ABSTRACT Introduction Perseverance—doing something despite difficulty—is an asset to healthcare professionals, yet not all providers display this trait. The literature offers no conceptualization of perseverance as it relates to military caregivers. This research sought to explore the perseverance displayed by members of military interprofessional healthcare teams (MIHTs) and to construct a framework for explaining the role of perseverance in MIHTs’ collaborative work. Methods Using Grounded Theory, this interview-based study collected insights from 30 individuals who had participated in MIHTs and/or led MIHTs. Participants represented 11 different health professions, both officers and enlisted military members, and three branches of the U.S. military (i.e., Army, Navy, and Air Force). Data were collected and analyzed in iterative cycles until theme saturation was achieved. Results We identified practices through which perseverance was exhibited by members of MIHTs: (1) humility, (2) mission focus, (3) team effort, (4) failure is not an option, (5) comfort with discomfort, and (6) continuous improvement. We then clustered these practices into three subcategories, reflecting how certain practices may connect to support MIHT perseverance. Conclusions These six practices appear connected in significant ways to support the MIHTs’ efforts. Humility, mission focus, and team effort can help team members collaborate as a unified and mutually supportive team. From this base, MIHTs seemed able to become comfortable with discomfort and to refuse failure as an option. These orientations then allowed the team to engage in continuous improvement. Together, these practices may enhance team perseverance and ultimately team performance.

Background: As healthcare becomes increasingly team based, we need new ways of educating trainees to be collaborative team members. One approach is to look to other professions that have developed highly effective ways of collaborating. Doctors have already turned to musicians for specific lessons; however, as of yet, there has been little empirical study of the ways that musicians interact in ensembles, or analysis of how this might provide insights for healthcare. Our hypothesis is that healthcare teams might learn from understanding collaborative practices of chamber musicians.Methods and Findings: We undertook an exploratory study of professional musicians playing in non-conducted ensembles. We used semi-structured interviews to explore factors the musicians considered important for effective group function. The interviews were transcribed and coded thematically. We identified three prominent themes that have relevance for healthcare teams.Conclusions: The highly individual nature of each musical group’s identity suggests that a focus on generic interprofessional education skills development may be insufficient. Furthermore, musicians’ understanding of the fundamental role of non-melodic parts provides the possibility of more nuanced leadership models. Finally, essential differences between musicians’ interactions in rehearsals and performances highlight the importance of varied forms of group interactions.

In 2008, the authors’ team started an ongoing project to administer music therapy sessions for patients with neurodegenerative diseases. Studies were made conducted from the “caring” perspective to evaluate the effects of music therapy on the mental health of the patients (Inomata, 2008a, Inomata 2008b) and on the role of nurses in integrative medicine (Inomata, 2008c). On the basis of the findings from these studies, music therapy programs were designed and conducted to meet the different needs of various neurodegenerative diseases. This project was the first ever reported music therapy initiative undertaken as a multi-disciplinary collaborative work and in partnership with a patients’ group (Saji, 2010). The findings from four years of running the project are summarized as follows: (1) Music therapy helped maintain/improve the QOL(Quality of Life) level of neurodegenerative disease patients, which would otherwise deteriorate with the progress of symptoms; (2) There was an improvement in the patients’ psychological and spiritual health as exemplified by the expansion of consciousness and rebuilding of relationships; (3) The project increased the feeling of partnership among the multi-disciplinary team members; (4) Care providers shared values such as self-belief and respect for both the self and others; (5) Caring for patients’ emotional side by being compassionate and staying with them and/or listening to them resulted in a stronger care provider-patient bond; (6) Nurses were engaged in the building a healing environment as “healers,” and the patients found more hope in everyday life.

It also discusses the Integrated Health Care for an Aging Population (IHCAP) report of 2008, competencies related to integrated care, interprofessional healthcare teams, team development, and the role of the psychologist in integrated care.

ABSTRACT Introduction The need to maintain medical ethical standards during conflict and peace has been the source of considerable academic discourse. Although still an unsolved challenge, scholars have made significant contributions to the literature, constructing categorizations that can help military providers contend with ethical conflicts. However, insights into the ethical comportment of military interprofessional healthcare teams (MIHTs) have yet to be reported. Materials and Methods This interview-based study collected insights from 30 military healthcare providers who participated in and/or led MIHTs. Altogether, participants represented 11 health professions, both officers and enlisted military members, and the U.S. Army, Navy, and Air Force. Following Grounded Theory methodology, data were collected and analyzed in iterative cycles until theme saturation was reached. Results The research team identified two themes of ethical bearing that enable MIHT success in and across care contexts. One theme of successful ethical bearing is “raising concerns,” referring to speaking up when something needs to be addressed. The other is “making compromises,” where individuals have to make sacrifices (e.g., lack of equipment, non-sterile environment, etc.) to give patient care. Conclusions These data suggest that effective MIHTs have a collective moral compass. This moral compass is the team’s ability to judge what is ethically right and wrong, as well as the team’s willingness and ability to act accordingly—to consistently “do the right thing.” There is a collective moral compass, and while the team may not all agree on what exactly is true north—they are all bending that way.

Collaborative groups, by their very nature, involve the interactions of many participants which can vary in time, place, culture, knowledge, and ability. The design of collaborative technology for work groups must occur in conjunction with the users while avoiding the dangers of technology-centered product development. This paper focuses on applying a group-centered approach to the development of Computer-Supported Collaborative Work (CSCW) products. This approach emphasizes the importance of team members” participation in the design process such that the resulting technology is centered around the users” needs and capabilities. By allowing collaborative groups to elaborate on real issues related to their actual work context, and to suggest specific ways to improve their work, a group-centered approach to the design of CSCW systems can be realized. The utilization of the Advanced Knowledge And Design Acquisition Methodology (AKADAM), consisting of concept mapping and design storyboarding techniques, was suggested as a basis for achieving this level of team participation. The concept mapping technique elicited insights into the collaboration process, identified problem areas, and generated suggestions for improving group work. With the aid of the design storyboarding technique, these suggestions for improvement can be directly translated into CSCW tools.

ABSTRACT Introduction Multiple aspects of interpersonal dynamics can help or hinder the success of teams, particularly those in a Military Interprofessional Healthcare Team (MIHT). One specific mechanism for MIHTs’ success is camaraderie and how these military teams are able to achieve, maintain, and enable the development of this important characteristic. Despite our understanding of military service members as being bonded like a family, we have a limited understanding of how this bond is translated into their MIHT experiences. Materials and Methods This study conducted interviews among 30 individuals who had participated in, led individual, and/or led many MIHTs, using a grounded theory methodology. Participants represented 11 different health professions, including officers and enlisted military members, and three branches of the U.S. military (e.g., army, navy, and air force). Data were collected and analyzed in iterative cycles until saturation was achieved. Results We identified six themes that shaped the overarching concept of camaraderie in MIHTs. These themes were (1) confidence in competent peers, (2) shared goals, (3) mutual respect, (4) desire to help one another improve, (5) personal is professional, and (6) bonds of military service. This paper describes each of these themes, provides illustrative examples from the data, and describes how these components contribute to MIHTs’ team dynamics. We present a model for how to understand these themes. Conclusions Through the identification and exploration of these aspects of camaraderie, we are able to better understand how MIHTs are able to be successful. MIHTs that demonstrated confidence in their brother/sister in arms possessed shared goals and missions, while maintaining mutual respect, a desire to help one another do better, and creating a personal and professional overlap tended to form stronger bonds of military service. Critically, these six aspects support a more nuanced understanding of the spirit of camaraderie and how it underpins MIHT success.

The Royal Air Force (RAF) Critical Care Air Support Teams (CCASTs) aeromedically evacuate seriously injured service personnel. Long casualty evacuation chains create logistical constraints that must be considered when aeromedically evacuating patients. One constraint is the length of a CCAST mission and its potential effect on team member performance. Despite no evidence of patient care compromise, the RAF has commissioned a study to investigate whether CCAST mission length influences performance. Describing and understanding the role of a CCAST enabled fatigue to be defined. Factors essential to studying fatigue were then identified that were used to develop a theoretical model for designing a study to measure the effects of fatigue on CCAST performance. Relevant factors include the patient’s clinical condition, team members’ cognition and vigilance levels, and the occupational aviation environment. Further factors influencing overall performance include the duration and complexity of patient interventions, mission length, circadian influences, and fatigue countermeasures.

ABSTRACT The importance of successful interprofessional collaboration for effective patient care is generally acknowledged. Research into interprofessional collaboration has thus far been mainly situated in the civilian context and has mostly indicated barriers that prevent successful interprofessional collaboration. However, military interprofessional healthcare teams (MIHTs) seem to be exceptionally successful. Building on the overarching finding of the studies within this special edition—i.e., that MIHTs’ readiness and excellence are in part due to healthcare professionals’ “shared understanding” of what is needed to effectively serve on an MIHT—this commentary uses the theory of Landscape of Practice as a lens to further explain the processes through which healthcare professionals attain this shared understanding. Used within the fields of Health Professions Education and workplace learning, Landscapes of Practice (LoP) helps to explain how learning occurs within practice. It highlights how, by engaging within the various working environments belonging to a profession, social interactions between the various professionals within that environment form the conduit for learning. LoP highlights that the outcome of this learning process is “knowledgeability,” i.e., understanding of how to engage within the field and with its players, resulting in being an acknowledged member of the field. Fostered through a process called “identification,” professionals learn to see how their professional practice aligns with that of others and how to effectively collaborate with others. The commentary explains how the findings of the separate studies within this special edition strongly resonate with knowledgeability and identification. It is concluded that civilian interprofessional healthcare teams may benefit from incorporating characteristics of MIHTs in their training programs.

Information asymmetry is omnipresent within strategic alliances. This diversity in knowledge structure and content between team members comprising the alliance is complicated by the increasing trend of physical distribution of team members in international strategic alliances. While technical solutions to distance work abound, a strong and theoretically grounded approach to how best to implement these solutions is lacking. However, research in the area of shared cognition holds promise as an appropriate solution. This chapter presents a three-part framework for organizing research in the areas of shared cognition and CSCW (Computer Supported Collaborative Work) so that strategic alliance managers will have a resource for implementing their technologically mediated teams.

Objective: We examine the construct of collective orientation, develop a measure to assess individual differences in collective orientation, and examine the extent to which the collective orientation of team members predicts performance on a variety of team tasks. Background: Scholars increasingly emphasize the importance of teamwork in collaborative work environments, and evidence indicates that a lack of teamwork is a prominent factor in many real-world accidents. Although it is clear that some persons are more team oriented than others are, there are few instruments available to assess individual differences in collective orientation in a team context. Method: We develop a scale to measure collective orientation in teams, gather evidence on reliability and construct validity, and examine the extent to which collective orientation predicts team performance. Results: Results indicate that the Collective Orientation Scale is reliable, correlates with cognate measures, and predicts performance on a variety of team tasks. Conclusion: We discuss the role of collective orientation in teams and the application of this scale to assess and diagnose teamwork deficiencies in work groups. Application: This research should contribute to a further understanding of factors that influence collaboration and coordination in teams.
