that CRUK discontinue its use of such strategies in its future social marketing endeavours.

Purpose: EatRight Ontario (ERO), a multi-modal dietitian service (phone, email, web), provided the public and health intermediaries with healthy eating advice, professional support, and health promotion tools from 2007 to 2018. An evaluation of ERO was conducted to assess the impact of the model on knowledge, attitudes, and behaviour for consumers, utilization, and support levels and satisfaction provided to health intermediaries. Methods: Consumer clients were sent a survey 1–4 weeks after using the ERO service to capture self-reported dietary changes, intentions, nutritional knowledge, and satisfaction. Health intermediaries were recruited through an electronic ERO newsletter and asked about how ERO supported their practice. Results: Of the 867 consumer respondents, 92% had either made a change or indicated that information from ERO confirmed their present behaviour, and 96% indicated they would recommend the services to others. Of the 337 health intermediaries who responded 71% indicated that ERO provided services they could not deliver. Conclusions: ERO’s multi-modal dietitian contact centre provides a model for implementing successful remote service access for consumers and professionals to support healthy eating across diverse demographics and geographies, including those in geographically underserved areas.

Vast numbers of crustaceans are produced by aquaculture and caught in fisheries to meet the increasing demand for seafood and freshwater crustaceans. Simultaneously, the public is increasingly concerned about current methods employed in their handling and killing. Recent evidence has shown that decapod crustaceans probably have the capacity to suffer because they show responses consistent with pain and have a relatively complex cognitive capacity. For these reasons, they should receive protection. Despite the large numbers of crustaceans transported and slaughtered, legislation protecting their welfare, by using agreed, standardized methods, is lacking. We review various stunning and killing systems proposed for crustaceans, and assess welfare concerns. We suggest the use of methods least likely to cause suffering and call for the implementation of welfare guidelines covering the slaughter of these economically important animals.

ABSTRACTThe religious reforms of the sixteenth century exerted a profound impact upon the liturgy of baptism in England. While historians' attention has been drawn to the theological debates concerning the making of the sign of the cross, the new baptism liturgy contained within the Book of common prayer also placed an innovative importance on the public performance of the rite in the presence of the whole congregation on Sundays and other holy days. Both religious radicals and conservatives contested this stress on ceremony and publicity throughout the early modern period. Through the collection of large numbers of baptism dates from parish registers, it is possible to measure adherence to these new requirements across both space and time. Before the introduction of the first prayer book in 1549, there was considerable uniformity among communities in terms of the timing of baptism, and the observed patterns are suggestive of conformity to the requirements of the late medieval church. After the mid-sixteenth century, parishes exhibited a range of responses, ranging from enthusiastic adoption by many communities to complete disregard in religiously conservative parts of Lancashire and Cheshire. Additionally, the popularity of saints' festivals as popular days for baptism fell markedly after 1660, suggesting a decline in the observance of these feasts.

Policy makers take action largely on issues that attain the pinnacle of the policy agenda (Pertschuck, 2001). As a result, how decision makers choose which issues are important has been the subject of much research. Agenda-setting conceptualizes the process of how issues move from relative unimportance to the forefront of policymakers’ thoughts (Dearing & Rogers, 1996). An area within agenda-setting research, Health Promotion Agenda-Setting, provides Health Promotion practitioners with an innovative framework and strategy to set agendas for sustained courses of action (Kozel, Kane, Rogers, & Hammes, 1995). In this interdisciplinary and bi-national exploratory study, funded by the Center for Border Health Research of the Paso del Norte Health Foundation, we examine agenda-setting processes in the Paso del Norte Region and evaluates how the public health agenda is determined within the U.S.-Mexico border population. Integrating both quantitative and qualitative data collection methods, the current research is focused on identifying deficiencies in the public health infrastructure in the U.S.-Mexico border area, and identifying channels that exist for working toward the bi-national goals presented in Healthy Border 2010 (U.S.-Mexico Border Health Commission, 2003). Research directions, design, and methodologies for exploring health promotion agenda-setting in applied settings, such as Healthy Border 2010, provide health practitioners and policy makers the potential to improve public health leadership by influencing the public health and policy agendas.

The thoughtful and cautionary pieces by Newacheck et al1 and Perrin et al2 remind us of how much we have achieved in piecing together care for vulnerable children, how far there is yet to go, and how the transition to the long-overdue health care reform might worsen, rather than improve our present arrangements. In the absence of a rational, planned care system for children, especially for those who are poor or who require extensive services, pediatricians and child advocates in both the public and private sectors have managed to cobble together at least the possibility of decent services for large numbers of children, with some payment to those who provide those services.

Health opinion leaders have widely embraced social media for health promotion and public health communication which can make a strong influence on the public decision making. However, despite the growing relevance of public health threats such as infectious diseases, pandemic influenza and natural disasters, research has paid little attention to the qualities of opinion leaders. Moreover, there is limited evidence that public health organisations use social media appropriately to engage in meaningful conversations with audiences. Thus, the aims of the study are to describe principles of communication practised by online opinion leaders to promote health issues, to discuss the strategies of social media used, to explain the opinion leaders’ influence attributes in health decision making and finally to develop a model of online opinion leader in the contemporary health promotion era. Content analysis was conducted on Facebook postings of five selected health opinion leaders. In addition, semi-structured interviews with 10 followers were conducted. The findings of the study revealed that there are three principles of communication practised by online opinion leaders, which are language, interaction and themes. There are three strategies opinion leaders use to influence the public: social connectivity, social support and social consultation. Furthermore, there are five opinion leaders’ attributes that influence the public’s decision making, namely personality, authenticity, trust/credibility, professional knowledge and social position. The model would be beneficial in educating and guiding the current public health opinion leaders in order to establish health and social well-being. COVID-19 has shown us clearly that the world must commit to a transformative approach that promotes health and wellbeing. Living in the Anthropocene – an epoch defined by human impact on our ecosystems – moves us into unknown territory. The challenge is to find a way of living that aims to meet the needs of all people within the means of the living planet. We will require foresight, agility and resilience to be well prepared. The global risks we face are enormous and they are interconnected – yet the opportunity to accelerate change for the better is extraordinary as well. We have models, knowledge and technologies at our disposal that could significantly improve health and wellbeing and create fairer and more sustainable societies – yet they have not been used widely to serve the public purpose and to address inequities.
