Abstract Background Childhood encephalitis causes severe morbidity and mortality. Difficulty identifying causative organisms and a lack of effective therapies leads to variability in management. The recent emergence of novel diagnostic tools and the increased recognition of auto-immune encephalitides has the potential to change the approach to this disease. Our objective was to assess the current evaluation and management of childhood encephalitis. Methods An 11-question confidential, web-based survey was distributed by the Emerging Infections Network (EIN) of the Infectious Disease Society of America to 370 Pediatric Infectious Disease (ID) physicians between January 29th and February 17th, 2020. Respondents were characterized by practice region, experience since fellowship, place of employment and primary hospital type. Responses were analyzed with SAS, v 9.4. Results Responses were obtained from 222 of 370 members (60%) (Table). Of the 222 respondents, 196 (88%) reported caring for children with suspected encephalitis and form the basis for this report. Multi-plex PCR testing of cerebrospinal fluid (CSF) in the initial evaluation of most children with suspected encephalitis was reported by 56% (with 65% of these respondents reporting the use of pathogen-specific confirmatory testing). CSF metagenomic next generation sequencing (mNGS) had been used by 47%, with 74% of all respondents stating they would use this test if likely diagnoses were excluded via standard testing (and 64% of these only using the test if the child was not improving). Variability in the interpretation of negative CSF mNGS results was reported (Fig 1a). Primary involvement of ID physicians in the diagnostic evaluation of auto-immune encephalitis was reported by 33%, yet only 55% of ID physicians reported feeling comfortable diagnosing this condition. Marked variation existed regarding criteria for the use of immunomodulating agents (Fig 1b) and the need for testing for autochthonous tropical viruses in the U.S. (Fig 1c). Abstract We surveyed pediatric infectious disease physicians through the Infectious Disease Society of America’s Emerging Infections Network regarding the diagnosis and management of encephalitis. We identified practice variations, particularly with the use of new diagnostic modalities and management of autoimmune encephalitides. These findings may inform the creation of updated management guidelines.

Covid -19 is an infectious disease caused by the newly discovered strain of coronavirus. As there is no vaccine discovered, the only way to prevent the spread is through following the practice of social isolation. But prolonged isolation may also lead to psychological stress and problems. The objective of the survey was to assess the knowledge and awareness of preventive measures against Covid 19 amongst small shop owners. A web-based cross-sectional study was conducted amongst the small shop owners.  A structured questionnaire comprising 15-17 questions had been put forth to assess the Covid 19 related knowledge and perception. The shopkeepers were contacted telephonically and responses recorded. The data analysis was performed using IBM SPSS statistics. Although the majority of the population had a positive perception about the preventive measures against the Covid spread, 36% of the shopkeepers were not aware of the preventive measures against the Covid spread. This study found optimal knowledge and perception of the preventive measures against Covid spread among the shopkeepers but misinformation and misunderstanding still prevailing. The shopkeepers are crucial in the prevention of the spread of Covid 19 and educating them might aid us in the fight against Covid- 19.

BACKGROUND Lung transplantation recipients (LTx) are more susceptible to severe acute respiratory syndrome-corona virus-2 (SARS-Cov-2) and suffer severer outcomes than healthy subjects. OBJECTIVE Here we aim to analyze whether it was appropriate to maintain lung transplant programs in medical institutions accepting coronavirus disease 2019 (COVID-19) patients. METHODS Methods: the clinical characteristics, laboratory testing, and epidemiology survey results of 10 LTx recipients undergoing allograft lung transplantation surgeries in the First Affiliated Hospital of Zhengzhou University during the COVID-19 pandemic were collected. A web-based epidemiology questionnaire was used to collect the information of LTx recipients after discharge. RESULTS A total of 10 LTx recipients were identified. The main cause of lung transplantation was idiopathic interstitial pneumonia (60%), with another rare case of cystic fibrosis. Comorbidities involved hyperlipidemia, subclinical hyperthyroidism, diabetes, viral hepatitis of type B. The average white blood cell (WBC) count and average lymphocyte count were 9.5±3.9×109 cells/liter and 1.7±1.1×109 cells/liter, respectively. 40% of the LTx recipients had lymphopenia. Impaired alanine aminotransferase (ALT) and aspartate transaminase (AST) were observed in LTx recipients. Good habitats of hand hygiene (100%), wearing protective masks behaviors (100%), indoor ventilation behaviors (100%), indoor disinfection measures (83%), personal tableware (67%), separate room (100%), personal bedsheets/ quilts (100%) and drinking glasses (100%) were observed during the follow-up. None of the LTx recipients or their family members get infected with SARS-CoV-2 during the novel coronavirus pandemic. CONCLUSIONS Under the premise of taking appropriate preventive measures during hospitalization and after discharge, the lung transplant program can be maintained in the medical institution that accepts patients with COVID-19. Heat-related illnesses (HRIs), such as heatstroke (HS) and heat exhaustion (HE), are common complications during Hajj pilgrims. The Saudi Ministry of Health (MoH) developed guidelines on the management of HRIs to ensure the safety of all pilgrims. This study aimed to assess healthcare workers’ (HCWs) adherence to the updated national guidelines regarding pre-hospital and in-hospital management of HRIs. This was a cross-sectional study using a questionnaire based on the updated HRI management interim guidelines for the Hajj season. Overall, compliance with HE guidelines scored 5.5 out of 10 for basic management and 4.7 out of 10 for advanced management. Medical staff showed an average to above average adherence to pre-hospital HS management, including pre-hospital considerations (7.2), recognition of HS (8.1), case assessment (7.7), stabilizing airway, breathing, and circulation (8.7), and cooling (5). The overall compliance to in-hospital guidelines for HS management were all above average, except for special conditions (4.3). In conclusion, this survey may facilitate the evaluation of the adherence to Saudi HRIs guidelines by comparing annual levels of compliance. These survey results may serve as a tool for the Saudi MoH to develop further recommendations and actions.

When controlling an emerging outbreak of an infectious disease, it is essential to know the key epidemiological parameters, such as the basic reproduction number R 0 and the control effort required to prevent a large outbreak. These parameters are estimated from the observed incidence of new cases and information about the infectious contact structures of the population in which the disease spreads. However, the relevant infectious contact structures for new, emerging infections are often unknown or hard to obtain. Here, we show that, for many common true underlying heterogeneous contact structures, the simplification to neglect such structures and instead assume that all contacts are made homogeneously in the whole population results in conservative estimates for R 0 and the required control effort. This means that robust control policies can be planned during the early stages of an outbreak, using such conservative estimates of the required control effort.

This study explored the impact of recent demographic changes on clinical service delivery in the state of Minnesota. A Web-based survey was used to ask speech-language pathologists in Minnesota about their training, clinical caseloads, and professional experiences with respect to diversity. Primary survey goals were to (a) determine if, and how, recent changes in the overall state demographics were reflected on caseloads of speech-language pathologists, and (b) identify challenges clinicians faced in meeting the needs of an increasingly diverse population. The authors considered the breadth or range of economically, racially, ethnically, and linguistically diverse populations represented on caseloads, relative to depth of diversity. Survey results are discussed with respect to challenges inherent in providing services to a diverse caseload, together with the competencies needed in order to meet these professional challenges.

Extant literature has demonstrated that suicide safety planning is an efficacious intervention for reducing patient risk for suicide-related behaviors. However, little is known about factors that may impact the effectiveness of the intervention, such as provider training and comfort, use of specific safety plan elements, circumstances under which providers choose to use safety planning, and personal factors which influence a provider’s decision to use safety planning. Participants were (N = 119) safety plan providers who responded to an anonymous web-based survey. Results indicated that most providers had received training in safety planning and were comfortable with the intervention. Providers reported that skills such as identifying warning signs and means safety strategies were routinely used. Providers who reported exposure to suicide were more likely to complete safety plans with patients regardless of risk factors. In addition, almost 70% of providers indicated a need for further training. These data provide important considerations for safety plan implementation and training.
