Case Study: Nutrition and Hydration Status during 4,254 km of Running Over 78 Consecutive Days

The aims of this study were to assess the dietary intake and monitor self-reported recovery quality and clinical symptomology of a male ultra-endurance runner who completed a multiday ultra-endurance running challenge covering 4,254 km from North Scotland to the Moroccan Sahara desert over 78 consecutive days. Food and fluid intakes were recorded and analyzed through dietary analysis software. Body mass (BM) was determined before and after running each day, and before sleep. Clinical symptomology and perceived recovery quality were recorded each day. Whole blood hemoglobin and serum ferritin were determined before and after the challenge. Total daily energy (mean ± SD: 23.2 ± 3.2MJ·day−1) and macronutrient intake (182 ± 31g·day−1 protein, 842 ± 115g·day−1 carbohydrate, 159 ± 55 g·day−1 fat) met consensus nutritional guidelines for endurance performance. Total daily water intake through foods and fluids was 4.8 ± 2.0L·day−1. Water and carbohydrate intake rates during running were 239 ± 143ml·h−1 and 56 ± 19g·h−1, respectively. Immediately after running, carbohydrate and protein intakes were 1.3 ± 1.0g·kg BM−1 and 0.4 ± 0.2g·kg BM−1, respectively. Daily micronutrient intakes ranged from 109 to 662% of UK RNIs. Prerunning BM was generally maintained throughout. Overall exercise-induced BM loss averaged 0.8 ± 1.0%; although BM losses of ≥ 2% occurred in the latter stages, a reflection of the warmer climate. Varying degrees of self-reported perceived recovery quality and clinical symptomology occurred throughout the challenge. This case study highlights oscillations in dietary habits along 78 consecutive days of ultra-endurance running, dependent on changes in ambient conditions and course topography. Nevertheless, nutrition and hydration status were maintained throughout the challenge. Despite dietary iron intake above RNI and iron supplementation, this alone did not prevent deficiency symptoms.

Pacing has been investigated in elite and master runners competing in marathon and ultra-marathon races up to 100 km and 100 miles, but not in longer ultra-marathons. In this case study, a 54-year-old master ultra-marathoner—intending to achieve as many kilometers as possible in a 48 h run—was examined. The changes in running speed during the race and selected anthropometric characteristics using bioelectrical impedance analysis (i.e., body mass and body water), during and after the race, were analyzed. The runner achieved a total distance of 230 km and running speed decreased non-linearly during the race. Body mass decreased, while percent body water increased, non-linearly, across the race. There was no statistically significant relationship between the decrease in body mass and the increase in percent body water. Considering the popularity of ultra-endurance running races, the findings of the present study offered valuable insight in the pacing and changes of body mass and body water during a 48 h run, and this information can be used by ultra-endurance runners and practitioners working with them.

The Rioja Bike Race is an individual mountain bike competition that is carried out in 3 stages, with variable distances (60-83km). The event provides refreshment areas and technical assistance to meet the nutrition and hydration needs of the participants. The dietary-nutritional preparation is of great importance in order to reduce or even prevent gastrointestinal problems, dehydration, hyponatremia, fatigue, among others. The aim of this case study is to describe the dietary and nutritional planning of an athlete in this event. Test subject: Male, 37 years old and 10 years of sports experience and very important sport achievements. His eating habits, supplement intake and body composition were evaluated. For nutrition planning of the Rioja Bike Race, we took into account the dietary and nutritional recommendations for athletes in competition, test time, refreshment areas and possible food intake / supplements. The dietary- utritional planning in ultra-endurance events is important for successful performance in the competition, to decrease nutritional risk and avoiding dehydration, fatigue, gastrointestinal disturbances, etc.

Purpose: Sleep deprivation (SD) is very common during ultraendurance competitions. At present, stimulants such as caffeine and naps are the main strategies used to reduce the negative effects of SD on ultraendurance performance. In this case study, the authors describe the application of a novel strategy consisting of the intermittent repetition of SD (SD training [SDT]) during the weeks preceding an ultraendurance competition. Methods: A male ultraendurance runner underwent a 6-week SDT program (consisting of 1 night SD every Sunday) in addition to his regular physical training program before taking part in a 6-day race. Before and after SDT, the participant performed 5 consecutive days of daily 2-hour constant-pace running with SD on the first and third night. Psychological and physiological responses were measured during this multiday test. Results: SDT was well tolerated by the athlete. A visual analysis of the data suggests that including SDT in the weeks preceding an ultraendurance competition may have beneficial effects on sleepiness and perceived mental effort in the context of 5 consecutive days of prolonged running and 2 nights of SD. This multiday test seems a feasible way for assessing ultraendurance athletes in the laboratory. Conclusions: The results provided some encouraging initial information about SDT that needs to be confirmed in a randomized controlled trial in a group of ultraendurance athletes. If confirmed to be effective and well tolerated, SDT might be used in the future to help ultraendurance athletes and other populations that have to perform in conditions of SD.

AbstractThis study aimed to investigate training-induced fitness changes and their relationship with training-competition load during half a soccer season (18 wks). Training load [heart rate (HR) and ratings of perceived exertion (RPE)] and match time were monitored, including 108 training (3 223 individuals) and 23 match sessions, in 38 youth elite male soccer players. Fitness variables were assessed before and after the study. Yo-Yo intermittent recovery test 1 (Yo-Yo IRT1) improved (P<0.001; 90%CI: 418–632 m; ES: 2.14). Anthropometrical, jump, sprint, and change-of-direction measures remained unchanged. Jump test correlated with sprint (r=0.74; P<0.001; SEE=3.38 m·s−1) and Yo-Yo IRT1 (r=−0.58; P=0.005; SEE=4.11 m) tests. Initial sum of 6 skinfolds was associated with changes in this same measure (r=−0.51; P<0.001; SEE=21%). Initial Yo-Yo IRT1 results were related to changes in Yo-Yo IRT1 (r=−0.84; P<0.001; SEE=10%) and match time played (r=0.44; P=0.033; SEE=445 m). Mean RPE records were related to training spent within 75–90% maximal HR (r=0.54; P<0.001; SEE=4%). The half-season was beneficial for endurance running performance but not for lower-limb strength-velocity production capacity. The more aerobically deconditioned players played fewer minutes of match, although they showed the greatest improvements in endurance performance. Non-soccer-specific, scientifically based, and individualized fitness programs in addition to soccer-specific training are recommended.

ABSTRACT Introduction: The popularity of ultra-endurance events is increasing worldwide. However, information about hematological parameters during repeated bouts of long-duration running, such as stages during a multi-stage ultramarathon (MSUM) is scarce. Objective: The purpose of the study was to monitor daily hematologic assessment in a 230-km multistage ultramarathon (MSUM) in hot environmental conditions. Methods: Eleven male and three female (n = 14) ultra-endurance runners entered this MSUM, which was conducted over five stages, covering a total distance of 230 km in hot ambient conditions. Peripheral blood samples were collected before and after each stage to determine leukocyte and erythrocytes parameters. Results: Multilevel modelling considering all measurement points showed an increase for whole blood leukocyte and granulocyte counts at five stages of the MSUM and for monocytes until Stage 3 of the race. For erythrocyte parameters, decreases across stages were observed in hemoglobin concentration and hematocrit responses when computing/considering all measurement points. Conclusions: The results indicate that MSUM in a hot environment leads to a greater impact on leukocyte population responses and platelet variation in the initial stages of the race. We suggest that athletes may have a decrease in immune function in the early stages of the MSUM, probably with some transient risk of infectious. Nevertheless, a physiological adaptation to physical exertion and heat mitigates these changes in the subsequent stages of MSUM. Level of Evidence III; Case-control study.

Although dehydration of ≥ 2% body weight (BW) loss significantly impairs endurance performance, dehydration remains prevalent among athletes and may be owing to a lack of knowledge in relation to fluid requirements. The aim of this study was to assess the hydration status of university/club level athletes (n = 430) from a range of sports/activities (army officer cadet training; bootcamp training; cycling; Gaelic Athletic Association camogie, football and hurling; golf; hockey; netball; rugby; running (sprinting and endurance); Shotokan karate and soccer) immediately before and after training/competition and to assess their nutritional knowledge. Urine specific gravity (USG) was measured immediately before and after exercise and BW loss during exercise was assessed. Nutritional knowledge was assessed using a validated questionnaire. 31.9% of athletes commenced exercise in a dehydrated state (USG >1.020) with 43.6% of participants dehydrated posttraining/competition. Dehydration was particularly prevalent (>40% of cohort) among karateka, female netball players, army officer cadets, and golfers. Golfers that commenced a competitive 18 hole round dehydrated took a significantly higher number of strokes to complete the round in comparison with their euhydrated counterparts (79.5 ± 2.1 vs. 75.7 ± 3.9 strokes, p = .049). Nutritional knowledge was poor among participants (median total score [IQR]; 52.9% [46.0, 59.8]), albeit athletes who were euhydrated at the start of exercise had a higher overall score in comparison with dehydrated athletes (55.2% vs. 50.6%, p = .001). Findings from the current study, therefore, have significant implications for the education of athletes in relation to their individual fluid requirements around exercise.

Ultra-endurance running provides numerous physiological, psychological, and nutritional challenges to the athlete and supporting practitioners. We describe the changes in physiological status, psychological condition, and nutritional intake over the course of two 100-mile running races, with differing outcomes: non-completion and completion. Athlete perception of pain, freshness, and motivation differed between events, independent of rating of perceived exertion. Our data suggest that the integration of multiple sensations (freshness, motivation, hunger, pain, and thirst) produce performance. Increases in carbohydrate feeding (+5 g·h−1) and protein intake (+0.3 g·kg−1) also likely contributed to successful completion of a 100-mile race, by reducing the fractional utilization of maximal oxygen uptake and satiating hunger, respectively. Nutritional data support the notion that the gut is a trainable, and critical organ with respect to ultra-endurance performance. Finally, we propose future research to investigate the rate at which peak feeding occurs throughout ultra-endurance events, as this may further serve to personalize sports nutrition strategies.

The present industrial food-production system is not suitably ecological for the environment. Mindful nutrition in sport is a relevant emergent sub-discipline that could help reduce environmental degradation. This case study describes a sustainable support diet during an ultra-endurance running (UR) event called the “Indoor Everest Challenge”. This UR challenge involved attaining the altitude of Mount Everest (8849 m) in a simulated way, in less than 24 h, without using ultra-processed food and without wasting plastics. During this challenge, a male athlete (34 years, weight: 78 kg, and height: 173 cm) wore a SenseWear Armband® (BodyMedia Inc., Pittsburg, PA, USA) accelerometer on his right arm to estimate energy expenditure. To supply his nutritional requirements, the athlete consumed only specially prepared homemade and organic food. All consumption was weighed and recorded in real-time; we determined nutrients using two databases: a food composition software, Dial Alce Ingenieria® (Madrid, Spain), to measure energy and macro- and micro-nutrients, and Phenol Explorer Database® (INRA Institut National de Recherche pour l’Alimentation, Paris, France) precisely to determine polyphenolic content. Most energy intake (up to 96%) came from plant foods. We found that subject consumed 15.8 g/kg−1/d−1 or 1242 g of carbohydrates (CHO), (2.4 g/kg−1/d−1) or 190 g of proteins (P), and 10,692 mL of fluid. The total energy intake (7580 kcal) showed a distribution of 65% CHO, 10% P, and 25% lipids (L). Furthermore, this sustainable diet lead to a high antioxidant intake, specifically vitamin C (1079 mg), vitamin E (57 mg), and total polyphenols (1910 mg). This sustainable approach was suitable for meeting energy, CHO, and P recommendations for UR. Physical and mental training (mindfulness) were integrated from the specific preliminary phase to the day of the challenge. The athlete completed this challenge in 18 h with a low environmental impact. This sports event had an educational component, as it awakened curiosity towards food sustainability.
