Abstract WP358: Comprehensive Integrated Stroke System: A Novel Approach to Optimizing Cerebrovascular Disease Care

Introduction: There is a need to deliver endovascular stroke therapy (EST) locally and rapidly, due to the time sensitivity of acute ischemic stroke (AIS). However, the optimal distribution of resources, and means to maintain quality outcomes in patients with diseases that benefit from concentrated care at specialized centers, including subarachnoid hemorrhage (SAH), is unknown. Methods: We performed a multicenter, observational cohort study across our 11-hospital network from Jan 2017 - Feb 2019, identifying AIS patients who received EST. From Jan 2017 - Aug 2017, there was one CSC in the system. Starting in Aug 2017, we implemented an Integrated Stroke System (ISS), in which 3 additional centers became CSCs, practices were standardized across a single physician group covering all 4 CSCs, and SAH care was centered at the original CSC, while EST was performed at 3 new CSCs. Logistic regression adjusted for age, sex, NIHSS, direct vs. transfer arrival, and time from onset to recanalization was used to assess likelihood of good outcome, defined as discharge to home or rehabilitation. Results are given as median [IQR] and OR [95% CI]. Results: Among 478 patients who received EST, median age was 68 [57-78], 47% were female, and 37% were white. Over the course of the study (Fig. 1), the number of monthly EST cases increased; EST volume at our original CSC remained stable, and an increasing EST was performed at the new CSCs (p<0.01). Monthly SAH case volume remained unchanged at the original CSC (29 cases vs. 30 cases, p=0.68). After implementation of our ISS, there was a decrease in the time from arrival to groin puncture (107 min [88-125] vs 92 min [67-120]; p<0.01). Among patients presenting 0-6 hours, there was a significant improvement in likelihood of good outcome after ISS (OR 2.59 [1.06-6.35]; p<0.05). Conclusions: By restructuring our stroke system of care and extending EST capability, we observed increased EST utilization and improvement in quality of care for AIS patients.

ABSTRACT Reimbursement of long-term permanent disability following a dental injury can lead to claims and legal involvement by the injured person. This will delay the treatment the patient's quality of life and the court system. A new formula has been hypothesized to address the problem. This might help the stakeholders including patients, insurance companies. The details of calculating the index and its significance are discussed. Introduction: In order to have a good assessment of the quality of maternal and child health care, it is essential that there is up-to-date and reliable information. Objective: To evaluate the impact of the implementation of a computerized database of clinical processes in the admission, archive and medical statistics section, of Maternity hospital Irene Neto/Lubango-Angola. Methodology: A descriptive study with a quantitative and qualitative approach to carry out a retrospective case study deliveries and newborns, records from 2014 to 2017. Final considerations: The implementation of this project may contribute to the improvement of clinical management support management of the hospital as well as facilitating access to information for research and scientific production.

Quality outcomes from vocational education and training (VET) are fundamental to ensuring a skilled workforce and supporting a productive economy. The quality of vocational training from the demand’s perspective is defined as meeting the customer specification, needs or requirements. Using the method of Exploratory Factor Analysis (EFA) and Multiple Regression Analysis to get the analytical results of 275 respondents as graduates of University of Transport and Communication, University of Transport Technology (the two universities training on transportation in the North of Vietnam), the results show that factors are measured by the observed variables in the study that ensure reliability and relevance. The descriptive statistics result of vocational training quality is 3.28, while all independent variables are at average level (between 3.1 to 3.48) with the highest value related to the quality of the teacher and the lowest are facilities and training programs and management capacity of university. However, the regression results show that the qualification of the teacher, material facilities and training program, management capacity and job opportunities at the training school do not affect the quality of the training while “Learners-NH” have a great positive effect and the labor market information has a negative effect on the training quality of the school significantly. Base on those data, some policy recommendations have been given out in order to improve the training quality of transport technology universities in the north of Vietnam in particular and the vocational training quality of Vietnam in general.

The article has considered the calculation of the unified glare rating (UGR) based on the luminance spatial-angular distribution (LSAD). The method of local estimations of the Monte Carlo method is proposed as a method for modeling LSAD. On the basis of LSAD, it becomes possible to evaluate the quality of lighting by many criteria, including the generally accepted UGR. UGR allows preliminary assessment of the level of comfort for performing a visual task in a lighting system. A new method of "pixel-by-pixel" calculation of UGR based on LSAD is proposed.

TThe distribution of the limited financial resources in the state hospitals in Kyrgyzstan is uneven. The problems associated with the current method of distribution of resources: the poor quality of services at the level of polyclinics and high hospitalization rates that require an evaluation of the budget allocation of healthcare organizations operating in the Single Payer system. In order to improve the efficiency of resource use it is suggested to review the principles of allocation of resources to the primary level of patient care.

Background: Brief psychosocial intervention (BPI) is a treatment for adolescent depression that has recently demonstrated clinical effectiveness in a controlled trial. The aim of this study is to explore experiences of adolescents with major depression receiving BPI treatment in the context of good treatment outcomes. Method: A subsample of five interviews from a larger study of adolescents’ experiences of BPI was purposively selected, focusing on good-outcome cases. Interviews were analysed using interpretative phenomenological analysis to provide a richer understanding of participants’ experiences of overcoming depression in the BPI group. Results: Four central themes were identified: ‘Being heard and feeling safe’, ‘Collaborative working enhancing therapy’, ‘Gaining a different perspective on one’s self and relationships’ and ‘A positive therapeutic relationship’. Conclusion: BPI is a novel approach with promising clinical effectiveness. Utilising adolescents’ experiences has revealed potential psychological mechanisms of good treatment response to BPI. Overall implications for clinical practice with depressed adolescents are discussed.

Abstract Choral singing is a novel approach to reduce dementia stigma and social isolation while offering participants a sense of purpose, joy and social connection. The pervasiveness of stigma surrounding dementia remains one of the biggest barriers to living life with dignity following a diagnosis (Alzheimer Society of Canada, 2018). This paper examines how a social inclusion model of dementia care involving an intergenerational choir for people living with dementia, their care partners and high school students can reduce stigma and foster social connections. Multiple methodologies are used to investigate the effects of choir participation on cognition, stress levels, social connections, stigma, and quality of life. Results demonstrate the positive impact of choir participation and indicate that this socially inclusive intervention offers an effective, non-pharmacological alternative for older adults living with dementia in the community. Discussion focuses on the importance of instituting meaningful and engaging dementia-friendly activities at the community level.

Traditional single-pixel imaging systems are aimed mainly at relatively static or slowly changing targets. When there is relative motion between the imaging system and the target, sizable deviations between the measurement values and the real values can occur and result in poor image quality of the reconstructed target. To solve this problem, a novel dynamic compressive imaging system is proposed. In this system, a single-column digital micro-mirror device is used to modulate the target image, and the compressive measurement values are obtained for each column of the image. Based on analysis of the measurement values, a new recovery model of dynamic compressive imaging is given. Differing from traditional reconstruction results, the measurement values of any column of vectors in the target image can be used to reconstruct the vectors of two adjacent columns at the same time. Contingent upon characteristics of the results, a method of image quality enhancement based on an overlapping average algorithm is proposed. Simulation experiments and analysis show that the proposed dynamic compressive imaging can effectively reconstruct the target image; and that when the moving speed of the system changes within a certain range, the system reconstructs a better original image. The system overcomes the impact of dynamically changing speeds, and affords significantly better performance than traditional compressive imaging.
