Development of the Family Stigma Stress Scale (FSSS) for Detecting Stigma Stress in Caregivers of People With Mental Illness

People with mental illness and their family caregivers often perceive public stigma, which may lead to stigma-related stress (or stigma stress). However, no instruments have been developed to measure this stress for family caregivers of people with mental illness. We modified an instrument that measures the stigma stress of people with mental illness (i.e., the cognitive appraisal of stigma as a stressor) and examined the psychometric properties of the scores of the newly developed instrument: the Family Stigma Stress Scale (FSSS). Primary family caregivers of people with mental illness in Southern Taiwan ( n = 300; mean age = 53.08 ± 13.80; 136 males) completed the FSSS. An exploratory factor analysis showed that the FSSS score had two factors; both factor scores had excellent internal consistency (α = .913 and .814) and adequate test–retest reliability ( r = .627 and .533; n = 197). Significant correlations between FSSS factor scores and other instruments supported its concurrent validity and the ability of the FSSS to differentiate between clinical characteristics, for example, having been previously hospitalized or not. The FSSS is a brief and effective measure of the stigma stress of family caregivers of people with mental illness.

Introduction: Caring for a person with mental disorder is a challenge for the caregivers. Little is known about the ways in which families cope while caring for a relative with mental illness in developing countries. This study has tried investigating the family burden and coping behaviour among the family caregivers of the people with mental illness. Material And Method: This is a descriptive cross-sectional study conducted at Devdaha Medical College during the period of July 2019-December 2019. The research was approved by the Ethical Committee of the Institutional Review Board of Devdaha Medical College. The protocol approval number is 028/2019. Convenient sampling method was used. Data analysis was done in SPSS (Version 23, SPSS Inc, Chicago, USA). Results were presented as frequencies and percentages where required. Results: Moderate Burden was experienced by 72% of the caregivers while the rest 28% experienced severe burden. People whose spouses were mentally ill had severe burden. Males coped with higher degree of problem and emotional focused coping strategy than females. Males felt more burden in comparison to the females and burden was found to be significantly higher among the caregivers who were taking care of their mentally ill spouses. Conclusion: Caregiving is physically and psychologically demanding as it requires an enormous physical and emotional commitment as well as some basic skills. Teaching skills in the form of problem solving and communication are needed to promote the coping abilities and lessen the burden of caregivers.

Purpose This study aims to assess the awareness about the rights of people with mental illness in the main psychiatric hospital in Tunisia among the service users, the family members and the staff. Design/methodology/approach The Convention of Rights of People with Disabilities mandates that State Parties initiate and maintain campaigns and human rights training to promote understanding of the rights of people with mental illnesses, considered as a main factor for their fulfillment. Service users, family members and staff evaluated, through a survey, the importance of ten rights for persons with mental illness, stated in the convention. Findings Disparities were found in the perception of the different rights by and between the three groups. The highest levels of awareness were associated with the freedom from torture or degrading treatment and the right to live with dignity and respect, whereas the lower importance were assigned to the right to participation in recovery plans, to give consent and to exercise legal capacity. Originality/value The lack of awareness and the poor perception of rights of people with mental illness is one of the barriers to their achievement. More training and awareness raising is necessary.

Objetivos: avaliar o impacto da sobrecarga familiar nos cuidados ao paciente psiquiátrico crônico. Metodologia: pesquisa bibliográfica por meio de referências publicadas em documentos, livros, revistas, artigos científicos, internet, ou seja, aborda tudo o que foi publicado sobre o tema proposto. Resultados: a convivência com a pessoa acometida por uma doença mental é desgastante para o familiar e, muitas vezes, há dificuldade de compreensão dos sintomas apresentados pelo doente. Approximately 24 million people worldwide experiencing schizophrenia (The World Health Report, 2001). Several people with mental disorder have to rely on support of family and friends to help them in their day-to-day happenings. In that era, caregivers are at risk for physical and mental health dilemma.1 Caring for a person with any mental illness often creates physical, emotional dilemma among the family caregivers more than they think. That is why it is also essential to rationalize that issue too. Family care givers of patients with any mental illness have different perspectives and coping strategies about the situation that may lead to feeling of sadness, loneliness, helplessness, hopeless at a variance among the care givers. OBJECTIVES: • To study levels of psychological problems faced by the family caregivers of patients with schizophrenia. • To study association between demographic variables and levels of psychological problems of family caregivers with schizophrenia. METHOD: A descriptive study was carried out to examine the psychological problems faced by family caregivers of patients with schizophrenia. 200 family caregivers were selected by applying stratified systematic sampling method from the government hospitals of mental health of Gujarat state with a criterion of minimum facility of 100 beds. A self-structured interview schedule was designed for study purpose by referring the Burden Assessment Tool of Thara et.al (1998) and Zarit Burden Interview. RESULT: Majority of the respondents (51%) feel anxious, depressed and frustrated due to caregiving responsibility. Majority of the respondents (52.5%) believed that care giving responsibility is mentally tiring for the family caregivers. Majority of the respondents (40.5%) agree with the statement that their contacts with family & friends have lessened due to the illness of care receiver.

Background: The care for people with mental illness falls on the family and community. The support system is a quintessential aspect of patient’s recuperation. However, some families experience the feeling of burden, loss, and stigma having a significant impact on the family’s quality of life.Purpose: This meta-synthesis explored the available literature on the experiences of the family about mental illness.Methods: A web-based literature search was conducted. An initial 1,952 studies were retrieved from Web of Science, Scopus, PsycINFO, CINAHL and Ovid-based MEDLINE. Six qualitative studies were included in the review.Results: Findings revealed that family empowerment as the grand theme emerged from the perceived effects of mental illness on the family, the process of coping and their perspectives on family empowerment.Conclusion: Family empowerment offers an openness, readiness, and acceptance towards a holistic care for a family member with a mental illness.

This study describes the development and the preliminary validation of the Family Caregiver Anticipatory Grief Clinical Interview (FcAG-CI). This instrument, conceptually based in Adult Attachment Theory, evaluates Anticipatory Grief response patterns of cancer family caregivers; semistructured interviews were conducted in a sample of family caregivers in palliative care ( n = 72, mostly women, adult children, or spouses, mean age of 52.37) and coded according to the rating guidelines. FcAG-CI, composed of eight domains, shows convergent and concurrent validity with self-reported predeath prolonged grief disorder symptoms as well as divergent validity with the Zarit Burden Interview. The tests for reliability suggest that it has reasonable consistency (Cronbach’s α = .750) and very good interrater reliability. Two factors were identified, corresponding to the two major sources of distress: traumatic and relational aspects of end-of-life caregiving. This instrument, allows the categorization of respondents in four different profiles (Avoidant, Adjusted, Traumatic, and Intense Anticipatory Grief), with clear implications for clinical practice.

Decreasing the Capabilities of the Breadwinner in Meeting the Economic Needs of Families: A phenomenology Study of the Mental Illness People Post-Pasung. The breadwinner of the family economically responsible for meeting the needs of all family members, including clothing, food, and housing needs. A breadwinner of the family with mental illness after pasung needs to adjust to social functions including the ability to work and be involved in social relations. Pasung is physical restraint and confinement by families of people with mental illness in the community. The study aimed to describe the experience of the breadwinner of family post-pasung to supply the economic needs of the family. This qualitative descriptive phenomenological study applied a purposive sampling in selecting the participants. Data were obtained through an in-depth interview with six participants. The data analyzed by Colaizzi’s method. The findings revealed that three themes, namely low self-esteem as a response to changes in the role after pasung, reduction of self-capacity as an obstacle to meeting family's financial needs, and utilization of social and spiritual support in returning confidence. Pasung has an impact on the decline of people with mental illness' ability to carry out the role of the breadwinner of the family. Restoring self-confidence and improving the quality of life for the breadwinner with mental illness after pasung can be obtained from community support, creating employment opportunities and accepting the people with mental illness in the community. Keywords: Breadwinner of the family, Mental Illness, Pasung, Phenomenology Abstrak Kepala keluarga bertanggungjawab secara ekonomi memenuhi kebutuhan hidup anggota keluarga yang meliputi kebutuhan sandang, pangan dan papan. Seorang kepala keluarga yang mengalami gangguan jiwa selepas dari pemasungan perlu menyesuaikan diri terhadap fungsi sosial yang meliputi kemampuan untuk bekerja dan terlibat dalam hubungan sosial. Tujuan penelitian adalah ini untuk mendeskripsikan pengalaman kepala keluarga paska pasung dalam memenuhi kebutuhan ekonomi keluarga. Pendekatan fenomenologi diterapkan untuk mengekplorasi pengalaman kepala keluarga. Sejumlah enam orang berpartisipasi dalam penelitian ini yang dipilih melalui metode purposive sampling. Pengumpulan data dilakukan dengan wawancara mendalam kemudian dianalisis dengan metode Colaizzi. Tiga tema dihasilkan dalam penelitian ini yaitu harga diri rendah sebagai respons perubahan peran paska pasung, penurunan kapasitas diri sebagai hambatan pemenuhan kebutuhan finansial keluarga, dan pemanfaatan dukungan sosial dan spiritual dalam pengembalian kepercayaan diri. Pasung berdampak pada penurunan kemampuan orang dengan gangguan jiwa (ODGJ) menjalankan peran sebagai kepala keluarga. Pemulihan kepercayaan diri dan peningkatan kualitas hidup ODGJ pasca pasung sebagai kepala keluarga dapat diperoleh dengan adanya dukungan masyarakat, penciptaan lapangan kerja serta penerimaan ODGJ di masyarakat. Kata Kunci: Fenomenologi, Gangguan Jiwa, Kepala Keluarga, Pasung
