WHO first recognised extranodal NK/T-cell lymphoma (ENKTCL) in 2001, thanks to technical advances in anatomopathology and immunohistochemistry. It is divided into nasal and extranasal subgroups depending on the primary site. Primary isolated NK/T-cell lymphoma of the testis is rare. Typical recurrence sites of primary testicular NK/T-cell lymphoma are the gastrointestinal tract, lymph nodes, skin, spleen and central nervous system. Nasal relapses of a primary NK/T-cell lymphoma of the testis are very rare and according to our knowledge, no other case has been reported yet in the literature. The authors report the case of a 35-year-old Caucasian man relapsing twice in the nasal cavity 1 year after initial diagnosis and treatment of a primary isolated, stage IE, ENKTCL of the testis. We report the clinical and radiological presentation of the nasal relapses and the different modalities of treatment that were applied. Sinonasal relapses of an isolated primary NK/T-cell lymphoma of the testis are very rare. ENKTCL is a very aggressive entity, even at an early stage, therefore, requiring a multimodal treatment approach including chemotherapy and radiotherapy. New strategies to treat this disease are needed.

Abstract The present study investigated the survival benefit of non–anthracycline (ANT)-based vs ANT-based regimens in a large-scale, real-world cohort of patients with extranodal natural killer (NK)/T-cell lymphoma, nasal type (ENKTCL). Within the China Lymphoma Collaborative Group (CLCG) database (2000-2015), we identified 2560 newly diagnosed patients who received chemotherapy with or without radiotherapy. Propensity score matching (PSM) and multivariable analyses were used to compare overall survival (OS) and progression-free survival (PFS) between the 2 chemotherapy regimens. We explored the survival benefit of non–ANT-based regimens in patients with different treatments in early-stage disease and in risk-stratified subgroups. Non–ANT-based regimens significantly improved survivals compared with ANT-based regimens. The 5-year OS and PFS were 68.9% and 59.5% for non–ANT-based regimens compared with 57.5% and 44.5% for ANT-based regimens in the entire cohort. The clinical advantage of non–ANT-based regimens was substantial across the subgroups examined, regardless of stage and risk-stratified subgroup, and remained significant in early-stage patients who received radiotherapy. The survival benefits of non–ANT-based regimens were consistent after adjustment using multivariable and PSM analyses. These findings provide additional evidence supporting non–ANT-based regimens as a first-line treatment of patients with ENKTCL.

Abstract Background Extra nodal NK/T cell lymphoma (ENKTL) is rarely occurred in the gallbladder, and it is generally difficult to diagnose before surgery. Here we present a case of primary ENKTL of gallbladder. Case presentation A 61-year-old female with pain in the right upper abdomen, and Murphy's sign was positive. Ultrasonography showed that a submucosal tumor-like lesion in the neck of the gallbladder, and the gallbladder wall thickness was 0.6cm; there were several high-signal stone shadows. Microscopic examination revealed coagulative necrosis in the lamina propria of the mucosa, the tumor cells were grown diffusely infiltrating, which with medium to large size and inconspicuous or small nucleoli. Immunohistochemistry showed that the tumor cells were positively expressed CD3, CD2, CD56, CD43, TIA-1, Perforin, Granzyme-B, and EBER-ISH (-). Based on these findings, the pathological diagnosis was ENKTL of the gallbladder. The patient received CHOP chemotherapy regimens, who has been free of recurrence for 8 months after the surgery. Conclusion In the early stage, histological diagnosis of ENKTL is difficult, and the patient may not have obvious symptoms, for patients with the gallbladder wall thickens and has protrusions, ENKTL should be regarded as one of the differential diagnosis.
