Background. Emergency physicians (EPs) face critical admission decisions, and their judgments are questioned in some developing systems. This study aims to define the factors affecting mortality in patients admitted to the hospital by EPs against in-service departments’ decision and evaluate EPs’ admission diagnosis with final discharge diagnosis. Methods. This is a retrospective analysis of prospectively collected data of ten consecutive years (2008–2017) of an emergency department of a university medical center. Adult patients (≥18 years-old) who were admitted to the hospital by EPs against in-service departments’ decision were enrolled in the study. Significant factors affecting mortality were defined by the backward logistic regression model. Results. 369 consecutive patients were studied, and 195 (52.8%) were males. The mean (SD) age was 65.5 (17.3) years. The logistic regression model showed that significant factors affecting mortality were intubation (p<0.0001), low systolic blood pressure (p=0.006), increased age (p=0.013), and having a comorbidity (p=0.024). There was no significant difference between EPs’ primary admission diagnosis and patient’s final primary diagnosis at the time of disposition from the admitted departments (McNemar–Bowker test, p=0.45). 96% of the primary admission diagnoses of EPs were correct. Conclusions. Intubation, low systolic blood pressure on presentation, increased age, and having a comorbidity increased the mortality. EPs admission diagnoses were highly correlated with the final diagnosis. EPs make difficult admission decisions with high accuracy, if needed.

BACKGROUND While there is a strong association between adhering to a healthy dietary pattern and reductions in blood pressure, adherence still remains low. New technologies aimed to help facilitate behavior change may have an effect on reducing blood pressure among individuals with hypertension. OBJECTIVE To evaluate characteristics of participants with stage 2 hypertension who used Foodsmart and to assess changes in systolic blood pressure (SBP) and diastolic blood pressure (DBP). METHODS We analyzed demographic, dietary, and clinical characteristics collected from 11,934 adults with at least two blood pressure readings who used the Foodsmart platform. Stage 2 hypertension was defined as SBP ≥ 140 mmHg or DBP ≥ 90 mmHg. We calculated mean changes in blood pressure among participants with stage 2 hypertension and stratified by length of follow-up and the covariates associated with achieving blood pressure levels below stage 2 hypertension. We compared changes in diet quality and weight between participants with stage 2 hypertension at baseline who achieved stage 1 hypertension or below and those who did not. RESULTS We found that 47.2% (5,627/11,934) and 10.6% (1,269/11,934) of the participants with at least two SBP and DBP readings had stage 1 and stage 2 hypertension, respectively, at baseline. Among Foodsmart participants with stage 2 hypertension at baseline, SBP and DBP had an absolute reduction, on average, of 5.6 mmHg and 4.0 mmHg, respectively. 33.0% (419/1,269) of participants with stage 2 hypertension at baseline achieved blood pressure levels below stage 2 hypertension (SBP < 140 mmHg and DBP < 90 mmHg). Using a multivariable ordinal logistic regression model, changes in Nutriscore and weight were statistically significantly associated with changes in blood pressure levels for users with stage 2 hypertension at baseline. Using a multivariable logistic regression model, we found that baseline Nutriscore, change in Nutriscore, and change in weight were all associated with the likelihood of users with stage 2 hypertension at baseline to achieve a lower blood pressure category. Among Foodsmart participants with stage 2 hypertension at baseline, systolic blood pressure and diastolic blood pressure had an absolute reduction, on average, of 5.6 mm Hg and 4.0 mm Hg, respectively. 33.0% (419/1,269) of participants with stage 2 hypertension at baseline achieved blood pressure levels of stage 1 hypertension (systolic blood pressure < 140 mm Hg and diastolic blood pressure < 90 mm Hg) in their last blood pressure entry. CONCLUSIONS This study evaluated changes in SBP and DBP among users of the Foodsmart platform with hypertension and found that users with stage 2 hypertension, on average, improved their blood pressure levels over time.

Renewable energy sources (RES) have gained increased popularity across the world mainly due to their ability to contribute to environmental protection through the generation of infinite ‘clean’ energy. To achieve a greater diffusion of renewables, however, small-scale investments implemented by individuals are critically important. In contrast to citizens whose attitudes have been consistently explored by research, there is little evidence on the attitudes towards investments among environmental students who will occupy positions of responsibility and play key roles in the environmental sector in the future. Hence, the purpose of the present study is to identify the most important factors that affect environmental students’ willingness to invest in renewable energy (RE) by developing a logistic regression model. According to our analysis, the participants in their majority expressed their willingness to invest, while environmental values, the low risk and profitability of renewable investments, as well as preferences for certain energy types were significant factors determining this willingness. However, willingness to invest was irrespective of the current taxation and subsidies, suggesting that significant improvements are required in these areas. The present study could be particularly useful for policymakers since the necessary steps to create favorable investment environments in Greece and elsewhere are highlighted.

The ability to identify factors that influence serious injuries and fatalities would help construction firms triage hazardous situations and direct their resources towards more effective interventions. Therefore, this study used odds ratio analysis and logistic regression modeling on historical accident data to investigate the contributing factors impacting occupational accidents among small electrical contracting enterprises. After conducting a thorough content analysis to ensure the reliability of reports, the authors adopted a purposeful variable selection approach to determine the most significant factors that can explain the fatality rates in different scenarios. Thereafter, this study performed an odds ratio analysis among significant factors to determine which factors increase the likelihood of fatality. For example, it was found that having a fatal accident is 4.4 times more likely when the source is a “vehicle” than when it is a “tool, instrument, or equipment”. After validating the consistency of the model, 105 accident scenarios were developed and assessed using the model. The findings revealed which severe accident scenarios happen commonly to people in this trade, with nine scenarios having fatality rates of 50% or more. The highest fatality rates occurred in “fencing, installing lights, signs, etc.” tasks in “alteration and rehabilitation” projects where the source of injury was “parts and materials”. The proposed analysis/modeling approach can be applied among all specialty contracting companies to identify and prioritize more hazardous situations within specific trades. The proposed model-development process also contributes to the body of knowledge around accident analysis by providing a framework for analyzing accident reports through a multivariate logistic regression model.

Abstract Objectives The objectives of this study were to examine the dietary salt-related knowledge, attitudes, and behaviors associated with the prevalence of hypertension among adults residing in rural Northern Thailand. Methods A cross-sectional study utilizing convenience sampling was administered in San Pa Tong District, Chiang Mai, Thailand. The one-on-one interview was conducted to assess subject's knowledge, attitudes, and behaviors relating to their dietary salt intake. Various food frequencies for foods high in salt were also measured. Each subject's blood pressure was measured twice before and after the interview. A logistic regression model was used in the analysis to determine potential hypertension risk factors. Results A total of 403 adults participated in the study. A majority of participants were female (73.2%), and the average age was 62.5 years old. Half of participants reported a family history of hypertension and 32% of participants were hypertensive. The major results from the logistic regression model indicated positive attitudes towards decreasing salt intake lead to a lower chance of becoming hypertensive (OR = 0.934). However, a family history of high blood pressure (OR = 1.417), a higher knowledge score about foods high in salt (OR = 1.254), daily use of Monosodium Glutamate (MSG) in food preparation (OR = 1.959) and buying outside food to eat at home (OR = 5.692) lead to a higher chance of becoming hypertensive. Conclusions Our findings suggest higher knowledge does not decrease the chance of becoming hypertensive. However, there is a positive association between hypertension and dietary salt-related behaviors among adults living in rural Thai communities. More specifically, salt-reduction interventions should focus on promoting home-cooked meal preparation with lower salt substitutes to MSG. Background: Endovascular treatment (EVT) has been shown to significantly improve functional outcome in patients with acute large cerebral vessel occlusions. To date, no evidence-based recommendations on blood pressure management after successful EVT exist. Previous studies showed an association between high pre-EVT systolic blood pressure (SBP) and functional outcome, but do not answer the question on how to manage blood pressure after successful recanalization. The purpose of this study was to determine the role of blood pressure measurements as a predictor for early functional outcome in patients with successful EVT. Methods: Prospectively derived data from patients with acute large vessel occlusion within the anterior circulation and EVT was analyzed in this monocentric study. Mean systolic- and maximum SBP as well as SBP-peaks have been obtained for the first 24 h after successful EVT. Predictive value of SBP for discharge modified Rankin Scale (mRS) ≤2 has been investigated using logistic regression models. Results: From 168 patients with successful EVT, 74 (44%) had a favorable outcome with an mRS ≤2. Mean- (127 vs. 131 mm Hg, p = 0.035) and maximum SBP (157 vs. 169 mm Hg, p < 0.001) as well as the number of SBP-peaks (0 vs. 1.5, p = 0.004) were lower in patients with favorable outcomes. Multivariable logistic regression showed high mean- and maximum SBP to predict unfavorable outcomes. Cutoff mean SBP was 141 mm Hg and maximum SBP 159 mm Hg. Conclusions: High SBP in the first 24 h after recanalization of acute anterior cerebral vessel occlusions is associated with unfavorable functional outcome. Interventional studies are needed to determine the role of SBP management as a modifiable parameter in the early phase after successful EVT.

Introduction: Traumatic brain injuries (TBI) are a real social problem, with an upward trend worldwide. The most frequent consequence of a traumatic brain injury is extra-axial hemorrhage, i.e., an acute subdural (SDH) and epidural hematoma (EDH). Most of the factors affecting the prognosis have been analyzed on a wide group of traumatic brain injuries. Nonetheless, there are few studies analyzing factors influencing the prognosis regarding patients undergoing surgery due to acute subdural and epidural hematoma. The aim of this study was to identify the factors which have the strongest prognostic value in relation to the 6-month outcome of the patients undergoing surgery for SDH and EDH. Patients and methods: The study included a group of 128 patients with isolated craniocerebral injuries. Twenty eight patients were operated upon due to EDH, and a group of 100 patients were operated upon due to SDH. The following factors from the groups were analyzed: demographic data, physiological factors, laboratory factors, computed tomography scan characteristics, and time between the trauma and the surgery. All of these factors were correlated in a multivariate analysis with the six-month outcome in the Glasgow outcome scale. Results: The factors with the strongest prognostic value are GCS score, respiration rate, saturation, glycaemia and systolic blood pressure. Conclusion: Initial GCS score, respiratory rate, saturation, glycaemia and systolic blood pressure were the factors with the strongest prognostic value.

Abstract Background and Objective: One of the causes of short-term mortality in patients is the lower quality of services provided by hospital emergency departments. Given the particular importance of the hospital emergency system and the presence of numerous problems, as well as short term mortality rates in hospitals, this study aimed to investigate the risk factors affecting short term mortality of patients presenting to the Emergency Department at Nemazi Hospital in Shiraz, Fars province in 2019.Methods: This is a retrospective study with a case control-analytical design. The sample size was 768 subjects. In the present study, the emergency department overcrowding was measured by the NEDOCS (National Emergency Department Overcrowding Scale) criterion. The severity of the disease was also evaluated based on the level of the triage of patients through the Emergency Severity Index (ESI) system and vital signs.Results: With each year increase in age, the chance of short-term mortality increases by 0.8%. People with O2 sat% <90% are 7.3 times more likely to experience short term mortality in an emergency department compared to people with O2 sat%> 90%. A significant relationship was noted between short term mortality and SBP (systolic blood pressure) in the hospital's emergency department. It was also found out that as the triage score increases, short term mortality decreases significantly. As hospital stay increases, the chance of the patients' mortality decreases by 0.5%.Conclusion: The percentage of arterial blood oxygen saturation, systolic blood pressure, respiration rate per minute, triage score, the way the patient arrives at the hospital, working shifts, hospitalization duration, age, and comorbidities were regarded as the risk factors for short term mortality. Therefore, promoting professional knowledge and skills of nurses and physicians in the hospitals' emergency department and up-to-dating and reviewing emergency protocols as well as similar research can greatly help reduce short term mortality in the hospital's emergency department.

Abstract The nature of urban and rural accidents has been different from each other in some of the factors and even the severity of damage rate, mayhem, and death. In this research, using statistical methods and binary logistic regression model, we have addressed to analyze important parameters such as age, gender, education level, the color of the pedestrian dress, season of accident, time of accident, the speed of the vehicle colliding with pedestrians and road surface conditions at the time of accident on the way of death (at the scene of the incident or in the hospital) pedestrians who have been traumatized. After the creation of the binary logistic regression model, it was determined that only the parameters of speed and the accident time have been significant in the level less than 5%. And other parameters such as age, gender, the season of accident occurrence, the color of the pedestrian dress, road surface conditions and education level had no significant effect in terms of statistical on the incidence of mortality arising from a pedestrian accident with the motor vehicle. The results revealed that by adopting decisions related to the traffic calming, attention to passages lighting and brightness the mortality rate of a pedestrian due to the urban accidents can be reduced.

Analysis of factors that influence the company's bankruptcy is one of the main tasks for companies that want to assess their financial situation and prevent possible bankruptcy in a timely manner. This article analyzes the factors that affect the company's bankruptcy. A logistic regression model was constructed based on the indicators of both bankrupt and financially stable companies. During the development of the model, significant factors were identified for predicting the bankruptcy of the organization. The results will be useful both for future bankruptcy researchers and for those companies that want to assess their financial situation.

This study was initiated to evaluate the effects of agri-environment program implemented in the Sultan reeds area of Kayseri province, Turkey. The specific objectives of the study were to compare the farmers who enrolled in the program with those who didn’t enroll regarding their application of different sustainable agricultural practices, and to determine factors affecting their enrolment in the program. The main comparative indicators were selected from different sustainable agricultural practices either promoted by the agri-environmental program or not promoted but considered very useful for the locality. Two stratified samples of farmers (enrolled and not enrolled) were selected based on their farm size. Chi-square tests of independence were used to compare farmers on the selected sustainable agricultural practices. Logistic regression model was used to determine factors affecting the enrolment of the agri-environment program. The findings of the chi-square test showed that enrolled farmers use grow more forage legumes, are more conscious about pesticides use and chemical applications, and they use more pressurized irrigation systems. Findings of the logistic regression model sowed that using rental land negatively, but contacts with extension personnel, and using long term loans for farming investments positively influenced the enrolment of the agri-environment program. Governmental effort must concentrate on these issues when promoting agri-environmental programs in the region.
