A 66-year-old man was referred to the oncological pneumology consultation due to a mass in the right upper lobe observed in a routine X-ray of the chest. The CT scan confirmed a mass in the same location. The biopsy revealed a lung adenocarcinoma. It was decided to start chemotherapy adapted to kidney function. In April 2020, the patient contracted SARS-CoV-2 infection and developed bilateral pneumonia with partial respiratory failure. He was transferred to the intensive care unit, where he had a positive evolution. In the next 5 months, there was a clinical improvement; however, the CT scan of the chest showed disease progression. After a new multidisciplinary approach, it was decided to start a second line with atezolizumab. After four cycles of atezolizumab, there was a clear clinical improvement, and a reduction by more than 50% in the tumour size, without significant adverse effects.

TPS7111 Background: There are no robust data published on the follow-up after surgery for non-small cell lung cancer (NSCLC). Current international guidelines are informed by expert opinion. Most of them recommend regular follow-up with clinic visit and thoracic imaging, either chest X-ray of Chest CT-scan. The IFCT-0302 trial addresses the question whether a surveillance program with chest CT-scan and fiberoptic bronchoscopy can improve survival compared to a follow-up only based on physical examination and chest x-ray. There is no such trial ongoing over the world. Methods: The IFCT-0302 trial is a multicenter open-label controlled randomized phase III trial. The objective of the trial is to compare two follow-up programs after surgery for stage I-IIIa NSCLC. The primary endpoint is overall survival. Patients are randomly assigned to arm 1, minimal follow-up, including physical examination and chest x-ray; or arm 2, a follow-up consisting of physical examination and chest x-ray plus chest CT scan and fiberoptic bronchoscopy (optional for adenocarcinomas). In both arms, follow-up procedures are performed every 6 months during the first two postoperative years, and every year between the third and the fifth years. The main eligibility criteria include: completely resected stage I-IIIA (6th UICC TNM classification) or T4 (in case of nodules in the same lobe as the tumor) N0 M0 NSCLC, surgery within the previous 8 weeks. Patients who have received and/or who will receive pre/post-operative chemotherapy and/or radiotherapy are eligible. Statistical considerations: 1,744 patients is required. Accrual status: 1,568 patients from 119 French centers had been included. The end of accrual can be expected for September 2012. Ancillary study: Blood samples are collected in 1000 patients for genomic high density SNP micro-array analysis. This collection will contribute to the French genome wide association study (gwas) of lung cancer gene susceptibility, and the genetic factors predictive of survival and lung cancer recurrence will be analyzed.

Mafucci’s syndrome is a rare genetic but non-hereditary disorder, characterized by multiple enchondromas (enchondromatosis), hemangiomas and, rarely, lymphangiomas. The risk for malignant transformation of enchondromas is very high, and chondrosarcomas can develop which mainly metastatize to the lungs. A case report of a 61-years-old male, whose enchondromatosis developed at his age of 10 years, is described. The initial diagnosis had been Ollier’s disease at that time, and it was modified to Mafucci’s syndrome only in 1995 when hemangiomatosis developed on the right hand. He had a unilateral disorder affecting his right upper and lower extremities. In 2010, a chondrosarcoma developed on his right leg and amputation was performed. In 2012, a thoracic CT scan revealed pulmonary metastases on both sides. This case report underlines the importance of the multidisciplinary approach and cooperation between various specialties in diagnosing and early detecting this type of cancer. Orv. Lung cancer is most often located in the right upper lobe and is called a Pancoast tumor. On many occasions, the Pancoast tumor begins with osteomyoarticular manifestations. A 72-year-old white male patient of peasant origin is presented who attends the Guard Corps because for a few weeks an increase in volume has been noted on the back of the right hand accompanied by pain in the absence of trauma. An X-ray of the hand was performed, observing osteolysis of the carpal bones, in the preoperative preparation a chest X-ray was performed where an image of condensation of the upper lobe of the right lung was observed. An excisional biopsy of the carpal lesion was performed, the result of which was metastasis of highly undifferentiated lung adenocarcinoma.

A 27-year-old man was first seen 4 weeks after his right eye being accidently hit by branches of tree. He complained of diplopia which was significant on the right gaze. There were partial thickness superior and inferior eyelid rupture and full thickness superior eyelid margin laceration (which got repaired), hematoma, and swelling of the right eye. Orbital x-ray demonstrated no abnormality. However, orbital CT Scan was eventually obtained and it showed medial wall and orbital floor fracture of the right eye, hence, we planned to do the reconstruction of orbital fracture. We concluded that patient with severe soft tissue swelling, unclear ocular movement restriction and diplopia with normal orbital X-ray should undergo orbital CT scan, as it is the best radiologic imaging in establishing an orbital wall fracture. This case report will discuss the importance on determining a proper imaging modality in blowout fracture.

Abstract: In general, lung cancer is all kinds of malignancy of the lung. It consists of malignancy derived from the lung itself (primary) and from out of the lung (metastasis). Clinically, primary lung cancers are malignant tumors derived from bronchial epithelium (bronchial carcinoma). Lung cancer is the main cause of death due to malignancy worldwide. We reported a male of 55-year-old male diagnosed as lung cancer. Diagnosis was based on anamnesis, physical examination, and supporting investigations. Anamnesis included smoking for 10 years ±15 cigarettes/day and complaints of shortness of breath, coughing, chest pain radiating to the back, and significant weight loss. Physical examination revealed enlargement of the right supraclavicular gland and decreased breath sounds in the right lung at the fifth intercostal space. Thorax photo, thorax CT-scan, and histopathological examination confirmed the diagnosis of lung cancer (adeno-carcinoma). Chemotherapy was administered with a combination of gemcitabine-cisplatin regimens for 12 cycles. The prognosis of this patient was poor because the disease had reached stage 4. However, the patient felt some clinical improvement after one month of chemotherapy.Keywords: lung cancer Abstrak: Kanker paru dalam arti luas adalah semua penyakit keganasan di paru, mencakup keganasan yang berasal dari paru sendiri (primer) maupun keganasan dari luar paru (metastasis). Dalam pengertian klinis yang dimaksud dengan kanker paru primer adalah tumor ganas yang berasal dari epitel bronkus (karsinoma bronkus). Kanker paru merupakan penyebab utama kematian akibat keganasan di dunia Kami melaporkan sebuah kasus kanker paru pada seorang laki-laki berusia 55 tahun. Diagnosis ditegakkan berdasarkan anamnesis, pemeriksaan fisik, dan pemeriksaan penunjang yang meliputi adanya riwayat merokok (sigaret) selama 10 tahun sebanyak ±15 batang rokok/hari, dengan sesak nafas, batuk, nyeri dada menjalar sampai ke punggung, dan penurunan berat badan yang nyata. Pada pemeriksaan fisik didapatkan pembesaran kelenjar supraklavikular kanan dan suara nafas menurun pada paru kanan setinggi sela iga V. Pada pemeriksaan penunjang foto toraks, thorax CT-scan, dan histopatologik didapatkan hasil yang menyokong diagnosis kanker paru (adenokarsinoma). Pada pasien ini, telah diberikan penata-laksanaan kemoterapi dengan kombinasi regimen gemcitabine-cisplatin selama 12 siklus. Progno-sis pasien ini buruk karena sudah sampai pada stadium 4, namun dengan kemoterapi yang dijalani sampai saat ini selama 1 bulan, pasien merasakan adanya perbaikan secara klinis.Kata kunci: kanker paru

Abstract Abstract 4964 POEMS syndrome is characterized by chronic progressive polyneuropathy (P), organomegaly (O), endocrinopathy (E), serum monoclonal protein (M), and skin changes (S). The bone lesions are typically sclerotic and may be minor in number or aymptomatic. Autologous peripheral blood stem cell transplant (APBSCT) has been reported to reverse major clinical manifestations of this disease. The peripheral neuropathy is often motor and sensory, painful in many cases, and asymmetric in distribution, and is often the major disease manifestation that affects patient performance status, and requires treatment. We report a patient case of POEMS where polyneuropathy was debilitating, confirmed by nerve conduction studies (NCS) with rapid clinical improvement on lenolidomide treatment and objective reversal of findings on NCS. The patient is a 54 year old man who developed slight leg weakness while exercising, and progressive severe lower back pain radiating down his right leg more than left. Plain X-ray showed an abnormality of the T4 and L3 vertebrae, and MRI showed a hypodense abnormality of the T3 vertebra and sclerotic area at the L2-3 vertebra. CT guided biopsy of these lesions showed plasma cells staining positive for CD 138 ; karyotype on the biopsy lesion was negative. The skeletal X-ray series was negative other than T4 and L3. Bone marrow biopsy showed no increase in plasma cells. Body CT showed multiple sclerotic lesions at the bodies of the pubis bones, left ischial tuberosity, femoral heads (left more than right), right sacrum, L3 and T12 vertebral body. PET scan showed uptake in these areas, SUV 3-4.6 The next MRI suggested 2 new sacral lesions and a new T7 lesion. At presentation, he had very remarkable motor and sensory changes in the right leg greater than the left with sensory neuropathy, proprioception and parasthesias. He had dysesthesias, parasthesias and loss of sensation. Nerve conduction studies showed complete loss of motor potentials in the right peroneal and right posterior tibial area and sensory abnormalities in the right sural nerve. He had bilateral foot drop with poor of balance and used a rolling walker, and subjective sensory loss and numbness of the distal extremities with dysthesias and parasthesias. Motor exam of lower extremities notable for 5/5 in the proximal muscles, foot and toe flexion 4/5 bilaterally. Upper extremities and cranial nerves intact except for poor hand grip strength. Laboratory Data SPEP showed small monoclonal IgG lambda band of 0.3 g/dl, immunofixation (urine) showed monoclonal free lambda light chains. Serum free light chains showed a ratio reversal of Lambda 44 to kappa 28. Quantitative immunoglobulins UPEP, CBC and ESR were within normal range. Patient's bone marrow was negative for multiple myeloma and amyloidosis. Lenalidomide was started at 15 mg/day for 21 days of a 28 day cycle with once weekly dexamethasone 40mg. By the second month patient was on therapy, his performance status improved to ECOG 1, with resolution of pain within the first cycle. Repeat nerve conduction studies after 4 cycles of Len/Dex confirmed marked improvement of motor and sensory nerve potentials. Patient underwent APBSCT and had a continued clinical improvement for two years, and was able to return to full time work. Conclusion Lenalidomide is an effective treatment for POEMS syndrome, with rapid and objectively documented improvement in polyneuropathy. As POEMS may be a cytokine-mediated disorder, the immunomodulatory effects of lenolidomide may explain its benefit in these patients. Disclosures Mohrbacher: Celgene: Speakers Bureau. Lung cancer has the highest mortality of all cancers in the United States. The incidence of lung cancer with metastases to the skin varies between 1–12%, with the highest incidence seen in men. Here, we present two cases of lung cancer presenting as skin metastasis. The first patient was an 80-year-old African American male who presented to the hospital for evaluation of a right upper back mass. A few months prior to admission, he was found to have a left lung mass on CT scan of the chest, he underwent biopsy which showed poorly differentiated SCC of the lung. He also had a skin biopsy which showed poorly differentiated carcinoma in the dermis consistent with metastatic SCC. He was started on chemotherapy, but could not tolerate it. He was accepted to hospice. The second patient was a 78-year-old Hispanic female who presented to the hospital with dyspnea, and a dry cough. Upon physical examination, a 2 × 2 cm ulcerated, wart-like nodule on the right palm was noted. Subsequent CT scan of the chest showed a partial collapse of the right middle lobe. A biopsy of the hand mass revealed well-to-moderately differentiated metastatic SCC favoring lung origin. A bronchoscopy biopsy showed invasive SCC. Subsequently her condition worsened and she passed away. Metastasis to the skin is an unusual presenting symptom of lung cancer. It is therefore essential to consider metastasis as a diagnosis in a patient with both a skin lesion and a smoking history.

A 70-year-old man was admitted for severe hypoxia, haemoptysis and cough. Chest-X-ray and CT-scan indicated a right-lower-lobe collapse. Bronchoscopy showed its occlusion by whitish dense mucus. Aspiration revealed a Bronchial Cast (BC) and a stenotic and inflamed orifice of the right-lower-lobe-bronchus which was biopsied. Histopathologic examination of BC showed fibrin with lymphocytes and neutrophils, and, surprisingly, also the presence of lung cancer. Although the association between BC and benign, myxoid-soft-tissue, tracheobronchial tumors has been described, the association with lung cancer has not previously been reported, and it remains unclear whether it is causal or casual.

Lung cancer is the most common neoplasm and one of the deadliest in the world. A clinical case of a 74-year-old patient with a history of being a former smoker is presented. He comes in with a dry cough, fever of 38.5°C and tiredness. The physical examination revealed the respiratory system: slightly decreased vesicular murmur towards the right parailiary region with the presence of crackles at that level. A Chest X-ray is performed, observing an image of inflammatory condensation towards the right iliary region; it is treated as bronchopneumonia worsening the clinical state. A thoracentesis is performed with extraction of pleural fluid for cytohistological study. The cytohistological study diagnosed adenocarcinoma-type non-small cell lung cancer. The patient died before starting cancer treatment. The bronchopneumonic form of lung cancer is not the most frequent, but it is one of the forms that most masks the picture.
