Introduction: An accurate measurement of blood pressure (BP) is critical to diagnosing and treating hypertension (HTN). Manual office BP (MOBP) often results in higher readings than automated office BP (AOBP). In previous studies, a repeat MOBP by a physician resulted in a lower BP than the initial MOBP by nursing staff. We evaluated our hypothesis that a repeat MOBP by a physician is statistically equivalent to AOBP. Methods: In an ambulatory outpatient setting, patients were roomed and at least a 5-minute interval lapsed before an AOBP was performed using a Welch Allyn Connex Vital Signs Monitor. The physician was blinded to the AOBP. The physician then entered the room and obtained a MOBP with a manual aneroid sphygmomanometer. The difference between the AOBP and the MOBP was calculated. A Wilcoxon signed rank sum test was used to determine if a significant difference between AOBP and MOBP exists. Results: A total of 186 patients (112 females, 74 male) had BP measured with a mean age of 66 years. AOBP resulted in a median systolic BP (SBP) 136 mmHg (IQR 121-150 mmHg) and median diastolic BP (DBP) of 78 mmHg (IQR 72-85 mmHg). MOBP SBP had a median of 132 mmHg (IQR 120-142 mmHg) and DBP had a median of 76 mmHg (IQR 70-81 mmHg). SBP and DBP were significantly lower in the MOBP group with a mean difference between AOBP and MOBP of 4.0 and 2.7 mmHg respectively (p-value of <0.0001). Conclusions: Repeat MOBP performed by the physician resulted in a significantly lower BP compared to AOBP. The lower BP may be due to an overall longer interval between the AOBP measurement and MOBP measurement. MOBP may be a viable option for accurate diagnosis and treatment of HTN clinics without access to a AOBP machine.

Abstract: Hypertension and anxiety are among the group of the most common chronic disease worldwide, and according to numerous studies they are oftentimes associated each other. Patients suffered from chronic illnesses, such as hypertension, may have negative emotion that increases the risk of mental disorders, most commonly anxiety disorder. This study was aimed to assess the difference of anxiety degree between uncontrolled and controlled hypertensive patients. This was an observational analytical study with a cross-sectional design. Subjects were divided into two groups: controlled and uncontrolled hypertensive patients. Measurement of blood pressure parameter was performed by using office blood pressure monitoring. Anxiety parameter was classified based on the scoring of the Generalized Anxiety Disorder Scale (GAD-7). Data were analyzed by using the Mann-Whitney test. Subjects consisted of 60 hypertensive patients (35 males and 25 females), aged 30-70 years (mean 56.48 years). There were 35 controlled hypertension patients and 22 uncontrolled hypertensive patients. The results showed that the difference in anxiety degree based on GAD-7 between controlled hypertensive and uncontrolled hypertensive groups obtained a p-value of 0.000. In conclusion, there was a significant difference in anxiety degree between uncontrolled and controlled hypertensive patients. Screening for anxiety among hypertensive patients is a simple and cost-effective tool that may improve outcomes.Keywords: anxiety, uncontrolled hypertension, controlled hypertension Abstrak: Hipertensi dan ansietas merupakan kelompok penyakit kronik yang paling umum di seluruh dunia. Penelitian ini bertujuan untuk menge-tahui apakah terdapat perbedaan derajat ansietas antara penyandang hipertensi belum terkontrol dengan hipertensi terkontrol. Jenis penelitian ialah analitik observasional dengan desain potong lintang. Subyek penelitian dibagi menjadi dua kelompok, yaitu kelompok penyandang hipertensi belum terkontrol dan hipertensi terkontrol. Pengukuran parameter tekanan darah dilakukan dengan menggunakan alat Oscillometric digital dengan cara Office Blood Pressure Monitoring (OBPM). Parameter ansietas diklasifikasikan berdasarkan skala Generalized Anxiety Disorder Scale (GAD-7). Adanya perbedaan derajat ansietas antara kedua kelompok dinilai dengan uji Mann-Whitney. Subyek penelitian terdiri dari 60 penyandang hipertensi (35 laki-laki dan 25 perempuan) berusia 30-70 tahun (rerata 56,48 tahun). Terdapat 25 penyandang hipertensi yang belum terkontrol dan 35 penyandang hipertensi terkontrol. Hasil penelitian menunjukkan bahwa terdapat perbedaan derajat ansietas berdasarkan GAD-7 antara kedua kelompok (p=0,000). Simpulan penelitian ini ialah terdapat perbedaan bermakna dalam derajat ansietas antara penyandang hipertensi yang belum terkontrol dengan yang terkontrol. AbstractBackgroundThe vital signs reflect the physiological state of patients in various clinical conditions. The purpose of this study was to compare the effects of cold compress and Xyla-P cream on hemodynamical changes during venipuncture in hemodialysis patients.Methods and MaterialIn this clinical trial study, 50 patients under hemodialysis were selected by simple random sampling. The patients were then randomly assigned to either Xyla-P cream, cold compress or placebo groups. The vital signs (blood pressure and pulse) were measured upon two intermittent hemodialysis sessions before and after venipuncture. Data were analyzed using repeated measures analysis of variance.ResultsThe mean alternation in systolic blood pressure was significantly different comparing the placebo and cold compress groups before and after intervention (p<0.001). However, the difference was not significant between the Xyla-P cream group and either placebo (p=0.402) or ice compress (p=0.698) groups. The difference of the mean diastolic blood pressure was significant comparing the placebo group with either the Xyla-P cream group (p=0.003) or cold compress group (p<0.001) before and after intervention. In addition, there was a significant difference in the mean number of heartbeats comparing the control group with either the Xyla-P cream group (p<0.001) or cold compress group (p<0.001) before and after the intervention.ConclusionsConsidering the beneficial effects of ice compress and the Xyla-P cream on reduction of cardiovascular parameters, it is recommended to use these methods in hemodialysis patients during venipuncture.

Abstract Background It is most important to measure blood pressure (BP) exactly in treating hypertension. Recent recommendations for diagnosing hypertension clearly acknowledge that an increase in BP attributable to the “whitecoat response” is frequently associated with manual BP recordings performed in community-based practice. However, there was no data about after-consult (AC) BP that could reduce whitecoat effect. So we evaluated before-consult (BC) and AC routine clinic BP and research based automated office blood pressure (AOBP) measured. Methods The study population consisted of 82 consecutive patients with hypertension between April 2019 and December 2019. We measured routine clinic BP and AOBP before and after see a doctor, respectively. Seated blood pressure and pulse are measured at each time after a rest period using an automated device as it offers reduced potential for observer biases. AOBP was measured and measuring BP 3 times un-observed. We compared each BP parameter for identifying exact resting BP state. Results There was significant difference between BC and AC systolic BP (135.37 ± 16.90 vs. 131.95 ± 16.40 mmHg, p = 0.015). However there was no difference in the BC and AC diastolic blood pressure (73.75 ± 11.85 vs. 74.42 ± 11.71 mmHg, p = 0.415). In the AOBP comparison, there was also significant difference (BC systolic AOBP vs. AC systolic AOBP, 125.17 ± 14.41 vs. 122.98 ± 14.09 mmHg, p = 0.006; BC diastolic ABOB vs. AC diastolic AOBP, 71.99 ± 10.49 vs. 70.99 ± 9.83, p = 0.038). Conclusions In our study, AC AOBP was most lowest representing resting state. Although AC BP was higher than BC AOBP, it might be used as alternative measurement for reducing whitecoat effect in the routine clinical practice.

BACKGROUND: Dyssomnia is the most frequent sleep disturbance and associated with increased blood pressure. There has been no study determining the difference in mean blood pressure based on dyssomnia types among adolescents.OBJECTIVE: To determine the difference in mean blood pressure among adolescents based on dyssomnia types.METHODS: a Cross-sectional study was conducted in SMP Negeri 1 Muara Batang Gadis in April 2016. Samples were students having sleep disturbance based on Sleep Disturbance Scale for Children (SDSC) questionnaire. Stature and blood pressure data were collected along with demographic data and sleep disorder questionnaire. Analyses were done with Kruskal-Wallis test and logistic regression. P - value < 0.05 was considered significant.RESULTS: Seventy-six samples were obtained with mean age 13.9 (SD 1.14) years - old. Dyssomnia proportion and hypertension were 72/76 and 20/76 respectively. Mean systolic (SBP) and diastolic blood pressure (DBP) was 111.1 (SD 16.46) mmHg and 70.3 (SD 11.98) mmHg respectively. Mean SDSC score was 49.7 (SD 8.96), and the most frequent dyssomnia type was disorders of initiating and maintaining sleep. Age and sex were not the risk factors of hypertension in dyssomnia. There was a significant difference in mean SBP (P = 0.006) and DBP (P = 0.022) based on dyssomnia types. Combination dyssomnia type had the highest mean blood pressure among dyssomnia types.CONCLUSION: There is a significant difference in mean blood pressure among adolescents based on dyssomnia types.

Abstract Background: recently, use of HT35 receptor antagonists to prevent postoperative shivering has attracted a great deal of attention. Therefore, the present study was conducted with the aim of investigating the effectiveness of granisetron as an HT35 antagonist when compared with ondansetron and pethidine in preventing postoperative shivering. Methods: in this triple blind random clinical trial study, 90 patients 18-50 years of age with ASA Class I and II undergoing general anesthesia were randomly assigned into one of the three drug groups: O (4-mg ondansetron), G (40 μg/kg of granisetron), and P (25 mg pethidine), immediately before induction of anesthesia. After anesthesia induction, at the end of the surgery, after the entrance and after leaving the recovery state, central temperature, peripheral temperature, systolic blood pressure, diastolic blood pressure, heart rate, and shivering were measured and recorded. P-value less than 0.05 was considered statistically significant. Results: in the pethidine, ondansetron, and granisetron groups, 4 (13.3%), 3 (10%), and 10 (33.3%) of patients experienced shivering during recovery, where the difference between the ondansetron and granisetron groups was significant (p-value=0.02). The variations in the mean arterial pressure during the investigation stages were not significant only in the ondansetron group (p>0.05). At the beginning of recovery, the reduction of peripheral temperature was significantly lower in the ondansetron group (p<0.05), while reduction of the central temperature was higher in the granisetron group (p<0.05). By the end of the recovery, the changes in the peripheral temperature across the three groups were consistent with the changes at the beginning of recovery, but no significant difference was observed variations of the central temperature across the three groups. Conclusion: Granisetron was not found to be much effective in preventing postoperative shivering. Ondansetron and pethidine were equally effective in preventing postoperative shivering. Ondansetron also causes less hemodynamic changes compared to other drugs, while granisetron is more effective in terms of preventing nausea and vomiting

The purpose of the Zero to Five study was to compare automated office blood pressure (AOBP) readings obtained after either 0 or 5 minutes of antecedent rest in relation to the awake ambulatory blood pressure. AOBP is recommended in different jurisdictions following either a 0- or 5-minute rest. This was a prospective, randomized, 2 arm, trial with blinded outcomes, recruiting adult patients referred for ambulatory blood pressure monitoring (ABPM). Participants had an AOBP measurement performed according to clinical practice guidelines with an OMRON HEM-907XL set to measure after 0 or 5 minutes of rest. The primary outcome was the difference between the mean AOBP and mean awake ABPM in the 0-minute wait group versus that in the 5-minute wait group. The study enrolled 618 participants, mean age 57.1 years, 52% women. For the 0-minute rest group, the mean AOBP was 141.2/83.1 (17.1/12.1 mm Hg) and the awake ABPM was 141.3/83.8 (16.1/10.2 mm Hg), with difference −0.02/0.52 (17.4/11.4 mm Hg). For the 5-minute rest group, the mean AOBP was 138.2/81.7 (16.9/12.4 mm Hg) and the awake ABPM was 143.4/83.6 (17.3/10.3 mm Hg), with difference −5.16/−0.8 (18.6/11.6 mm Hg). The difference of differences in systolic blood pressure (AOBP-awake ABPM) for the 0 versus the 5-minute wait group was 5.1 mm Hg (95% CI, 2.3–8.0, P =0.005) with the 0-minute AOBP measurement closest to the awake ABPM. The Zero to Five study demonstrated that a wait time of 0 minute before an AOBP measurement was closer to the daytime ABPM result than a 5-minute wait before the AOBP. Background: Ambulatory blood pressure (ABP) is known to provide prognostic information about cardiovascular disease better than office BP. Not much is known about the correlation of ABP control and patient engagement with gamification. Objective: To examine the relationship between ambulatory blood pressure and patient engagement with a nationwide ABP kiosk network. Methods: De-identified historic data from a nation wide ABP kiosk network (higi SH, llc www.higi.com) was analyzed from September 2012 to May 2015. At time of this abstract submission 9,926 ABP kiosks were deployed within the network. Only patients with initial BP readings in the hypertensive range and those who opted-in to share data for research purposes were included in the study. Level of engagement was defined as the total number of achievements, or badges, earned on the gamification platform. BP changes were defined as the difference between patients first and last reading on the kiosk network. Patient demographics, level of engagement with gamification platform and their ABP trends were analyzed and correlation measured. Results: A total of 153,092 patients qualified the inclusion criteria for the study. Mean age was 52 years with 56% (85,361) male and 44% (67,731) female. Almost half the patients were obese (49%, 74,587). The patients on the gamification platform earned a total of 898,130 achievements. There was a statistically significant difference in drop in systolic and diastolic blood pressure with number of achievements earned by patients (systolic BP: p-value < 0.0001, diastolic BP: p-value = 0.0033). Patients achieving greater than 20 achievements showed an average drop of systolic BP of 16.2 mmHg (p<0.01) and a drop of diastolic BP of 10.6 mmHg (p<0.01). Of the patients earning greater than 20 achievements 84.8% moved from hypertensive classification to normotensive classification. Conclusion: The results showed a statistically significant relationship between level of achievements earned on the gamification platform and lowering of blood pressure.

Hypertension is one of the number one causes of death, globally. Hypertension is the most common cause of cardiovascular events and is a major problem in both developed and developing countries. Cardiovascular is also the number one cause of death in the world every year. Respondents in this study were patients with a diagnosis of hypertension in hospitalized patients at Mardi Rahayu Kudus Hospital, totalling 41 people. the characteristics of hypertension patient respondents seen from the age of the most patients are in the late adult age category (36-45 years), namely 31 people (75.6%), while for early adulthood (26-45 years). 35 years) as many as 10 people (24.4%). The characteristics of hypertension patient respondents seen from the sex of the most patients were male, namely 25 people (61.0%), while the female gender was 16 people (39.0%). Results The highest systolic blood pressure was 170 mmHg and the lowest was 145 mmHg. The average systolic blood pressure was 152.90 mmHg. In hypertensive patients on the left arm, the highest systolic blood pressure was 170 mmHg and the lowest was 145 mmHg. The average systolic blood pressure was 152.90 mmHg. Based on the dependent t-test, the t value is -12.491 with a p-value of 0.000. It can be seen that the p-value is 0.000 < (0.05), this indicates that there is a significant difference in the results of blood pressure measurements between the right arm and the left arm in hypertensive patients in the inpatient room of Mardi Rahayu Kudus Hospital. The difference in blood pressure variations obtained in this study is still considered normal because the difference in MAP between the right and left arms is 6.11 mmHg (normal 10-20 mmHg).

Abstract: Routine blood examination is the earliest blood test or screening test to determine the diagnosis of an abnormality. Blood easily froze if it is outside the body and can be prevented by the addition of anticoagulants, one of which Ethylene Diamine Tetra Acetate (EDTA). Currently available vacuum tubes containing EDTA anticoagulants in the form of K2EDTA and K3EDTA. K3EDTA is usually a salt that has better stability than other EDTA salts because it shows a pH approaching a blood pH of about 6.4. The purpose of this research is to know the difference of erythrocyte index results include MCH, MCV and MCHC using K3EDTA anticoagulant with K2EDTA. This research is a cross sectional design. This study used venous blood samples mixed with K2EDTA anticoagulant and venous blood mixed with K3EDTA anticoagulants, each of 30 samples. Data were collected and analyzed using paired different test. Based on data analysis that has been done on MCH examination, p value <0,05 then there is a significant difference between samples with K3EDTA anticoagulant with K2EDTA to erythrocyte index value. Then on the examination of MCV and MCHC obtained p value <0.05 then there is no significant difference between samples with K3EDTA anticoagulant with K2EDTA to erythrocyte index value.Abstrak: Pemeriksaan darah rutin merupakan pemeriksaan darah yang paling awal atau screening test untuk mengetahui diagnosis suatu kelainan. Darah mudah membeku jika berada diluar tubuh dan bisa dicegah dengan penambahan antikoagulan, salah satunya Ethylene Diamine Tetra Acetate (EDTA). Dewasa ini telah tersedia tabung vakum yang sudah berisi antikoagulan EDTA dalam bentuk  K2EDTA dan  K3EDTA. K3EDTA  biasanya berupa garam yang mempunyai stabilitas yang lebih baik dari garam EDTA yang lain karena menunjukkan pH yang mendekati pH darah yaitu sekitar 6,4. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan hasil indeks eritrosit meliputi MCH, MCV dan MCHC menggunakan antikoagulan K3EDTA dengan K2EDTA. Penelitian ini merupakan penelitian dengan desain cross sectional. Penelitian ini menggunakan sampel darah vena yang dicampur dengan antikoagulan K2EDTA dan darah vena yang dicampur dengan antikoagulan K3EDTA, masing-masing sebanyak 30 sampel. Data dikumpulkan dan dianalisis menggunakan uji beda berpasangan. Berdasarkan analisis data yang telah dilakukan pada pemeriksaan MCH didapatkan nilai p < 0,05 maka ada perbedaan yang signifikan antara sampel dengan antikoagulan K3EDTA dengan K2EDTA terhadap nilai indeks eritrosit. Kemudian pada pemeriksaan MCV dan MCHC didapatkan nilai p < 0,05 maka tidak ada perbedaan yang signifikan antara sampel dengan antikoagulan K3EDTA dengan K2EDTA terhadap nilai indeks eritrosit.
