Physical Fitness and the Metabolic Syndrome in Adults From the Quebec Family Study

Physically fit individuals have a lower risk of cardiovascular disease and type 2 diabetes. However, it is unknown whether the associations between physical fitness and the metabolic syndrome are independent of total and abdominal adiposity. Physical fitness was defined as the physical work capacity on a cycle ergometer at heart rate of 150 bpm (PWC 150) in a sample of 158 men and 198 women 20-60 years of age. PWC 150 was adjusted for fat-free mass prior to the analyses. Percent body fat was estimated by hydrostatic weighing. Visceral and subcutaneous abdominal fat were measured by computed tomography. The prevalence of the metabolic syndrome was based on two definitions. The metabolic syndrome decreased with increasing fitness in men (approx. 6 times higher in less fit vs. most fit fertile, p <  0.05) and in women (approx. 4 times higher in less fit vs. most fit tertile, p <  0.05). Fitness was negatively associated with most individual components of the metabolic syndrome, except HDL-cholesterol for which the correlation was positive. However, in men and in women, the effects of physical fitness on the individual components of the metabolic syndrome were attenuated after considering total and abdominal adiposity. Abstract As it is mentioned in the action direction Healthy and Workable Individual of the National Development Plan of Latvia 2012 –2020: 30% of all the dead in Latvia have lost their life at the age of being able to work. The main causes of death rate are different diseases (cardio-vascular a.o.), as well as outer death causes (injuries) (Pudule et al, 2012; Štale et al, 2013). Only 8% of the EU and 6% of the Latvian population regularly perform moderate and very difficult physical activity (PA) (Eurobarometer, 2013). Therefore it is necessary to implement purposeful and effective health promoting and risk limiting events to develop the physical capital of one. The understanding of the impact of physical activity on one’s physical capital is not well explored in the previous studies (Hutson, 2012; Maguire, 2008; Goldenberg, 2003; Hedblom, 2009). Therefore, the aim of the study is to determine the relationship between PA and the objective indicators of physical capital for women in fitness. In the study voluntarily participated 33 women (30±4.1 years) from 2 fitness clubs in Riga. In order to determine the respondents’ objective indicators of physical capital were applied: bioimpedance method (TANITA SC330, Japan, 89/336/EC), heart rate variability analysis data processing software ‘Omega’, complex load test with expiratory gas analysis (VIASYS Healthcare GMBH, Germany) and Eirofit tests for adults. In order to determine the level of PA the IPAQ (Craig et al, 2003) short version in Latvian (Kaupuzs & Larins, 2010) was used. Descriptive and inferential statistics were made (SPSS ver.18.0). Several moderately close (0.5<|rs|<0.69; p<0.01) and close correlations (0.7<|rs|<0.99; p<0.01) are determined between the level of PA and indicators of body composition, physical work capacity, physical fitness and functional state of the body. The level of PA has a positive impact on the objective indicators of physical capital. The more physically active the respondent is, the better physical fitness, higher work capacity, better the indicators of his body composition are and better functional state.

The purpose of this study was to examine the possible influence of childhood physical fitness on physical activity level and some of its psychosocial determinants as an adult. Childhood (age 10–12 years) data from the longitudinal Trois-Rivières Growth and Development Study (body mass index, Physical Work Capacity (PWC170)), number of sit-ups/min, left + right hands grip strength) were correlated with adult data (age 35.0 ± 0.3 years) for physical activity (PA) level, attitude toward PA, intention to exercise, perceived barriers to exercise and support of an active lifestyle by significant others. No significant relationships between childhood physical fitness and adulthood PA were found. Although the sample size is relatively small, our data suggest that the preadolescent physical fitness level has no measurable impact on adult habitual PA, attitudes toward PA, intentions to exercise, perceived barriers to exercise or support from the individual’s entourage.

Background:There is controversy regarding whether moderately-intense sports can improve physical fitness, which declines throughout adolescence among girls. The objective was to estimate the association between moderate and vigorous sports participation and cardiorespiratory fitness in a racially diverse sample of adolescent girls.Methods:Cardiorespiratory fitness was measured using a modified physical work capacity test in 1029 eighth-grade girls participating in the Trial of Activity for Adolescent Girls. Girls reported sports in which they participated in the last year on an organized activity questionnaire. Using general linear mixed models, the study regressed absolute and relative fitness on the number of vigorous and moderate sports in which girls participated, race/ethnicity, age, treatment group, fat mass, fat-free mass, and an interaction between race and fat-free mass.Results:The number of vigorous sports in which girls participated was positively associated with absolute fitness (β = 10.20, P = .04) and relative fitness (β = 0.17, P = .04). Associations were reduced, but not eliminated, after controlling for MET-weighted MVPA. Participation in moderate sports was not associated with either fitness measure.Conclusions:Vigorous sports participation is positively associated with cardiorespiratory fitness. Future longitudinal research should analyze whether promoting vigorous sports at an early age can prevent age-related declines in cardiorespiratory fitness among adolescent girls.

We studied the physical fitness of 71 patients with renal disease before dialysis, after starting dialysis, and after successful renal transplantation. Maximal muscle strength was determined in 58 patients and maximal cardiovascular capacity was assessed in 36 patients. Before dialysis there was a 31 percent reduction in muscle strength in men and 47 percent in women. The loss of muscle strength was most pronounced in the leg extensors. Cardiovascular capacity and muscle strength was positively correlated in the dialysis patients (p ≤ 0.05). The cardiovascular capacity was decreased 29% in predialysis patients, 45% in dialysis patients and 37% in transplanted patients compared to untrained healthy controls. The predialysis patients had a 19% greater cardiovascular capacity than the dialysis patients and a 9% greater cardiovascular capacity than the transplanted patients. While hemoglobin concentrations showed a positive correlation (p ≤ 0.05) with maximal cardiovascular capacity and creatinine concentration showed a negative correlation (p ≤ 0.05) with maximal cardiovascular capacity in the predialysis patients, hemoglobin concentrations and creatinine concentrations did not correlate with cardiovascular capacity in the dialysed and transplanted patients. In dialysis patients as well as in patients after kidney transplantation hemoglobin and creatinine concentrations, muscle strength, height, weight, and age are factors influencing the physical work capacity, but none of these variables alone seems to be a strong determination factor. Besides the complications of kidney disease which may itself reduce physical fitness, the lack of physical training is an important factor in determining the residual muscle strength and cardiovascular capacity for all patients with renal insufficiency.

BackgroundLittle is known about the effects of physical activity and fitness on sleep timing parameters in adolescence.MethodsWe investigated the development of sleep timing between age 8 and 15 and its association with physical fitness at age 15 in 787 adolescents (408 males, 379 females). Physical fitness was measured using the physical work capacity (PWC) protocol. Information on sport activity was collected at ages 11 and 15. Finally, the contribution of other covariates (sex, body mass index (BMI), parental education and occupational skill level) to the association between sleep parameters and physical fitness was evaluated. The correlation of BMI and physical fitness was assessed separately.ResultsMild correlation of sleep duration at ages 8 and 15 was observed (r=0.08–0.16). Higher sport activity participation and physical fitness were found to be mildly associated with delayed bedtime and reduced sleep duration; the association with bedtime was significant after adjustment for all covariates. Sport activity at age 11 was not associated with sleep timing at age 15. Interestingly, higher BMI was linked to delayed bedtime and higher physical fitness.ConclusionOur findings do not support existing hypotheses suggesting the association of low physical activity and fitness with shorter sleep duration and high BMI in a generally non-obese adolescent population without severe sleep restriction.

       Some studies indicated a relationship between increased serum levels of osteoprotegerin with arterial calcification and as a result, it leads to the risk of cardiovascular disease. In our study group we selected patients with osteoporosis, with similar age and body mass index for the assessment of the relationship between cardiovascular disease and osteoprotegerin serum level. We took into account the analysis of correlation and association between the presence of distinct patterns of atherosclerosis and associated diseases like high blood pressure,  diabetes mellitus, low HDL cholesterol, increased LDL cholesterol, increased triglycerides and was the case of presence of any type of dyslipidemia, in case of pre-existent treatment. Objective of study was the assessment of osteoprotegerin value as predictive marker for cardiovascular and metabolic risk in osteoporotic patients. Our results showed significant correlations of parathyroid hormone, osteocalcin and biochemical markers of bone with glucose metabolism and lipid were found in our research, maintaining crosstalk between calcium and biochemical markers of bone and cardiovascular risk. The serum level of Osteoprotegerin has been shown to have a large predictive value for the metabolic syndrome as a cardiovascular risk standard in patients with osteoporosis. The osteoprotegerin serum levels were increased in the patients with metabolic syndrome as a protective response facing the atherosclerotic lesions.

Abstract Background Evidence is limited about the joint health effects of the Mediterranean lifestyle on cardiometabolic health and mortality. The aim of this study was to evaluate the association of the Mediterranean lifestyle with the frequency of the metabolic syndrome (MS) and the risk of all-cause and cardiovascular mortality in Spain. Methods Data were taken from ENRICA study, a prospective cohort of 11,090 individuals aged 18+ years, representative of the population of Spain, who were free of cardiovascular disease (CVD) and diabetes at 2008–2010 and were followed-up to 2017. The Mediterranean lifestyle was assessed at baseline with the 27-item MEDLIFE index (with higher score representing better adherence). Results Compared to participants in the lowest quartile of MEDLIFE, those in the highest quartile had a multivariable-adjusted odds ratio 0.73 (95% confidence interval (CI) 0.5, 0.93) for MS, 0.63. (0.51, 0.80) for abdominal obesity, and 0.76 (0.63, 0.90) for low HDL-cholesterol. Similarly, a higher MELDIFE score was associated with lower HOMA-IR and highly-sensitivity C-reactive protein (P-trend < 0.001). During a mean follow-up of 8.7 years, 330 total deaths (74 CVD deaths) were ascertained. When comparing those in highest vs. lowest quartile of MEDLIFE, the multivariable-adjusted hazard ratio (95% CI) was 0.58 (0.37, 0.90) for total mortality and 0.33 (0.11, 1.02) for cardiovascular mortality. Conclusions The Mediterranean lifestyle was associated with lower frequency of MS and reduced all-cause mortality in Spain. Future studies should determine if this also applies to other Mediterranean countries, and also improve cardiovascular health outside the Mediterranean basin.

SummaryGrowing evidence suggests that metabolic syndrome (MetS) has both genetic and environmental bases. We estimated the heritability of the MetS and its components in the families from the Tehran Lipid and Glucose Study (TLGS). We investigated 904 nuclear families in TLGS with two biological parents and at least one offspring (1565 parents and 2448 children), aged 3–90 years, for whom MetS information was available and had at least two members of family with MetS. Variance component methods were used to estimate age and sex adjusted heritability of metabolic syndrome score (MSS) and MetS components using SOLAR software. The heritability of waist circumference (WC), HDL-cholesterol (HDL-C), triglycerides (TGs), fasting blood sugar (FBS), systolic blood pressure (SBP) and diastolic blood pressure (DBP) as continuous traits after adjusting for age and gender were 27, 46, 36, 29, 25, 26 and 15%, respectively, and MSS had a heritability of 15%. When MetS components were analysed as discrete traits, the estimates of age and gender adjusted heritability for MetS, abdominal obesity, low HDL-C, high TG, high FBS and high blood pressure (BP) were 22, 40, 34, 38 and 23%, respectively (P < 0·05). Three factors were extracted from the six continuous traits of the MetS including factor I (BP), factor II (lipids) and factor III (obesity and FBS). Heritability estimation for these three factors were 7, 13 (P < 0·05) and 2%, respectively. The highest heritability was for HDL-C and TG. The results strongly encourage efforts to identify the underlying susceptibility genes.

Background: Metabolic syndrome is a disorder which is categorized by the presence of various features like hypertension, obesity, insulin resistance and dyslipidemia. One of the basic features of this syndrome is hypertension that may lead to increased incidence of cardiovascular incidents. Aim: To determine the gender based comparison of metabolic syndrome among the hypertensive patients who reported in the outpatient department. Study Time: The present study was conducted from January 2019 to June 2019 over a period of six months. Methods: Known hypertensive patients aged between 20 to 50 years who fulfilled the selection criteria were included in this study. After informed written consent, the physical examination and required laboratory investigation were done. The data was entered and analysed by using SPSS version 23 Mean±standard deviation was recorded for the quantitative variables while frequency was utilized for the qualitative variable. The p-value of ≤0.05 was taken as significant. Results: A total of 85 known patients of hypertension were included in the study comprising (70.12%) males and (35.40%) females. They were evaluated for the metabolic syndrome using the Adult Treatment Panel III- A (ATP –III A) criteria. Their blood pressure was recorded and fasting blood sample were taken to determine the levels of serum glucose, HDL-cholesterol and triglyceride.
