Otosclerosis: Vestibular Symptoms and Sensorineural Hearing Loss

Of 546 patients with otosclerosis, 500 who had not had ear surgery were divided into four groups according to the relative and absolute amounts of sensorineural hearing loss in the hearing-impaired ears. The first three groups of patients had progressively less of a conductive component and progressively more of a sensorineural component to their hearing losses. The fourth group of patients had pure sensorineural hearing impairments. The remaining 46 patients were placed in group V. They had bilateral hearing losses and had had a prior unilateral stapedectomy for otosclerosis and they had received medical therapy for otosclerosis. The following conclusions were reached: 1 Further clinical evidence was obtained supporting the existence of the entity “pure labyrinthine otosclerosis.” However, there can be difficulties in the diagnosis of this condition. 2 The incidence of vestibular symptoms in patients with otosclerosis increased as the relative and absolute amounts of sensorineural hearing loss increased and was much higher than one would expect in the normal population. In addition, with increasing sensorineural hearing loss, the severity of vestibular symptoms increased along with the incidence of depression in vestibular function determined by the bithermal caloric test in patients with vestibular Symptoms. 3 A preliminary study on the administration of a combination of calcium gluconate, sodium fluoride, and vitamin D to patients with otosclerosis who had vestibular symptoms indicated that the therapy controlled vestibular symptoms in a high percentage of these patients. 4 A preliminary study on the administration of calcium gluconate, sodium fluoride, and vitamin D to patients with otosclerosis indicated that in a high percentage of patients the deterioration in hearing was stopped. A small but significant number of patients had reversal of their hearing loss. 5 Side effects of treatment with a combination of calcium gluconate, sodium fluoride, and vitamin D were not serious and were reversible, and there appears to be no contraindication to this therapy for healthy adults.

Abstract Objectives Idiopathic sudden sensorineural hearing loss (SSNHL) represents a frequently encountered otological entity, of various types and severity, with an array of associated symptoms including vertigo. This is a devastating life-changing condition with a blurry prognosis. The objective of this study was to determine the clinical association of vestibular impairment by electronystagmography (ENG) and caloric tests, and their ability to predict prognosis. Methods An observational, crossectional study was carried out amongst patients admitted with SSNHL. Each consenting patient had an audiometry test performed on admission as well as ENG and caloric tests. Treatment included oral steroids and carbogen with intratympanic steroids used only as salvage treatment. Follow-up was completed after 6 months when hearing gains were evaluated. Finally, an association was sought between the rate of recovery and ENG and caloric test results. Results Of 35 patients included, marked recovery was seen in patients without vertigo when compared to those with vertigo (p=0.003). A statistically significant association was found between the presence of vertigo and hearing deterioration (p=0.008). More so, normal electronystagmography results were associated with marked recovery (p=0.04). Conclusions The vestibular end organs are both subjectively and objectively affected in SSNHL as demonstrated by the abnormal ENG and caloric tests in our study despite the small sample size. Concomitant vestibular involvement carries poorer prognosis and routine identification may help foresee the recovery of patients with SSNHL and as such, aid in patient counseling. ENG and caloric tests are easily available and may be recommended for all patients with SSNHL.

The effect of vitamin D supplementation to patients with sudden sensorineural hearing loss (SSNHL), treated with hyperbaric oxygen (HBO) therapy, on the markers of the oxidant-antioxidant equilibrium was investigated. Patients were divided into two groups: those who did and did not receive vitamin D (cholecalciferol at 4000 IU/24 h). Concentrations of the following compounds, thiobarbituric acid reactive substances (TBARS), malondialdehyde (MDA), conjugated dienes (CD) in plasma and erythrocytes and activities of catalase (CAT), superoxide dismutase (SOD) and glutathione peroxidase (GPx) in erythrocytes, were determined. Haemoglobin (HGB) and haematocrit (HCT) were measured. Blood for analyses was collected from the basilic vein at three time points: before the first HBO procedure, up to 5 min after the first procedure, and after 14 procedures. No statistically significant differences in parameters tested were found between patients who did and did not receive vitamin D. In patients without supplementation, an increase of 53.2% ( P ≤ 0.05 ) in erythrocyte TBARS was observed after the first HBO treatment. In patients receiving vitamin D, a reduction of 27.6% ( P ≤ 0.05 ) was observed in erythrocyte MDA after 14 HBO treatments vs. that after the first treatment. In both groups, a decrease of 33.3% in plasma CD was observed after 14 treatments vs. that after the first treatment ( P ≤ 0.05 and P ≤ 0.01 , respectively). No statistically significant changes were observed in the erythrocyte SOD, GPx, and CAT activities and in HCT. A reduction of HGB concentration of 10.9% ( P ≤ 0.05 ) was demonstrated in nonsupplemented patients after 14 treatments compared with baseline. The results confirm that the effect of HBO therapy on oxidative stress markers is inconclusive and complex. Repeated HBO procedures can induce adaptive changes which protect against disruption of the oxidant-antioxidant equilibrium. It is possible that vitamin D supplementation inhibits the process of lipid peroxidation in erythrocytes.

ABSTRACT Introduction Radiotherapy either primary or adjuvant, is a commonly used modality of treatment in head and neck malignancies. The audiovestibular apparatus is often within the fields of radiation treatment, and hearing loss is a possible complication. This study was undertaken to assess the audiovestibular functions in patients undergoing radiation therapy for head and neck malignancies to determine the type and severity of hearing loss and vestibular dysfunction following radiation therapy. Materials and methods Fifty patients with head and neck malignancies reported to the malignant disease treatment center of INHS Asvini and received radiotherapy as a primary modality of treatment or in combination with surgery during the period May 2003 to Sep 2004 were included in this study. None of these patients had prior treatment by chemotherapy. Conclusion A significant number of patients who were subjected to radiation therapy for head and neck malignancies develop conductive hearing loss is predominant in the immediate postradiation period. Conductive hearing loss is reversible and improves with the conservative line of treatment. Sensorineural hearing loss more commonly affects the higher frequencies and is more common in older patients. Sensorineural hearing loss is more common when radiation doses exceed 60 Gy. There is no conclusive evidence of vestibular dysfunction in patients undergoing radiotherapy for head and neck cancers.

Abstract Introduction The cochlea and the vestibular receptors are closely related in terms of anatomy and phylogeny. Patients with moderate to profound sensorineural hearing loss (MPSHL) should have their vestibular organ functions tested. Objective To evaluate the incidence of vestibular abnormalities in patients with MPSHL and to study the correlation between the etiology of hearing loss (HL) and a possible damage to the labyrinth. Methods A case-control retrospective study was performed. In the case group, 20 adults with MPSHL of known etiology were included. The control group was composed of 15 adults with normal hearing. The case group was divided into 4 subgroups based on the etiology (bacterial meningitis, virus, vascular disease, congenital). Cervical vestibular-evoked myogenic potentials (cVEMPs) were used to rate the saccular function and lower vestibular nerve. Results The study was performed in 70 ears, and it highlighted the presence of early biphasic P1-N1 complex in 29 (71.5%) out of 40 ears in the study group, and in all of the 30 ears in the control group (p = 0.001). Regarding the presence or absence of cVEMPs among the four subgroups of patients with MPSHL, the data were statistically significant (p < 0.001). The comparison between the latencies and amplitude of P1-N1 in case and control groups from other studies and in the four subgroups of cases in the present study did not detect statistically significant differences. Conclusion The present study demonstrates that patients with MPSHL have a high incidence of damage to the labyrinthine organs, and it increases the current knowledge about the etiopathogenesis of sensorineural HL, which is often of unknown nature.

The aim of the study is a spatial and statistical analysis of hearing loss of various types (conductive, sensorineural, mixed) and deafness in the adult population of the Chelyabinsk region, assessment of its dynamics and the impact of environmental stress in the place of residence. The study materials were data from the regional audiological center of the Chelyabinsk regional clinical hospital for the period from 2016–2020. Epidemiological issues of the spread of diseases were studied using the methods of spatial analysis, in the R software environment. The main tasks of spatial analysis were to visualize a point pattern from the geographical coordinates of the patient’s places of residence, assess the properties of the spatial process that generates this point pattern, test the hypothesis about the spatial randomness of this process and the influence of the ecological situation on it. It was found that more than half of all primary visits were patients with bilateral sensorineural hearing loss. The number of patients who applied for the year during the observation period fluctuates within the average. Of all hearing impairments, the highest values of relative incidence are characteristic of sensorineural hearing loss, which has a direct relationship with age. The intensity of detection in patients with hearing impairment and deafness is directly related to the size of the living population. The existing division into zones with different ecological situations does not have a statistically significant (p <0.05) effect on the ratio of the number of patients with hearing impairments to the population.

The problem of acute otitis media in children is relevant because of the prevalence, social significance, possibility of rapid development of both life−threatening intracranial and intralabyrinthine, mastoid complications, as well as acute or prolonged dysfunction of adjacent cranial nerves, especially auditory, vestibular ones. In order to determine the frequency of various forms of acute otitis media, the nature and extent of its complications in children of different ages there was conducted a retrospective analysis of medical histories of patients treated at the City Clinical Hospital № 30 Kharkiv within 2015−2019. There was stated that the biggest group was composed by the patients with a purulent form of the disease, a slightly smaller number of patients had secretory and viral processes. But when analyzing the data over the years, of interest is the fact that during the period when the number of purulent forms decreased, the number of viral ones increased. Complications include mastoiditis, acute sensorineural hearing loss, cerebrospinal fluid and hypertension and intoxication syndromes. It is important that in acute purulent and viral otitis there is an intoxication syndrome along with the manifestations of acute sensorineural hearing loss. Signs of vestibular dysfunction in children can be disguised as symptoms of intoxication, they are much more difficult to detect. Considering the fact that the snail and the labyrinth are washed by the same liquid, it is logical to assume that in this situation the vestibular analyzer suffers together with the auditory one. Therefore, along with otoneurological research it is necessary to apply instrumental methods for establishment of latent disturbances of function of the vestibular analyzer. According to the research results it was concluded that a more detailed study of vestibular analyzer dysfunction in children with acute otitis media, as they are disguised as symptoms of intoxication. The coronavirus disease 2019 (COVID-19) pandemic resulted in widespread unprecedented changes to the health care system. Herein, we sought to assess the impact of the viral outbreak on clinical presentations of sudden sensorineural hearing loss (SSNHL) at a single academic center. Our results demonstrate a decrease in the absolute number of patients presenting with SSNHL to our institution during the initial onset of the COVID-19 pandemic compared to an analogous time frame 1 year prior. However, the ratio of patients with SSNHL compared to total patients evaluated was largely similar during the 2 time periods. Based on data from our institution, the COVID-19 virus does not appear to confer a significantly increased risk for the development of SSNHL.
