Influence of Food Security Status and Anemia-Related Knowledge on Perceptions About 2 Nutritious Underutilized Foods Among Ghanaian Caregivers

Background: Nutritious underutilized foods (NUFs) significantly contribute to sustainable dietary diversity but are often unused for many reasons. Objective: We assessed the influence of food security status (FSS) and anemia-related knowledge (ARK) on perceptions about Solanum torvum (turkey berry) and Rhynchophorus phoenicis Fabricius (palm weevil larvae) among Ghanaian caregivers. Methods: A cross-sectional survey was conducted among 891 caregivers (aged 20-49 years), who have children 6 to 59 months old, from Upper Manya Krobo district (Eastern region), Kumasi metropolitan (Ashanti region), Ho municipality (Volta region), La-Nkwantanang-Madina, and Ga West municipality (Greater-Accra region), Ghana. Food security status, ARK, and perceptions about the 2 NUFs were obtained using pretested semi-structured questionnaire. Logistic regression models were used to determine effect of FSS and ARK on perception outcomes. Results: Thirty-six percent of caregivers were food secure, while 13.9%, 28.4%, and 21.7%, respectively, were mildly, moderately, and severely food insecure. Most caregivers (62.0%) scored above 70% on ARK. High favorable perception was significantly lower for palm weevil larvae than that for turkey berry. Food secure caregivers were 4.5 times more likely to have poor favorable perceptions about palm weevil larvae than food insecure caregivers ( P = .03). However, food secure caregivers were 2.9 times more likely to have high favorable perceptions about turkey berry than food insecure caregivers ( P < .001). Caregivers’ knowledge about anemia was associated with high favorable perception about turkey berry by 3.3-fold (95% confidence interval: 2-5.5, P = .001). Conclusions: Nutrition education about turkey berry and palm weevil larvae is needed to encourage their use for promoting nutrient density of complementary and household foods.

Abstract Objectives Availability and access to diversity of foods that are affordable for all peoples always is important for achieving food security status (FSS). Hence, this study examined the influence of FSS on women's perception on underutilized foods: iron-rich plant, solanum torvum (turkey berry, TUB) and edible insect, Rhynchophorus phoenicis fabricius (palm weevil larvae, PWL). Methods This cross-sectional study involved 891 women of reproductive age (WRA) from communities in Upper Manya Krobo district, Kumasi Metropolitan, La-Nkwantanang-Madina and Ho municipality in the Eastern, Ashanti, Greater-Accra and Volta regions of Ghana respectively. Data was collected on socio-demographic characteristics, FSS and perceptions about TUB and PWL using a pretested semi-structured questionnaire. Logistic regression models were used to determine effect of FSS on perception outcomes. Also, 2 focus group discussions (FGD) of 10 WRA each were conducted in each municipality/district and analyzed for major themes and patterns. Results Mean age was 33 ± 8 y and 36% were food secure. High favorable perception about TUB was 60.9% compared to 17.4% for PWL. WRA residing in big towns in the Greater-Accra region were 2.5 times more likely to have high favorable perceptions about TUB and 4.8 times more likely to have low favorable perceptions about PWL compared to WRA in small towns in the Volta region (P = 0.03). Compared to food insecure WRA, food secure WRA were 2.9 times more likely to have high favorable perceptions about TUB and 4.5 times more likely to have low favorable perceptions about PWL (P = 0.001). FGD participants identified TUB as “the blood-giving plant” and PWL as “the meat of delight”. TUB use barriers included its “hard to grind numerous seeds”, “bitter taste” and “funny smell”. PWL use barriers included “it's like a maggot” and “it's scarce”. FGD participants requested education on nutritional benefits of TUB and PWL and processing of these foods into products that can be liked by everyone. Conclusions Drivers of favorable perception about TUB and PWL were region of residence and FSS. Nutrition education and local processing of TUB and PWL into ready-to-use products is recommended as a low-cost sustainable strategy for improving nutrient density of complementary and household foods in communities where these foods are culturally accepted. AbstractObjectiveTo describe: household dietary diversity across four zones in Ethiopia; the relationship between household dietary diversity and consumption of vitamin A-rich foods; and the relationship between household dietary diversity and food security status.DesignThis was a cross-sectional survey. Data were collected using structured questionnaires in the local language. Household dietary diversity scores measured types of foods households consumed, and households were classified by food security status using a modified version of the Household Food Insecurity Access Scale. An ordinal logistics regression model was created to assess the relationship between three tiers of dietary diversity (low, medium and high) and food security while controlling for agricultural zone, educational variables and household characteristics.SettingRural households in Tigray, Ethiopia.SubjectsThree hundred households in Tigray, Ethiopia, were interviewed.ResultsOf the households, 23, 47 and 30 % had low, medium and high dietary diversity, respectively. Among households with high dietary diversity, eggs and fruit were the most common foods added to the diet. In the fully adjusted model, participants who reported being food secure had 1·8 increased odds of greater dietary diversity (95 % CI 1·0, 3·2) compared with participants who were food insecure.ConclusionsFood security was positively associated with dietary diversity. In order to enhance health, interventions that improve dietary diversity and vitamin A consumption should remain important areas of focus for health leaders in the region.

Background: Malnutrition and food insecurity are major challenges in Somalia, particularly among small children living in internally displaced person (IDP) camps. Poor diet has been identified as a key driver of malnutrition in young children who depend for their diets on their household’s socioeconomic standing and access to food, as well as on the family’s caring and feeding practices. Objective: To assess the dietary diversity and identify the factors associated with it among children (6-23 months) in Somalian IDP camps. Methods: We used a cross-sectional survey conducted in 11 IDP camps in Somalia in June 2014 and in June 2015. A total of 3188 children aged 6 to 23 months were surveyed. Child diets were assessed using food frequency questionnaires, and dietary diversity was categorized using the minimum child dietary diversity (MDDC) indicator. Multivariable logistic regressions were used to identify the factors associated with the children’s dietary diversity. We built and compared 2 models using alternatively the household dietary diversity score (HDDS) and the food consumption score (FCS) as food security proxies. Results: Around 15% of children in IDP camps reached the minimum dietary diversity. Overall, our results confirm that not only are food security proxies the factors most associated with MDDC, but HDDS performs better than FCS. In addition, results identify that women as key decision-maker in the household, duration of household permanence in the settlement, women’s physiological status, frequency of milk feeding to child, type of toilet, and measles vaccination are positively associated with MDDC. Conclusions: To improve child dietary diversity in IDP camps, food security interventions should be broadened to include female empowerment and inclusive nutrition education (encouraging male participation) programs, as well as initiatives targeting children who do not live with pregnant or lactating women and that can support families beyond the first months after their arrival.

Food security at the household level remains a major issue in South Africa and for many other developing countries, particularly those in Africa. As a means of ensuring food security in KwaZulu-Natal province, various food security intervention programmes were launched. Nonetheless, food security remains an issue among households in the province. This paper estimates the household food security status of the “One Home One Garden” (OHOG) beneficiaries against that of non-beneficiaries and assesses the determinants of household food security status in Maphumulo. A stratified random sampling technique was used to sample 495 households (including 330 OHOG beneficiaries and 165 non-beneficiaries). The status of household food security was estimated by means of a “Household Dietary Diversity Score” (HDDS). Additionally, a Household Food Consumption Score” (HFCS) tool was employed to supplement the HDDS. The results showed that food consumption patterns were characterized by medium (4.89) and average (4.22) HDDS for the OHOG beneficiaries and non-beneficiaries, respectively. Taking HDDS as a proxy for household food security, an independent samples t-test (Levene’s test—equal variances assumed) reveals a significant (p < 0.001) relationship between the sample means of the two groups. A greater proportion (65%) of the OHOG beneficiaries had an acceptable (≥35) HFCS level, whereas just over half (54%) of the non-beneficiaries fell in the borderline (21.5 to 35) HFCS level. The determinants of household food security status were elicited by means of a binary logistic regression model. The results revealed that education (p = 0.036), receiving infrastructural support (irrigation) (p = 0.001), and participation in the OHOG programme (p = 0.000) positively influenced the food security status of households, yet household income (p = 0.000) and access to credit (p = 0.002) showed a negative correlation. This paper proposes that government and developmental agencies, in their efforts to enhance food security through food security intervention programmes, should support households by investing in education and agricultural infrastructure, as well as giving priority to smallholder infrastructural irrigation support for households that largely rely on rain-fed systems.

Food security is one of the most severe challenges facing the majority of African countries. The objective of this study was to explore household food dietary diversity and food security in a rural area in the Eastern Cape Province, South Africa. A total of 296 household heads were randomly sampled to participate in the study. The Household Dietary Diversity Score (HDDS) and Household Food Consumption Score (HFCS) were used to identify the consumption patterns of the households and their food security status. Meanwhile, a binary model was used to identify the variables that had an impact on household food security. Findings from the Household Dietary Diversity Score (HDDS) revealed that 61 percent of the households had lower dietary diversity and were consuming at least three food groups, which mainly include pulses, milk, and cereals. The results from the Household Food Consumption Score (HFCS), however, showed that the majority of the households had adequate levels of food consumption. The binary model revealed that age, household income, access to credit, and gender are statistically significant in influencing household food security status in the study area. It can be concluded that household dietary diversity is not guaranteed by food security, as proven by the regression model. Therefore, the government should consider the impact of low income on food security and it should intensify efforts directed at helping rural households to reduce incidences of food insecurity.

We explore the association between quality of diet and food security status among older persons. Cross-sectional survey of large national samples, from the National Health & Nutrition Examination Surveys (1988-1994 & 2001-2002), in the United States. A total of 5,039 and 2,040 men and women aged 60 and older, respectively. The dependent variable was the healthy eating index (HEI; 2000 and 2005) as a measure of diet quality. The independent variable of interest was food security status and was categorized as food secure, food insecurity receiving food stamps and food insecurity not receiving food stamps. In multivariate analyses, food insecure individuals not receiving food stamps had lower total HEI-2000 scores compared to food secure individuals (b=-3.91, SE=1.81, P=0.0310). Factors independently associated with high HEI (2000 or 2005) scores were female gender, being married, highly educated or wealthy, with very good/ excellent perceived health, or without functional limitations. Food secure individuals had better quality of diet than food insecure individuals. Among food insecure individuals, receiving food stamps was not associated with diet quality. Food insecure families with older individuals may need new methods of dissemination of food and nutrition programs – methods that yield positive and promising changes in the health status of this special population group.

The discourse on food security in India has concentrated upon availability and accessibility of cereals, neglecting the nutritional significance of fruits, vegetables and meat food products. This article attempts to assess the food security level both quantitatively and qualitatively, and level of dietary diversity among socio-economic groups. A cross sectional survey covering 304 households selected through stratified and proportional random sampling methods was conducted in six villages of Uttar Pradesh, India in 2013–2014. Household Food Insecurity Access Scale and Dietary Diversity approach were used to indicate food security. Subjective assessment highlights that despite sufficient availability of food, most of the households were unable to eat the quality of food they preferred. Findings also show predominant concentration of diet towards cereals, milk, root and tuber food products. Muslims, scheduled caste, labourers and households with lower wealth index consumed a less diversified diet, which is statistically significant. This article provides important insights into the strength and limitations of food policy measures and underlines the need to move beyond the dominant cereal-based interventions for food security in emerging economies like India. Abstract BackgroundGlobally nations are advocating for universal health coverage which argues for health access for all however, inequalities in child health remain a threat to this global initiative. Even though malnutrition and food insecurity are now dominating the global development agenda, there are substantial gaps on literature about patterns and trends of socioeconomic inequalities in food insecurity and malnutrition in many developing countries. Globally an estimated 3.1 million children die annually as a result of undernutrition, shockingly sub-Saharan Africa accounts for majority of the most nutritionally insecure and food insecure children in the world. In previous decades’ prevalence of stunting in Zimbabwe has been erratic. This paper assessed socioeconomic inequalities in child health focusing on malnutrition and food insecurity in Zimbabwe.MethodsThe study used Demographic Health Survey (DHS) data sets of 2010\11 and 2015. Food insecurity in children was determined based on the WHO dietary diversity score. The study adopted the WHO dietary diversity score informed by the Infant and Young Child Feeding (IYCF) practices. Minimum dietary diversity as an indicator for food security is defined by a cut- off point of >4, therefore for this study children with less than 3 of the 13 food groups were defined as food insecure. Malnutrition was assessed using weight-for-age Z-scores, with children whose weight-for-age Z-score below minus two standard deviations (-2 SD) from the median considered malnourished. Concentration indices were computed to understand if malnutrition was dominant among the poor or rich. The paper used the Theil index, which is a generalized entropy measure and decomposed the indices by population subgroups (geographical clusters and socioeconomic status) so as to separate total inequality in the distribution between the selected groups and remaining within-group inequalities.ResultsFor the period under review malnutrition prevalence increased by 1.03 percentage points (p.p) [2010/11(3.73%); 2015(4.76%)], while food insecurity prevalence decreased by 4.35p.p [2010/11(78.29%);2015(73.94)]. Prevalence of malnutrition and food insecurity increased by; 9.6p.p and 2p.p among poor children & 10.23p.p and 0.5p.p among rural children. Concentration indices showed that; children from wealthy households were more likely to be food secure (pro-rich) while, children from poor households were more likely to be nutritious (pro-poor). For nutrition status socioeconomic inequality gaps appear to be widening as the concentration indices between the two time periods increased, while for food security status socioeconomic inequality gaps appear to be contracting as the concentration indices between the two time periods reduced. Decomposed Theil indices by geographical clusters (urban & rural) for; food security status shows contracting socioeconomic inequality gaps in both geographical clusters (urban & rural), while for nutrition status the Theil indices reflect widening socioeconomic inequality gaps among urban children and contracting socioeconomic inequality gaps among rural children.ConclusionThe study concluded within-group inequalities to be driving most of the socioeconomic inequalities in nutritional status and food security status of children in Zimbabwe. Therefore, Zimbabwean government should design policies that focus on addressing within-group inequalities and direct food security interventions for food insecure children through availing food aid.
