Measurement of Knee-Joint-Position Sense in Women with Genu Recurvatum

Context:Proprioception of the knee joint.Objective:To determine the difference in knee-joint-angle reproduction in women with and without genu recurvatum (GR).Design:Between-subjects.Setting:Clinic.Subjects:Twenty-four women divided into 2 groups according to their standing knee-extension angle.Main Outcome Measures:Each subject's ability to actively reproduce active positioning of 3 knee angles (10, 30, and 60°) was measured. Pearson correlation coefficients were calculated to determine correlation values for standing GR angle and absolute angular error (AAE). A1 -way repeated-measures MANOVA was computed to evaluate differences in group, angle, limb, and trial.Results:Standing GR angle correlated significantly to the AAE angle at 10° (r= .48). The high-recurvatum group consistently scored worse, with the highest error rate occurring at 10°.Conclusion:Individuals with GR might have diminished proprioceptive sense at end-range extension that could potentially lead to knee injury.

An impaired joint position sense (JPS) causes activity limitations, postural imbalance, and falls. This study compares the reliability of knee JPS measurements between the iPhone’s “Measure” application and VICON motion capture system. Eleven healthy participants were recruited for the study. To conduct the study measures, the blindfolded participant, with an iPhone fixed to the lower non-dominant leg, was seated with their lower limbs in a relaxed position. The examiner held the participant’s leg at the target angle (30°/60° from initial position) for 5 s before releasing it. The participant was then instructed to move the leg to the same target angle and hold it for 5 s (replicated angle). Absolute angular error (AAE), i.e., the difference between the target and replicated angles, was measured. Intraclass and Pearson correlation coefficients established statistically significant relationships. The study comprised 6 males and 5 females of mean age 27.6±5.6 years, mean height 1.67±0.10 m, and mean body weight 60.7±10.3 kg. Strong correlations existed between iPhone and VICON 30° (ICC = 0.969, r = 0.960, P < 0.001) and 60° AAEs (ICC 0.969, r = 0.960, P < 0.001). Bland-Altman plots showed a mean difference of 0.43° and 0.20° between the AAE measurements at 30° and 60°, respectively. The iPhone’s “Measure” application is a simple and reliable method for measuring JPS in clinical practice and sports/fitness settings.

Context: Clinicians require portable, valid, and cost-effective methods to monitor knee joint-position-sense (JPS) ability. Objective: To examine the criterion-related validity of image-capture JPS measures against an isokinetic-dynamometer (IKD) procedure. Design: Random crossover design providing a comparison of knee JPS measures from image capture and IKD procedures. Participants: 10 healthy participants, 5 female, age 28.0 ± 13.29 y, mass 60.3 ± 9.02 kg, height 1.65 ± 0.07 m, and 5 male, 29.6 ± 10.74 y, mass 73.6 ± 5.86 kg, height 1.75 ± 0.07 m. Main Outcome Measures: The dependent variables were absolute error scores (AES) provided by 2 knee directions (flexion and extension). The independent variables were the method (image capture and IKD). Results: There was no significant difference between clinical and IKD AED into knee-extension data (P = .263, r = 0.55). There was a significant difference between clinical and IKD AES into knee-flexion data (P = .016, r =.70). Conclusions: Analysis of photographic images to assess JPS measurements using knee flexion is valid against IKD techniques. However, photo-analysis measurements provided a lower error score using knee-extension data and thus may provide an optimal environment to produce maximal knee JPS acuity. Therefore, clinicians do not need expensive equipment to collect representative JPS ability.

The purpose of this study was to determine if a difference exists in the reproducibility of knee joint flexion angles in an open versus a closed kinetic chain. Thirty generally healthy subjects (12 males, 18 females; mean age 23.8 years) participated. Subjects actively reproduced small, medium, and large knee flexion angles (with target angles of 15°, 45°, and 75°, respectively) in an open and a closed kinetic chain while being videotaped. Goniometric measurements were taken from the videotape of initial and reproduced joint angles. Data were analyzed using ANOVA with repeated measures on kinetic chain test position and joint angle. Subjects more accurately reproduced knee flexion angles in a closed kinetic chain position. The main effect for angle and the interaction of angle and test position were nonsignificant. The results indicate that knee joint position is more accurately reproduced in closed kinetic chain. Closed kinetic chain testing is also a more functional assessment of joint position sense, and thus closed kinetic chain assessment of lower extremity joint position sense is recommended.

Context:During sport activity, knee proprioception might worsen. This decrease in proprioceptive acuity negatively influences motor control and therefore may increase injury risk. Hiking is a common activity characterized by a higher-intensity-exercise phase during uphill walking and a lower-intensity-exercise phase during downhill walking. Pain and injuries are reported in hiking, especially during the downhill phase.Objective:To examine the effect of a hiking-fatigue protocol on joint-position sense.Design:Repeated measures.Setting:University research laboratory.Participants:24 nonprofessional sportswomen without knee injuries.Main Outcome Measures:Joint-position sense was tested at the beginning, after 30 min uphill walking, and after 30 min downhill walking on a treadmill (continuous protocol).Results:After downhill walking, joint-position sense was significantly worse than in the test at the beginning (P = .035, α = .05). After uphill walking, no differences were observed in comparison with the test at the beginning (P = .172, α = .05) or the test after downhill walking (P = .165, α = .05).Conclusion:Downhill walking causes impairment in knee-joint-position sense. Considering these results, injury-prevention protocols for hiking should focus on maintaining and improving knee proprioception during the descending phase.

Context:The effect of externally bracing chronically unstable ankles on joint position sense awareness has had limited study yet is clinically relevant.Objective:To determine active joint position sense awareness, effects of bracing, and differences between flexible and semirigid braces.Design:Between-ankle-group (chronic and healthy), repeated-measures, including brace condition for the joint position of 15° inversion from subtalar neutral.Setting:Research laboratory.Participants:24 subjects with healthy and 12 with chronically unstable ankles.Main Outcome Measures:Blindfolded subjects attempted to find a passively placedjoint position under conditions of unbraced, flexible, and semirigid braced. A 3-way repeated-measures ANOVA and post hoc Tukey test for significant interaction effects were conducted.Results:Significant main effects were observed for ankle status, brace, and gender, but no significant interaction effects.Conclusion:Chronically unstable ankles have greater error in joint position sense awareness. However, this deficit might be reduced by using an external semirigid brace.

Context Foam-rolling exercises are frequently included in warmups due to their benefits for increasing range of motion (ROM). However, their effects on proprioception and vertical jump have not been analyzed and therefore remain unclear. Moreover, the effects of performing practical-duration foam-rolling exercises after typical warmup exercises such as jogging are unknown. Objective To analyze the effects of jogging and practical-duration foam-rolling exercises on the ROM, knee proprioception, and vertical jump of athletes. Design Randomized controlled study. Setting Sports laboratory and university track. Patients or Other Participants Thirty athletes were randomly classified into an experimental group (EG) or control group (CG). Intervention(s) The EG performed 8-minute jogging and foam-rolling exercises. The CG performed 8-minute jogging. Main Outcome Measure(s) Knee flexion, hip extension, active knee extension, ankle dorsiflexion (ADF), knee-joint position sense, and countermovement jump (CMJ) were evaluated before the intervention (baseline), after (post 0 min), and 10 minutes later. Results The EG exhibited higher values for ADF and CMJ at post 0 min (ADF: P &lt; .001, d = 0.88; CMJ: P &lt; .001, d = 0.52) and 10 minutes later (ADF: P = .014, d = 0.41; CMJ: P = .006, d = 0.22) compared with baseline. Although the CG also showed increased CMJ at post 0 min (P = .044, d = 0.21), the EG demonstrated a greater increase (P = .021, d = 0.97). No differences were found in the remaining ROM variables (knee flexion, hip extension, active knee extension: P values &gt; .05). For knee-joint position sense, no differences were found (P &gt; .05). Conclusions Combining jogging and practical-duration foam rolling may increase ADF and CMJ without affecting knee proprioception and hip or knee ROM. Jogging by itself may slightly increase ADF and CMJ, but the results were better and were maintained after 10 minutes when foam rolling was added.

Research  background  and  hypothesis.  Proprioception  is  important  in  the  prevention  of  injuries  as  reduced proprioception  is  one  of  the  factors  contributing  to  injury  in  the  knee  joint,  particularly  the  ACL.  Therefore, proprioception appears not only important for the prevention of ACL injuries, but also for regaining full function after ACL reconstruction.Research aim. The aim of this study was to understand how proprioception is recovered four and five months after anterior cruciate ligament (ACL) reconstruction.Research methods. The study included 15 male subjects (age – 33.7 ± 2.49 years) who had undergone unilateral ACL reconstruction with a semitendinosus/gracilis (STG) graft in Kaunas Clinical Hospital. For proprioceptive assessment, joint position sense (JPS) was measured on both legs using an isokinetic dynamometer (Biodex), at knee flexion of 60° and 70°, and at different knee angular velocities of 2°/s and 10°/s. The patients were assessed preoperatively and after 4 and 5 months, postoperatively.Research results. Our study has shown that the JPS’s (joint position sense) error scores  to a controlled active movement is significantly higher in injured ACL-deficient knee than in the contralateral knee (normal knee) before surgery and after four and five months of rehabilitation.  After 4 and 5 months of rehabilitation we found significantly lower values in injured knees compared to the preoperative data. Our study has shown that in injured knee active angle reproduction errors after 4 and 5 months of rehabilitation were higher compared with the ones of the uninjured knee. Proprioceptive ability on the both legs was  independent of all differences angles for target and starting position for movement. The knee joint position sense on both legs depends upon the rate of two different angular velocities and the mean active angle reproduction errors at the test of angular velocity slow speed was the highest compared with the fast angular velocity. Discussion and conclusions. In conclusion, our study shows that there was improvement in mean JPS 4 and 5 months after ACL reconstruction, but it did not return to normal indices.Keywords: knee joint, joint position sense, angular velocity, starting position for movement.

Relationships between physiological parameters and 5-km running performance were examined in 15 male runners (17.3 ± 0.9 years). Running economy (RE) and blood lactate concentration ([BLa]) at 241.2 m/min, VO2max, velocity at VO2max (vVO2max), vertical jump height and muscle power, and isokinetic knee extension strength at 60°/sec and 240°/sec were measured. The participants’ best 5-km race time over the last month of the cross-country season (16.98 ± 0.76 min) was used in the analysis. The data were analyzed using Pearson correlation coefficients. Significant relationships to run time were observed for VO2max (r = -.53), RE (r = .55), and vVO2max (r = -.66), but not [BLa], isokinetic muscle torque, or vertical jump. Identifying the unique strength and power characteristics related to running performance in this age group is warranted.
