The Oxford Handbook of Digital Technologies and Mental Health

This book provides an academically oriented and scientifically based description of how technological advances may have contributed to a wide range of mental health outcomes, covering the spectrum from problems and maladies to improved and expanded healthcare services. Digital technologies covered include internet use (including social networking, shopping, gambling, viewing of pornography, etc.), digital gaming, smartphones, mobile applications, virtual reality, and computer-based therapeutic interventions. Both the positive and negative aspects of these various types of digital technologies are discussed. Further, some chapters include a discussion of future developments in the field. The book has been written by international experts in the applications of digital technology across many countries and quality universities. This book consists of eight sections. The first focuses on digital technology in the general population. The second covers problematic conceptual issues in the impact of technology use. The third discusses potential positive impacts of digital technology. The fourth covers potential negative impacts of digital technologies. The fifth evaluates utilizing digital technologies for assessment and legal considerations. The sixth explores using digital technologies to improve overall well-being. The seventh discusses the use of digital technologies to treat specific disorders, and the eighth presents a discussion on treatments of problematic technology use. The book concludes with a brief summary as well as a discussion of future directions for digital technologies.

Although the time adolescents spend with digital technologies has sparked widespread concerns that their use might be negatively associated with mental well-being, these potential deleterious influences have not been rigorously studied. Using a preregistered plan for analyzing data collected from a representative sample of English adolescents ( n = 120,115), we obtained evidence that the links between digital-screen time and mental well-being are described by quadratic functions. Further, our results showed that these links vary as a function of when digital technologies are used (i.e., weekday vs. weekend), suggesting that a full understanding of the impact of these recreational activities will require examining their functionality among other daily pursuits. Overall, the evidence indicated that moderate use of digital technology is not intrinsically harmful and may be advantageous in a connected world. The findings inform recommendations for limiting adolescents’ technology use and provide a template for conducting rigorous investigations into the relations between digital technology and children’s and adolescents’ health.

BACKGROUND A growing number of apps to support good mental health and well-being are available on digital platforms. However, very few studies have examined older adults’ attitudes toward the use of these apps, despite increasing uptake of digital technologies by this demographic. OBJECTIVE This study sought to explore older adults’ perspectives on technology to support good mental health. METHODS A total of 15 older adults aged 50 years or older, in two groups, participated in sessions to explore the use of digital technologies to support mental health. Interactive activities were designed to capture participants’ immediate reactions to apps and websites designed to support mental health and to explore their experiences of using technology for these purposes in their own lives. Template analysis was used to analyze transcripts of the group discussions. RESULTS Older adults were motivated to turn to technology to improve mood through mechanisms of distraction, normalization, and facilitated expression of mental states, while aiming to reduce burden on others. Perceived barriers to use included fear of consequences and the impact of low mood on readiness to engage with technology, as well as a lack of prior knowledge applicable to digital technologies. Participants were aware of websites available to support mental health, but awareness alone did not motivate use. CONCLUSIONS Older adults are motivated to use digital technologies to improve their mental health, but barriers remain that developers need to address for this population to access them.

Psychological well-being is a major global concern receiving more scholarly attention following the 2008 Great Recession, and it becomes even more relevant in the context of COVID-19 outbreak. In this study, we investigated the impact of economic uncertainty resulting from natural disasters, epidemics, and financial crisis on individuals’ mental health. As unemployment rate exponentially increases, individuals are faced with health and economic concerns. Not all society members are affected to the same extent, and marginalized groups, such as those suffering from chronic mental illnesses or low-income families cannot afford the downsizing, mass lay-offs and lack of access to public health services. Psychiatric profession is familiarized with the phenomenon of intolerance of uncertainty (IU), and we examine how this concept is associated with job uncertainty and social identity disturbance. Several studies have formally investigated the effects of IU, but to our knowledge, this is the first research integrating the psychological well-being, job uncertainty and identity disturbance caused by economic breakdown. Literature points to many reported cases of PTSD, anxiety, depression and suicidal tendencies following major social disasters. Yet, we have undertaken to analyze the subjective experiences underlying the self-harming behaviors in an attempt to fill the methodological gap by drawing insights from prominent psychological, sociological and economic theories. We find economic uncertainty to have a positive relation to job uncertainty and identity disturbance, and a negative relationship with psychological well-being. Psychological well-being depends on coherency between both abstract subjective and concrete objective identity, and when these perceptions are inconsistent, cognitive dissonance arises resulting in identity disturbance. We argue that stability is not associated with monetary advantage only, but also with a wide range of other benefits that are crucial for individuals’ growth, satisfaction and sense of identity. Therefore, we propose the implementation of social support and public welfare policies to mitigate health risks during the turbulent socio-economic changes.

IntroductionResearch indicates that stressors introduced by the COVID-19 pandemic have negatively impacted mental health, particularly among young people.1 Time spent on digital technology (e.g., social media, smartphones) has also increased2 as schools, workplaces, and social gathering sites have closed, thus intensifying pre-pandemic concerns regarding the putative effects of digital technology use (DTU) on mental health. Indeed, recent academic and newspaper articles have both directly and indirectly asserted that increased DTU is a source of the heightened psychological distress observed during the pandemic.3–5 However, these claims are dubious for two primary reasons. First, these articles rely on self-report measures of DTU, which are inaccurate6 and prone to systematic bias.7 Second, since the pandemic has impacted both mental health and DTU for many, the observed association between the two may be attributable to a shared common cause, rather than causality. Thus, we investigated the longitudinal associations between objectively measured DTU and mental health while accounting for important COVID-19-related effects.MethodsThis study was approved by the University of Pittsburgh and followed Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) reporting guidelines. In this four-wave panel study, participants were recruited from Prolific (https://www.prolific.co/), an online participant-recruitment platform. Waves of data collection were launched on August 14, September 12, October 14, and November 9 of 2020. Eligible participants were U.S. residents, 18-35 years old, iPhone users, and had ≥ 10 previous submissions on Prolific with approval rating ≥95%. At each wave, participants uploaded screenshots of their “Screen Time” application (which passively tracks device usage) and completed self-reports of mental health (depression, anxiety, suicidal ideation [SI]), COVID-19-related stressors, and perceived COVID-19-related impact on well-being and DTU (Table 1). We extracted three elements from the “Screen Time” screenshots: (1) total screen time, (2) total time spent on social media, and (3) total number of pickups. We estimated separate random-intercept multilevel models for each mental health outcome using Mplus. Predictors were entered hierarchically in blocks (see eTable 3 in Supplement) to assess ΔR2 at the within- and between-person levels. See Supplement for methodological details. ResultsA total of 384 young adults participated in this study (Mage = 24.5, SDage = 5.1; 57% female; 54% white; 48% Bachelor’s degree education or above). Overall, participants averaged 47.5 hours of Screen Time, 677 pickups, and 15.5 hours of social media over the past week. On average, participants reported experiencing between 4 and 5 pandemic-related stressors per wave. Mean depression and anxiety t-scores were 54.6 and 56.7, respectively, and nearly 29% of participants reported past-week SI at least once. See eTable 1 for summary statistics of sample demographics and primary variables.Results of the multilevel analyses revealed that objectively-measured total screen time and social media use were unrelated to within- or between-person differences in mental health, while between-person difference in pickups was negatively associated with depression (see Figure 1). Together, the objective DTU variables explained, at most, 2.8% of the within- or between-person variance in any of the mental health outcomes (eTable 3 in Supplement). COVID-related impacts on well-being had the largest effects across models—accounting for about 45% and 10%, respectively, of the between- and within-person variance in depression and anxiety, and 21%/28% of the between/within variance in SI. DiscussionAmong a sample of young adults, a population with particularly high rates of DTU8 and COVID-19-related distress,1 we found that objectively-measured DTU did not contribute to increases in depression, anxiety, or SI—refuting the popular notion that increases in DTU may be contributing to young peoples’ psychological distress during the pandemic. Rather, depression, anxiety, and SI were driven mostly by young peoples’ reports of the pandemic’s impact on their well-being. The convenience-based sample, retrospective (past week) assessments of mental health outcomes, and single-item measures of COVID-19-related impacts are limitations of the study. Nevertheless, results indicate that current speculations about the direct harms of DTU on mental health may be unfounded and risk diverting attention from a more likely cause: pandemic-related stressors.

This review provides an overview of the literature regarding digital technology use and adolescent well-being. Overall, findings imply that the general effects are on the negative end of the spectrum but very small. Effects differ depending on the type of use: whereas procrastination and passive use are related to more negative effects, social and active use are related to more positive effects. Digital technology use has stronger effects on short-term markers of hedonic well-being (eg, negative affect) than long-term measures of eudaimonic well-being (eg, life satisfaction). Although adolescents are more vulnerable, effects are comparable for both adolescents and adults. It appears that both low and excessive use are related to decreased well-being, whereas moderate use is related to increased well-being. The current research still has many limitations: High-quality studies with large-scale samples, objective measures of digital technology use, and experience sampling of well-being are missing.

BACKGROUND Digital technologies have been used to support mental health services for two decades, but the COVID-19 pandemic creates a unique opportunity for greater utilization and more data-driven assessment of these digital technologies. OBJECTIVE This research aims to provide a systematic review of the characteristics and effectiveness of digital inventions that were employed to improve mental health in the real context of COVID-19 pandemic. METHODS A combination of search terms was applied for automatic search of publications in the relevant databases. The key features of included studies were extracted, including the intervention, participant, and study details. RESULTS A total of 14 eligible studies were included in the final review, which were conducted across different geographic regions and among diverse cultural groups. Ten studies mainly examined the impact of digital technologies on healthy individuals, while only one study developed specific interventions for the isolated COVID-19 depressed patients in hospitals. Digital technologies identified in this study mainly include web-based interventions, smartphone or social media applications such as SMS messages, mobile games, and even video conferencing app. But only five of them were aligned with theoretical approaches from standardized psychological treatments. Most of the studies have reported positive effects of digital technologies, either on improving general mental and emotional well-being or addressing specific conditions (e.g., depression, loneliness). CONCLUSIONS This systematic review suggests that digital technologies hold promise in bridging the mental health-care gap during and after the COVID-19 pandemic, and calls for more rigorous studies to identify pertinent features that are likely to achieve more effective mental health outcomes.

AbstractThere has been significant disruption to the lives and mental health of adolescents during the COVID-19 pandemic. The purpose of this study was to assess the psychological and lifestyle impact of the pandemic on Australian adolescents, using an online survey, administered during the outbreak. Self-report surveys were administered online to a sample of 760 Australian adolescents aged 12–18 years assessing impact on a range of domains including behaviour, education, relationships, lifestyle factors (exercise, technology use, and sleep), and mental health outcomes (psychological distress, loneliness, health anxiety and well-being). Results showed that three quarters of the sample experienced a worsening in mental health, since the pandemic began, with negative impacts reported on learning, friendships and family relationships. There were also high higher levels of sleep disturbance, psychological distress and health anxiety, relative to normative samples. Effects on mental health were worse among those who reported a previous diagnosis of depression and/or anxiety relative to those without no such history. Adolescents are already vulnerable to the onset of mental illness at this developmental stage, and the current research underscores the need to find rapid and accessible ways to support adolescent mental health during times of crisis.

Digital technologies that create innovation have the capacity to leverage economic development. The integration of digital technologies in daily operations has become a key success factor in sustainable development, market positioning, and advancement. This is particulary challenging for female companies in developing countries, which face numerous biases on their entrepreneurial path, including in innovation. This specific group of entrepreneurs faces additional barriers in implementing digital technologies and digital transformation, which are considered part of their innovation capacity. However, some companies perform well according to financial parameters, regardless of the level of digitization and innovation potential. This research paper explores the correlation between financial performance and the innovative implementation of digital technologies. The main assumption in this research is that companies that do not use digital technologies to innovate can not expect to improve their financial performance. We focus on the impact of digital technology on the financial performance of female companies. We also consider the indirect impact of digital technology use on financial performance, with product innovation as the mediator variable. The sample consists of 46 female companies. The results of the analysis confirm the hypotheses that digital technology has no direct influence on financial performance, and that product innovation mediates the relationship between digital technology and financial performance.

  Since the evolution of the internet in 1969 and the inception of the personal computer in the early 1970s, a significant body of research has emerged that highlights the impacts of digital technology on education, health and human development. There is extensive research examining the physical, mental, and social health effects on young people and adolescents as well as the impacts of digital technology on their educational achievements. The aim of the review reported on here was to examine the impact of digital technology, and more specifically the use of Bring Your Own Device (BYOD) policies in schools. BYOD allows students to bring phones, tablets and personal computers to school. Numerous components with a negative impact on physical health have been identified. Outcomes of this study will be of interest to school leaders as policy makers. There is a need for trustworthy information on which to base reviews and revisions of school policy to ameliorate the risks of digital technology. This research report is also of relevance for tertiary providers and the government with the implementation of more online learning in class and online distance learning during COVID-19 restrictions.
