Information on menstrual cycle dependent variation of tumor markers in healthy women is a subject of diagnostic efficiency and has an impact in elucidating the normal function of these markers. In this study midfollicular and midluteal concentrations of serum CEA, AFP, CA 19-9, CA 125, CA 15-3 and their relations with LH, FSH, prolactin, estradiol and progesterone were evaluated during ovulatory cycles in a group of 23 healthy female individuals. Samples were collected on the 7th and 21st day of the same menstrual cycle. Tumor marker and hormone concentrations were determined with chemiluminescence or electrochemiluminescence EIA methods. A significant phase-dependent difference was observed for CA 15-3, midluteal concentrations (mean ± SEM; 26.33 ± 1.56 U/ml) higher than the midfollicular (mean ± SEM; 19.27 ± 1.49 U/ml) concentrations (p < 0.001). But an obvious difference for other tumor markers investigated did not exist. Significant correlations of follicular and luteal CA 125 levels with body mass index of the subjects were observed (r:0.52, p < 0.05 and r:0.57, p < 0.005, respectively).CA 15-3 antigen is a product of the MUC-1 gene which is expressed in abundance by endometrial epithelial cells in the secretory phase of the menstrual cycle which may be the potential source of variability. The association of CA 125 levels with obesity suggests a possible role of adipose tissue in CA 125 metabolism. In conclusion our data suggest that in healthy women serum CA 15-3 levels are significantly elevated in the midluteal phase of the menstrual cycle compared to midfollicular phase. Therefore, consideration of menstrual cycle dependent variability for CA 15-3 appears indicated in interpretation of individual results.

Background: Kisspeptin is a critical hypothalamic regulator of reproductive function. Chronic kisspeptin administration causes profound tachyphylaxis in male monkeys and in women with functional hypothalamic amenorrhea. The pharmacological effects of chronic kisspeptin exposure in healthy women with normal menstrual cycles have not been studied previously. Aim: Our aim was to determine the effects of follicular-phase kisspeptin-54 treatment on menstrual cyclicity in healthy women. Methods: We performed a prospective, single-blinded, 1-way crossover study. Healthy women received twice-daily sc injections of kisspeptin (6.4 nmol/kg) or 0.9% saline during menstrual days 7–14 (n = 5 per treatment arm). Serial assessments of basal reproductive hormones, ultrasound parameters, LH pulsatility, and acute sensitivity to GnRH and kisspeptin-54 injection were performed. Results: Menstrual cyclicity persisted in all women after follicular-phase kisspeptin-54 treatment. Chronic exposure to kisspeptin-54 did not abolish acute stimulation of LH after injection of kisspeptin-54 or GnRH. In addition, kisspeptin-54 treatment was associated with a shorter mean length of the menstrual cycle (mean length of menstrual cycle was 28.6 ± 1.4 days with saline vs 26.8 ± 3.1 days with kisspeptin, P &lt; .01), earlier onset of highest recorded serum LH (mean menstrual day of highest LH was 15.2 ± 1.3 with saline vs 13.0 ± 1.9 with kisspeptin, P &lt; .05), and earlier onset of the luteal phase (mean menstrual day of progesterone increase was 18.0 ± 2.1 with saline vs 15.8 ± 0.9 with kisspeptin, P &lt; .05). Conclusion: Our data suggest that 1 week of exogenous kisspeptin-54 does not abolish menstrual cyclicity in healthy women. Further work is needed to determine whether kisspeptin could be used to treat certain anovulatory disorders.

A lack of sensitive and specific tumor markers for early diagnosis and treatment is a major cause for the high mortality rate of ovarian cancer. The purpose of this study was to identify potential proteomics-based biomarkers useful for the differential diagnosis between ovarian cancer and benign pelvic masses. Serum samples from 41 patients with ovarian cancer, 32 patients with benign pelvic masses, and 41 healthy female blood donors were examined, and proteomic profiling of the samples was assessed by surface-enhanced laser desorption/ionization time-of-flight (SELDI-TOF) mass spectroscopy (MS). A confirmatory study was also conducted with serum specimens from 58 patients with ovarian carcinoma, 37 patients with benign pelvic masses, and 48 healthy women. A classification tree was established using Biomarker Pattern Software. Six differentially expressed proteins (APP, CA 125, CCL18, CXCL1, IL-8, and ITIH4) were separated by high-performance liquid chromatography and identified by matrix-assisted laser desorption/ionization (MALDI)-MS/MS and database searches. Two of the proteins overexpressed in ovarian cancer patients, chemokine CC2 motif ligand 18 (CCL18) and chemokine CXC motif ligand 1 (CXCL1), were automatically selected in a multivariate predictive model. These two protein biomarkers were then validated and evaluated by enzyme-linked immunosorbent assay (ELISA) in 535 serum specimens (130 ovarian cancer, 64 benign ovarian masses, 36 lung cancer, 60 gastric cancer, 55 nasopharyngeal carcinoma, 48 hepatocellular carcinoma, and 142 healthy women). The combined use of CCL18 and CXCL1 as biomarkers for ovarian cancer had a sensitivity of 92% and a specificity of 97%. The multivariate ELISA analysis of the two putative markers in combination with CA 125 resulted in a sensitivity of 99% for healthy women and 94% for benign pelvic masses, and a specificity of 92% for both groups; these values were significantly higher than those obtained with CA 125 alone (P<0.05). We conclude that serum CCL18 and CXCL1 are potentially useful as novel circulating tumor markers for the differential diagnosis between ovarian cancer and benign ovarian masses.

The issue of serum concentrations of tumor markers in hemodialyzed patients is a widely discussed one due to the fact that these patients are usually at high risk of developing concurrent malignancies on one hand and due to the fact that half-time of these markers is strongly influenced by an impaired renal function. This is a literature review focusing on the role of determination of tumor markers such as CA 125, CA 15-3, AFP in hemodialyzed patients.

Introduction: Ovarian tumors pose a diagnostic predicament as it is difficult to differentiate benign from malignant without a histopathology report. Appropriate tumor markers may serve as diagnostic aid to better decision making in the management of these cases. We attempted to determine the relationship between age, serum markers, and histopathological sub types of ovarian tumors to help distinguish benign from malignant tumors.Methods: A retrospective cross sectional study of all cases with ovarian tumors that had available histopathology reports and tumor marker levels was done at a single centre. Variables examined were age, histopathology report and serum tumor markers CA-125, CEA, CA19-9, LDH, and βHCG. Results: Histopathological analysis revealed 26% teratomas, 28% cystadenomas, 14% corpus luteal cysts, 26% carcinomas and 6% endometriomas. CA-125 was the only marker that was significantly raised in malignant versus benign tumors (p=0.008) and increased with increasing age. All women with raised CEA reports had teratomas, and none with cancers had a raised CEA. CA19-9, LDH and βHCG were not significantly different in benign versus malignant tumors.Conclusions: CA-125 may be used as an adjuvant diagnostic tool for ovarian cancer in older women. The role of CEA as a marker for teratomas needs further evaluation.

Materials and methods. 28 patients with GE of I-II degrees of prevalence (R-AFS) were included in the study. Control group consisted of 11 healthy women. All the patients underwent laparoscopy and hysteroscopy with histological examination of ovarian tissue, peritoneum and endometrium; patients with GE also were performed histological examination of endometrioid of heterotopies. Definition of levels of kisspeptin, FSH, LH, prolactin, AMH, estradiol, estrone, androgens (free testosterone, dehydroepiandrosterone – DHEA) in peripheral blood in healthy women and in patients with GE were conducted by ELISA on the 2nd and 8th days of menstrual cycle. Immunohistochemical method was used to evaluate expression of metastine (kisspeptin) and its receptor in ovarian tissue, peritoneum, in endometrioid heterotopies and in endometrium. Results. Hormonal survey has revealed that the level of kisspeptin in peripheral blood was reliably higher compared to its level in the control group. Immunohistochemical study found that in patients with GE area of expression of KISS and KISS1R receptor in endometrium was reliably lower when compared with these values in the control group. In the foci of endometrioid heterotopies, which were located on the pelvic peritoneum, it was observed a reliable increase in protein КІЅЅ1 expression and receptor KISS1R compared to fragments of intact peritoneum. The area of expression of receptor KISS1R in endometrioid heterotopies was reliably higher than the area of its expression in endometrium of patients with GE and in endometrium of women of the control group. Direct correlation between the area of protein expression (KISS) in endometrium with level kisspeptin on the 8th day of menstrual cycle in peripheral blood (rs = 0,90) has been revealed. It has been determined that the level of kisspeptin on the 2nd day of menstrual cycle in peripheral blood correlates with the area of expression of receptor KISS1R in peritoneum (rs = 0,57). A direct correlation between the area of expression of receptor KISS1R in peritoneum with the level of kisspeptin on the 8th day of menstrual cycle in serum has been found (rs = 0,90). Conclusion. The obtained data show a definite role of kisspeptin in pathogenesis of genital endometriosis. It can be assumed that the increase in the level of metastine (kisspeptin) in peripheral blood and increased expression of KISS1 in endometrioid heterotopies and especially its receptor KISS1R are compensatory-adaptive response aimed at deterrence of further spread of endometriosis. The obtained results justify the need to continue research in this direction for a deeper understanding of pathogenesis of the disease and possible use of kisspeptin as biomarker for non-invasive diagnostics as well as potential targeted therapy of GE.

Peculiarities of vegetovascular malfunction at functional dysmenorrhea has been investigated. 107 women with dysmenorrhea and 10 healthy women have been examined. Methods of pelvis minor organ rheography, rheoencephalography and cardiointervalography have been used. It has been revealed that at women with dysmenohhrea a disorganization  of  pelvis  minor  regional circulation developed during the first phase of menstrual cycle. It was evident in simultaneous decrease of cerebral vessel blood filling and increase of venous drainage, development of microcirculation disturbances. Simultaneously the enhancement  of  sympathetic effect and decrease of the effect degree of parasympathetic nervous system have been observed. On the contrary, at healthy women it is observed the enhancement of parasympathetic effect and the hemodynamic factors remain fixed.The obtained results are evidence that at women with dysmenorrhea the marked vegetovascular disturbances connected with sympathetic tone prevalence and being evident mainly at cerebral hemodymanics level take place in the first  days  of  menstrual cycle. The nature of revealed disturbances is connected with the peculiarities of disease clinics.
