Clinically, a reconstruction plate can be used for the facial repair of patients with mandibular segmental defects, but it cannot restore their chewing function. The main purpose of this research is to design a new three-dimensionally (3D) printed porous titanium mandibular implant with both facial restoration and oral chewing function reconstruction. Its biomechanical properties were examined using both finite element analysis (FEA) and in vitro experiments. Cone beam computed tomography images of the mandible of a patient with oral cancer were selected as a reference to create 3D computational models of the bone and of the 3D-printed porous implant. The pores of the porous implant were circles or hexagons of 1 or 2 mm in size. A nonporous implant was fabricated as a control model. For the FEA, two chewing modes, namely right unilateral molar clench and right group function, were set as loading conditions. Regarding the boundary condition, the displacement of both condyles was fixed in all directions. For the in vitro experiments, an occlusal force (100 N) was applied to the abutment of the 3D-printed mandibular implants with and without porous designs as the loading condition. The porous mandibular implants withstood higher stress and strain than the nonporous mandibular implant, but all stress values were lower than the yield strength of Ti-6Al-4V (800 MPa). The strain value of the bone surrounding the mandibular implant was affected not only by the shape and size of the pores but also by the chewing mode. According to Frost’s mechanostat theory of bone, higher bone strain under the porous implants might help maintain or improve bone quality and bone strength. The findings of this study serve as a biomechanical reference for the design of 3D-printed titanium mandibular implants and require confirmation through clinical investigations.

Background. There is no general consensus in restorative dentistry about which lateral guidance should be established. Some studies have shown that canine guidance decreases the tension of masticatory muscles. Others have reported that group function might achieve a better physiologic distribution of occlusal forces. Also, some reports have shown that both guidances are equally acceptable. Despite all discussions, clinical evidence of one guidance being superior to another is limited. Thus, this study aimed to analyze the electromyographic (EMG) activity of masseter muscles in individuals with group function and canine guidance. Methods. Twenty volunteers of both genders, aged 20-25, were divided into two groups: GF (group function guidance, n=10) and CA (canine guidance, n=10). EMG activity of masseters was captured using surface electrodes during habitual maximum intercuspation (HMI) and right and left lateral jaw movements and recorded using EMG amplitude values (RMS – root means square). Student’s t-test was used to compare mean RMS values between the groups and lateral movements in each group. Results. During HMI, there was no difference in masseter EMG activity between the groups. Both masseters showed higher activity in group GF only on the right side during lateral movements, while the left masseter exhibited higher activity on the nonworking side in both groups. The activity of both masseters distributed by tooth was higher in group CA. Conclusion. During tooth restorative procedures, any guidance is acceptable considering HMI. However, group function guidance is more favorable during lateral movements due to greater dissipation of occlusal pressures.

Abstract There is a growing emphasis to use a transdisciplinary team approach to accelerate innovations in science to solve complex conditions associated with aging. However, the optimal organizational structure and process for how to accomplish transdisciplinary team science are unclear. In this study, we illustrate our team’s experience using transdisciplinary approaches to solve challenging and persistent problems for older adults living in urban communities. We describe our challenges and successes using the National Institutes of Health four-phase model of transdisciplinary team-based research. Using a de-identified survey, the team conducted an internal evaluation to identify features that created challenges including structural incongruities, interprofessional blind spots, group function, and group dynamics. The team then identified responses to address the features that created challenges and determined indicators for success. Indicators for success were identified by the team as a place for continued evaluation of the teams’ collaborative effectiveness, transdisciplinary integration, and impact on the university and aging community. This work resulted in the creation of the team’s Transdisciplinary Conceptual Model. This model became essential to understanding the complex interplay between societal factors, community partners, and academic partners. Conducting internal evaluations of transdisciplinary team processes is integral for teams to move beyond the multi- and interdisciplinary niche and to reach true transdisciplinary success. More research is needed to develop measures that assess team transdisciplinary integration. Once the process of transdisciplinary integration can be reliably assessed, the next step would be to determine the impact of transdisciplinary team science initiatives on aging communities.

In sport teams, humor is an essential element that influences communication processes, and plays an important role in group dynamics. Despite this, no current instrument is presented in the literature to measure humor climate in sport teams. Therefore, the current study presents the development and initial validation of the Humor Climate in Sport Scale (HCSS). The aim was to assess content, structural and concurrent validity of the developed instrument, and to examine differential item functioning (DIF) as a function of sex. Three different phases were completed in this study. The first phase involved focus groups (n = 5) that explored humor as communication in a team sport context. In phase 2, information from the focus groups was used to create a pool of potential items for the questionnaire. Two discussion groups with sport science students contributed to the development of 80 potential items, that two different expert groups then assessed for item quality. The final version of the instrument after this phase contained 14 items, representing three different humor dimensions. In phase 3, two independent samples with a total number of 776 participants were recruited for the psychometric evaluation of the instrument. EFA, ICM-CFA, and ESEM analysis were performed, supporting a three-factor structure with positive humor, negative humor in-group, and negative humor out-group. In addition, partial DIF as a function of sex on the negative humor dimensions was found, indicating differences in how male and female interpret the negative humor items. The findings in the current study expand our understanding of humor in sport teams and may be a starting point for further research on humor climate in sport teams and its role in group function.

Dentin dysplasia is a rare hereditary disorder, transmitted by autosomal dominant mode, affecting both dentin and pulp. In Type I crown morphology is normal, but root dentin organization loss leads to shorter roots. Mutations in the SSUH2, VPS4B and SMOC2 genes have been reported as responsible for this condition. Orthodontic treatment was conducted on an 11-year-old female patient presenting the disorder along with bilaterally impacted permanent maxillary canines, in close proximity to the roots of the lateral and central incisors. Treatment plan included lateral incisors extraction, surgical exposure and traction of the impacted canines. Light forces were applied from a custom-made trans-palatal arch. Comprehensive orthodontic treatment was performed using edgewise appliances. After 3 years and 2 months, group function occlusion was achieved. The canines underwent composite resin restorations. At one year post-retention, the dentition remained stable. Family-based genetic analysis did not reveal any mutations in the aforementioned genes pointing to further genetic heterogeneity of this disorder. As dental medicine becomes more sophisticated and personalized, the association between mutation type/function and orthodontic treatment response may provide useful therapeutic insights. The positive treatment response of the presented case could be attributed to a more “benign” mutation awaiting to be identified.

Abstract Background Osteoarthritis of the knee is a common degenerative joint disorder in our ageing population. A combination of thermal therapy with a self-management exercise have shown a positive effect in the management of osteoarthritis of the knee. This study aimed to compare the effectiveness of topical heat pack versus focal application of heat therapy at the acupressure points in the treatment of osteoarthritis of the knee. Methods A randomized controlled trial was conducted in 76 patients with osteoarthritis of the knee, diagnosed by an experienced orthopedic surgeon. Following inclusion and exclusion selection, patients were randomly allocated to group 1 (Heat pack) or group 2 (Thermal gun). All patients received 30 min of treatment in each session, twice a week for 4 weeks. They also received an education program and taught home knee exercises. Outcome measurements were the visual analog scale (VAS) for pain intensity, muscle power, knee ROM, WOMAC and SF-12v2. Results In the Thermal gun group, function and total scores (WOMAC) and Physical Composite Scale (SF-12v2) were significantly improved after 8 sessions. Quadriceps strength was significantly improved after 8 weeks (from 4.42 to 4.63; p = 0.02). In the Heat pack group, flexion was significantly improved after 8 sessions (p = 0.02). Mean VAS scores after Heat pack treatment was consistently better (lower) than mean VAS scores after Thermal gun treatment. Conclusion The combination of focal thermal therapy at acupressure points is a viable conservative treatment in osteoarthritis of the knee. The pressure at the acupressure points has a synergistic benefit than topical thermal therapy alone. The advancement of science has made computer technology and the education industry more and more closely related, and the development of intelligent teaching systems has also opened a new path for classroom teaching. This paper studies the application of fuzzy control based on genetic algorithms in the intelligent psychology teaching system. Facing the complicated variables in the teaching process, the improved genetic algorithm can better realize dynamic teaching decisions through fuzzy control. This article aims to improve the quality of psychology classroom teaching, and develops an intelligent psychology teaching system based on the fuzzy control theory of genetic algorithm. Combined with the current development of fuzzy control theory, the problems existing in the intelligent teaching system are studied and analyzed, and they have been optimized and improved. This paper proposes a control algorithm based on a teaching management system. The algorithm can implement fuzzy control on student models, knowledge organization structure, intelligent test papers and teaching decision-making. While restoring the real teaching process, it can better realize teaching students in accordance with their aptitude and improve teaching. The intelligence of the system. According to the system test data, the proportions of the difficulty of the system’s automatic test paper are 30.1%, 51.6%, 18.3%, which are in line with the designer’s set expectation of 3 : 5:2, which shows the improved genetic algorithm. It can realize the intelligent volume group function very well.

Abstract Background Collaboration among different categories of health professionals is essential for quality patient care, especially for individuals with cleft lip and palate (CLP). This study examined interprofessional collaboration (IPC) among health professionals in all CLP specialised centres in South Africa’s public health sector. Methods During 2017, a survey was conducted among health professionals at all the specialised CLP centres in South Africa’s public health sector. Following informed consent, each member of the CLP team completed a self-administered questionnaire on IPC, using the Interprofessional Competency Framework Self-Assessment Tool. The IPC questionnaire consists of seven domains with 51 items: care expertise (8 items); shared power (4 items); collaborative leadership (10 items); shared decision-making (2 items); optimising professional role and scope (10 items); effective group function (9 items); and competent communication (8 items). STATA®13 was used to analyse the data. Descriptive analysis of participants and overall mean scores were computed for each domain and analysed using ANOVA. All statistical tests were conducted at 5% significance level. Results We obtained an 87% response rate, and 52 participants completed the questionnaire. The majority of participants were female 52% (n = 27); with a mean age of 41.9 years (range 22–72). Plastic surgeons accounted for 38.5% of all study participants, followed by speech therapists (23.1%), and professional nurses (9.6%). The lowest mean score of 2.55 was obtained for effective group function (SD + -0.50), and the highest mean score of 2.92 for care expertise (SD + -0.37). Explanatory factor analysis showed that gender did not influence IPC, but category of health professional predicted scores on the five categories of shared power (p = 0.01), collaborative leadership (p = 0.04), optimising professional role and scope (p = 0.03), effective group function (p = 0.01) and effective communication (p = 0.04). Conclusion The seven IPC categories could be used as a guide to develop specific strategies to enhance IPC among CLP teams. Institutional support and leadership combined with patient-centred, continuing professional development in multi-disciplinary meetings will also enrich IPC.

How individuals in a group lead to collective behavior is a fundamental question across biological systems, from cellular systems, to animal groups, to human organizations. Recent technological advancements have enabled an unprecedented increase in our ability to collect, quantify, and analyze how individual responses lead to group behavior. However, despite a wealth of data demonstrating that collective behavior exists across biological scales, it is difficult to make general statements that apply in different systems. In this perspective, we present a cohesive framework for comparing groups across different levels of biological organization, using an intermediate link of “collective mechanisms” that connects individual responses to group behavior. Using this approach we demonstrate that an effective way of comparing different groups is with an analysis hierarchy that asks complementary questions, including how individuals in a group implement various collective mechanisms, and how these various mechanisms are used to achieve group function. We apply this framework to compare two collective systems—cellular systems and honey bee colonies. Using a case study of a response to a disturbance, we compare and contrast collective mechanisms used in each system. We then discuss how inherent differences in group structure and physical constraints lead to different combinations of collective mechanisms to solve a particular problem. Together, we demonstrate how a hierarchical approach can be used to compare and contrast different systems, lead to new hypotheses in each system, and form a basis for common research questions in collective behavior.

Abstract Background This article systematically summarizes the present evidences, published in the last 20 years, regarding the pre- and post-operative factors, which may influence recovery of the function of the quadriceps muscle group following total knee arthroplasty (TKA). We following the PRISMA methodology, including meta-analyses and high-level evidence studies (prospective trials, and, when unavailable, retrospective studies). Sources of data Pubmed and Cochrane databases: 582 articles were identified and 54 of them were selected. Areas of agreement Tourniquets inflated at high pressure exert a detrimental effect on the quadriceps muscle group. Faster quadriceps recovery takes place using mini-invasive approaches, an eight-week rehabilitation period including balance training and the use of nutraceuticals. Areas of disagreement Pre-habilitation and pre-operative factors, analgesic methods and different TKA implants. Growing points and areas timely for developing research Telerehabilitation seems a cost-effective tool for rehabilitation after TKA. Patients’ optimization protocols before TKA should include standardized nutraceuticals intake.
