ObjectivesThis study investigated the frequency of electronic media (e-media) usage by preschool children and the risks of high-dose e-media use on young children’s psychosocial well-being.DesignLongitudinal associations between e-media use at 18 months and psychosocial symptoms at 5 years of age were studied, as well as cross-sectional associations between e-media use and psychosocial symptoms at 5 years.SettingBetween 2011 and 2017 in Finland.ParticipantsChildren aged 5 years (n=699).Primary and secondary outcome measuresChildren’s psychosocial symptoms were determined at the age of 5 years using the parent-reported questionnaires Five-to-Fifteen (FTF) and the Strengths and Difficulties Questionnaire (SDQ).ResultsBased on our results, 95% of the preschool children exceeded the daily recommended use of e-media set by health professionals. Our results indicate that increased screen time at 5 years of age is associated with a risk of multiple psychosocial symptoms (OR 1.53–2.18, 95% CI 1.05 to 3.34, p<0.05), while increased levels of e-media use at 18 months was only associated with FTF peer problems (OR 1.59, 95% CI 1.04 to 2.41, p=0.03). Moreover, high-dose use of electronic games at the age of 5 years seems to be associated with fewer risks for psychosocial well-being than programme viewing, as it was only associated with SDQ hyperactivity (OR 1.65, 95% CI 1.08 to 2.51, p=0.02).ConclusionIncreased screen time has multiple risks for children’s psychosocial well-being. These risk factors seem to be significant in the long term, and are related to problems in children’s socio-emotional development later on. Health professionals and paediatricians have an important role as communicators of the current research results on the safe usage time of e-media for families, and enhancing parents’ skills as regulators of children’s safe e-media use. More research is needed on the family conditions of high-dose e-media users.

BACKGROUND Excessive screen time is detrimental to the child’s health. However, screen time situation among Malaysian children is poorly understood. OBJECTIVE This study aims to identify the prevalence and determinants of screen time among children under five years old using the latest WHO guidelines. METHODS A cross sectional design was used to randomly select 489 children from nine government health clinics. Total screen time and factors were assessed using validated self-administered questionnaires and analyzed using multiple logistic regression. RESULTS Results show an overall prevalence of 91.4% with a median of 3.00 hours (IQR: 1.36-5.04). Majority of children watched television (66%), followed by mobile phones (30%) and computers (4%). The determinants of screen time were Malay ethnicity, (AOR 3.56, 95% CI: 1.65-7.68), parental age of 30 years or more (AOR 3.12, 95% CI: 1.58-6.16), parental screen time exceeding 2 hours a day (AOR 2.42, 95% CI: 1.24-4.73), parent’s moderate self-efficacy to influence child’s physical activity (AOR 2.29, 95% CI: 1.01-5.20) and parent’s positive perception on influence of screen time on child’s cognitive well-being (AOR 1.15, 95% CI:1.01-1.32). CONCLUSIONS Parents played an important role in determining their child’s screen time. Future interventions that focus on the parents may ensure age appropriate screen time for their children.

BackgroundRelationship between recurrent wheeze and airway function and inflammation in preschool children is not fully known.ObjectiveTo investigate the relationship between recurrent wheeze and airway inflammation, lung function, airway hyper-reactivity (AHR) and atopy in preschool children.DesignObservational study, comparing forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC) and mid-forced expiratory flow (FEF25%–75%), dose–response slope (DRS), exhaled nitric oxide (eNO) and atopic sensitisation between children with recurrent wheeze and those without.SettingPopulation-based, cross-sectional study in Seoul and the Gyeonggi province of Korea conducted as a government-funded programme to perform standardised measurement of the prevalence of allergic diseases, and related factors, in preschool children.Participants900 children aged 4–6 years.Primary and secondary outcome measureseNO, FEV1/FVC, FEF25%–75%, DRS, atopic sensitisation and allergic diseases.MethodsChildren completed the modified International Study of Asthma and Allergies in Childhood questionnaire and underwent eNO assessments, spirometry, methacholine bronchial provocation tests and skin prick tests. Recurrent wheeze was defined as having a lifetime wheeze of more than three episodes, based on the questionnaire. The frequency of hospitalisation and emergency room visits was also obtained by means of the questionnaire. ‘Current’ wheeze was defined as having symptoms or treatments within the past 12 months.ResultsThe prevalence of recurrent wheeze was 13.4%. Children with recurrent wheeze showed a higher prevalence of lifetime or current allergic rhinitis (p=0.01 and p=0.002, respectively) and lifetime atopic dermatitis (p=0.007). Children with recurrent wheeze showed lower FEV1/FVC (p=0.033) and FEF25%–75%(p=0.004), and higher eNO levels (p=0.013) than those without recurrent wheeze. However, the DRS, prevalence of atopic sensitisation and serum IgE levels were not significantly different between the two groups.ConclusionsRecurrent wheeze in preschool children may be associated with airway inflammation and diminished airway function, but not with AHR or atopy.

Abstract Background Social media use becomes pervasive and universal among young adults including medical personnel. Furthermore, it become established that more frequent use of the social media is associated with negative psychological well-being, anxiety which represents the second leading cause of disability among all psychiatric disorders and depression which represents the single largest contributor to global disability according to the World Health Organization. The Aim of this study To analyze the relationship between the use of social media, stress level, anxiety and depression among residents in Ain Shams University Hospitals (ASUH) and academic demonstrators in faculty of medicine. Patients and Methods This is an observational cross-sectional study, Conducted in Ain Shams University hospitals (ASUH), we included 200 of staff and medical school academics in faculty of medicine, starting from February 2018 till August 2019. Results 33 participants showed average social media use representing 16.5% while 167 participants showed problematic social media use representing 83.5%. 11 participants showed low stress levels representing 5.5%, 127 participants showed moderate stress levels representing 63.5% while 62 participants showed high stress levels representing 31%. Conclusion Social media use by clinicians is widespread, especially by younger clinicians for personal and reference purposes. Use of multiple SM platforms is independently associated with symptoms of depression and anxiety, even when controlling for overall time spent in their use.

Aims: To examine prospective and cross-sectional associations between screen time and blood pressure (BP) in preschool children. Methods: The Odense Child Cohort study started in January 2010. Children who were born in the municipality of Odense underwent a clinical examination at 3 and 5 years of age and their parents were asked to complete a questionnaire. A total of 628 children were included in the prospective analysis and 964 children were included in two cross-sectional analyses at 5 years of age. Multivariable adjusted linear and logistic regression models were computed to examine prospective and cross-sectional associations between screen time and BP with adjustment for putative confounding factors. Results: No significant prospective association was found between a 2-year change in screen time and systolic BP (0.55 BP percentile change per 1 h increase in screen time, 95% confidence interval (CI) −1.51 to 2.60) and diastolic BP (0.74 BP percentile change per 1 h increase in screen time, 95% CI −1.09 to 2.57). No significant cross-sectional association was observed between screen time (⩽1 h/day, >1–2 h/day, >2 h/day) and the prevalence of high BP at 5 years of age. Exposure to screen time before bedtime 2–5 days/week and ⩾6 days/week was significantly associated with a greater prevalence of high BP compared with screen time before bedtime 0–1 day/week (odds ratios 1.57 (95% CI 1.02–2.42) and 1.82 (95% CI 1.18–2.89), respectively. Conclusions: No prospective association was found between screen time and BP. However, a significant cross-sectional association was found between screen time before bedtime and high BP in preschool children.

ObjectiveThe objective of this study was to examine associations between depression, anxiety and binge drinking among a large sample of Canadian youth, while testing the moderating effect of flourishing. This research uses data from the Cannabis, Obesity, Mental health, Physical activity, Alcohol, Smoking, Sedentary Behaviour (COMPASS) study (2012–2021) with a large sample size collecting data on youth health behaviours within Canadian secondary schools.DesignCross-sectionalSetting14 secondary schools across Ontario and British Columbia, Canada.ParticipantsA sample of grade 9–12 students (n=6570) who participated in the Mental Health pilot of the COMPASS studyPrimary and secondary outcome measuresSelf-reported questionnaires assessed student binge drinking behaviours (5≥drinks), symptoms of depression (Center for Epidemiologic Studies Depression Scale (Revised)−10 scores≥10) and anxiety (Generalised Anxiety Disorder 7-item Scale scores≥10), and flourishing (Diener’s Flourishing Scale: 8–40).ResultsIn our sample of 6570 students, 37.0% of students reported binge drinking in the last year, and 41.4% and 31.7% of students report clinically-relevant symptoms of depression and anxiety, respectively. Anxiety (adjusted OR (AOR): 0.57, (99% CI 0.15 to 2.22)) and depression (AOR: 1.98, (99% CI 0.76 to 5.13)) symptoms were not found to be associated with binge drinking and we did not detect any moderating role of flourishing. Rather, factors that were associated with increased odds of binge drinking included sports team participation (AOR: 1.67, (99% CI 1.37 to 2.03)) and use of other substances (tobacco (AOR: 3.00, (99% CI 2.12 to 4.25)) and cannabis (AOR: 7.76, (99% CI 6.36 to 9.46))). Similar associations were found for frequency of binge drinking.ConclusionsConsistent with existing literature, binge drinking behaviours were problematic, as well as clinically-relevant symptoms of depression and anxiety. However, mental health problems and well-being may not be responsible for explaining patterns of binge drinking in youth. Targeted intervention efforts towards student athletes and concurrent substance users are necessary for addressing binge drinking in youth populations.

Increase in usage of digital devices and mandatory e-learning imposed during the Covid 19 pandemic curfew may have given rise to an emerging public health threat of digital eyestrain among undergraduate medical students in the state. The study aimed to assess the distribution of digital eyestrain among undergraduate medical students and to determine the association between digital eyestrain and screen time spent on different digital devices for e-learning and social media use.A cross sectional, questionnaire based online study was done on undergraduate medical students of a teaching hospital in Central Kerala during the month of October, 2020. Survey form included CVS-Q questionnaire, OSDI questionnaire and questions regarding screen time spent on different digital devices for e-learning and social media use.Around three -fourth of the 113 participants experienced at least one symptom of digital eyestrain. Headache (76.1%) was the most common symptom followed by neck/shoulder/back pain (67.3%), tearing(49.6%), itching(48.7%), eye pain (41.6%) and increased sensitivity to light (38.1%).OSDI score revealed that 67.7% of the students had dry eye disease. Screen time of more than 6 hours spent on mobile/tablet was significantly associated with feeling of foreign body sensation, excessive blinking and itching. Lens /spectacle use was significantly associated with a feeling that eyesight is worsening and difficulty focusing for near vision. No association was seen between screen time spent for e- learning and digital eyestrain. Steps towards awareness and prevention of digital eyestrain should be taken to promote ocular health and to avoid this occupational hazard.

Background There is mixed evidence as to the effects of different types of social media use on mental health, but previous research has been platform-specific and has focused on an oversimplified distinction between active and passive use. This study aimed to identify different underlying subgroups of adolescent social media user based on their pattern of social media activities and test associations between user type and future mental health. Methods Students from nineteen schools (N=2,456) in south-west England completed an online survey measuring thirteen social media activities and four psychosocial outcomes (past year self-harm, depression, anxiety and poor well-being) at age 13 years (October 2019) and repeated a year later (October 2020; aged 14 years). Latent class analysis using Mplus identified distinct classes of social media user. A bias-adjusted three-step model was used to test associations between class membership at baseline and mental health at follow-up. Analyses were adjusted for gender, ethnicity, sexual orientation, socio-economic status, disability, social media screen-time and baseline mental health.Results A four-class model of social media user at baseline was selected based on fit statistics and interpretability. User types were labelled High Communicators; Moderate Communicators; Broadcasters; and Minimal users. Broadcasters at age 13 had the poorest mental health outcomes at age 14, with mental health and well-being generally better in the two Communicator groups. Conclusions Findings suggest that adolescents with high levels of content sharing – in addition to socialising and browsing online – are most likely to be experiencing poor mental health a year later. Recommendations regarding social media use should move beyond screen-time to consider different user types, and mental health implications of their engagement with different online activities.
