The current case presents a patient with a tumor emboli stroke as a presenting symptom of a liposarcoma metastasis to the heart that was treated successfully with endovascular mechanical retrieval, followed by subsequent cardiac surgery. The patient is still alive, under chemotherapy treatment, 3 years following the interventions. This scenario should be considered as a part of the differential diagnosis of oncology patients presenting with new central neurological symptoms. This active approach can be an effective treatment if the patient is fit and there is no evidence of widespread disease.

Ocular albinism is an X-linked melanosome biogenesis disorder, leading to mild cutaneous symptoms and persistent visual impairment in affected males. As a result of mutations in the GPR143 gene, a defect occurs in the transformation of melanosomes into macromelanosomes with loss of function at the GPBR143 receptor. Clinically, manifests with nystagmus, which typically occurs in infants until the sixth month from birth. Optic nerve misdirection which can be detected by VEP is important in the differential diagnosis of another infantile nystagmus. Foveal hypoplasia is the most responsible finding for poor vision. Today, functional losses are tried to be minimized by differential diagnosis and early treatment. It is predicted that more effective treatment can be provided with gene therapy in the future.

This case study presents a patient living in a suburban/rural community who received appropriate referral to secondary and tertiary care for nausea and vomiting, accompanied by waxing and waning neurological symptoms, yet proved difficult to diagnose. This patient is presented to draw attention to a rare neurological disorder which should be included in the differential diagnosis of nausea and vomiting with some key neurological complaints, even in the absence of physical findings.

Chronic fatigue syndrome (CFS) is a common and disabling problem; although most likely of biopsychosocial origin. The nature of the pathophysiological components remains unclear. It is an illness characterized by persistent and relapsing fatigue, often accompanied by numerous symptoms involving various body systems. The etiology of CFS remains unclear. Prognosis in neonatal endocarditis depends on early diagnosis and the status of the patient at the time of diagnosis, and effective treatment with appropriate antibiotics. ✓ Whipple's disease is infrequently considered in the differential diagnosis of patients presenting with progressive neurological deterioration. This is in part a result of the relative rarity of this entity and in part due to the more frequent initial presentation of the disease with gastrointestinal, musculoskeletal, or cardiovascular symptoms. A case is described in which the neurological symptoms of progressive dementia and weakness were seen in the relative absence of non-neurological symptomatology. The diagnosis of Whipple's disease was made from a brain biopsy. The neuropathology of Whipple's disease of the central nervous system is described and the importance of considering it as a treatable entity in the differential diagnosis of progressive neurological deterioration, despite the absence of systemic symptomatology, is stressed.

Background Pemphigoid nodularis is a rare clinical variant of bullous pemphigoid characterized by overlapping clinical features of both prurigo nodularis lesions and bullous pemphigoid blisters. The condition appears to be more common in females and is often resistant to treatment. Objective To raise awareness of this rare variant of bullous pemphigoid. Methods Case report. Results and Conclusion Dermatologists should include this variant in differential diagnosis of prurigo nodularis because early recognition can lead to an effective treatment for the prurigo component.

India has been termed as Diabetic hub of the globe. Along with other complications, one of the underestimated complication is cognitive decline and memory loss associated with poor diabetes control. In the Indian context, structural imaging like MRI-Magnetic Resonance imaging is more commonly used in Neurological disorders such as stroke, head injury, Functional imaging of the human brain has been underutilized in the Indian scenario. One such technique is PET CT which has been typically used as a cancer biomarker in India. By virtue of current case study, we propose i) association between poor control of Diabetes and poor cognition ii) role of PET CT brain in differential diagnosis of Dementia. We present two case reports providing strong findings for utilization of PET CT brain in Dementia protocols.
